ABSTRACT

Objective To explore the clinical value of Doppler flow spectrums of umbilical artery and ductus venosus in fetus with arrhythmia by Doppler ultrasound in evaluating the fetal prognosis.Methods Umbilical artery and ductus venosus flow spectrums were obtained in a total of 796 pregnancies with gestation age between 16-41 weeks who were found abnormal heart rhythm by maternity auscultation,fetal electronic monitoring or ultrasound.All these fetuses were followed up by electrocardiography and echocardiography examination until the end of pregnancy and the pregnancy outcomes were recorded.Results Among 796 fetuses,136 cases of fetal arrhythmia were detected.There were 18 cases appeared with umbilical artery absent or reversed end-diastolic flow,and 9 cases with absent or reversed wave A.Among 18 fetuses with arrhythmia whose blood flow spectrums of umbilical artery were abnormal,twelve were compared with congenital heart disease.The proportion of fetal bradycardia associated with abnormal flow spectrums and congenital heart disease were higher than other types of arrhythmias.Conclusions Pregnancy outcome of the fetus with arrhythmias who appeared with normal umbilical artery and ductus venosus will be good,and conversely is poor.So Doppler flow spectrums of the fetal umbilical artery and ductus venosus is one of the important indicators in early intervention.

ABSTRACT

PURPOSE: To evaluate hemodynamic effects of mannitol infusion in patients with acute intracerebral hemorrhage. METHODS: Thirty patients with acute intracerebral hemorrhage were enrolled. Transcranial doppler was used to detect variables of bilateral middle cerebral arteria (MCA) including mean velocity (Vm) and pulsitility index (PI) before and after125ml and 250ml mannitol infusion (0, 30, 60, 90, 120, 180, 240 min). RESULTS: When 125ml or 250ml mannitol was infused in patients with acute intracerebral hemorrhage, Vm of bilateral MCA elevated, and reached the top at 30min, and then decreased. PI decreased in the affected MCA (250ml) and in the unaffected MCA (125ml and 250ml). CONCLUSION: Mannitol infusion in patients with acute intracerebral hemorrhage can improve cerebral blood flow in bilateral hemispheres and decrease intracranial pressure in the hemorrhagic hemisphere (250ml) and in the nonhemorrhagic hemisphere (125ml and 250ml).

ABSTRACT

Objective To evaluate the value of N-terminal prohormone brain natriuretic peptide (NT-proBNP) and tissue Doppler imaging (TDI) in patients with left ventricular diastolic dysfunction.Methods Forty-three patients with diastolic heart failure were enrolled. Pulsed wave (PW) Doppler technology was used to detect early diastolic mitral velocity (E) and TDI was performed to determine the mitral annulus velocity of early diastolic phase (Em). According to the E/Em, patients were divided into three groups: group Ⅰ (E/Em <8),group Ⅱ (E/Em 8-15) and group Ⅲ (E/Em > 15). The plasma NT-proBNP was measured. Results The plasma NT-proBNP in group Ⅲ [( 1119 ± 3123) ng/L] was higher than that in group Ⅱ [(684± 1742) ng/L] and group Ⅰ [(276±438) ng/L] (P<0.01),and the plasma NT-proBNP in group Ⅱ was higher than that in group Ⅰ (P < 0.01 ). The plasma NT-proBNP had positive correlation with E/Em (r = 0.438,P < 0.01 ). Conclusion The plasma NT-proBNP and TDI are useful to evaluate the left ventricular diastolic dysfunction.

ABSTRACT

Objective To evaluate the effects of statins on the cerebral haemodynamics of ischemic stroke patients with transcranial Doppler(TCD). Methods 70 inpatients with acute ischemic stroke were divided into two groups according whether they had taken statins after stroke.The change of the cerebral haemodynamics was studied in the two groups with TCD three months after stroke. Results Three months later,the systolic flow velocities and mean flow velocities of bilateral middle cerebral arteries in the statins groups significantly increased (P<0.05). The pulse index of bilateral middle cerebral arteries in statins group significantly decreased (P<0.05).The flow velocities and pulse index of bilateral middle cerebral arteries in control group don't change significantly(P>0.05). Conclusions Statins may have the effects of improving the situation of cerebral haemodynamics of patients with ischemic stroke.

ABSTRACT

Objective Peripheral arterial diseases(PAD)are most common conditions in patients with end stage renal disease(ESRD). However, PAD hasn′t been extensively studied like other cardiovascular or cerebrovascular diseases such as coronary heart disease and cerebral infarction among ESRD patients. The present study aims to investigate the ultrasonic characteristics of PAD and risk factors related to peripheral arterial intima thickening and plaques formation. Methods Seventy-three ESRD patients and 21 healthy individuals (as control) were involved in the investigation, and their carotid and lower-extremity arteries including tunica intima, lumen diameter, plaques and Doppler spectrums were examined with color Doppler ultrasonic technique. Then, the risk factors related to intima thickening and plaques formation among ESRD patients were studied combining with their clinical data and biochemical makers. Results The lesions of varying degrees in peripheral arteries occured in 52.1%(38/73) ESRD patients, including intima thickening, coarse and chaotic tunica intima, strong echo masses or atheromatous plaques with different shapes and sizes, widened and deformed Doppler spectrums, accelerated peak systolic velocity and lowered or disappeared diastolic reverse peak at narrow sites, low resistant blood flow at apo-stenosis sites (prolonged systolic accelerative time and decreased acceleration).
