ABSTRACT

Objective:To investigate the predictive value of stone composition prediction method based on dual-source stone energy spectrum CT for uric acid stones.Methods:The clinical data of 204 patients with urinary stones, 159 males and 45 females, admitted to Shanghai Sixth People's Hospital from July 2020 to July 2022 were retrospectively analyzed. The average age was (50.7±14.3) years. There were 187 cases of upper urinary tract (kidney, ureter) stones and 17 cases of lower urinary tract (bladder, urethra) stones. All patients underwent preoperative dual-source stone energy spectroscopy CT, measuring CT values at 150 kV and 100 kV, respectively, and the calculated dual energy ratio (Ratio) was used to predict stone composition. Of the 204 cases in this group, 33 cases underwent percutaneous nephrolithotomy and 171 cases underwent ureteroscopic lithotripsy. Postoperative stone specimens were analyzed for stone composition by infrared spectroscopy. Subject work characteristic (ROC) curves were used to analyze the efficacy of preoperative dual-source stone energy spectrum CT to predict uric acid stones.Results:In 204 patients, preoperative CT predicted 28 cases of uric acid stones and 176 cases of non-uric acid stones, including 136 cases of calcium oxalate stones, 38 cases of hydroxyapatite, and 2 cases of cystine stones. Postoperative IR spectral analysis detected 26 cases of uric acid stones and 178 cases of non-uric acid stones, including 129 cases of calcium oxalate stones, 47 cases of hydroxyapatite, and 2 cases of cystine stones. Compared with the preoperative CT results, there were 2 false positives and no false negatives in the classification of uric acid stones. The ROC curve showed that the sensitivity of the CT value for predicting uric acid stones at 100 kV was 96.2%, the specificity was 99.5%, and the area under the ROC curve (AUC) was 0.995, with the best prediction value of 620 HU.The sensitivity of the CT value for predicting uric acid stones at 150 kV was 96.2%, specificity was 85.5%, AUC was 0.916, and the best predictive value was 597.5 HU. The sensitivity of using Ratio values for predicting uric acid stones was 100.0%, specificity was 97.9%, AUC was 0.996, and the best predictive value was 1.14. The CT values for uric acid stones at 100 kV and 150 kV were (558.58±77.07) HU and (521.12±83.54) HU, CT values for calcium oxalate stones were (1 335.26±301.82) HU and (878.21±200.21) HU, CT values for hydroxyapatite were (1 104.09±203.61) HU and (710.38±178.44) HU, respectively.The CT values of cystine stones were (684.5±67.18) HU and (573.5±67.10) HU, respectively, and the differences between uric acid stones and other components were statistically significant ( P<0.01). The Ratio values of uric acid stones, calcium oxalate stones, hydroxyapatite, and cystine stones were 1.08±0.06, 1.52±0.08, 1.62±0.40, and 1.19±0.02, respectively, and the differences were statistically significant ( P<0.01) when comparing uric acid stones with other components. Conclusion:The CT and Ratio values of dual-source stone energy spectrum CT can effectively distinguish uric acid stones from other types of stones, and the accuracy, sensitivity, and specificity of this method for predicting uric acid stones are high.

ABSTRACT

The research highlights and latest developments on urinary tract stones at the 2023 American Urological Association (AUA) annual meeting mainly include several aspects, including stone metabolism and basic research, drug treatment and dietary prevention, application of new technologies in lithotripsy, development of new equipment, and innovation in the combination of artificial intelligence and medical industry. In term of basic research, new breakthroughs have been made in the formation mechanism of Randall plaque and stones microbiology and proteomics. The update and iteration of new lithotripsy instruments make minimally invasive technology efficient and safe. The widespread application and in-depth expansion of artificial intelligence machine learning and imageomics have shown strong advantages in preoperative diagnosis and postoperative prediction. The drug treatment and dietary prevention for genetic related stone diseases have been given special attention. In addition, public social media platforms and medical self media have begun to provide more active science popularization and education for the public.

ABSTRACT

Objective:To discuss the clinical experience and efficacy of minimally invasive anterograde multiple endoscopic treatment of upper urinary tract stones in allograft kidney.Methods:We retrospectively analyzed 14 patients with upper urinary tract stones in allograft kidney admitted to our hospital from June 2018 to August 2020. 8 cases were female and 6 cases were male, with an average age of 47.3±11.1 years. 3 patients had hyperuricemia and four had hypertension and diabetes. The history of renal transplantation was over ten years in 3 cases, three to five years in 4 cases, two years in 3 cases, 1 year in 3 cases, and 4 months in 1 case. 3 patients had a history of RIRS, and the operation was terminated due to severe ureteral tortuosity resulting in failure of sheathing or ureteroscopy. Ureteral stent was performed in 2 cases due to stone obstruction and hydronephrosis. Serum creatinine was normal in 7 patients before operation, and serum creatinine was 91-139μmol/L in 4 patients in the compensatory stage of chronic renal insufficiency. The serum creatinine was 292, 544 and 708μmol/L respectively in 3 patients in the decompensated stage of chronic renal insufficiency or renal failure stage. The preoperative average hemoglobin was 117.5g/L. 3 cases were partial staghorn calculi, 4 cases were single caliceal or renal pelvis calculi, 2 cases were renal pelvis or caliceal calculi with upper ureteral calculi, and 5 cases were renal pelvis or renal caliceal calculi with multiple calculi. Stone size were 1 case of single upper caliceal stone of 0.7cm, 3 cases of lower caliceal stones of 1.5, 1.6 and 2.0cm, 1 case of renal pelvic stone of 1.5cm, 2 cases of middle and upper caliceal stones of 2.8 and 3.1cm, 2 cases of middle and lower caliceal stones of 1.5 and 3.2cm, respectively. 3 cases of middle upper caliceal and renal pelvic stones were 2.2, 2.5 and 2.6cm. 2 cases of renal pelvis with upper ureteral stones were 1.3 and 1.7cm, 0.7 and 0.5cm respectively. Preoperative routine urine examination showed that 9 cases had urinary tract infection, among which 5 cases had positive urine culture. Surgery was performed after therapeutic improvement with sensitive antibiotics. According to the size and distribution of stone, the combination of single access or multi-access PCNL in different diameters was adopted, supplemented by FURS. Surgical methods selection and performance: 2 cases performed in single S-PCNL with stone size were 2.2cm and 2.6cm, 2 cases performed in single M-PCNL with stone size were 1.5cm and 1.5cm, 1 case performed in Needle-perc with stone size was 0.7cm, 2 cases performed in S-PCNL combined M-PCNL with stone size were 2.8cm and 3.1cm, 3 cases performed in S-PCNL combined Needle-perc with stone size were 2.0cm, 2.5cm and 3.2cm, 2 cases performed in M-PCNL combined Needle-perc with stone size were 1.5cm and 1.6cm, 2 cases performed in S-PCNL combined anterograde FURS with stones size were 1.3cm and 1.7cm in allograft kidney and ureter stone were 0.7cm and 0.5cm, a total of 7 kinds of way, and postoperative stone free rate, laboratory indexes (serum creatinine, blood hemoglobin), surgical complications, postoperative hospital stay were analyzed.Results:All 14 patients (mean age was 47.3±11.1 years) were successfully operated. Postoperative examination revealed 1 case had 0.6cm residual stone and it was cleared at the second stage anterograde FURS through the original access. The mean operative time and postoperative hospital stay were 68.2±21.6min and 6.2±1.3 days. Compared with preoperative serum creatinine changes, 2 cases showed slight increase (mean 12.6±0.3μmol/L), 3 cases showed significant decrease (mean 329.6±216.6μmol/L), and the other 9 cases showed no significant change (range<10μmol/L), among which 5 cases showed an increase (mean 5.4±0.7 μmol/L) and 4 cases showed a decrease (mean 3.7±0.4 μmol/L). The mean decrease of hemoglobin was 9.3±4.1g/L. Two patients had fever and their body temperature returned to normal after anti-inflammatory treatment. No blood transfusion, abdominal organ injury or urogenic sepsis occurred.Conclusions:Invasive anterograde multiple endoscopic treatment of upper urinary tract stones in allograft kidney is a single or combined operation using single-channel PCNL, multi-channel PCNL of different sizes and diameters and anterograde FURS according to individual differences, which can effectively reduce renal function injury is safe, efficient and feasible.

ABSTRACT

Objective To investigate the value of heparin-binding protein(HBP) in predicting urosepsis after endoscopic lithotripsy for upper urinary tract stones.Methods From January to November 2018,635 cases of upper urinary tract stones treated by endoscopic lithotripsy in our hospital was analyzed.According to the 2014 edition Chinese Guidelines of the diagnosis and treatment of urosepsis,all patients were divided into two groups,the definite urosepsis group (n =53,8.3%) and the no urosepsis group (n =582,91.7%).The serum of HBP level was detected by fluorescence immunoassay technology.The serum of procalcitonin (PCT) level was detected by cyclic enhanced immunofluorescent assay.C-reactive protein (CRP) level was detected by automatic chemiluminescence immunoassay analyzer.White blood cells (WBC) count was performed by hematology analyzer.Receiver operating characteristic(ROC) curves were constructed by SPSS 22 and MedCale,and areas under curves (AUC) were calculated and compared to illustrate the diagnostic power of HBP,PCT,CRP and WBC.The correlation between HBP and WBC was also significant (r =0.68,P < 0.05).Results The level of HBP,PCT,CRP and WBC were significantly higher in the definite urosepsis group compared to the no urosepsis group.AUC of HBP,PCT,CRP and WBC were 0.930,0.878,0.727 and 0.617.When the cutoff of HBP,PCT,CRP and WBC were set as 51.5 ng/ml,0.41 ng/ml,64.2 mg/L and 11.7 × 109/L,the sensitivities were 89.2%,83.3%,83.5% and 44.7%,and the specificity were 95.9%,75.6%,64.3% and 77.1%,respectively.Conclusions The value of heparin-binding protein was superior to that of PCT,CRP and WBC in predicting urosepsis after endoscopic lithotripsy for upper urinary tract stones.Heparin-binding protein could be acted as a diagnostic marker for urosepsis.

Background: Urinary stones are one of the most common and earliest known human diseases. Extracorporeal shock wave lithotripsy (ESWL) is known as the most common method for treating urinary stones less than 20-25 mm. The aim of this study was to evaluation the success rate of ESWL in patients with urinary stones.Methods: In this cross-sectional descriptive study 200 patients with urinary stones between 7-25 mm which were candidates for lithotripsy, enrolled in the study. Patients with coagulation disorders, urinary or other organ transplantation, pregnant women, uncontrolled blood pressure patients, and those who are contraindicated for drug use have been excluded from the study. The process was carried out by an operator and by the Dornier Compact Delta II lithotriptor. Two weeks later, the patients were re-visited, and ultrasound was performed, and the success rate of stones was measured and recorded and classified in three groups complete response, partial response and failure in treatment. Collected data analysed by statistical methods in SPSS version 19.Results: Efficacy of Arian 101 lithotriptor in removing of renal stones, were found to be 74% complete response, 25.5% partial response while 0.5% of subjects had no response. There was significant positive correlation between the size of stones before and after lithotripsy (r=0.49, p=0.001). In stones upper than 10 mm, the upper calices stones had 100% complete response. Also, in stones lower than 10 mm, the upper calices stones had 100% complete response.Conclusions: The results showed that Dornier Compact Delta II lithotriptor has more efficacy in treatment of stones. In stones below 10 mm, the success rate of treatment was generally higher, especially in the upper calices stones and pelvic. In stones larger than 10 mm, the success rate was slightly lower, but in the case of upper calices stones, the complete response was 100%.

ABSTRACT

Objective To investigate the distribution characteristics and changing tendency of urinary tract stones.Methods From January 2011 to May 2017,clinical data of 15 269 patients treated in our center was retrospectively reviewed.The stone components were detected by the automatic stone infrared spectroscopy system and the predominant components were recorded.There were 9 019 male patients and 6 250 female patients.The patients were divided into four groups according to their age,including group A ≤ 18 years;group B 19-40 years;group C 41-60 years;and group D > 60 years.Compared the distribution characteristics of urinary tract stones of patient in different groups of sex,age and calendar year.Results Calcium oxalate stones were more prevalent in males than females [6 221 (69.0%)vs.3 582 (57.3%),P < 0.001],but calcium phosphate stones [210 (3.4%) vs.210 (2.3%)],magnesium ammonium phosphate stones [230(3.7%) vs.165 (1.8%)] and carbonate apatite stones [1 328 (21.3%) vs.1 030 (11.4%)] were more common in females than males (P < 0.001,respectively).The proportion of uric acid stones in group D [679(20.7%)] was higher than that in group A [23(9.1%)],group B[260(7.9%)],group C [1 163 (13.8%)] (P <0.001,respectively).The peak of carbonate apatite stones was showed in group B [652(19.7%)] (P<0.001,respectively).Ammonium urate stones [9(3.5%)] and cystine stones [36 (14.2%)] were more frequent in group A(P <0.001,respectively).In adults,the percentage of uric acid stones increased with age,such as group B [260(7.9%)],group C [1 163(13.8%)],group D [679 (20.7%)].And the carbonated apatite stones decreased with age,such as group B [652 (19.7%)],group C [1 270(15.1%)],group D [416(12.7%)] (P <0.001,respectively).Further analysis showed the proportion of calc ium oxalate (OR =0.944,95 % CI 0.927-0.962,P < 0.001),ammonium urate stones (OR =0.854,95% CI 0.742-0.982,P =0.027) decreased,while calcium phosphate (OR =1.192,95% CI 1.127-1.261,P <0.001),uric acid (OR =1.042,95% CI 1.015-1.069,P =0.002) and ammonium magnesium phosphate (OR =1.078,95% CI 1.019-1.141,P =0.009) stones increased with time.Conclusions The distribution of stones was different in genders and age.Calcium oxalate stones were more common in male patients,while ammonium magnesium phosphate and carbonate apatite stones were more common in female patients.Uric acid stones were more frequent in patients older than 60,while carbonate apatite were more frequent in the 19-40 age group.The proportion of calcium oxalate and ammonium urate stones showed a downward trend,whereas calcium phosphate,uric acid and magnesium ammonium phosphate stones increased with time.

ABSTRACT

Objective To retrospec the efficacy of percutaneous nephrolithotomy on treatment of urinary tract stones in solitary kidney.Methods From June 2008 to April 2011,percutaneous nephrolithotomy technique was used to treat 20 cases of urinary tract stones,with kidney stone or cast form staghorn calculi in 4 cases,single pelvis,calyceal stones 10 cases,6 cases of upper ureteral calculi;Fourteen cases used the standard channel F24;6 cases used F16 Micro Channel.Among the 20 patients,16 cases performed stage Ⅰ single channel surgery;2 cases performed dual-channel surgery in Ⅰ stage;2 patients with severe infection performed stage Ⅱ single channel surgery.Results The average duration of the operation was 66mins.The stone clearance rate was 88.36% on average(18/20),without laceration or bleeding of pelvis or renal calyces mucosa or any other complications.Postoperative indwelling nephrostomy tubes were removed 6 -7 days later,without complications found during the follow-up period.For the 6 cases with preoperative renal dysfunction before surgery,the levels of blood Cr decreased at 2 weeks after surgery.Conclusion Compared to open surgery,percutaneous nephrolithotomy is safe and effective with small wound in treatment of solitary kidney with invasive upper urinary tract calculi.

Los pacientes con litiasis renal requieren de investigaciones para identificar las condiciones médicas subyacentes y otras anomalías metabólicas predisponentes. Los resultados de estas investigaciones se utilizan para guiar el tratamiento preventivo. La profundidad del estudio necesario depende de varios factores, incluyendo la edad, la historia clínica de la persona y el número y la frecuencia de los cálculos. Una variedad de factores dietéticos y metabólicos pueden contribuir o causar la formación de litiasis renal. Los factores dietéticos incluyen una alta ingesta de proteínas animales, oxalato y sodio, y una baja ingesta de líquidos y de productos cítricos que contienen potasio. Las alteraciones metabólicas más frecuentemente asociadas a litiasis son la hipercalciuria, la hipocitraturia, la gota, la hiperoxaluria y la hiperuricosuria. Las modificaciones en la dieta deben aplicarse en todos los pacientes con litiasis renal, y consisten en una elevada ingesta de líquidos, la restricción de oxalato y sodio, una dieta balanceada en proteínas animalesy complementadas por una ingesta adecuada de frutas y verduras. Cuando las modificaciones en la dieta no son suficientes en prevenir la formación de litiasis o en la presencia de alteraciones metabólicas importantes, es necesaria una intervención farmacológica especifica.


People who form kidney stones require investigations to identify underlying medical conditions and to detect other predisposing metabolic abnormalities. The results of these investigations can also be used to help guide therapy to prevent future stone formation. The extent of testing required dependson several factors including age and medical history of the person and the number and frequency of stones. A variety of dietary and metabolic factors may contribute or cause stone formation in nephrolithiasis. Dietary factors include a high intake of animal proteins, oxalate and sodium, and a low intake of fluids and potassium containing citrus products. Some of the metabolic causes of stones are hypercalciuria, hypocitraturia, gout, hyperoxaluria, and hyperuricosuria. Dietary modification, to be applied in all patients with stones includes a high fluid intake, restriction of oxalate and sodium, and balanced diet with animal proteins complemented by adequate intake of fruits and vegetables. When dietary modification is ineffective in controlling stone formation or in the presence of severe metabolic derangements, a pharmacologic intervention may be necessary.

ABSTRACT

4 mm)in the kidney,and were then treated by ESWL combined with oral dissolution agents.The cure rate of this series was 97.7%(86/87).The patients were discharged from the hospital 4 to 16 days(mean,6.3 days)after the treatment.One patient developed massive hemorrhage and was then cured by selective embolization of the small renal vessels.86 patients were followed up for 3 to 18 months(mean,6.3 months),none of them had residual stones during this period.The symptoms of hydronephrosis was improved in all of the cases.Conclusions B-ultrasonography guided MPCNL with holmium:YAG laser is convenient,safe,and effective for upper urinary tract stone.

ABSTRACT

PURPOSE: In the era of shock wave lithotripsy(SWL), most of the urinary stones are treated with SWL. But, there are some controversies in being SWL always the optimal treatment method for urinary stones, especially for lower urinary stones. To determine what is the most optimal initial treatment method for the lower urinary tract stones, we compared the results of SWL and laser lithotripsy in several aspects. MATERIALS AND METHODS: We compared retrospectively the success(stone free) rate, prevalence day, number of treatment sessions, cost between two groups, SWL and Laser lithotripsy. There were 42 men and 18 women in SWL group(total 60), and 47 men and 20 women in Laser lithotripsy group(total 67). RESULTS: The result were obtained as followings; The success rate were 83.3% in ESWL group during the follow up period, 94.0% in Laser lithotripsy group. The average prevalance days were 20.55 days in ESWL group, 5.5 days in Laser lithotripsy group. The average number of treatment session were 1.37 in ESWL group, 1 in Laser lithotripsy group. The average cost for the treatment of stone were 597,117 won in ESWL group, 652,887 Won in Laser lithotripsy group. CONCLUSIONS: For the lower urinary tract stones, laser lithotripsy got higher success rate in a shorter period with a single treatment session and is more effective for bladder and urethral stones.
