ABSTRACT

ObjectiveTo describe the neuropsychological development screening of 0‒2 years in Tongzhou from 2017 to 2021 so as to understand the status and trend of developmental delay (DD). MethodsAnnual report data of 21 community health service centers in Tongzhou District from 2017 to 2021 were clustered, Chi square test was used to analyze the differences in positive rate and DD rate of children aged 0‒2 years with different ages and household registration, and Chi square trend test was used to analyze the linear trend of each age group and household registration. The Gesell test results in 762 children with developmental delay were analyzed, and Chi square test was used to compare the age distribution differences in gross motor, fine motor, language and personal-social behaviors. ResultsThe DD rate of children aged 0‒2 years in 2017‒2021 was 0.43%. A decreasing trend of DD rate in the 0‒ age group was observed (χ2=14.135, P<0.001), while an increasing trend of DD rate in the 1‒ and <3 age groups was observed (χ2=5.375, P=0.020; χ2=5.558, P=0.018). The DD rate of children aged 0‒2 years with Beijing household registration was higher (χ2=12.504, P<0.001). The DD rate of gross motor was the highest in the 0‒ age group (64.60%), the DD rate of language was the highest in the <3 age group (85.97%), and a statistically significant difference of gross motor and language was separately found in the three age groups (χ2=183.061, P<0.001; χ2=78.450, P<0.001). ConclusionAge and Beijing household registration are the influencing factors of DD for children aged 0‒2, and 0‒ years and <3 years are the critical periods for guidance and intervention to promote the development of gross motor and language abilities.

ABSTRACT

Objective:To analyze the association between HbA 1C variability and the incidence of diabetes retinopathy in patients with type 2 diabetes mellitus(T2DM). Methods:All the patients with type 2 diabetes receiving regular follow-up were enrolled from Lee′s Joint Clinic from 2002 to 2014. Demographic and laboratory data like HbA 1C were collected including fundal examination. According to HbA 1C variability, which was defined as the difference between baseline and last available HbA 1C, participants were divided into three groups, stable group with HbA 1C variability of ±10%, increase group(>10%), and decline group(<-10%). Results:A total of 3 657 T2DM participants were recruited including 662(13.4%) participants with diabetes retinopathy. Blood glucose gradually rose from ideal level [HbA 1C(7.04±1.35)%] and HbA 1C was up to (9.11±1.96)% at the end of follow-up in increase group. HbA 1C gradually fell to (7.27±1.12)% from (10.05±1.99)% of baseline in decline group. HbA 1C of the third group remained relatively stable. Adjusted for age, body mass index, systolic blood pressure, pulse pressure, duration of diabetes, mean HbA 1C of follow-up, glaucoma and so on, logistic regression revealed that participants in stable group( OR=0.800, 95% CI 0.645-0.992) and increase group( OR=0.706, 95% CI 0.548-0.909) had a lower risk of diabetes retinopathy than decline group( P<0.05). Conclusion:HbA 1C variability is an important risk factor of diabetes retinopathy in patients with T2DM. Patients with blood glucose declined had increased risk of diabetes retinopathy as compared to those with rising HbA 1C.

ABSTRACT

【Objective】 To investigate the diagnostic value of severe acute respiratory syndrome coronavirus 2 (SARSCoV-2) specific antibodies IgM and IgG on coronavirus disease 2019 (COVID-19). 【Methods】 1) The test results of SARS-CoV-2 IgM/IgG antibodies and nucleic acid(NAT), which were tested by colloidal gold test and fluorescent quantitative PCR respectively, were collected from 145 febrile outpatients during early March, 2020, named Fever group, in which retrospective analysis and paired chi-square test were performed. 2) 612 cases of SARS-CoV-2 IgM/IgG antibodies test results, which were done on March 5, 2020, were collected. They were named COVID-19 group (Our hospital was provisionally assigned as a specialized hospital for COVID-19, and 1500 COVID-19 patients admitted to our hospital from February 12, 2020 to March 18, 2020). The SARS-CoV-2 IgM/IgG antibodies and NAT were respectively tested on the 30th and the 60th day after the date of discharge. The clinical application values of the antibodies was clarified by statistical analysis. 【Results】 1) In the fever group, the positive rate of SARS-CoV-2 IgM, IgG and IgM+ IgG antibodies were 26.21% (38/145), 54.48% (79/145) and 26.21% (38/145), respectively(P<0.01), and the positive rate of NAT was 4.14% (6/145), which was lower than that of antibody (P<0.01). One (1/145, 0.69%) positive NAT was implicated in initially negative IgM and IgG antibodies samples. 2) In the COVID-19 group, the positive rate of IgM antibody was low (5%) and IgG antibody was high (65%) during 2~14 days after infection, and stably increased during the 15~56 days [IgM 47.68%(277/581) vs IgG 94.15% (547/581) ], then both decreased after 57 days. The positive rates of IgM antibody and IgG antibody were 45.8% (280/612) and 93.1% (570/612) in 612 patients during hospitalization. 15 patients′ data after dischange were not collected as they were later transferred to Huoshenshan Hospital for treatment. The coronavirus NAT results of the rest 597 COVID-19 patients, tested on the 30th and 60th days after the date of discharge, were negative, and the positive rates of IgG antibody and IgM antibody were still ≥80% and ≥40% respectively at the second month after discharge. 【Conclusion】 IgM, IgG antibody against SARS-CoV-2 can be well detected by Colloidal gold method(Innovita), whose positive rate is higher than that of NAT. IgG antibody is produced earlier than IgM, and it keeps high positive rate and persists for a long time. The combination of colloidal gold antibody test and NAT can improve the diagnose rate of COVID-19 and the exclusion of suspected cases.

ABSTRACT

Objectives@#To analyze the status quo and trends on the burden of cerebrovascular diseases between 1990 and 2016 in China.@*Methods@#Morbidity mortality, years of life lost (YLL), years of lived with disability (YLD) and disability-adjusted life year (DALY) related to cerebrovascular diseases between 1990 and 2016, were collated and analyzed, according to the results of the Global Burden of Diseases Study 2016 (GBD 2016). Numbers on incidence and morbidity were used to assess the incidence of diseases, while the numbers of death and mortality were used to assess the death of diseases. Years of life lost due to premature death (YLL), years lost due to disability (YLD) and disability-adjusted life year (DALY) were used to assess the burden of diseases. Changing trend on the burden of cerebrovascular disease from 1990 to 2016 was also analyzed.@*Results@#In 2016 and 1990, the numbers of new cases/morbidity and the number of deaths/mortality on cerebrovascular diseases in the country showed an upward trend. Rates regarding YLL and DALY on cerebrovascular diseases remained stable from 1990 to 2016, however, the YLD rate showed a slow upward trend. The changing rate of DALY was mainly influenced by YLL. Both DALY and YLL crude rates in males showed a slow upward trend, with the highest DALY rate appearing in the ≥70 age group. Disease burden on males was heavier than that of the females and in the 50-60 age group, which taking the largest proportion. As for the composition in DALY, YLL appearing much larger than YLD and slowly increasing. Analysis on the subtypes of diseases, proportions of YLL and DALY in hemorrhagic stroke were greater than that in ischemic stroke while the proportion of YLD in ischemic stroke was in the opposite.@*Conclusions@#The burden of disease on cerebrovascular diseases remained heavy and the differences appeared in age, gender and subtypes of diseases. Our findings called for the adoption of measures including screening, intervention and rehabilitation to be taken on target populations, in order to reduce the burden on both individuals and the society.

ABSTRACT

Objective To analyze the disease constitution and trend of inpatients with malignant tumors in Shanxi Provincial Cancer Hospital. Methods The clinical data of inpatients with malignant tumors in Shanxi Provincial Cancer Hospital from 2008 to 2017 were collected, and the characteristics of disease composition and variation trend of patients were analyzed retrospectively by using Excel, SPSS 22.0 and R3.5.0 statistical software. Results The top 10 malignant tumors from 2008 to 2017 in Shanxi Provincial Cancer Hospital were successively lung cancer (22.92%), gastric cancer (18.34%), cervical cancer (14.31%), esophageal cancer (11.41 %), breast cancer (10.85 %), rectal cancer (6.90 %), thyroid cancer (6.06 %), colorectal cancer (4.30%), ovarian cancer (2.89%) and uterine cancer (2.03%), accounting for 120319 cases in the top 10 malignancies in total. The number of cases presented an increasing trend from 2008 to 2017 (P<0.05). The constitutions of top 10 malignant tumors between men and women were different, and the peak age of malignant cancers was from 51 to 70 years old. Conclusions The number of inpatients with malignant tumors in Shanxi Provincial Cancer Hospital is increasing year by year. Except the special malignant tumors in females, the disease spectrums of malignant tumors inpatients show a consistent trend. Respiratory and digestive system malignancies are the main malignant tumors in Shanxi Province. It is of great significance to master the characteristics of cancer and the changing situation, adjust prevention measures, optimize the allocation of hospital beds as well as medical resources, strengthen clinical specialized subject construction.

ABSTRACT

Objective To analyze the variation and influencing factors of distant vision of students during the recruitment selection and medical examination in two Air Force youth aviation schools in Northeast China in order to protect the youth aviation school students against myopia and to provide reference for interventions.Methods The distant vision of students of each grade in two schools in Northeast China was observed during the recruitment selection and during 2017 medical examination,and was statistically analyzed.The influencing factors of vision were investigated and analyzed.Results There was no statistically significant difference in the distant vision constituent ratio of students in Grade One and Grade Two during the recruitment selection and 2017 medical examination (P ≥ 0.05),but the difference was statistically significant in the distant vision constituent ratio of students in Grade Three during the recruitment selection and 2017 medical examination(P <0.01).There was statistically significant difference in the paired variable differences of students in each grade(P <0.01).The Logistic analysis result showed that reading distance and eating fruits and vegetables were the two important factors that influenced the visual acuity of the students.Conclustion The distant vision gets worse in the second semester of Grade Two,and the range of visual fluctuation is large.So regular examinations of visual acuity should be conducted and feasible measures of myopia protection should be taken to preserve hight-qnality sources of students.

ABSTRACT

Objective To understand the birth conditions of newborns in four cities of China and its trend. Methods The physical development data of single live newborns in 4 cities of Beijing, Harbin, Changsha and Guangzhou were investi-gated by random cluster sampling method, and was compared with the national neonatal growth standard developed from the investigation in 15 cities from 1986 to 1987. Results The study investigated a total of 5 539 newborns:1 412 in Beijing, 1 410 in Harbin, 1 274 in Changsha and 1 443 in Guangzhou. Except for the head circumference of male newborns, the weight, height and head circumference of full-term newborns in four cities in 2012 were all more than the national neonatal growth standard of 1986-1987 (P<0.05). Compared with the data obtained from 1986-1987, the sex ratio and the prevalence of macrosomia and premature were higher, but the rates of post-term delivery and low birth weight were lower (P<0.01). Conclusions The new-borns in four cities of China are in good condition during recent 25 years. More attention should be paid to improve the birth sex ratio and to decrease rates of macrosomia and premature.
