ABSTRACT

At present, there is no consensus on the diagnosis, treatment, and management of pulmonary nodules with integrated traditional Chinese and Western Medicine in China. To establish the clinical management strategy of pulmonary nodules with integrated medicine, a consensus was formed after full discussion by the experts from the Cancer Committee of Chinese Association of Integrative Medicine and the Cancer Committee of Beijing Association of Chinese Medicine according to the guidelines for pulmonary nodules issued in recent years and the clinical practice of multiple hospitals in the treatment of cancers with integrated Chinese and Western medicine. This consensus involves the existing clinical management guidelines of pulmonary nodules and the intervention methods of traditional Chinese medicine (TCM). Considering the therapeutic positioning and value of TCM, this consensus standardizes the whole-process management of pulmonary nodules with integrated traditional Chinese and Western medicine, which will help more patients.

ABSTRACT

Liver transplantation is one of the best approaches for the treatment of hepatocellular carcinoma. The concept of transplant oncology could shed light on the whole process management of hepatocellular carcinoma patients. The success rate of operation and perioperative safety were the major concerns in the past, whereas the focus of treatment is gradually shifting to cancer treatment and improving patient survival and quality of life, with the promotion of neoadjuvant and postoperative adjuvant therapy for hepatocellular carcinoma. The prognosis of different group of patients might be heterogeneous. Therefore, refined stratification should be carried out for heterogeneous patients before and after liver transplantation to achieve the best prognosis. The present study classified patients for three clusters: primary hepatocellular carcinoma patients within the transplant criteria, hepatocellular carcinoma recurrence after liver resection, and patients after down-staging or neoadjuvant therapy. Fine stratified management are essential for the whole process management in the new era of transplant oncology.

ABSTRACT

We retrospectively analyzed the clinical characteristic of one patient with metastatic prostate cancer and the relative literatures were reviewed. A 40-year-old man was admitted and diagnosed as prostate cancer on March 20, 2018(T 4N 1M 1a) with prostate-specific antigen (PSA) at 47.99 ng/ml. The first 68Ga-PSMA PET/CT showed multiple nodular lesions in the bilateral peripheral bands of the prostate, multiple nodular lesions in the right apex, abnormal uptake of nuclides in multiple lymph nodes in the abdominal aortic wandering zone, the abdominal aortic bifurcation zone, and the bilateral iliac artery wandering zone at the level of the lumbar 2-5 vertebral body, and metastasis was considered. The patient was treated with six cycles of drug castration combined with antiandrogenic treatment and pre-operative system chemotherapy(docetaxel). Six months later, the PSA decreased to 0.225ng/ml. Robot-assisted laparoscopic prostatectomy and expanded pelvic lymph node dissection was performed. Postoperative total androgen blocking therapy was maintained, and PSA slowly increased. Ten months after operation, salvage radiotherapy for enlarged lymph nodes was performed in pelvic extension field, prostate tumor bed area and pelvic cavity. PSA remained stable for 7 months postradiotherapy, and then increased. The patient developed castration-resistant prostate cancer and was treated with triptorelin combined with abiraterone. PSA was decreased, and local radiotherapy was performed for new lymph node metastases in the neck. 68Ga-PSMA PET/CT could provide a decision-making basis for accurate clinical staging, therapeutic effect evaluation and distant metastatic lesions location with guiding value for the formulation of individualized treatment plans.

ABSTRACT

@#Clinical trials are the key step in verifying the effectiveness, safety and usability of novel oral medical devices. With improvements in invention capability, the number of clinical trials of medical devices has steadily increased domestically, indicating the need for new requirements for quality management. Here, on the basis of summarizing the characteristics of clinical trials for oral medical devices in China, we proposed a whole-process quality-control model for good clinical practice (WQGCP). This model clarified the management specifications for oral medical device clinical trials in the new era. It indicates that the clinical trial design is the breakthrough point, while whole-process risk control is the key point. This suggests the need for the establishment of a quality management cooperation system, evaluation and postevaluation systems and the training of high-level personnel who conduct clinical trials. It also provides suggestions to promote the participation of oral clinicians in high-quality research and invention of novel clinical medical devices with the aim of accelerating the development of oral medical devices in China.

ABSTRACT

Perioperative whole-process management (WPM) for patients with advanced gastric cancer (AGC) mainly focuses on some clinical issues which are easily neglected or underappreciated. WPM is helpful in making a scientific and rational therapeutic plan, and avoiding inadequate communication in multi-disciplinary participation, so that the diagnosis, treatment and rehabilitation for AGC patients can be integrated organically. Based on the current clinical practice for AGC patients, eight key issues in WPM should be emphasized.(1) Preoperative clinical staging. An accurate preoperative staging by endoscopy and imaging technique is helpful in setting up a rational therapeutic plan, and is also a prerequisite to start WPM. (2) Indications and value of diagnostic laparoscopy. Laparoscopic exploration is beneficial to find intraperitoneal micro-metastases so as to avoid unnecessary laparotomy. For cases of AGC infiltrating serosal layer or suspected of peritoneal metastasis, preoperative laparoscopic exploration should be routinely performed. (3) Neoadjuvant therapy. Multiple RCT studies have shown that neoadjuvant chemotherapy can benefit a majority of patients with AGC, improving prognosis and prolonging their overall survival. Therefore, neoadjuvant therapy should be considered first for stage III and IVA AGC patients. (4) Prediction of efficacy in neoadjuvant chemotherapy. Endoscopy, MDCT scan, PET-CT and liquid biopsy have certain predictive value individually, which can be used together or separately to improve the accuracy of prediction. (5) Effective prevention of postoperative peritoneal metastasis. Extensive intraoperative peritoneal lavage (EIPL), neoadjuvant intraperitoneal and systemic chemotherapy (NIPS), hyperthermic intraperitoneal chemotherapy (HIPEC), early postoperative intraperitoneal chemotherapy (EPIC), and normothermic intraperitoneal chemotherapy (NIPEC) have been shown to be of various efficacy in preventing peritoneal metastases. (6) Prediction of postoperative prognosis of AGC patients. The key pathological indicators are tumor regression grade (TRG) and ypTNM staging, especially if there is lymph node metastasis. Usually for AGC patients who received neoajuvant chemotherapy with TRG 0 or ypN0, their prognosis was comparable to that of patients with cTNM stage I.(7) Postoperative adjuvant chemotherapy. Postoperative adjuvant therapy is always an important part of the WPM management of AGC patients. Several recent RCT studies have shown that duplet chemotherapy can significantly reduce the risk of death after D2 radical gastrectomy compared to singlet chemotherapy, especially for stage III patients. (8) Perioperative nutritional support. Due to different degrees of malnutrition in AGC patients, enhanced nutritional treatment in the perioperative period can not only reduce surgical complications, but also enable patients to complete necessary course of chemotherapy, and ultimately further improve their survival rate.

ABSTRACT

Endoscopic retrograde cholangiopancreatography (ERCP) is the most challenging technique of gastrointestinal endoscopy. Patients are most likely to benefit from this mini invasive treatment, meanwhile they bear high risk of complications, which might be life-threatening. Standard operation, standardized training, quality control and whole-process management of perioperative period are critical factors for ERCP training, and should be popularized in medical centers, where ERCP are performed. This paper has illustrated the whole-process management of perioperative period for ERCP, including preoperative preparation, decision-making during operation, postoperative management, as well as prevention and treatment of complications. By performing whole-process management of ERCP, patient can benefit more from ERCP operation, and which will serve more patients.
