ABSTRACT

Objective To investigate the effect of Yanhuning injection antiviral therapy in the treatment of acute viral myocarditis patients with Xiedu Qinxin syndrome.Methods From May 2016 to August 2017,84 acute viral myocarditis patients with Xiedu Qinxin syndrome in the Integrated Traditional Chinese and Western Medicine Hospital of Wenzhou were enrolled in this study.According to the digital table,they were randomly divided into observation group and the control group,with 42 cases in each group.The control group received conventional antiviral therapy,and the observation group was given Yanhuning injection.The heart type free fatty acid binding protein (H-FABP),serum troponin Ⅰ (cTnI),macrophage migration inhibitory factor(MIF)and interleukin 4(IL-4)were measured before and after treatment for 4 weeks.The clinical symptoms score and the effective rate were compared between the two groups at the same time.Results The serum levels of cTnI,H-FABP,MIF and IL-4 of the two groups after treatment were lower than those before treatment(all P<0.05 ),which of the observation group after treatment were significantly lower than those of the control group (t=3.012,P=0.039;t=2.835,P=0.040;t=3.534,P=0.032;t=3.323,P=0.033).The effective rate of the observation group was significantly higher than that of the control group (90.48% vs.80.95%,χ2=3.432,P=0.038).The scores of palpitations,sore throat,upsetting the chest tightness of the observation group after treatment were significantly lower than those of the control group (t=3.045,P=0.038;t=2.946,P=0.039;t=3.467,P=0.031;t =3.358,P=0.032).Conclusion Yanhuning injection antiviral therapy in the treatment of acute viral myocarditis patients with Xiedu Qinxin syndrome can signifi-cantly improve the efficacy of patients.

ABSTRACT

Objective To discuss the effect of Yanhuning Injection combined with amoxicillin and clavulanate potassium in treatment of infantile pneumonia.Methods Totally 1 00 infants with pneumonia were selected.They were randomly divided into two groups,the observation group (52 cases) were given Yanhuning Injection combined with amoxicillin and clavulanate potassium.The control group (48 cases) were given Yanhuning Injection.The effect of Yanhuning Injection combined with amoxicillin and clavulanate potassium in treatment of infantile pneumonia was evaluated using efficacy,cough,pulmonary rales disappeared time,pyretolysis time,pulmonary function indexes and adverse reaction during treatment as indexes.Results The effective rate was 92.3% in the observation group and 75.0% in the control group.The effective rate in the observation group was higher than that of the control group (P < 0.05).The cough,pulmonary rales disappeared time of the observation group was shorter than that of the control group (P < 0.05).There was no statistical significance on pyretolysis time between two groups.Before treatment,there was no statistical significance on FVC,FEV1 and FEV1/FVC% between two groups.After treatment,the FVC,FEV1 and FEV1/FVC% were increased and higher in the observation group (P < 0.05).During treatment,there was no statistical significance on adverse reaction between two group.Conclusion Yanhuning Injection combined with amoxicillin and clavulanate potassium has a good effective on infantile pneumonia.It could improve the symptoms of pneumonia and lung function while inhibiting inflammation with good safety.It was worthy of clinical application.

ABSTRACT

Objective To observe the clinical curative effect of aerosol inhalation of Xiyanping combined with Potassium Sodium Dehydroandroan drographolide Sucinate in treatment children with herpangina. Methods A total of 106 cases of children with herpangina were collected in Zichang County People's hospital from July 2014 to Dec. 2016. All cases were randomly divided into the treatment group (54 cases) and the control group (52 cases) by double blind method. The treatment group was given inhalation treatment of Xiyanping combined with Potassium Sodium Dehydroandroan drographolide Sucinate, the control group were treated with Xiyanping plus Ribavirin Spray. The clinical efficacy and recurrence rate were compared before and after treatment. Results The total effective rate of the treatment group was significantly higher than that of the control group (P<0.05), and the adverse reactions and recurrence rate were significantly lower than that of the control group (P<0.05). Conclusion Inhalation treatment of Xiyanping combined with Potassium Sodium Dehydroandroan drographolide Sucinate in children with Herpangina has significant clinical efficacy with less adverse reaction and lower recurrence rate.

ABSTRACT

Objective To investigate the effect of potassium sodium dehydroandroandrographolide succinate injection on helper T cells in peropheral blood and the content of products of oxidative stress in patients with pulmonary infection.Methods 80 community patients with acquired pneumonia in our hospital were selected as the research subjects.They wererandomly divided into observation group and control group,40 cases in each group.The control group was treated with routine treatment of community-acquired pneumonia,patients in the observation group were given conventional treatment combined with potassium sodium dehydroandroandrographolide succinate injection.The peripheral blood T cell subsets,inflammatory cytokines and oxidative stress were compared in two groups.Results After treatment,malondialdehyde,advanced oxidation protein products of the observation group were (14.61 ± 1.65)U/L,(42.67 ± 5.62)μmol/L,respectively,which were lower than those of the control group.Glutathione peroxidase,superoxide dismutase of the observation group were (243.57 ± 33.64) μg/mL,(21.63 ± 3.78) μg/mL,respectively,which were higher than those of the control group,the differences were statistically significant(t =15.155,15.432,7.651,4.726,all P < 0.05).After treatment,Thl7 of the observation group was (1.03 ± 0.15) %,Thl7/Treg was (0.21 ± 0.03),which were lower than those of the control group,and Treg was higher than that of the control group,the differences were statistically significant (t =7.315,14.805,4.541,all P <0.05).The total effective rate of the observation group was 87.50%,which was significantly higher than 77.50% of the control group (x2 =2.385,P < 0.05).Conclusion Potassium sodium dehydroandroandrographolide succinate injection can contribute to the improvement of community acquired pneumonia in patients with peripheral blood T cell subpopulation structure,reduce the level of oxidative stress,improve the therapeutic effect,it is worthy of clinical application.

ABSTRACT

Objective To investigate the the curative effect of pidotimod combined with Yanhuning for children with refractory mycoplasma pneumonia(RMPP).Methods90 patients with RMPP from January 2013 to January 2015in shengzhou maternal and child health care hospital were divided into 3 groups, including Yanhuning group(A group), azithromycin combined with Yanhuning group(B group)and pidotimod combined with Yanhuning group(C group).And the total effective rate, the clinical symptoms, immune function parameters were analyzed.ResultsThe regulation of immune function in C group was more significant than that in A group and B group, the prognosis was better, and the repeated times and continuous time were decreased (P<0.05).ConclusionPidotimod combined with Yanhuning treatment of RMPP can regulate the immune function of children, improve clinical symptoms, it is worthy of popularization.

ABSTRACT

OBJECTIVE:To systematically evaluate therapeutic efficacy of Yanhuning injection in the treatment of epidemic mumps,explore valid way of evidence-based review,provide reference for the selection of new essential medicine list. METH-ODS:Retrieved from PubMed,CJFD,Wanfang database and VIP,randomized controlled trials(RCTs)about Yanhuning injection alone or combined with other positive drugs(trial group)vs. other positive drugs(control group)in the treatment of epidemic mumps. Meta-analysis was conducted by using Rev Man 5.3 statistical software after quality evaluation by modified Jadad scale. RE-SULTS:Totally 26 RCTs were included,involving 2613 patients. Results of Meta-analysis showed,overall response rate [OR=5.28,95%CI(3.92,7.11),P<0.001] of trial group was significantly higher than that of control group;defervescence time [MD=-1.97,95%CI(-2.83,-1.12),P<0.001],swelling disappearance time [MD=-2.05,95%CI(-2.53,-1.58),P<0.001] and pain disappearance time [MD=-1.39,95%CI(-2.02,-0.75),P<0.001] of trial group were significantly shorter than those of control group,there was statistical significance. CONCLUSIONS:Yanhuning injection has good curative effect for epidemic mumps,and can significantly shorten defervescence time,swelling disappearance time and pain disappearance time.

Objective To investigate the clinical effect of combined treatment of children with hand foot and mouth disease Yanhuning combined with interferon, psychological intervention. Methods 80 children with HFMD in our hospital from January 2015 to March 2017 were selected,and according to the different treatment and nursing intervention divided into observation group and control group,40 cases in each groups. control group with routine nursing intervention in the observation group, interferon, interferon ,psychological nursing intervention mode of Yanhuning; the two groups of children with mental state improvement of clinical symptom, clinical resolution the treatment time and total efficiency, and the relevant data for comparative analysis. Results The psychological nursing mode of interferon Yanhuning (observation group) the clinical effect is better than that of interferon plus routine nursing intervention in children with hand foot and mouth disease (control group) the clinical effect of intervention, the psychological state of patients better than the control group, the clinical symptoms disappeared time was shorter than the control group, the total efficiency of treatment was higher than the control group, there was statistical significance the difference (P<0.05). Conclusion Children with HFMD selection effect of interferon psychological nursing intervention model of Yanhuning significantly, can effectively improve the clinical symptoms in children with mental state, fades in a short time, it is worthy of widely used intervention in children with hfmd.

ABSTRACT

Objective To investigate the curative effect of pidotimod combined with Yanhuning for children with refractory mycoplasma pneumoniae pneumonia(RMPP).Methods 150 patients with RMPP in our hospital were divided into 3 groups,including Yanhuning group(A group),azithromycin combined with Yanhuning group(B group) and pidotimod combined with Yanhuning group(C group).And the total effective rate,the clinical symptoms,immune function parameters were analyzed.Results Compared to A group,the effective rate of B group and C group were sig-nificantly higher(all P <0.05),but there was no statistically significant difference between B group and C group. Compared to A group,the defervescence time,cough disappeared time and rales disappeared time were significantly shorter(all P <0.05),and the effect in C group was better than that in B group (all P <0.05).Compared to normal control group,CD +3 ,CD +4 ,CD +4 /CD +8 and CD +16 CD +56 in A group,B group and C group before the treatment were sig-nificantly lower(P <0.05),but IgG,IgM,CD +8 and CD +19 were significantly higher(all P <0.05).And CD +3 ,CD +4 , CD +4 /CD +8 and CD +16C D +56 in C group were significantly higher(all P <0.05)after the treatment.During follow -up, compared to A group,the recurrence rate and time in B group and C group were significant reduction(all P <0.05), and the recurrence rate and time in C group were less than that in B group (all P <0.05).So the regulation of immune function in C group was more significant than that in A group and B group,the prognosis was better,and the repeated times and continuous time were decreased.Conclusion Pidotimod combined with Yanhuning treatment of RMPP can regulate the immune function of children,improve clinical symptoms,which should be promoted.

OBJECTIVE:To systematically review the efficacy and safety of the aerosol inhalation of Yanhuning for injection in adjuvant treatment of children respiratory system infections,and provide evidence-based basis for the administration way of off-label drug use in clinic. METHODS:Retrieved from PubMed,Science direct,EMBase,CHKD,VIP and Wanfang Database, randomized controlled trials(RCT)or quasi-randomized controlled trials(qRCT)about the aerosol inhalation of Yanhuning for in-jection in adjuvant treatment of children respiratory system infections were collected. Meta-analysis was performed by using Rev Man 5.3 software after data extraction and quality evaluation. RESULTS:Totally 11 RCTs were enrolled,involving 982 patients. Results of Meta-analysis showed,the aerosol inhalation of Yanhuning for injection in adjuvant treatment of children respiratory sys-tem infections can significantly improve the clinical total effective rate [OR=4.47,95%CI(2.80,7.16),P<0.001],shorten breath shortness duration [MD=-2.05,95%CI(-2.86,-1.24),P<0.001],cough duration [MD=-1.73,95%CI(-2.34,-1.13),P<0.001] and pulmonary rales duration [MD=-2.13,95%CI(-3.38,-0.89),P<0.001],reduce the hospitalization days [MD=-2.38, 95%CI(-2.86,-1.89),P<0.001],and not increase the incidence of adverse reactions [OR=0.65,95%CI(0.21,2.00),P=0.45], nor shorten the fever duration[MD=-1.18,95%CI(-2.58,0.22),P=0.10]. CONCLUSIONS:The aerosol inhalation of Yanhun-ing for injection can improve the clinical efficacy in adjuvant treatment of children respiratory system infection,it helps to improve respiratory symptoms in children and does not increase the incidence of adverse reactions.

ABSTRACT

Objective To study the clinical efficacy and safety of Yanhuning injection in the treatment of bronchial pneumonia in children.Methods 102 cases of bronchial pneumonia were randomly divided into the observation group and the control group,51 cases in each group.The control group was given conventional treatment, the observation group was given Yanhuning injection on the basis of the treatment of control group.The clinical efficacy after treatment,clinical symptoms and signs disappeared time and adverse drug reaction were compared between the two groups.Results In the observation group after treatment,the cured,improvement,invalid cases were 27 cases, 23 cases,1 case,the total effective rate was 98.04%,which in the control group were 18 cases,25 cases,8 cases,the total effective rate was 84.31%.The total effective rate of the observation group was significantly higher than that of the control group (χ2 =5.97,P 0.05).Conclusion Yan-huning injection for the treatment of children with bronchial pneumonia can significantly improve the clinical curative effect,shorten the disappearance time of the clinical symptoms and signs,the curative effect is reliable,high security.

ABSTRACT

The interaction betweenYan-Hu-Ning (YHN) and bovine serum albumin (BSA) was investigated in order to provide further theoretical evidences on action mechanism study between YHN and proteins within the organism. Under optimal conditions, the interaction between YHN and BSA was studied by fluorescence quenching, ultraviolet absorption spectrometry and synchronous fluorescence spectroscopy. At the temperature of 283.15 K, 298.15 K and 313.15 K, quenching constant (KSV) and speed constant (Kq) were calculated by S-V curves. Static quenching constant (KLB) was obtained by L-B double reciprocal equation. Double logarithmic equation was used to calculate the binding constants (Kb) and the number of binding site (n). Thermodynamic equation was used to obtainΔH,ΔS,ΔG. Hill’s coefficients (nH) was obtained by Hill equation. The results showed that at three different temperatures, along with the increasing of YHN concentration, the fluorescence intensity of BSA decreased regularly. The value of KSV, Kq, KLB, Kb, n and nH decreased with the increasing of temperature;ΔG 1. It was concluded that YHN-BSA complex quenched the intrinsic fluorescence of BSA. The mechanism of fluorescence quenching was static quenching. The main binding forces were deduced as hydrogen bonds and Van der Waals forces from calculated values of thermodynamic parameters. YHN and BSA can form a binding site, which indicated certain binding interaction between YHN and BSA. YHN can be stored and transported by protein within the body. Free energy was produced to transformΔG into negative value. It showed that the process of binding between YHN and BSA was spontaneous. The nH was more than 1. It indicated that YHN had positive cooperative effect. The primary binding site was located at subdomainⅡA. The synchronous fluorescence spectra showed certain influence on the conformation of BSA by YHN. It led to the weakening of polarity within BSA and the binding site to be closer to the tyrosine.

ABSTRACT

Objective To compare the clinical effect and safety of children with acute upper respiratory tract infection treated with Yanhuning and ribavirin.Methods 182 cases with acute upper respiratory tract infection treated were selected and divided into observation group and control group randomly.All children were given general symptomatic and supportive treatment.Yanhuning was added in observe group,10mg · kg-1 · d-1,ivgtt,qd,and ribavirin was added in control group,10mg · kg-1 · d-1,ivgtt,qd.Changes and disappearance time of clinical symptoms were observed.Results 89 cases (97.80%) were treated effectively in observation group,while 73 cases (80.22%) in control group,the efficacy of Yanhuning was higher than that of ribavirin (P < 0.05).The time of clinical symptoms disappeared and blood examination recovering were shorter in observation group than that in control group (all P < 0.05).Scores of fever and cough in observation group were (2.90 ± 0.04) and (2.87 ± 0.06),while those in control group were (2.37 ± 0.11) and (2.26 ± 0.13).The scores in observation group were higher than those in control group (all P < 0.05).Conclusion Yanhuning applied in children with acute upper respiratory tract infection treatment is efficacy and safe,and it could adjust systemic immunity of children,which is worthy of clinical application.

ABSTRACT

Yanhuning is a semisynthetic preparation of andrographolide. The chemical name of yanhuning is monopotassium anhydroandro-grapholide disuccinate (DASK). The urine and feces samples containing DASK were extracted with chloroform. The chloroform solutions were spotted on silica gel plates (F254) and developed with chloro form-petroleum ether-ethanol-glacial acetic acid (6:3:0.5:0.5). After development the plate was placed on a Shimadzu dualwavelength TLC scanner CS-930. A zigzag scanning was then carried out for each spot developed at s=290nm andR=360nm under ultraviolet reflection. The DASK in each spot was tdus quantitated. The re suits obtained in present paper indicate that the method is sensitive, accurate and specific and it is applicable for quuntitative analysis of DASK in biological samples
