ABSTRACT

@#Workplaces in tropical countries are associated with increasing temperature and humidity, thus, the workers are susceptible to heat hazards. The inability for self-cooling properly toward exposure to head hazards can lead to severe dehydration and even death. To date, there are several indices and models to assess heat stress, such as WBGT, ISO 7933, discomfort index, HIS, PhSI, etc. However, their accuracy and suitability for tropical regions are still under investigation since they are mostly developed for subtropical regions. This review assessed the indices to estimate heat exposure in tropical regions based on the various online database. Among those indices, WBGT is the most suitable despite its inability to estimate human thermal response in tropical regions with several adjustments. Based on this review, it showed that there is a need for improvement of current indexes to be used in tropical regions since they are more suitable for the subtropical countries.

ABSTRACT

Background: Dengue is a major public health problem throughout subtropical and tropical regions. In more severe or complicated dengue, patients present with a severe febrile illness characterized by abnormalities of hemostasis and increased vascular permeability, which in some instances results in a hypovolemic shock. Dengue is endemic in more than 100 countries in tropical and subtropical regions with an estimated 390 million infections occurring worldwide, among which 96 million infections are clinically apparent. The objective of this study is to assess the usefulness of serum albumin as a prognostic factor in dengue.Method: This observational study enrolled 100 patients who were admitted in KIMS Hospital from June 2017 to June 2018 without any co morbidities as mentioned in the exclusion criteria. On day 4 of illness hemoglobin, packed cell volume, platelet and serum albumin were done, and the participants were classified into 3 groups based on clinical manifestationResults: In this study, hemoglobin was elevated on day 4 in group 3 when compared to other groups and it was statistically significant. Hemoconcentration and low platelet values were also seen in group 3 when compared to other groups showing statistical significance.Serum albumin was also low in group 3 when compared to other groups, which was strongly significant.Conclusion: Hence serum albumin can be used as a prognostic factor in dengue.

ABSTRACT

BACKGROUND: Earthquakes, floods, droughts, storms, mudslides, landslides, and forest wild fires are serious threats to human lives and properties. The present study aimed to study the environmental characteristics and pathogenic traits, recapitulate experiences, and augment applications of medical reliefs in tropical regions. METHODS: Analysis was made on work and projects of emergency medical rescue, based on information and data collected from 3 emergency medical rescue missions of China International Search and Rescue Team to overseas earthquakes and tsunamis aftermaths in tropical disaster regions Indonesia-Aceh, Indonesia-Yogyakarta, and Haiti-Port au Prince. RESULTS: Shock, infection and heat stroke were frequently encountered in addition to outbreaks of infectious diseases, skin diseases, and diarrhea during post-disaster emergency medical rescue in tropical regions. CONCLUSIONS: High temperature, high humidity, and proliferation of microorganisms and parasites are the characteristics of tropical climate that impose strict requirements on the preparation of rescue work including selective team members suitable for a particular rescue mission and the provisioning of medical equipment and life support materials. The overseas rescue mission itself needs a scientific, efficient, simple workflow for providing efficient emergency medical assistance. Since shock and infection are major tasks in post-disaster treatment of severely injured victims in tropical regions, the prevention and diagnosis of hyperthermia, insect-borne infectious diseases, tropic skin diseases, infectious diarrhea, and pest harms of disaster victims and rescue team staff should be emphasized during the rescue operations.
