ABSTRACT

La hernia inguinal indirecta (HHI) es la anomalía congénita más frecuente en la infancia y niñez. En las niñas, las HII deslizantes contienen principalmente el ovario con o sin trompa de Falopio. Presentamos el caso de una niña de 6 meses, que acude por tumoración inguinal izquierda, irreductible, asociada a vómitos. Es intervenida de emergencia con diagnóstico de hernia inguinal indirecta izquierda complicada, conteniendo trompa de Falopio y ovario izquierdo gangrenados. Este tipo de hernia tiene una incidencia más alta en prematuros y de sexo femenino menores de un año. La ecografía preoperatoria puede ser útil para el diagnóstico. El procedimiento para la reparación es la herniorrafia clásica con ligadura alta del saco herniario, a través de un abordaje inguinal.


Indirect inguinal hernia (IHH) is the most frequent congenital anomaly in infancy and childhood. In girls, the sliding IIHs contain mainly the ovary with or without a fallopian tube. We present the case of a 6-month-old girl who was presented with an irreducible left inguinal tumor associated with vomiting. She was operated of emergency with a diagnosis of complicated left indirect inguinal hernia containing gangrenous fallopian tube and left ovary. This type of hernia has a higher incidence in prematures and females under one year of age. Preoperative ultrasound may be helpful for diagnosis. The repair procedure is the classic herniorrhaphy with high ligation of the hernia sac, through an inguinal approach.

ABSTRACT

INTRODUCCIÓN: La torsión aislada de la trompa de Falopio ocurre en aproximadamente una de cada 50.000 a 1.500.000 mujeres, por ende, es una afección extremadamente rara. Fue originalmente descrita por Bland-Sutton en 1890. El cuadro clínico suele ser inespecífico y puede simular una amplia variedad de dolencias. CASO CLÍNICO: Paciente de 18 años de edad que acude al servicio de urgencias por dolor abdominal intenso, la cual se decide ingresar por cuadro agudo doloroso abdominal de posible etiología ginecológica, se realiza laparoscopía de urgencia por la posibilidad de un cuadro de torsión quística, durante esta se observa una imagen laparoscópica de la trompa izquierda violácea y torcida, además un quiste paratubárico hemorrágico, de color violacio; por las características laparoscópicas de la trompa y el quiste paratubárico que se relacionan con necrosis de estos elementos, se decide realizar a la paciente una anexectomía total izquierda con cistectomía del quiste paratubárico. DISCUSIÓN: La rotación de la trompa alrededor de su pedículo vascular es el evento fisiopatológico descrito, lo que provoca inicialmente la obstrucción del retorno venoso y linfático, con el consiguiente edema, ingurgitación y trombosis. Si la torsión progresa, y no se resuelve, se ve comprometida la vascularización arterial y se desencadena la necrosis del órgano y, finalmente la instauración de peritonitis, la conducta se basa en la viabilidad de la trompa: si no tiene signos de necrosis, la conducta indicada es destorcerla, si ya aparecieron signos de necrosis deberá ser extirpada.


INTRODUCTION: Isolated torsion of the Fallopian tube occurs in approximately one in every 50,000 to 1,500,000 women, therefore, it is an extremely rare condition. It was originally described by Bland-Sutton in 1890. The clinical picture is usually nonspecific and can mimic a wide variety of ailments. CLINICAL CASE: An 18-year-old patient who came to the emergency service for intense abdominal pain, which was decided to enter due to acute abdominal pain of possible gynecological etiology, an emergency laparoscopy was performed due to the possibility of a cystic torsion condition, During this, a laparoscopic image of the violaceous and crooked left tube is observed, as well as, a hemorrhagic paratubal cyst, violet in color; Due to the laparoscopic characteristics of the tube and the paratubal cyst that are related to necrosis of these elements, it was decided to perform a total left adnexectomy with cystectomy of the paratubal cyst. DISCUSSION: Rotation of the tube around its vascular pedicle is the described pathophysiological event, which initially causes obstruction of venous and lymphatic return, with the consequent edema, engorgement and thrombosis. If the torsion progresses and does not resolve, the arterial vascularization is compromised and the necrosis of the organ is triggered and, finally, the establishment of peritonitis, the behavior is based on the viability of the tube: if there are no signs of necrosis, the indicated conduct is to untwist it, if signs of necrosis have already appeared, it must be removed.

ABSTRACT

RESUMEN: Objetivos: 1) Determinar la prevalencia de embarazo ectópico en el Servicio de Alto Riesgo Obstétrico del Hospital Carlos Andrade Marín. 2) Identificar los factores de riesgo asociados con esta patología. Métodos: se realizó un estudio descriptivo, de corte transversal, retrospectivo, tomando en cuenta a las pacientes ingresadas con diagnóstico de Embarazo Ectópico al Servicio de Alto Riesgo del Hospital Carlos Andrade Marín, en el periodo del 1 de enero del 2017 al 31 de mayo del 2019. Resultados: durante el período comprendido entre el 1 de enero de 2017 y el 31 de mayo de 2019, se registró un total de 7.596 nacimientos en el Servicio de Alto Riesgo Obstétrico del Hospital Carlos Andrade Marín; de ellos, hubo 117 pacientes diagnosticadas de embarazo ectópico. La prevalencia del embarazo ectópico en el Hospital Carlos Andrade Marín, entre los años 2017 y 2019, fue de 1,5%; la tasa de embarazo ectópico es de 15,40 / 1.000 gestaciones. Conclusiones: el análisis de los datos obtenidos de las historias clínicas evidenció que la tasa de prevalencia de embarazo ectópico en el Servicio de Alto Riesgo Obstétrico del Hospital Carlos Andrade Marín es similar a la reportada por diferentes estudios realizados alrededor del mundo.Palabras claves: embarazo ectópico, prevalencia, trompa de falopio, factores de riesgo


ABSTRACT: Objective: To determine the prevalence of ectopic pregnancy in the high-risk obstetric service of the Carlos Andrade Marín Hospital and to know the associated risk factors. Methods: A descriptive, cross-sectional, retrospective study was carried out, taking into account patients admitted with a diagnosis of ectopic pregnancy to the high-risk service of the Carlos Andrade Marín Hospital, from January 1, 2017 to May 31 of 2019. Results: During the period from January 1, 2017 to May 31, 2019, a total of 7596 births were recorded in the High Obstetric Risk Service of Hospital Carlos Andrade Marín. Of all of them, a total of 117 patients were registered diagnosis of ectopic pregnancy. The prevalence of ectopic pregnancies in the Hospital Carlos Andrade Marín in a period between 2017 and 2019 was 1.5%, with an ectopic pregnancy rate of 15.40 per 1000 pregnancies. Conclusions: When performing an analysis of the data obtained from the medical records it was possible to obtain similar results of prevalence rates of ectopic pregnancy in the High Risk Obstetric Service of the Hospital Carlos Andrade Marín in relation to the data obtained by different studies conducted around of the world.Keywords:ectopic pregnancy, prevalence, fallopian tube, risk factors

ABSTRACT

Abstract Femoral hernias comprise a small proportion of all groin hernias. They are more common in women and have a high rate of incarceration and strangulation, leading to emergency repair. A 61-year-old female patient was admitted to the emergency department complaining of a 2-day painful lump in the right groin, that had become more intense in the last 24 hours. Physical examination suggested the presence of a strangulated femoral hernia, and the patient underwent emergency surgical repair. Intraoperatively, the right fallopian tube was observed in the hernia sac. Since there were no signs of ischemia, the tube was reduced back into the pelvic cavity and the hernia was repaired. The postoperative period was uneventful, and the patient was discharged without complications, 3 days after surgery.


Resumo As hérnias femorais representamuma pequena fração de todas as hérnia da região inguinal. Elas são mais comuns entre as mulheres e estão associadas a elevadas taxas de complicações, como encarceramento e estrangulamento, com necessidade de cirurgia urgente. Uma paciente do sexo feminino, de 61 anos, recorreu ao serviço de emergência por quadro de dor e tumefação da região inguinal direita com 2 dias de evolução e agravamento nas últimas 24 horas. O exame objetivo sugeria a presença de uma hérnia femoral encarcerada, e a paciente foi submetida a cirurgia urgente. Intraoperatoriamente, confirmou-se o diagnóstico de hérnia femoral encarcerada, que continha a trompa de falópio direita no interior do saco herniário. Uma vez que a que a trompa não apresentava sinais de isquemia, o conteúdo da hérnia foi reduzido, e procedeu-se à sua reparação. O período pós-operatório decorreu sem intercorrências, e a paciente teve alta no 3° dia após a cirurgia.

ABSTRACT

Los quistes paratubáricos son estructuras quísticas simples llenas de líquido seroso ubicadas a lo largo de la trompa de Falopio, que generalmente se originan del ligamento ancho o el mesosálpinx. Es importante siempre considerar la torsión de la trompa como un diagnóstico diferencial en las pacientes que presentan dolor abdominal agudo y reconocer que se necesita un diagnóstico y tratamiento quirúrgico rápido para preservar la fertilidad de la paciente, especialmente aquellas en edad reproductiva. Se presenta el caso de una paciente de 16 años quien refirió dolor hipogástrico intenso junto con vómitos. Durante la laparoscopia, se encontró un quiste paratubárico torcido y se realizó la salpingectomía, debido a que la trompa derecha estaba deformada. El análisis histológico demostró que la tumoración era un quiste paratubárico.


Paratubal cysts are simple cystic structures filled with serous fluid that are located along the Fallopian tube, usually arising from the broad ligament or mesosalpinx. It is important to al ways consider tube torsion as a differential diagnosis for female patients presenting with an acute abdomen, and to be aware of the need for quick diagnosis and surgical treatment in order to preserve the patient's fertility, especially in younger patients of child-bearing age. We present the case of a 16 year-old female patient who complained of intense hypogastric pain together with vomits. During laparoscopy, a torted paratubal cyst was found in the right adnexa. Laparotomy was performed to remove the cyst and salpingectomy was carried out, since the right Fallopian tube was unsalvageable. Histological analysis showed that the mass was a paratubal cyst.

ABSTRACT

El tumor mülleriano mixto maligno es una neoplasia inusual que corresponde al 0,1 % - 0,5 % de los tumores malignos ginecológicos. Se presenta el caso de paciente de 73 años de edad que consulta por dolor y tumoración pélvica. Es llevada a mesa operatoria con el diagnóstico de tumor de anexo uterino izquierdo. Se realizó histerectomía total, salpingooforectomía bilateral y omentectomía, más lavado peritoneal. El estudio histopatológico informa tumor mülleriano mixto maligno primario de trompa uterina izquierda. La inmunohistoquimica reportó inmunomarcaje en las células neoplásicas con vimentina, Q7, PAX8, Ki67 15 % y concluye como tumor mixto mülleriano. La enfermedad fue clasificada como en estadio III (FIGO) y se decide quimioterapia sistémica de manera coadyuvante. A los 19 meses posteriores a la cirugía la paciente se encuentra estable y sin muestras de recidiva.


The malignant mixed Mullerian Tumor is an unusual neoplasia which corresponds to the 0.1 - 0.5 % of gynecologic malignancies. The case of female patient of 73-year-old consulting by pain and pelvic tumor arises. It is carried to the operating table with the left uterine annex tumor Diagnostics. He was total hysterectomy, bilateral salpingo-oophorectomy and omentectomy, most peritoneal washing. The histopathological study advises Tumor Mullerian mixed malignant primary left uterine Horn. Immunohistochemistry reported immunomarking in neoplastic cells with vimentin, Q7, PAX8, Ki67 15 % and concludes as mixed Tumor, Mullerian. The disease was classified as stage III (FIGO), and decided way adjuvant systemic chemotherapy. 19 months after the surgery, the patient is stable and no signs of relapse.

ABSTRACT

Cáncer epitelial de ovario es una enfermedad altamente letal. Constituye la quinta causa de muerte por cáncer en mujeres a nivel mundial. El subtipo histológico más frecuente es el carcinoma seroso de alto grado. Este es el responsable de la alta letalidad de la enfermedad. Se presenta evidencia que respalda el origen tubario de este tipo histológico desde lesiones precursoras. A partir de estos datos se ha establecido que el cáncer tradicionalmente conocido como cáncer ovárico seroso de alto grado, el cáncer de trompa de Falopio y el carcinoma peritoneal primario, corresponden a una misma entidad nosológica: cáncer seroso pélvico de alto grado. Se revisa además la evidencia disponible para establecer que la salpingectomía podría constituir una medida de prevención para este tipo de cáncer.


Epithelial ovarian cancer is a highly lethal disease. It is the 5th cause of cancer death in women worldwide. The most common histologic subtype is the high-grade serous carcinoma. This is the responsible for the high lethality of the disease. Evidence supporting the tubal origin of this histological type from precursor lesions is presented. From these data it has been established that cancer traditionally known as serous high-grade ovarian cancer, cancer of the fallopian tube and primary peritoneal carcinoma, correspond to a single disease entity: pelvic serous high-grade cancer. We also check the available evidence to establish that the salpingectomy could be a preventive measure for this type of cancer.

ABSTRACT

Se presenta el caso clínico de una paciente de 49 años de edad, quien fue ingresada en el Servicio de Obstetricia y Ginecología del Hospital General Docente "Dr. Juan Bruno Zayas Alfonso" de Santiago de Cuba con el diagnóstico de fibroadenoma uterino. Se realizó histerectomía total abdominal con doble anexectomía y se observó la presencia de un fibroma uterino y un tumor en la trompa de Falopio derecha. Se indicó quimioterapia y seguimiento en el Hospital Oncológico de esta ciudad.


The case report of a 49 year female patient is presented who was admitted in the Obstetrics and Gynecology Service from "Dr. Juan Bruno Zayas Alfonso" Teaching General Hospital in Santiago de Cuba with the diagnosis of uterine fibroadenoma. An abdominal total hysterectomy with double anexectomy was carried out and the presence of an uterine fibroma and a tumor in the right Fallopian tube was observed. Chemotherapy and follow-up in the Oncological Hospital of this city was indicated.

ABSTRACT

Se realizó un estudio descriptivo y prospectivo de 32 pacientes con embarazo ectópico, tratadas por medio de cirugía laparoscópica en el Hospital Clinicoquirúrgico Docente Dr Joaquín Castillo Duany de Santiago de Cuba desde enero de 2003 hasta diciembre de 2008. Esa gestación anormal prevaleció en mujeres de 15-31 años, en las trompas (tubárico) y en la forma complicada (roto). La técnica quirúrgica fue radical en la totalidad de la serie, lo cual garantizó la pronta recuperación de sus integrantes y el alta hospitalaria precoz, sin aumentar el riesgo de complicaciones.


A descriptive and prospective study of 32 patients with ectopic pregnancy treated through laparoscopic surgery at Dr Joaquín Castillo Duany Teaching Clinical Surgical Hospital in Santiago de Cuba was carried out from January, 2003 to December, 2008. The abnormal gestation, which prevailed in the Fallopian tubes (uterotubal) and in the complicated form (broken), was observed in women aged 15-31 years. Surgical technique was radical in the whole series. This guaranteed to patients their soon recovery as well as their earlier discharge from hospital, without increasing complication risks.

ABSTRACT

Se describe el caso clínico de una paciente de 53 años de edad con antecedente de sangrado vaginal por posible fibroma uterino. Se realizó histerectomía total con doble anexectomía. Los resultados de los exámenes complementarios, incluida la biopsia, confirmaron la presencia de un carcinoma primario de la trompa uterina derecha. Se indicó radioterapia y seguimiento en el Hospital Oncológico Conrado Benítez de Santiago de Cuba.


The clinical case of a 53 year old patient with history of vaginal bleeding for possible uterine fibroma is described. A total hysterectomy with doble anexectomy was performed. The complementary tests results, including the biopsy, confirmed the presence of a primary carcinoma of the right Fallopian tube. A radiotherapy and follow up were indicated in Conrado Benítez Oncology Hospital from Santiago de Cuba.

ABSTRACT

Female genital schistosomiasis is not uncommon in endemic areas for schistosomiasis, but there are few reports in the Brazilian medical literature. Here, we describe the case of a 31-year-old woman with lower abdominal pain who was diagnosed as presenting a fallopian tube tumor caused by Manson's schistosomiasis. The diagnosis was delayed because her symptoms were considered nonspecific. Involvement of the parietal peritoneum of the ovarian fossa was observed during laparoscopy and confirmed by histological analysis. The left tube and the tumor were excised and schistosomiasis was treated with praziquantel. She presented a full recovery and options for future reproduction are under evaluation.


Esquistossomose genital feminina não é incomum em áreas endêmicas para esquistossomose, mas, há poucos relatos na literatura médica brasileira. Descrevemos aqui, o caso de uma paciente de 31 anos com dor abdominal no baixo ventre que recebeu o diagnóstico de tumor da trompa de falópio causada pela esquistossomose mansônica. O diagnóstico foi retardado porque os sintomas foram considerados inespecíficos. Havia envolvimento do peritônio parietal da fossa ovárica durante a laparoscopia, confirmado à histologia. A tuba esquerda e o tumor foram extirpados e a esquistossomose foi tratada com praziquantel. A paciente apresentou recuperação completa e opções para reprodução futura estão sob avaliação.

Se reporta una paciente con diagnóstico histopatológico de Adenocarcinoma papilar infiltrante de la trompa de Falopio moderadamente diferenciado, que invade hasta la capa muscular, sin rebasar a ésta, con metástasis a un ganglio del epiplón y no invade endometrío. Operada en la clínica de Onco-Ginecología y Patología cervical del Departamento de Gineco-obstetricia Hospital Obrero No 2 de la Caja Nacional de Seguro, constituyendo el único caso conocido en nuestro Hospital. Se completa el tratamiento remitiendo al Servicio de Oncología.


Its report a patient with a diagnosis of papillary adenocarcinoma of the fallopian tubes moderately differentiated, than invades until the muscle layer, without exceeding this layer, metastasis to the ganglyon of epiplon and does not invades endometrial layer. Operated at the Onco-gynecology and cervix pathology clinic of the Department of Gynecology - obstetrics of the Hospital worker at the CNS, been the only one case Knowed in our hospital. The treatment its completed sent the patient to the oncology service.
