ABSTRACT

Aim To evaluate the estrogen-like activity of Semen Descurainiae aqueous extracts (SD-ae), to deter-mine its effective chemical separation components and to study the mechanisms. Methods The estrogen-like ac-tivity of SD-ae and its effective chemical separation com-ponents were evaluated by the animal experiment, uterine weight test and cell experiment, namely E-SCREEN ex-periment. Estrogen receptor antagonist ICI182,780 inter-vention blocking experiment was carried out to detect the pathway of estrogen-like action; the HEK293 cells were co-transfected with the report gene carrier and the ERα, ERβ expression vector by cationic liposome, the report gene carrier was constructed via the estrogen-responsive component (ERE) and the report gene luciferase (Luc), then the estrogen-like signaling pathway was evaluated with standardized Luc activity; the expression of estrogen receptor ERα, ERβ and estrogen-induced gene PR mR-NA was detected by real-time fluorescence quantitative PCR. Results Compared with normal control, SD-ae low and high dose could significantly improve the uterine coefficient of immature female mice(P<0.05), and the oligosaccharides composition of Semen Descurainiae aque-ous extracts(SD-ae-Oli) and the polysaccharide composi-tion of Semen Descurainiae aqueous extracts(SD-ae-Pol) also significantly improved the uterine coefficient of im-mature female mice (P<0.01 or P<0.05); SD-ae, SD-ae-Oli and SD-ae-Pol had a significant proliferative effect on MCF-7 cells ( P <0.01 or P <0.05), while ICI182,780 intervened to block its proliferative effect. The reporter gene technology showed that the standardized Luc activities of SD-ae, SD-ae-Oli and SD-ae-Pol were significantly higher than those of the normal control when they were induced by ERβ respectively (P<0.01); and the SD-ae significantly increased the expression of ERβ mRNA in mouse uterus than the normal control, but no effect was found on the expression of ERα and PR mR- NA. Conclusions The estrogenic effect of SD-ae may be found at the first time, and its effective chemical sepa-ration components are SD-ae-Oli and SD-ae-Pol. Their estrogenic effects are mediated by ERβ. The molecular mechanism of the estrogenic effects is probably that SD-ae promotes the expression of ERβ mRNA.

ABSTRACT

OBJECTIVE: To evaluate the effect of uterine weight on short-term outcome of total laparoscopic hysterectomy (TLH) for benign gynecological conditions. METHODS: A retrospective medical records review of 265 cases of patients with TLH was performed. Patients included in this study underwent TLH as benign uterine disorders at Kangnam Sacred Heart Hostpital, Hallym University from January 2008 through June 2009. These patients were stratified into three groups; Group 1 patients with uterus weighing less than 180 g (n=60), Group2 patients with uterus weighing 180 g to 350 g (n=141), Group 3 patients with uterus weighing more than 350 g (n=64). The groups were compared as regard postoperative stay, operative time, estimated blood loss, hemoglobin change, conversion to open surgery, and postoperative complications. RESULTS: There was no significant difference in age, gravidity, body mass index, previous pelvic surgery and past medical history. The overall complication rates were not significantly different. But operative time prolonged as uterine weight increased. CONCLUSION: The TLH can be performed successfully in case of enlarged uterus. Therefore the enlarged uterus is not an absolute contraindication for TLH.

ABSTRACT

Effects of graded dosages of methanolic extract of Aspilia Africana were examined on the estrous cycle, uterine weights and histology to determine its effects on the reproductive functions of 25 cyclic female rats. The rats were randomized into five groups A, B, C, D, and E. They were given Omg/kg body weight, 150 mg/kg body weight, 200 mg/kg body weight, 250 mg/kg body weight, and 300 mg/kg body weight, respectively. The effect on the estrous cycle was determined by vaginal lavage while routine histological preparations were done with haematoxylin-eosin stains. All values were statistically compared at appropriate confidence intervals. Estrous cycles were significantly reduced in a dose-dependent manner and histology revealed a dose-dependent toxicity.


Se examinaron los efectos de las distintas dosificaciones de extracto de metanol de Aspilia africana, en los ciclos estrales, peso uterino y en la histología para determinar sus efectos en las funciones reproductivas de 25 ratas hembra. Las ratas fueron randomizadas en cinco grupos: A, B, C, D y E y recibieron 0 mg/kg de peso corporal, 150 mg/kg de peso corporal, 200 mg/kg de peso corporal, 250 mg/kg de peso corporal y 300 mg/kg de peso corporal respectivamente. El efecto en el ciclo estral fue determinado por frotis vaginal, preparado con técnicas histológicas de rutina y teñido con hematoxilina-eosina. Todos los valores fueron estadísticamente comparados con intervalos de confianza adecuados. Los ciclos estrales fueron significativamente reducidos en forma dosis dependiente y la histología reveló una toxicidad dosis dependiente.

METHODS: A retrospective chart review of 549 cases of patients with vaginal totoal hysterectomy was performed. Patients included in this study underwent vaginal total hysterectomy as benign uterine tumors at Kangnam Sacred Heart Hospital, Hallym University, Seoul, Korea from June 2003 through June 2004. These patients were stratified into four groups; Group I patients with uterine weight of 380 gm (n=79). The groups were compared as regard age, paturity, previous pelvic operations, postoperative discharge day, postoperation hemoglobin change, operation time, postoperative complications. RESULTS: There was no significant difference between the four groups with repect to postoperative discharge day (Mean=9.08 day), concurrent surgical procedure, age (M=45.96 years old), parturity (M=1.88). The overall complication rate was not significantly different (postoperation hemoglobin change (M=1.25), Complication). But the morcellation rate increased 8.33% in group I, 34.18% in group II, 55% in group III, 83.5% in group IV, respectively. The operation time prolonged as uterus weight increased.; 73 minutes in group I, 79 minutes in group II, 85 minutes in group III, 91 minutes in group IV. CONCLUSION: The vaginal total hysterectomy can be performed successfully in case of greatly enlarged uterus. Uterus enlargement is not an absolute contraindication.

ABSTRACT

OBJECTIVES: To understand the importance of the serum level of Ca-125 among pelvic mass, we performed a study. METHOD: From January to December 1998, we performed the study. Before hysterectomy, we performed a blood sampling to know the serum level of Ca-125, After hysterectomy, we weighed the uterus and measured the thickness of endometrium and other histologic characteristics. RESULTS: We performed my research to 80 peoples. The relation between uterine weight and the serum level of Ca-125 is little, if ever(R2=0.0007), and the relation between the thickness of endometrium and the serum level of Ca-125 is also little, if ever(R2=0.0353). The relation between leiomyoma, the cycle of endometrium and the serum level of Ca-125 were also little, but there was a close relationship between adenomyosis and the serum level of Ca-125. CONCLUSION: There was little relationship between uterine weight and the serum level of Ca-125.
