ABSTRACT

Lesão por pressão relacionada a dispositivos médicos é uma complicação bastante comum e geralmente oriunda da fixação do dispositivo utilizado na intubação endotraqueal para ventilação mecânica. Desta forma, o presente estudo reportou o caso de um paciente do sexo masculino, 65 anos, leucoderma, internado em uma unidade de terapia intensiva, e que desenvolveu lesão ulcerada na região de fixação do dispositivo utilizado para intubação orotraqueal. Após alívio da pressão local exercida, terapia de fotobiomodulação foi proposta como única estratégia para cicatrização da lesão e diminuição do edema. Após 3 sessões diárias foi possível observar excelente curso de reparação tecidual e remissão dos sinais flogísticos. De acordo com o presente caso, a terapia de fotobiomodulação parece ser bastante eficiente para o tratamento de lesões por pressão relacionada a dispositivos médicos(AU)


Medical device-related pressure injuries are very common complications and usually arise from the fixation of the device used in endotracheal intubation for mechanical ventilation. Thus, the present study reports a case of a Caucasian male patient, 65 years old, who was admitted to an intensive care unit and developed later an ulcerated lesion on the region of fixation of the device used for orotracheal intubation. After relieving the local pressure exerted, photobiomodulation therapy was proposed as the only strategy for wound healing and edema reduction. After 3 daily sessions, it was possible to note an excellent tissue repair course and remission of phlogistic signs. According to the present case, photobiomodulation therapy appears to be a quite efficient treatment strategy for medical device-related pressure injuries(AU)

ABSTRACT

La bronquitis plástica es una enfermedad poco frecuente pero potencialmente mortal por obstrucción de la vía aérea, que puede ocurrir a cualquier edad. Debido a su poca frecuencia no tiene un tratamiento bien definido y el que se pueda encontrar está basado en experiencias anecdóticas. Ante un episodio de aumento de presiones en el ventilador con una marcada diferencia entre la presión pico y la plateau debe sospecharse de obstrucción del tubo orotraqueal y de ser necesario proceder a cambiarlo sobre todo si el paciente empieza a desaturar e inestabilizarse.


Plastic bronchitis is a rare but potentially fatal disease because of obstruction of the airway, which can occur at any age. Due to its infrequency, it doesn´t have a well-defined treatment and that we can found is based on anecdotal experiences. In the presence of increased pressure on the mechanical ventilator with a marked difference between the peak pressure and plateau pressure, we should suspect of endotracheal tube obstruction and if necessary, we must change it, especially if the patient begins to desaturate and become unstable.

ABSTRACT

Para determinar fracción espirada final de sevoflurane al retirar tubo orotraqueal y máscara laríngea sin compli-caciones en pacientes pediátricos, se elaboró estudio obser-vacional de tipo descriptivo y, transversal, seleccionando un total de 40 pacientes de pediatría quirúrgica (Hospital Universitario de Caracas) para cirugías urológicas y her-nioplastias, distribuidos en grupo T (tubo orotraqueal) y grupo M (máscara laríngea). Premedicados con Midazo-lam recibieron anestesia inhalatoria y bloqueo regional. Mantenimiento: O2/aire 50/50. Concluida la cirugía se retiró dispositivo según parámetros hemodinámicos, clí-nicos y ventilatorios. Hubo diferencias estadísticamente significativas entre ambos dispositivos para presión arte-rial sistólica y diastólica, tiempo retiro del dispositivo de la vía aérea ­ salida del quirófano (TR) y complicaciones, no para fracción espirada. Se concluyó que para retiro de máscara laríngea y tubo orotraqueal, la fracción espirada final de sevoflurane fue de 0,66 y 0,87 respectivamente, siendo la máscara laríngea el dispositivo más recomenda-do por menor TR y menor número de complicaciones (AU)


To determine end-tidal sevoflurane fraction by removing endotracheal tube and laryngeal mask in pediatric patients without complications was developed observational and descriptive, transversal, selecting a total of 40 surgical pedia-tric patients (Hospital Universitario de Caracas) for surgery hernioplasties urological and distributed in group T (endo-tracheal tube) and group M (laryngeal mask). Premedicated with midazolam received inhalation anesthesia and regional blockade. Maintenance: O2/aire 50/50. Completed surgery withdrew device according to hemodynamic parameters, clinical and ventilator. Significant differences between the two devices for systolic and diastolic blood pressure, while removing the device from the air - out of the operating room (TR), and complications, not fractional exhaled. It was con-cluded that removal of laryngeal mask and endotracheal tube, the end-tidal sevoflurane fraction was 0.66 and 0.87 respectively, and the laryngeal mask device smaller than recommended by TR and fewer complications (AU)
