ABSTRACT
OBJECTIVE: To develop the model of medication reconciliation (MR) service provided by clinical pharmacist for chronic kidney disease (CKD). METHODS: The definition, working process, development of MR were analyzed and the medication problems we reviewed were classified and intervened according to the newest Pharmaceutical Care Network Europe (PCNE) V9.0 classification scheme which was constructed for drug related problems (DRPs) to analysis and summary. We stated and summarized procedure of MR by several cases based on the realities of our hospital. RESULTS: A new closed loop model of outpatient clinic to hospital, then to home was developed, a working process was made according to three settings consist of outpatient clinic, hospital and home. CONCLUSION: The procedure of MR of several cases was summarized based on the realities on our hospital.

This article introduced the peritoneal dialysis nursing model of Home Dialysis Unit in the affiliated hospital of Colorado University,including:overall status,setting and distribution,responsibilities of staff,work model of PD (pre-dialysis education,catheter insertion,CAPD/CCPD training,clinic visit model,home visit model,etc),and explored the enlightenment of this model on PD nursing in our country.This model provided references for dialysis center distribution,staff arrangement,pre-dialysis education,home visit,increase of social support and so on.

OBJECTIVE To save rust remover's cost,reduce the material consuming and ensure the cleaning quality of the instruments by changing the work model.METHODS According to the key factors of the increasing cost of the medical rust remover,adjust the work model properly,that is to say,set up target by know about the problems;confirm the measurement data of the problems;analyze the problems and figure out key minority;make strategy to correct the mistakes;and make the controlling measures to ensure effective implementation.Enforce organization management,and remove the rust collectively and grouply by time,by ration and by certain number of people.RESULTS The rust remover cost had been saved greatly and the cleaning quality had been improved distinctly,the number of scrapped medical instrument had been reduced.Compared with the period before improvement,it had statistic significance on difference(P

OBJECTIVE: To provide reference for the management of central pharmacy in our hospital. METHODS: In daily work, performance score of a job was related with quality evaluation using grouping work model and management method of allocating a specific responsibility to a specific person. RESULT & CONCLUSION: The work process of central pharmacy is clear and plays a great role in economizing on resources relatively.
