ABSTRACT

Despite the availability for nearly twenty years of an effective vaccine, hepatitis B remains one of the most frequent viral diseases throughout the world. Mother to child transmission is one of the primary routes of transmission in children. To assess the vaccine response in children born to HBV infected mothers. HBsAg positive consenting mothers registered in the antenatal care (ANC) service database of Centre Hospitalier Dominicain St Martin de Porres, Yaounde were enrolled with their children. Socio demographic char acteristics were collected using a tested questionnaire. The 5 markers of hepatitis B were tested and the quantification of anti HBsAg antibodies was done by indirect ELISA method. The data collected was analyzed using Microsoft excel and Epi info softwares. Out of 5,996 women registered, 143 were identified as HBsAg positive (2.38% prevalence) and none was HBeAg positive. Of these 143 HBsAg positive women, 50 were enrolled in the study. Of the 50 positive mothers, 78 children were included with a mean age ± standard deviation of 2.33±2.86 years. No child was infected with HBV, but all have been exposed to the virus (HBeAb positive). Overall 64 (82.05%) received at birth both anti HBs immunoglobulin (HBIG) and a dose of vaccine, while 14 (17.95%) received only the birth dose of vaccine. 72 (92.31%) children received all three recommended doses of vaccine. Vaccine responders were 62.82% (above 10 IU/ml), while 37.18% of children were non responders; representing a higher risk group if not boosted. The coverage of the anti HBV vaccine in children in this study was 92.31%. The protection level of 62.82% is below the 95% recommended rate by WHO. The factors sustaining this suboptimal protection should be investigated

ABSTRACT

Introduction The yellow fever epidemic that occurred in 1972/73 in Central Brazil surprised the majority of the population unprotected. A clinical-epidemiological survey conducted at that time in the rural area of 19 municipalities found that the highest (13.8%) number of disease cases were present in the municipality of Luziânia, State of Goiás. Methods Thirty-eight years later, a new seroepidemiological survey was conducted with the aim of assessing the degree of immune protection of the rural population of Luziânia, following the continuous attempts of public health services to obtain vaccination coverage in the region. A total of 383 volunteers, aged between 5 and 89 years and with predominant rural labor activities (75.5%), were interviewed. The presence of antibodies against the yellow fever was also investigated in these individuals, by using plaque reduction neutralization test, and correlated to information regarding residency, occupation, epidemiological data and immunity against the yellow fever virus. Results We found a high (97.6%) frequency of protective titers (>1:10) of neutralizing antibodies against the yellow fever virus; the frequency of titers of 1:640 or higher was 23.2%, indicating wide immune protection against the disease in the study population. The presence of protective immunity was correlated to increasing age. Conclusions This study reinforces the importance of surveys to address the immune state of a population at risk for yellow fever infection and to the surveillance of actions to control the disease in endemic areas. .

ABSTRACT

Avian metapneumovirus (aMPV) is a respiratory pathogen associated with the swollen head syndrome (SHS) in chickens. In Brazil, live aMPV vaccines are currently used, but subtypes A and, mainly subtype B (aMPV/A and aMPV/B) are still circulating. This study was conducted to characterize two Brazilian aMPV isolates (A and B subtypes) of chicken origin. A challenge trial to explore the replication ability of the Brazilian subtypes A and B in chickens was performed. Subsequently, virological protection provided from an aMPV/B vaccine against the same isolates was analyzed. Upon challenge experiment, it was shown by virus isolation and real time PCR that aMPV/B could be detected longer and in higher amounts than aMPV/A. For the protection study, 18 one-day-old chicks were vaccinated and challenged at 21 days of age. Using virus isolation and real time PCR, no aMPV/A was detected in the vaccinated chickens, whereas one vaccinated chicken challenged with the aMPV/B isolate was positive. The results showed that aMPV/B vaccine provided a complete heterologous virological protection, although homologous protection was not complete in one chicken. Although only one aMPV/B positive chicken was detected after homologous vaccination, replication in vaccinated animals might allow the emergence of escape mutants.


O Metapneumovírus aviário (aMPV) é um patógeno respiratório associado à síndrome da cabeça inchada (SHS) em galinhas. Apesar de vacinas vivas contra o aMPV serem utilizadas no Brasil, os subtipos A e B (aMPV/A e aMPV/B) são ainda encontrados no país, com predominância do subtipo B. Este estudo foi conduzido com o intuito de estudar dois isolados brasileiros de aMPV (subtipos A e B) isolados de frango. Para isto, um desafio experimental em frangos foi conduzido com o intuito de explorar a capacidade de replicação dos subtipos A e B Brasileiros. Posteriormente, a protecção virológica conferida por uma vacina do subtipo B em pintos foi realizada com os mesmos isolados. Após o desafio experimental demonstrou-se, por isolamento viral e PCR em tempo real, que o isolado do subtipo B replicou por maior período de tempo e em quantidades maiores, em comparação com o subtipo A. Para o estudo de proteção, 18 pintos de um dia de idade foram vacinados e desafiados aos 21 dias. Usando isolamento viral e PCR em tempo real, em nenhuma ave vacinada e desafiada com aMPV/A foi detectado o vírus, ao passo que uma ave vacinada e desafiada com o aMPV/B foi positiva. Os resultados mostraram que a vacina do subtipo B forneceu protecção heteróloga completa, embora a protecção homóloga não tenha sido conferida em uma ave. Apesar de o aMPV/B ter sido detectado em apenas um frango após vacinação homóloga, a replicação viral em aves vacinadas pode resultar em emergência de mutantes de escape.
