ABSTRACT

El Covid 19 es un coronavirus que afecta directamente al sistema respiratorio, es de un elevado nivel de contagio, provocando que los hospitales han visto desbordado sus servicios de emergencia. El área de triage que atiende la unidad de emergencia pediátrica del hospital General Puyo, no escapa a esta realidad. Es por ello que el presente estudio tiene como fin establecer el nivel de conocimiento que presenta el personal de enfermería que labora en el área de triage sobre los procedimientos de clasificación de los pacientes en medio de la pandemia del Covid 19. El estudio fue descriptivo, de corte transversal. La técnica que se utilizó para la recolección de datos es la encuesta. Como instrumento se utilizó un cuestionario dicotómico aplicado a 21 enfermeras del Hospital Provincial General Puyo que desempeñan funciones en el servicio de emergencia. En conclusión, que el área de la emergencia pediátrica referida al triage de emergencia, le debe crear un mayor compromiso al personal de enfermería por mejorar su conocimiento sobre el nivel de atención que posee cada paciente y más aún él era de pediatría que es el sector más indefenso porque muchas veces no saben expresar que es lo que está sintiendo. Resalando finalmente, que durante el , desarrollo de esa investigación, se cumplieron las normas éticas establecidas por la OMS y OPS para el abordaje de pacientes positivo covid-19(AU)


Covid 19 is a coronavirus that directly affects the respiratory system, it is of a high level of contagion for this reason, hospitals have seen their emergency services overwhelmed, in this sense the triage area that attends the pediatric emergency unit of the hospital General Puyo, does not escape this reality. That is why the present study aims to establish the level of knowledge presented by the nursing staff working in the triage area about the procedures for classifying patients in the midst of the Covid 19 pandemic. The study was descriptive, cross-sectional. the technique used for data collection is the survey. The instrument used was a dichotomous questionnaire applied to 21 nurses from the Puyo Provincial General Hospital who perform functions in the emergency service. In conclusion, that the pediatric emergency area, referring to emergency triage, should create a greater commitment to the nursing staff to improve their knowledge about the level of care that each patient has and even more so he was pediatric, which is the sector more defenseless because many times they do not know how to express what they are feeling(AU)

ABSTRACT

Objective To observe the accuracy of the five-level adult emergency triage system (FLAETS) in determining the severity of critically ill patients,and to analyze the factors that influenced the accuracy of the triage.Methods The triage information of patients admitted to Huashan North Hospital Affiliated to Fudan University between 1 year before and 1 year (started in July 2014) after the implementation of the FLAETS were retrospective analyzed.The patients who triaged by FLAETS (from September 2014 to August 2015) were served as observational group,and those manual triaged by modified early warning score (MEWS,from July 2013 to June 2014) were set as control group.The patients with triage of Ⅰ,Ⅱ,and Ⅲ levels were enrolled (fatal,critical,urgent),and the triage results of emergency physicians-in-chief on duty were set as gold standard.The gender,age,triage level,the proportion of over-triage,the proportion of under-triage,and mortality were compared between two groups.The reasons for the overtriage and under-triage were analyzed.The accuracy of FLAETS in determining severity of critically ill patients was calculated.Results 18449 patients were enrolled in observational group,and 720,1641 and 16088 patients were triaged as level Ⅰ,level ⅡⅡ,level Ⅲ respectively;17 378 patients were triaged as critically ill patients according to gold standard,and level Ⅰ,level Ⅱ,level Ⅲ were 637,1476 and 15 265 patients respectively.6 352 patients were enrolled in control group,and level Ⅰ,level Ⅱ,level Ⅲ were 204,771 and 5 377 patients respectively;6002 patients were triaged as critically ill patients according to gold standard,and level Ⅰ,level Ⅱ,level Ⅲ were 308,836 and 4 858 patients respectively.There were no statistically significant differences in gender and age between two groups (both P > 0.05).Compared with the control group,the proportion of over-triage of level Ⅲ was significantly lowered [8.3% (1 329/16088) vs.12.5% (674/5 377),P < 0.01],the proportion of under-triage of level Ⅱ and level Ⅲ was significantly lowered [level Ⅱ:0.6% (9/1 641) vs.7.0% (54/771),level Ⅲ:0.4% (63/16088) vs.4.7% (254/5 377),both P < 0.01].The success rate of resuscitation in observation group was significantly higher than that of control group [80.70% (619/767) vs.75.23% (410/545),P =0.020],and the mortality was significantly lowered [1.11% (193/17 378) vs.2.35% (141/6002),P =0.037].Over-triages were mainly found in patients with chronic obstructive pulmonary disease (COPD),hypertension and the main complaint of chest tightness,shortness of breath,and under-triages were mainly found in patients with the multiple organ dysfunction in the elder,atypical heart disease and pneumothorax.The accuracy rates of level Ⅰ,level Ⅱ and level Ⅲ in observation group (99.37%,97.42% and 89.58%) were significantly higher than those of the control group (97.51%,92.54%,and 80.16%,all P < 0.01).Conclusion The FLAETS in determining severity of critically ill patients were objective and reliable,which enable the nurse to better handle the relationship of the emergency and the ordinary,the severe and the mild,the priority and the non-priority,which made the patients received timely and effective treatment.

ABSTRACT

BACKGROUND: Most current triage tools have been tested among hospital nurses groups but there are not similar studies in university setting. In this study we analyzed if a course on a new four-level triage model, triage emergency method (TEM), could improve the quality of rating in a group of nursing students.METHODS: This observational study was conducted with paper scenarios at the University of Parma, Italy. Fifty students were assigned a triage level to 105 paper scenarios before and after a course on triage and TEM. We used weighted kappa statistics to measure the inter-rater reliability of TEM and assessed its validity by comparing the students' predictions with the triage code rating of a reference standard (a panel of fi ve experts in the new triage method).RESULTS: Inter-rater reliability was K=0.42 (95%CI: 0.37-0.46) before the course on TEM, and K=0.61 (95%CI: 0.56-0.67) after. The accuracy of students' triage rating for the reference standard triage code was good: 81% (95%CI: 71-90). After the TEM course, the proportion of cases assigned to each acuity triage level was similar for the student group and the panel of experts.CONCLUSION: Among the group of nursing students, a brief course on triage and on a new in-hospital triage method seems to improve the quality of rating codes. The new triage method shows good inter-rater reliability for rating triage acuity and good accuracy in predicting the triage code rating of the reference standard.

ABSTRACT

Objective To Describe the queuing mathematical model and analyze the reasonable hospital application pattern of electronics voluntary system.Methods By scientifically arranging medical care personnel and equipment,patients queuing process was optimized.Results We successfully applied the outpatient real-time computer information management system of electronics voluntary based on this mathematical model.Conclusion It is a possible way to optimize the queuing process and improve the efficiency in electronic triage system based on queuing theory.
