ABSTRACT

Chronic atrophic gastritis (CAG) and gastric intestinal metaplasia (GIM) significantly increase the risk of gastric cancer. Blocking the development of CAG and GIM would help to decrease the incidence of gastric cancer. It was revealed that eradication of Helicobacter pylori could reverse gastric mucosa atrophy. Recent studies reported that GIM could be reversed to a certain extent. Clinical studies demonstrated that Lamb′s tripe extract and vitamin B12 capsule exhibited significant reversal effects on GIM. The present study systemically reviewed the diagnosis and treatment of CAG and clinical application of Lamb′s tripe extract and vitamin B12 capsule, and established the specialist instruction that would guide the reversal treatment of CAG and GIM.

ABSTRACT

Objective:To explore the therapeutic effects of lamb′s tripe extract and vitamin B12 (hereinafter referred to as lamb′s tripe) capsule on precancerous lesions of gastric mucosa in rats.Methods:Thirty-two rats of the 42 Wistar rats (model group) were selected for modelling, and in model group six rats died due to gavage, 10 rats were sacrificed for observing the results of modeling, and the left 16 rats were divided into administration group (eight rats) and non-administration group (eight rats). After modeling, five of the 10 rats without modelling treatment were selected as the normal control of the model group, the other five (negative control group) rats were included in drug intervention experiment. The drug intervention program was as follows: in administration group, rats were gavaged with lamb′s tripe 0.2 g/kg once per day for three months; in non-administration group and negative control group, rats were gavaged with 0.9% sodium chloride solution 0.2 g/kg once per day for three months. One rat died in each for the administration group and non-administration group due to gavage. Body weight gain, pH value of gastric juice and pathological changes of gastric mucosa of the three groups were evaluated. The number of nodules on gastric mucosal surface and the incidence and scores of precancerous lesions (intraepithelial neoplasia) in gastric mucosal were analyzed. The therapeutic effects of lamb′s tripe capsule on gastric mucosal precancerous lesions in rats were evaluated. Independent sample t test , Mann Whitney U test and chi-square test were used for statistical analysis. Results:The body weight gain of rats at the 6th week in the administration group was higher than that of rats in the non-administration group ((508.26±33.96) g vs. (495.50±23.01) g), and the pH value of gastric juice of rats in the administration group was lower than that of rats in the non-administration group (3.07±0.55 vs. 4.45±0.72), and the differences were statistically significant (both P<0.05). The number of proliferative nodules on the gastric mucosal surface of the rats in the administration group was less than that of rats in the non-administration group (the ratio of gastric fundus: 6.00(3.00, 7.00) vs. 11.00(7.00, 13.00); the ratio of gastric antrum: 0.00(0.00, 1.00) vs. 3.00(2.00, 4.00); the ratio of whole stomach: 7.00(3.00, 10.00) vs. 15.00(13.00, 17.00)), and the differences were statistically significant ( U=43.50, 49.00, 49.00, all P<0.05). The score of gastric mucosal precancerous lesions in the administration group was lower than that in the non-administration group(1.00±0.00 vs.1.14±0.38), and the incidence of precancerous lesions in the administration group was lower than that in the non-administration group (1/7 vs. 5/7), and the differences were statistically significant ( t=2.45, χ2=4.67, both P=0.031). Conclusions:Lamb′s tripe capsule can significantly inhibit the progression of precancerous lesions of gastric mucosa in rats, so as to play a role in preventing the occurrence of gastric cancer.

ABSTRACT

Objective:To investigate the efficacy and related factors of lamb′s tripe extract and vitamin B12 capsule (LTEVB12) in the treatment of chronic atrophic gastritis. Methods:From October 1st 2016 to April 30th 2021, 240 patients with chronic atrophic gastritis visited the Department of Gastroenterology at Xijing Hospital, Air Force Medical University were retrospectively analyzed. All patients regularly took LTEVB12 (110 U/day, 3 times/day) for six months. At the end of treatment, endoscopy and gastric mucosal biopsy were conducted. The therapeutic effects were evaluated by comparing the changes of operative link on gastritis assessment (OLGA) and operative link on gastritis assessment based on intestinal metaplasia (OLGIM) staging before and after treatment. The related factors affecting the efficacy of the drug were analyzed. Multivariate logistic regression analysis was used for statistical analysis. Results:After half a year of treatment, the reversal efficiency of atrophy and intestinal metaplasia was 45.4% (109/240) and 37.9% (91/240), respectively, and the total efficiency was 62.9% (151/240). The reversal efficiency of OLGA and OLGIM staging reversed from high stage (stage Ⅲ to Ⅳ) to low stage (stage 0 to Ⅱ) was 53.4% (63/118) and 54.5% (36/66), respectively. The results of multivariate analysis showed that female, vitamin supplementation (≥3 times/week), negative or successful eradication of Helicobacter pylori and mild inflammatory status (inflammation score: 1 to 2) were associated with improving the efficacy of LTEVB12 (odds ratio=1.798, 3.730, 2.817 and 4.631, 95% confidence interval 1.055 to 3.064, 1.197 to 11.627, 1.171 to 6.779, 1.480 to 14.493; all P<0.05). High consumption of pickled food (≥3 times/week) was associated with reducing efficacy of LTEVB12 (odds ratio=0.384, 95%confidence interval 0.200 to 0.740). Conclusion:LTEVB12 has better reversal therapeutic effect on atrophy and intestinal metaplasia, and may reduce the risk of gastric cancer in patients with chronic atrophic gastritis.

ABSTRACT

Usually gastrointestinal malignancies present with low SAAG ascites. But when there is diffuse liver infiltration following malignancy, high SAAG ascites can occur. So liver infiltration can masquerade as cirrhosis. Malignant acanthosis and tripe palm are the paraneoplastic manifestations seen in GI malignancies. We are reporting a case which was initially managed as a  straightforward case of cirrhosis ,but later turned out to be a case with tripe palms, malignant acanthosis and carcinoma stomach as primary with diffuse liver infiltration having high SAAG ascites.

ABSTRACT

This paper points to an open dialogue on issues that demand our attention concerning psychoanalytical training in our days. Besides the recognition of the unquestionable reputation of the classic tripod and its present in the different training models, its advantages and limitations, it supports the thesis that psychoanalytical training should enlarged (expand) its horizons without losing its specificity. The paper justifies this thesis as well as offer possible pathways and directions for implementation.

ABSTRACT

Objective To investigate the influencing factors and improve potency methods of chymosin, to verify the stability and applicability of the national standard of chymosin. Methods The effects of different formula milk powder substrate and enzyme concentration on the determination of the activity of chymosin were studied.3 ×3 dose-response parallel line method was established. The results were compared with the different methods of absolute and relative methods. Results The different formula milk powder had a significant effect on the determination of the absolute potency of the activity of chymosin. The concentration of the enzyme was a power function relationship with the milk clotting time.Compared with the absolute potency, reproducibility of the relative potency of the results was better in different laboratories. The suitable doses in 3 ×3 dose-response parallel line method were 0.35,0.44,0.55U/mL.The confidence limit rate was less than 5%.The potency of the national standard of chymosin (140712-201302) was not significantly different between 2013 and 2015.In a certain dose range, the dose-response of the national standard of chimosin and gastropylor complex or lamb'tripe extract was linear, and the two lines were parallel. Conclusion A lot of factors can affect on the potency of chymosin.Relative potency is determinate by reference standard which can eliminate the influence of different substrates, different operators and endpoint judgment on the determination in order to make results have comparability between laboratories. The test design of 3 ×3 dose-response parallel line can control the test deviation better than the single point determination. The stability of the national standard of chymosin(140712-201302) is good, and is suitable for the potency of chymosin of the products of gastropylor complex and the extract of the lamb.

ABSTRACT

This paper stems from the psychoanalytical transmission experience and training in the Psychoanalysis Department of Sedes Sapientiae Institute, in order to present the theoretical grounds of the so-called analytical tripod. The paper claims that an institutional project on psychoanalytical training should be grounded on epistemological convictions about the object of psychoanalysis, the analytical practice and the appropriate institutional conditions. All the elements of the tripod should result in analytical effects. Moreover, the subjacent institutional devices should also work in agreement with the tripod, creating a friendly atmosphere, however not leading to alienation...

ABSTRACT

Cutaneous paraneoplastic syndromes are the non-contiguous skin and mucous membrane changes that are caused by internal malignancy. Since they may be the first sign of a new or recurrent malignancy, recognizing their features and clinical implication is very important. We describe three patients with internal malignancy such as stomach adenocarcinoma or biliary tract adenocarcinoma who showed coexistence of acanthosis nigricans, tripe palms, and cutaneous florid papillomatosis. We reviewed the literatures about these cutaneous paraneoplastic syndromes and discussed their relationship between this striking cutaneous manifestations and internal malignancy.

ABSTRACT

Florid cutaneous papillomatosis(FCP) is a disease consisting of multiple acuminate keratotic papules in association with an underlying malignancy. Most common underlying malignancy is gastric adenocarcinoma. Other associated malignancies are breast cancer, bladder cancer, hepatobiliary cancer, ovarian cancer and uterine cancer. An association of FCP with other signs of internal cancer is common, with malignant acanthosis nigricans usually appearing many times with sign of Leser-Trelat. We experienced a florid cutaneous papillomatosis associated gastric adenocarcinoma in an 84 years old woman. The patient had a severe itching and showed numerous tiny papules on the trunk(FCP) and brown verrucose papules on the axillae(acanthosis nigricans) and hyperkeratotic yellowish tiny papules on the palms(tripe palm). Histopathologically, the lesion of the trunk exhibited papillomatosis, hyperkeratosis, and acanthosis in the epidermis. Gastroscopic biopsy revealed gastric adenocarcinoma.

ABSTRACT

The purpose of this study was to evaluate the postoperative outcomes between the glaucoma triple procedure and the small incision cataract surgery using foldable silicone intraocular lens(IOL). This prospective randomized study comprised of 42 eyes which had the glaucoma triple procedure and 41 eyes which underwent small incision cataract surgery at chonnam University Hospital from May, 1994 to December, 1996. At least 3 months follow-up period was needed. Corrected visual acuity of 0.5 of better was obtained in 50% of triple procedure group and 88% of small incision cataract surgery group with the average lines of improved visual acuity of 5.6 and 7.2 respectively. There was no significant change in the refractive cylinder during the early postoperative period in both group. In the glaucoma triple procedure group, posterior capsular opacification(PCO) developed in significantly low incidence in the Mitomycin C using group and the incidence of PCO in eyes with IOL decentration was greater than in those without IOL decentration. In conclusion, patients who underwent glaucoma triple procedure using foldable silicone IOL showed excellent postoperative IOP control with success rate of 81%, rapid visual rehabilitation and refractive stability. We also found that glaucoma triple procedure with Mitomycin C was associated with lower incidence of PCO.
