ABSTRACT

Resumen: El embarazo triple monocorial espontáneo tiene una incidencia muy baja, no identificándose factores causales que lo expliquen. La corionicidad determina el resultado perinatal de estas gestaciones, siendo la frecuencia de malos resultados mayor a medida que aumenta el número de fetos con una única placenta. Se presenta un caso de embarazo triple monocorial espontáneo cuyo seguimiento fue realizado en el Centro Hospitalario Pereira Rossell con excelente resultado perinatal.


Summary: Spontaneous monochorionic trigemellar pregnancies have a very low incidence, and there is no cause that may explain them. The chorionicity will determine the perinatal result of this gestations, but the frequency of poor results is higher as the number of fetuses sharing the same placenta increases. We present a case of a spontaneous monochorionic trigemellar gestation whose follow-up was carried out in the Pereira Rossell Pediatric Center with an excellent perinatal result.


Resumo: A tríplice gestação monocorial espontânea tem incidência muito baixa, não sendo identificados os fatores causais que a explicam. A corionicidade determina o resultado perinatal dessas gestações, e a frequência de resultados ruins é maior à medida que aumenta o número de fetos com uma única placenta. Apresentamos um caso de gravidez tríplice monocoriônica espontânea cujo seguimento foi realizado no Centro Hospitalar Pereira Rossell com excelentes resultados perinatais.

ABSTRACT

Los embarazos múltiples se consideran una entidad de alto riesgo obstétrico. Su incidencia ha ido en aumento debido a la utilización de técnicas de reproducción asistida y el aumento de la edad materna. Se presentan 2 casos de embarazos triples monocoriales triamnióticos, de sexo femenino y masculino. En ambos embarazos se manifestaron complicaciones, principalmente fetales y neonatales, atribuidas a embarazos múltiples descritas en la literatura.


Multiple pregnancies are considered a high-risk obstetric entity. Their incidence has been increasing due to the use of assisted reproductive techniques and increased maternal age. Here, we describe two cases of triamniotic monochorionic triplet pregnancies, female and male respectively. Both pregnancies demonstrated complications, mainly fetal and neonatal, attributed to multiple pregnancies reported in the literature.

ABSTRACT

El embarazo múltiple (triple) es infrecuente (1 porciento), aunque su incidencia se ha incrementado en los últimos 15 años debido al empleo de los inductores de la ovulación y del desarrollo las técnicas de fertilización in vitro y transferencia de embriones, elevándose entre el 5 y 35 porciento. Continuamente conllevan una preocupación para el ginecobstetra por la morbimortalidad materna y perinatal, fundamentalmente debido a la prematuridad. El objetivo de este artículo es presentar tres casos de gestantes con embarazo triple y evolución favorable, ocurridos entre los años 2010 y 2011. Se revisó la literatura sobre el tema, se exponen brevemente las características clínicas de este embarazo, complicaciones, manejo y pronóstico; posteriormente se describieron los elementos significativos de estas pacientes atendidas en el Hospital Docente Ginecobstétrico Dr. Julio R. Alfonso Medina de Matanzas. Se realizan comentarios sobre los casos con opiniones de los autores revisados y de los autores del artículo; es frecuente el nacimiento por cesárea y pretérmino


Multiple (triple) pregnancies is rare (1 per cent), although its incidence has increased in the last 15 years due to the use of ovulation induction and the development of in vitro fertilization techniques and embryo transfer (5 and 35 per cent). These pregnancies continually involve a concern for the obstetrician due to maternal and perinatal morbidity and mortality, mainly due to prematurity. The objective of this paper is to present three cases of pregnant women with triple and favorable pregnancy, occurring from 2010 to 2011. Literature on the subject was reviewed. The clinical features of this pregnancy complications, management and prognosis are briefly described. Significant elements of these patients are then described. They were treated at the Dr. Julio R. Alfonso Medina Teaching Hospital in Matanzas. Preterm cesarean delivery is frequent. Comments on the cases with opinions of the revised authors and the authors of the article

ABSTRACT

A complete hydatidiform mole coexisting with a fetus is a rare condition, particularly when diagnosed after IVF-ET. In spite of the wide spread use of assisted reproductive technology, there have been, to our knowledge, only few reported cases of molar pregnancy after IVF-ET. At present, there are limited data to guide the antenatal management of a complete hydatidiform mole coexisting with fetuses. A complete mole can coexist with a normal, healthy fetus who can be carried to term, with good outcome. But, patients who desire to continue the pregnancy after such a diagnosis must be cautioned about the potential for severe medical complications and developing persistent gestational trophoblastic tumor. We report here a case of complete hydatidiform mole in a triplet pregnancy coexistent with two live fetuses following in IVF-ET.
