ABSTRACT

OBJECTIVE@#To improve the rat model of cervical spondylosis of vertebral artery type (CSA) induced by injecting sclerosing agent. To evaluate the efficacy of injecting sclerosing agent to induce CSA.@*METHODS@#Forty Health SPF SD rats(20 males and 20 females), were randomly divided into two groups:the model group (20) and the blank group (20). All the animals were followed up for 4 weeks for the observation of general situation, transcranial Doppler(TCD) detection of blood flow velocity, pulsatility index and resistive index of the vertebral artery, measurement of mental distress by open-field test.@*RESULTS@#One to two days after establish the animal model, rats in the model group appeared apathetic with decreased autonomic activities, trembling, squinting, increased eye excrement, etc., and no rats died during the experiment. The mean blood flow velocity of the model group was lower than that of the blank group (P<0.05), and the pulsatilit index and resistive index of the model group were higher than that of the blank group (P<0.05). The mental distress of the model group was significantly higher than that of the blank group.@*CONCLUSION@#The modified injection of sclerosing agent is a practical method to establish the rat model of CSA, with high success rate, high stability, low mortality and simple operation.

ABSTRACT

Ferroptosis is a novel type of cell death, characterized by iron overload and lipid metabolism disorders, and is closely related to the occurrence of cardiovascular and cerebrovascular diseases, tumors, and acute injuries. When tissues and organs of the body are in a state of ischemia and hypoxia, iron ions initiate lipid peroxidation and produce a large number of oxygen free radicals. A series of oxidative stress reactions are more likely to trigger ferroptosis. In the process of cervical spondylosis of vertebral artery type, insufficient blood supply to the vertebral artery caused by degenerative changes of the cervical spine leads to imbalance of iron homeostasis and abnormal amino acid metabolism. Then ferroptosis is induced and plays an important role in the development of cervical spondylosis of vertebral artery type. This paper reviewed the mechanism of ferroptosis in the occurrence of cervical spondylosis of vertebral artery type, in order to provide new ideas and perspectives for the study of related problems of the disease.

ABSTRACT

Objective:To establish a model of cervical spondylosis of vertebral artery type (CSA) in rats by mixed modeling method, and observe the intervention effect of Panlongqi tablet (PLQT) on CSA rats. Method:SD rats were divided into a normal control group, a model group, low- (0.16 g·kg<sup>-1</sup>), medium- (0.32 g·kg<sup>-1</sup>), and high-dose (0.64 g·kg<sup>-1</sup>) PLQT groups, and a Jingfukang granule (JFK, 1.35 g·kg<sup>-1</sup>) group. The rats were treated correspondingly 24 hours after modeling for eight weeks, and those in the normal control group received an equal volume of normal saline by gavage. The limb movement was tested by the inclined plate assay, vertebral artery flow volume by multi-mode high-frequency sound wave for small animals, and microcirculatory blood flow in the pia mater by the laser Doppler. The imaging of the cervical spine was recorded and scored by X-ray micro-computed tomography (Micro CT). Enzyme-linked immunosorbent assay (ELISA) was used to detect serum levels of endothelin-1 (ET-1), nitric oxide (NO), tissue plasminogen activator (t-PA), and plasminogen activator inhibitor (PAI). Result:Compared with the normal control group, the model group showed decreased limb movement, vertebral artery flow volume, and microcirculatory blood flow in the pia mater, and increased imaging of the cervical spine and score (<italic>P</italic><0.05,<italic>P</italic><0.01). PLQT could dose-dependently improve the motor function, increase the vertebral artery flow volume and microcirculatory blood flow in the pia mater, and reduce the degree and score of imaging of the cervical spine in CSA rats(<italic>P</italic><0.05,<italic>P</italic><0.01). The serum levels of NO and t-PA were decreased and those of ET-1 and PAI were increased in the model group as compared with those in the normal control group, while such changes were reversed by PLQT treatment(<italic>P</italic><0.05,<italic>P</italic><0.01). Conclusion:PLQT can enhance the limb movement, promote the vertebral artery flow volume and microcirculatory blood flow in the pia mater, improve the degree of imaging of the cervical spine, regulate the vasomotor function, and improve the coagulation and fibrinolysis system of CSA rats, which shows good potential for the treatment of CSA.

OBJECTIVE@#To compare the efficacy difference among acupuncture at "seven acupoints on neck", acupuncture at regular acupoints and betahistine mesilate tablet for cervical spondylosis of vertebral artery type, and to explore the effective treatment for cervical spondylosis of vertebral artery type.@*METHODS@#Ninety patients were randomly divided into a regular acupuncture group, a medication group and a neck-seven-acupoint group, 30 cases in each group. According to the acupoints prescription for vertigo in "11th Five-Year" Nation Textbook , the patients in the regular acupuncture group were treated with acupuncture at Fengchi (GB 20) and Baihui (GV 20), etc.; the patients in the neck-seven-acupoint group were treated with Fengfu (GV 16), Fengchi (GB 20), Tianzhu (BL 10) and Wangu (GB 12); the two groups were treated once a day, 6 treatments were taken as one course; there was an interval of 1 day between course and totally two course were given. The patients in the medication group were treated with betahistine mesilate tablets, 1 tablet each time, 3 times a day, for 2 weeks. The vertigo symptom and function score, mean blood flow velocity of left vertebral artery (LVA), right vertebral artery (RVA), basilar artery (BA) as well as pulsatile index (PI) and resistance index (RI) were observed, and the clinical efficacy of the three groups was compared.@*RESULTS@#The total effective rate was 93.3% (28/30) in the neck-seven-acupoint group, which was superior to 76.7% (23/30) in the regular acupuncture group and 70.0% (21/30) in the medication group (both 0.05). The improvement of mean blood flow velocity in the neck-seven-acupoint group was superior to those in the medication group and regular acupuncture group (all <0.05); the improvement in the regular acupuncture group was superior to that in the medication group (all <0.05). PI and RI were all reduced after treatment in the three groups (all <0.05); the reduction in the neck-seven-acupoint group was more significant those those in the medication group and regular acupuncture group (all <0.05), and the reduction in the regular acupuncture group was superior to that in the medication group (all <0.05).@*CONCLUSION@#Acupuncture at "seven acupoints on neck" has better clinical efficacy than regular acupuncture and betahistine mesilate tablets, which could obviously improve vertigo and brain blood supply.

ABSTRACT

Objective To analyze differences in gene expression profile of pituitary tissue in cervical spondylosis of vertebral artery type(CSA)model rats and explore the mechanism of adrenal gland's regulation of CSA.Methods Ten SPF male Wistar rats were randomly divided into model group and blank group.The CSA model was established by compound modeling method.The total RNA was extracted from the pituitary;the gene expression profile was detected by whole gene chip,ontology(GO),and signal pathway analysis.Results Compared with the normal group,the differential genes'expression profile analysis showed that the total number of the differential genes was 321(fold change︱ > 2,P<0.05),with 203 up-regulated genes and 118 down-regulated genes.A total of 1 294 genes rich in GO function were involved in the regulation of intercellular signal activity and nerve cell function;the stress response to external stimuli;and the regulation of coagulation function,angiogenesis, endometrial system,and cell cycle;There were 145 signal transducers,including adipocytokine signaling pathway, TGF-β signal transduction,AMPK signaling pathway,PPAR signal pathway,Wnt signaling pathway,and MAPK signal transduction pathway.Conclusion The pituitary regulates CSA mainly through the inflammatory stimulation,immune regulation,regulation of vertebral artery function,and endometrial system.

ABSTRACT

<p><b>OBJECTIVE</b>Establishing a rabbit model of vertebral artery type of cervical spondylosis(CSA) and to observe the characteristics of timeliness in improving the blood flow of vertebral artery by massage, and discusse the material basis of this timeliness based on NPY and ET-1.</p><p><b>METHODS</b>Fifty New Zealand healthy and white rabbits, 6-month-old, the body mass of (2.0±0.5) kg, with half males and half females, were randomly divided into blank group, model group, three massage groups(including massage for 10 min, 20 min, and 30 min group by random number table), 10 rabbits in each group. In addition to the blank group, CSA rabbit model was made by injection of sclerosing agent in other groups. The rabbits of massage for 10 min, 20 min, and 30 min groups received the massage therapy of corresponding duration, one times a day, continuous 10 days. The blood flow of vertebral artery in each group was detected by PeriFlux5000 laser doppler, and the contents of NPY and ET-1 in serum were detected by ELISA before and after treatment.</p><p><b>RESULTS</b>Changes in blood flow of vertebral artery before and after treatment:there was no significant difference between model group and massage for 10 min group;there was significant difference between model group and massage for 20, 30 min groups(<0.01); there was significant difference between massage for 10 min group and massage for 20, 30 min groups(<0.01); there was no significant difference between massage for 20 min group and massage for 30 min group. Changes of NPY content before and after treatment: there was significant difference between model group and massage for 10 min group(<0.05); there was significant difference between model group and massage for 20, 30 min groups(<0.01); there was significant difference between massage for 10 min group and massage for 20, 30 min groups(<0.01); there was no significant difference between massage for 20 min group and massage for 30 min group. Changes of ET-1 content before and after treatment:there was no significant difference between model group and massage for 10 min group;there was significant difference between model group and massage for 20, 30 min groups(<0.01); there was significant difference between massage for 10 min group and massage for 20 min, 30 groups (<0.01); there was no significant difference between massage for 20 min group and massage for 30 min group.</p><p><b>CONCLUSIONS</b>Massage needed 20 min for rabbits with CSA can only significantly improve the blood flow of vertebral artery. However, prolonging the time of massage has no obvious effect. The material basis of this timeliness characteristic of massage is closely related to the change of NPY and ET-1 levels in serum.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To observe the differences in the clinical therapeutic effects on cervical spondylosis of vertebral artery type (CSA) between the modified acupuncture and the routine acupuncture at unilateral/bilateral Renying (ST 9) as well as the impacts on the concentrations of plasma neuropeptide Y (NPY) and urotensinⅡ(UⅡ) in the patients.</p><p><b>METHODS</b>A total of 160 patients were divided into a modified bilateral acupuncture group, a modified unilateral acupuncture group, a routine bilateral acupuncture group and a routine unilateral acupuncture group, 40 cases in each one according to the random number table. In the modified bilateral acupuncture group, the modified acupuncture was applied bilaterally to Renying (ST 9). In the modified unilateral acupuncture group, the modified acupuncture was applied unilaterally to Renying (ST 9). In the routine bilateral acupuncture group, the routine acupuncture was applied bilaterally to Renying (ST 9). In the routine unilateral acupuncture group, the routine acupuncture was applied unilaterally to Renying (ST 9). The treatment was given once every day, continuously for 6 days as one course. Two courses of treatment were required at the interval of 1 day. In each group, before and after treatment, we observed the peak systolic blood flow velocity (Vs) of the vertebral artery (VA) and the basilar artery (BA), cervical vertigo symptoms and functional assessment scales (ESCV) and the concentration of plasma NPY and UⅡ. The clinical therapeutic effects were compared among the groups.</p><p><b>RESULTS</b>After treatment, the clinical therapeutic effect in the modified bilateral acupuncture group was 90.0% (36/40), which was better than 80.0% (32/40) in the modified unilateral acupuncture group, 77.5% (35/40) in the routine bilateral acupuncture group and 65.0% (26/40) in the routine unilateral acupuncture group (all <0.05). After treatment, Vs of VA and BA was improved remarkably in every group (all <0.01), and the result in the modified bilateral acupuncture group was higher than those in the other groups (all <0.01). After treatment, ESCV scores were all increased remarkably in every group (all <0.01). ESCV score and improvement index in the modified bilateral acupuncture group were all higher than those in the other groups (<0.05, <0.01). After treatment, the concentrations of plasma NPY and UⅡ were all reduced remarkably in every group (all <0.01) and the differences were significant among the groups (all <0.01).</p><p><b>CONCLUSION</b>The modified bilateral acupuncture at Renying (ST 9) effectively regulates the blood supply of the vertebral basilar artery and improves the cerebral circulation. The effects are superior to those of the unilateral acupuncture at Renying (ST 9).</p>

ABSTRACT

Objective To explore the use of three-dimensional computed tomography angiography (3D CTA) in the diagnosis of cervical spondylosis with vertebral artery involvement.Methods The study group included 45 patients treated for cervical spondylosis with vertebral artery involvement,from January 2014 to July 2015;42 healthy volunteers were selected as the control group.3D CTA data of all research subjects were evaluated.Results The findings in the experimental group included 3 patients with normal vertebral artery,8 with unilateral narrowed lumen,2 with aberrant route,3 with vascular sclerosis,8 with local lumen stenosis,3 with left side compression,4 with right side compression,and 5 with vessel tortuosity.The right vertebral artery transverse diameter and radius,the left vertebral artery transverse diameter and radius,and 3D images of the left vertebral artery diameter,right transverse diameter,right radius,and left transverse process inner diameter showed statistically significant differences between the two groups (P < 0.05).Conclusion 3D CTA can be used for accurate diagnosis of cervical spondylosis with vertebral artery involvement and can clearly display the diameter of the vertebral artery and the transverse diameter of the vertebral artery.

ABSTRACT

This study was aimed to summarize the clinical medication rule of professor Shi Qi in the treatment of cervical spondylosis of vertebral artery type (CSA),in order to explore the academic ideas.A total of 265 CSA cases from professor Shi Qi's outpatient clinic were collected in Longhua Hospital from 2009 to 2015.Frequency analysis and cluster analysis were conducted on used herbal medicine from included cases by SPSS 21.00 software.The results showed that there were a total of 202 types of herbs used.The five most commonly used herbs were Rhizoma Ligustici Wallichii,Radix bupleuri,Radix Achyranthis Bidentatae,Codonopsis and Gastrodia elata.The cluster analysis revealed that professor Shi Qi frequently used Huo-Xue Hua-Yu herbs,Bu-Xu herbs,Ping-Gan Xi-Feng herbs,Qu-Feng-Shi herbs.Sheng-Yu decoction and Tian-Ma Gou-Teng decoction were the core prescriptions used by professor Shi Qi in the treatment of CSA.It was concluded that the cluster analysis showed that academic ideas of professor She in the treatment of CSA was to pay equal attention to both qi and blood,to focus on liver and kidney,as well as to remove phlegm and blood stagnation,to treat both the branch and the root.The cluster analysis revealed a certain medication rule of professor Shi Qi in the treatment of CSA.It can be used as guidance in the clinical practice.

The blood flow velocity was compared before and after the treatment, and the clinical efficacies of the two groups were compared.Result After the treatment, the systolic velocity (Vs) (left vertebral artery, LVA; right vertebral artery, RVA), mean velocity (Vm) (LVA, RVA) and diastolic velocity (LVA, RVA) of the observation group were better than those of the control group, and the differences were statistically significant (P<0.05); at the end of the intervention and 3 months after, the Dizziness Assessment Rating Scale (DARS) scores of the observation group were significantly lower than those of the control group (P<0.05); at the end of the intervention, the markedly effective rate was 80.0% in the observation group versus 62.3% in the control group, and the difference was statistically significant (P<0.05).Conclusion Mild moxibustion at bilateral Fengchi can improve the blood flow of vertebral artery in patients with cervical spondylosis of vertebral artery type, and produce a significant efficacy.

ABSTRACT

Objective To analyse effects of psychological intervention combined with celecoxib and physiotherapy on vertebral artery type of cervical spondylosis.Methods 134 patients with vertebral artery type of cervical spondylosis were grouped into two groups.Two groups were treated with physiotherapy and celecoxib,observation group was treated another with psychological intervention.Results Total efficiency of observation group was higher than that of control group(P<0.05).After treatment,symptom score,CASCS score,mean blood flow velocity in left vertebral artery and right vertebral artery,TNF-α and IL-6 level of observation group were better than that of control group(all P<0.05).During treatment, there was no obvious adverse reaction in the two groups.Conclusion Sychological intervention can improve the therapeutic effect of patients with cervical spondylosis of vertebral artery type.

ABSTRACT

Objective To study the improvement of hemorheology and prognosis of vertebral artery type cervical spondylosis by combination of strong Dingxuan tablet.Methods106 cases of vertebral artery type cervical spondylopathy were randomly divided into observation group and control group.Fifty-three patients in each group were treated with manipulation in the control group, and the patients in the observation group were enrolled.The blood viscosity, whole blood low shear viscosity, plasma viscosity, whole blood reduction viscosity, hematocrit, erythrocyte aggregation index, fibrinogen and so on were compared between the two groups before and after treatment.Rheological parameters of the situation, the two groups of patients for 1 year follow-up observation, compared with 1 year recurrence rate.ResultsThe total effective rate was 92.45% in the observation group and 77.36% in the control group (P<0.05).The whole blood low shear viscosity (7.01±1.52) mPa·s, the whole blood viscosity (1.18±0.28) g/L was significantly higher than that in the control group (7.88±1.65) mPa·s, (1.47±0.34) mPa·s, (4.51±0.98) g/L, respectively (P<0.05).The recurrence rate was 13.21% in the observation group and 30.19% in the control group (P<0.05).ConclusionThe combination of strong Dingxuan tablet combined with manipulation of vertebral artery type cervical spondylopathy is significant, the blood viscosity of the patients is reduced, the blood circulation function is improved, the recurrence rate after treatment is low, and it is worthy of clinical application.

ABSTRACT

Objective To analyze the effects of gentongping pill combined with physiotherapy on the patients with vertebral artery type of cervical spondylosis. Methods 134 patients with vertebral artery type of cervical spondylosis were grouped into two groups, the observation group and the control group. The two groups were treated with physiotherapy and the observation group was treated with Gentongping pill. The effects in the two groups were evaluated. Results The total efficiency in the observation group was higher than that in the control group(P<0.05). After treatment, the symptom score, CASCS score, mean blood flow velocity in left vertebral artery and right vertebral artery, TNF-α and IL-6 level in the observation group were better than those in the control group (all P<0.05). There was no obvious adverse reaction in the two groups. Conclusion Itcan improve the therapeutic effect which Gentongping pill is used on the treatment of patients with vertebral artery type of cervical spondylosis.

ABSTRACT

Objective To compare and evaluate the methods of building model of cervical spondylosis of vertebral artery type (CSA)rats by measuring the learning and memory ability,anxiety and depression,and degree of nervous tension of three kinds of CSA model rats.Methods We randomly divided 120 rats into mixed modeling group,bone graft compression modeling group,mechanical balance disorder modeling group,and blank control group with 30 rats in each.Morris water maze,elevated plus maze test and open field test were used to detect and compare the rats’abilities of learning and memory,anxiety and depression,and degree of nervous tension.We then evaluated the three CSA rat models.Results Compared with those in the blank group,the learning and memory abilities in the mixed modeling, bone graft compression and mechanical balance disorder modeling rats were significantly decreased,the anxiety and depression and degree of nervous tension were significantly increased (P<0.05).Compared with bone graft compression and mechanical balance disorder modeling groups,the mixed modeling group could restore the characteristics of CSA.Conclusion The three kinds of modeling methods can successfully reproduce the CSA animal model;the mixed modeling is superior and thus worthy of promotion.

ABSTRACT

Objective To observe the clinical efficacy of abdominal acupuncture plus moxibustion in treating cervical spondylosis of vertebral artery type.Method A hundred patients with cervical spondylosis of vertebral artery type were randomized into a treatment group and a control group, 50 cases in each group. The control group was intervened by abdominal acupuncture plus electroacupuncture; while the treatment group was by moxibustion in addition to the treatment given to the control group. After a treatment course, the onset and remission time of vertigo was observed, and the clinical efficacies were compared between the two groups.Result The total effective rate was 96.0% in the treatment group versus 93.8% in the control group, and the difference was statistically significant (P<0.05). There was a significant difference in comparing the onset and remission time of vertigo between the two groups (P<0.05). In comparing the relapse rates 3, 6, and 12 months after the intervention between the two groups, the difference were statistically significant (P<0.01).Conclusion Abdominal acupuncture plus moxibustion is an effective approach in treating cervical spondylosis of vertebral artery type.

ABSTRACT

Objective To observe the clinical effects of Huoxue Dingxuan Capsules on treatment of vertebral artery type of cervical spondylosis, and evaluate its efficacy and safety. Methods Totally 130 cases of vertebral artery type of cervical spondylosis were randomly assigned to treatment group and control group, and treated with Huoxue Dingxuan Capsules and Sibelium capsules respectively. Integration method was used to compare the differences of main symptoms and signs, the overall efficacy and safety were evaluated, and relapse after six months was followed up. Results Both groups had a significant effect on the improvement of signs and symptoms in vertebral artery type of cervical spondylosis, the total efficiency was 94.03%(63/67) in treatment group and 82.54% (52/63) in control group. The treatment group had a better effect on the overall efficacy and relieving symptoms and signs compared with the control group (P<0.01). The mean blood flow velocity had a significant change after treatment (P<0.05), and the treatment group was better than the control group (P<0.01). Within six months, the treatment group had 4 cases (6.34%) with mild recurrence of symptoms and signs, and the control group had 18 cases (34.61%), the difference between the two groups was statistically significant (P<0.05), and no severe recurrence was found. The two groups showed no side effects. Conclusion Huoxue Dingxuan Capsules is safe and effective for treating vertebral artery type of cervical spondylosis, and the long-term efficacy and subjects’ compliance are better than oral Sibelium capsules.

ABSTRACT

Objective To observe the therapeutic effect of treating cervical spondylosis of vertebral artery type (CSA) mainly by needling EX-B2 point & Tui na,and to investigate the advantage of needling EX-B2 point to treat CSA.Methods 60 patients with CSA were divided into a trial group (30cases) and a control group (30cases) by the sequence of visit.Patients in the trial group were treated by needling EX-B2 point combined with electro-acupuncture and Tui na,other patients in the control group were treated by Tui na,with 2 weeks as a therapeutic course.Then,the change of therapeutic effect,symptoms index,mono-symptomatic therapeutic effect and average blood flow velocities of vertebral artery and basilar artery were observed in both groups before and afer the treatment.Results The therapeutic effect constitution & cure rate showed significant differences between the two groups (P<0.05),but there was no difference in total efficacy.Significant differences were found in both groups in symptoms index before and after treatment,and after the treatment the symptoms index was lower.There was significant difference between the two groups in symptoms index difference (P<0.05),and the trial group was better.The mono-symptomatic therapeutic effect of vertigo,nausea and vomiting showed significant difference between the two groups (P<0.05),and the trial group was better.No marked differences were observed in avenge blood flow velocities of vertebral artery and basilar artery between groups and in each group.Conclusion The therapeutic effect of CSA mainly treated by needling EX-B2 point & Tui na was much better than just treated by Tui na,and this method showed obvious advantage in relieving vertigo.

ABSTRACT

@#Objective To explore the effect of silver needle thermo-conduction on cervical spondylosis of vertebral artery type (CSA). Methods 82 patients with CSA were randomly divided into control group (n=41) and treatment group (n=41). The control group was treated with cervical traction combined with microwave diathermy, and the treatment group was treated with silver needle thermo-conduction combined with cervical traction. Both groups were evaluated with Evaluation Scale for Cervical Vertigo (ESCV) and transcranial Doppler before and 1 course after treatment. Results The ESCV score, the peak blood flow velocity of period of contraction of left vertebral artery, right vertebral artery and basilar artery in both groups were significantly higher after treatment than before (P<0.01), the treatment group was better than the control group after treatment (P<0.01). Conclusion Silver needle thermo-conduction combined with cervical traction therapy can accelerate the blood flow of patients with CSA and improve their clinical symptoms

ABSTRACT

Objective To compare the clinical efficacy of triple needling and conventional acupuncture in the treatment of cervical spondylosis of vertebral artery type. Methods 49 cases with cervical spondylosis of vertebral artery type were randomly divided into two groups ,24 patients of control group treated with conventional acupuncture,25 patients of observation group treated with triple needling,the clinical results of two groups were compared. Results The obvious effective rate of the observation group was significantly higher than the control group, there were significant differences between two groups( x2 =4. 69 ,P < 0. 05). The improvement of average flow velocity(Vm) of the observation group was better than the control group, there was significant difference between two groups ( P < 0. 05 ).Conclusion Triple needling in the treatment of cervical spondylosis of vertebral artery type had better clinical results, and it was worthy of concern.
