ABSTRACT

Chagas disease is an endemic zoonosis that can cause chronic medical complications in a third of those infected, usually decades after infection. It mainly affects the peripheral nervous system of heart, esophagus and colon. At digestive level, motor dysfunction leads to the development of megaesophagus and megacolon whose predominant symptoms are dysphagia and constipation. It should be suspected in patients with epidemiological history and compatible symptoms. In the chronic phase it is confirmed with the detection of specific antibodies. The etiological treatment is effective in early stages after infection. In the chronic stage the management is symptomatic, medical and/or surgical.


La enfermedad de Chagas es una zoonosis endémica que puede producir complicaciones médicas crónicas en un tercio de los infectados, habitualmente décadas luego de la infección. Afecta principalmente el sistema nervioso periférico de corazón, esófago y colon. A nivel digestivo la disfunción motora lleva a la formación de megaesófago y megacolon cuyos síntomas predominantes son disfagia y constipación. Debe sospecharse en pacientes con antecedentes epidemiológicos y síntomas compatibles. En la fase crónica se confirma con la detección de anticuerpos específicos. El tratamiento etiológico es efectivo en las fases tempranas post-contagio. En la etapa crónica el manejo es sintomático, médico y/o quirúrgico.

ABSTRACT

Se presenta el caso de un paciente masculino con síntomas abdominales de corta evolución que ingresa con un síndrome coronario agudo con elevación del ST y cuadro sugestivo de pericarditis, el cual fallece pocas horas después de su ingreso. Se documentó en la autopsia una miocarditis linfocítica secundaria a infección por Tripanosoma.


The case of a male patient with a short evolution of abdominal symptoms, an acute coronary syndrome with ST elevation and a clinical suspicion of pericarditis, who dies shortly after his admission to the emergency room. The autopsy findings presented a linfocitic myocarditis secondary to Tripanosoma infection.

ABSTRACT

La enfermedad de Chagas o tripanosomiasis sudamericana, es una parasitosis crónica causada por el protozoario flagelado Tripanosoma cruzi. Este agente puede ser transmitido de la madre al hijo por vía transplacentaria durante la fase aguda y/o crónica de la infección materna. Objetivo: Evaluar la seroprevalencia de la enfermedad de Chagas en gestantes y en niños de madres seropositivas a T. cruzi y su relación con algunos factores condicionantes en el Municipio Roscio del estado Guárico. Métodos: Se realizó un estudio de tipo transversal, estudiando la enfermedad de Chagas en gestantes y en niños (as) nacidos de madres reactivas al T. cruzi. La muestra estuvo conformado por una población representada por todas las gestantes (925) que acudieron al primer control de embarazo en la red primaria de atención, y otra, por los hijos (as) (2) nacidos de madres seropositivas a T. cruzi. Resultados: Se detectaron dos gestantes reactivas utilizando Anticuerpos IgG, obteniéndose una seroprevalencia de 0,22 por ciento. Los dos hijos nacidos de estas madres reactivas resultaron No Reactivos mediante la técnica parasitológica directa (Microhematocrito), confirmando el diagnóstico con ELISA y Hemaglutinación Indirecta.

INTRODUCTION: Chagas disease is caused by Trypanosoma cruzi. Wild and perianthropic mammals maintain the infection/transmission cycle, both in their natural habitat and in the peridomestic area. The aim of this paper was to present the results from a study on wild rodents in the central and northern regions of San Luis province, Argentina, in order to evaluate the prevalence of this infection. METHODS: Sherman traps were set up in capture areas located between latitudes 32º and 33º S, and longitudes 65º and 66º W. The captured rodents were taxonomically identified and hemoflagellates were isolated. Morphological, biometric and molecular studies and in vitro cultures were performed. Infection of laboratory animals and histological examination of the cardiac muscle and inoculation area were also carried out. Parasites were detected in circulating blood in Calomys musculinus, Graomys griseoflavus, Phyllotis darwini and Akodon molinae. The parasites were identified using biological criteria. Molecular PCR studies were performed on some isolates, which confirmed the characterization of these hemoflagellates as Trypanosoma cruzi. RESULTS AND CONCLUSIONS: Forty-four percent of the 25 isolates were identified as Trypanosoma cruzi, and the remaining 56 percent as Trypanosoma cruzi-like. These findings provide evidence that wild rats infected with Trypanosoma cruzi and Trypanosoma cruzi-like organisms are important in areas of low endemicity.


INTRODUÇÃO: A doença de Chagas é causada pelo Trypanosoma cruzi e os mamíferos periantrópicos e silvestres mantêm o ciclo de infecção/transmissão, tanto no ambiente natural, como no peridomicílio. O objetivo deste trabalho foi mostrar os resultados de um estudo de roedores silvestres do centro e norte da Província de San Luis, Argentina, para avaliar a prevalência da infecção. MÉTODOS: Estabeleceram-se lugares de caça com armadilhas tipo Sherman entre os 32º - 33º de latitude S e 65º - 66º de longitude W. Identificou-se taxonomicamente os roedores, isolou-se os hemoflagelados e fizeram-se estudos morfológicos, biométricos, moleculares, cultivo in vitro, infecção a animais de laboratório, histologia de músculo cardíaco e de zona de inoculação. Observou-se parasitas em sangue circulante: Calomys musculinus, Graomys griseoflavus, Phyllotis darwini e Akodon molinae. A identificação dos parasitas foi feita utilizando critérios biológicos e, em alguns, realizou estudos moleculares por PCR que confirmaram a caracterização desses hemoflagelados como Trypanosoma cruzi. RESULTADOS E CONCLUSÕES: Dos 25 isolados, 44 por cento foram identificados como Trypanosoma cruzi e 56 por cento como Trypanosoma cruzi like. Este achado nos induz a considerar a importância dos ratos do mato infectados com Trypanosoma cruzi y Trypanosoma cruzi like, em área de baixa endemicidade.

ABSTRACT

As soon as they were published early in 1909, Chagas's articles on Trypanosoma cruzi and American trypanosomiasis became the topic of discussions in France. The description of T. cruzi and Chagas disease was added to parasitology textbooks as early as 1912, and elicited active research, particularly on the part of French parasitologist Emile Brumpt. He contributed towards eluciding the lifecycle of T. cruzi and the different ways it could infect humans. Laboratory research on T. cruzi was interrupted by First World War and was not resumed afterwards on the same scale, although interest in the epidemiology of Chagas disease continued.


Assim que os artigos de Carlos Chagas sobre o Tripanosoma cruzi e a tripanosomíase americana foram publicados em 1909, passaram a ser tema de discussões na França. A descrição do T. cruzi e da doença de Chagas foram adicionadas a livros de parasitologia ainda em 1912, e ajudaram a elucidar pesquisas, particularmente as que vinham sendo desenvolvidas pelo parasitologista Emile Brumpt. Ele contribuiu para o esclarecimento do ciclo de vida do T. cruzi e as diferentes maneiras que poderiam infectar osseres humanos. Pesquisa de laboratório sobre o T. cruzi foi interrompida durante a Primeira Guerra Mundial e não foi retomada mais tarde na mesma escala, a despeito do fato de os estudos epidemiológicos sobre a doença de Chagas continuarem despertando muito interesse.

La enfermedad de Chagas, es uno de los problemas de salud publica de mayor impotancia en America Latina. Aunque la OPS ha estimado una seroprevalencia alrededor de 0.25%, para la enfermedad de Chagas en el ecuador, analisis recientes destacan una prevalencia general, a nivel pais, de 1.38%. El presente estudio, realizado por un equpo interdiciplinario, compuesto por el Progama Nacional del Chagas, autoridades locales de salud de la provincia de Loja y con el apoyo de Médicos sin Fronteras, tomó tres cantones de la Provincia de Loja, (Chaguarpamba, Olmedo y Paltas), seleccionados de acuerdo con estudios anteriores, como de mayor importancia epidemiológica para esta enfermedad...


Chagas disease is one of the more important public health problems in Latin America. Albeit the World Health Organization (WHO) has estimated a seroprevalence for Chagas disease in Ecuador of 0.25%, recent analysis revealed a general prevalence of 1.38%.Previous studies about this disease in Loja Province, established a general-population seroprevalence of 5,2% and a triatomine-infestation index per house of 34 %. In this study, performed by an interdisciplinary team, integrated by the National Chagas Program, the LojaProvince health authorities and the aid of Doctors without Borders, we analyzed three cantons of Loja Province (Chaguarpamba, Olmedo y Paltas), selected according to previous studies which recognized them as epidemiological importance regions for this disease...
