ABSTRACT

Objective To explore the effect of thrombolysis with anticoagulation treatment for early stage of posttraumatic and postoperative deep vein thrombosis of lower extremity. Methods The clinical data of 38 palients with early stage of posttraumatic and postoperative deep vein thrombosis(DVT) of lower extemity treated by thrombolysis with anticoagulation and dispersion drugs were analyzed retrospectively. Results The thrombolytic effect was significant. After the treatment, the deep veins were recanalization without regorging in 71.0% of patients. The total effective rate was 100%. Only one patient had hemorrhagic complication, two patients had symptomatic pulmonary embolism, but none of the patients died. Conclusions Thrombolysis with anticoagulation is an effective and safe method for posttraumatic and postoperative DVT in the early stage.

ABSTRACT

Objective To evaluate the effect of ultrasound ablation transluminally(USATL) for deep venous thrombosis(DVT) of lower extremities. Methods The clinical features of 47 patients with DVT of lower extremities treated by USATL were retrospectively analyzed. Results 5 cases were changed to surger because of a long disease course(4 months~4 years) or chronic inflammation.42 cases had satisfactory results with USATL.Among them,36 cases restored to complete return venous flow;6 had partial return flow. The symptoms and signs of 42 cases were alleviated apparently compared with pretreatment.1 case died of the rectal cancer metastasis 2 months after the treatment. Conclusions USATL can ablate DVT safely ,effectively, and conveniently. Some other transluminal angioplasties are necessary to improve the therapeutic result.

ABSTRACT

Objective To study the diagnosis and treatment of superior mesenteric venous thrombosis (MVT). Methods The clinical data of 13 MVT cases were retrospectively analyzed. Results The diagnosis of MVT in all 13 cases were comfirmed by surgery. Of them, diagnosis for 6 cases was made preoperatively, and misdiagnosed preoperatively in 7.11 patients were cured, and 2 died. Conclusions Because the symptom does not accord with physical sign in the early stage of MVT, the diagnosis of MVT is difficult . B US and CT are useful in the diagnosis of MTV.Intra and post operative coagulation treatment is the key to prevent the recurrence and to reduce the mortality.

ABSTRACT

Objective To determine the collagen deposition in adventitia of venous wall and the influence of removal venous thrombus by Fogarty balloon catheter on venous wall at different time. Methods The collagen deposition in adventitia of venous wall in inferior vena caval thrombosis in rats, and the injury of venous wall caused by Fogarty balloon catheter removal of thrombus at different time were observed. Results Collagen deposition in adventitia of venous wall increased with time. The peak of collagen deposition was 10 days after thrombus formation, which was significantly different from the control group (P

ABSTRACT

Objective To explore the effective treatment of acute deep venous thrombosis(DVT) of left lower extremity accompanied with Cockett syndrome. Methods The data of 16 patients with acute DVT of left lower extremity accompanied with Cockett syndrome, who were admitted to our hospital from August,2004 to January,2008,were analyzed retrospectively.Inferior vena caval filters were inserted and thrombectomy was done to all of the 16 patients;PTA and stent insertion were done in 13 patients and PTA only in the other 3 patients.Anticoagulation, thrombolysis and antiplatelet therapy were given postoperatively to all the patients. Results There was no death or pulmonary embolism in all 16 patients;Forteen patieats had good outcome,2 had acute DVT of left lower extremity one day after surgery, and limbs swelling subsided after anticoagulation, thrombolysis and antiplatelet treatment before their discharge.Forteen patients were followed up from 1 month to 25 months(average 11 months),and 2 patients had post-DVT syndrome, but the others had no swelling or varicose veins of the lower extremity. Conclusions Most patients with acute DVT of left lower extremity accompanied with Cockett syndrome could get satisfactory outcome with thrombectomy,PTA and stent insertion.

ABSTRACT

Objective To observe the intimal changes of inferior caval vein(ICV) wall after combined thrombolysis and anticoagulation therapy for acute thrombosis in rats. Methods The inferior caval venous thrombosis model was established in 105 rats and they were randomly diride into 3 treated groups:heparin(A) treatment group(n=35),urokinase(B) treatment group(n=35),and combination of urokinase and heparin(C) treatment group(n=35),and also eslablished a sham(D) group(n=30).The thrombosed caval veins were taken 1,4,7,14,and 28 d after thrombosis.Changes of thrombus structure and orgainization and intimal hyperplasia were observed by light microscope. The expression areas of collagen fiber were measured by histochemistry.Scanning electron microscopy was used to observe the damage of endothelial cells. Results The intimal hyperplasia in A group was the most severe. The expression area of collagen fiber in group C was less than that of A and B groups(P
