ABSTRACT

Objective To summarize the medical rescue of Chinese Government Medical Team (Chongqing) in Nepal earthquake region in order to explore the work pattern of transnational medical rescue, and improve the rescue efficiency. Methods From the experience about the post-earthquake medical rescue of Chinese Government Medical Team (Chongqing) in Nepal in 2015,several aspects were worthy to summarize such as the establishment of medical team, the layout of camp site, the work algorithm and process, with the analysis of injury feature and outcome of treatment. Results Under the setting of efficient organization and rational assignment of professional work, special working rules, the mutual transfer treatment and multi-disciplinary treatment were employed for 737 emergency patients.Of them,128 patients were hospitalized (including 63 patients completely recovered,56 patients were of clinical improvement, and 9 patients were critically ill transferred to other hospital for advanced treatment),and post-traumatic complication occurred in 48 cases without death. In addition,148 operations were carried out successfully. Conclusions The earthquake disaster has specific feature such as suddenness,a host of casualties and poor rescue conditions, and overseas rescue is with the presence of the language barrier, the difference in living habit, and the lack of coordination with local rescue system, therefore, rationally assigning personnel and resources and establishing work pattern with flexibility, orderly and good communication are the key to promote the efficiency of transnational medical rescue for the injured patients in earthquake region.

ABSTRACT

Objective To explore the management model of chronic disease like myocardial infarction through the effect of collaborative work pattern of doctors and nurses on acute myocardial infarction patients with percutaneous coronary intervention(PCI) after 1 year follow up.Methods A total of 852 patients from October 1,2011 to September 30,2013 with collaborative work pattern of doctors and nurses were as collaborative work pattern group, and 734 patients without collaborative work pattern of doctors and nurses at the same time were as control group. The patients were followed up for 1 year. The differences of treatment compliance, risk factors control, and the incidence of cardiovasc~ar events were compared between collaborative work pattern group and control group. Results The improvement of treatment compliance in collaborative work pattern group was better than that in control group. The improvement rate of improve lifestyle initiatively, completely medication, check regularly were 83.3%(710/852),97.9%(834/852),75.4%(642/852) in collaborative work pattern group and 63.5%(466/734),93.7%(688/734),59.0%(433/734) in control group,x2=81.02,17.57,26.22,P<0.01.The control rate of risk factors including hypertension, diabetic mellitus, high blood low density lipoprotein cholesterol were 78.5% (252/321),68.4% (214/313),84.8% (491/579) in collaborative work pattern group and 55.8% (168/301),58.1% (168/289),79.9% (381/477) in control group,x2=36.47,6.79,4.41,P<0.01 or <0.05.The incidence of treatment compliance and total cardiovascular events in collaborative work pattern group were better than those in control group. There were significant differences in the above indices,P<0.05.Conclusions Collaborative work pattern of doctors and nurses is an effective method and supplement for patients with myocardial infarction after PCI to improve treatment compliance and control risk factors. The prognosis of patients can be improved and this pattern is an effective exploration of chronic disease management.

ABSTRACT

Objective To explore the psychological experience of ICU nurses who had worked follow different work patterns, and find the most appropriate staffing model. Methods A phenomenological approach of qualitative research was conducted in 5 ICU nurses by intensive interview, then the data collected were read, analyzed, reflected, finally the theme was got. Results In the work pattern of shifted every 12 hours, with low work frequency and strong feeling of fatigue. In the work pattern of shifted every 8 hours, with high work frequency and the disadvantage of nurse- patient relationship. In the short-time continuous APN work pattern, with continuous management of patients' condition, promotion of nurse-patient relationship, improvement of doctor-nurse relationship, comparatively stable biological clock. Conclusions The short-time continuous APN work pattern can concentrate nurses' work and rest time, relieve the press that caused by the shifted work, at the same time, promote the patients',patients' family and doctors' satisfaction degree with nursing work, increase nurses' effect in the ICU team, guarantee the quality of care, eventually ensure the patients' safety.

ABSTRACT

Some Bemorheogical indexes of the middle-aged persom and the ages of "mental work pattem" health state were compared with that of the middle aged persen and the aged in general health in the paper, The authors found the values of the former were higher than that of the latter in varied degrees. Except the both sexes hematocrit and the male's erythrocyte sedimentation rate, the other inedxes of the both groups showed very significant differences. According to the cpidemiological data which showed the incidences of ischemic cardio-cerebrovascular diseases in the mental works were higher than that in the physical labourers, it is suggested that the differences of hemorheological characterislics in the both may be the one of the causes. The authors suggest that it should pay atterntion to health states of the middle aged person and the aged who had elementary normal conclusion in physical examination but they had abnormal high values of one or some hemorheological indexes.
