ABSTRACT

ObjectiveTo observe the effects of Youguiwan on bone metabolism and bone morphogenetic protein-2 (BMP-2)/Smad signaling pathway in ovaries-removed rats with osteoporosis and study the mechanism of Youguiwan in the prevention and treatment of osteoporosis. MethodA postmenopausal rat model of osteoporosis was prepared by bilateral ovariectomy. The 40 female SD rats were randomly divided into five groups, including sham operation group, model group, alendronate sodium group (0.1 mg·kg-1), and high-dose and low-dose (5.36 and 2.68 g·kg-1) groups of Youguiwan. The drug was given seven days after modeling, once a day for 12 weeks. After the treatment, the changes in femur tissue structure were observed by micro-CT, including bone mineral density (BMD), bone volume/total volume (BV/TV), trabecular number (Tb.N), trabecular thickness (Tb.Th), bone surface/bone volume (BS/BV), and trabecular separation (Tb.Sp). Ossification was observed by saffrane-solid green staining, and serum levels of bone metabolism markers, including bone alkaline phosphatase (BALP), osteocalcin (BGP), type Ⅰ procollagen amino terminal propeptide (PINP), and tartrate-resistant acid phosphatase 5b (TRACP-5b), were determined by enzyme-linked immunosorbent assay (ELISA). The protein and mRNA expression levels of Runx2, BMP-2, and Smad1 in rat femur were detected by Western blot and Real-time fluorescence quantitative polymerase chain reaction (Real-time PCR). ResultCompared with the sham operation group, bone trabecula in the model group was sparse. BMD, BV/TV, Tb.N, and Tb.Th were decreased (P<0.05, P<0.01). BS/BV (P<0.05) and Tb.Sp were increased. The content of BGP, BALP, PINP, and TRACP-5b in serum was significantly increased (P<0.01). The mRNA and protein expressions of Runx2, BMP-2, and Smad1 in rat femur were significantly decreased (P<0.05, P<0.01). Compared with the model group, the number of bone trabeculae in the high-dose and low-dose groups of Youguiwan was increased, and the bone microstructure was improved. BMD, BV/TV, Tb.N, and Tb.Th were increased significantly (P<0.05, P<0.01), and BS/BV and Tb.Sp were increased. The content of bone metabolic markers decreased (P<0.05, P<0.01). ConclusionYouguiwan has certain preventive and therapeutic effects on postmenopausal osteoporosis, and its mechanism may be related to promoting bone formation by regulating the BMP-2/Smad signaling pathway.

ABSTRACT

ObjectiveTo explore the therapeutic mechanism of Bushen Huoxue prescription from the perspective of bone metabolism by observing the clinical efficacy of this prescription in treating femoral head necrosis (ONFH, syndrome of liver and kidney deficiency) and its influences on bone metabolism indexes: N-terminal propeptide (PINP) and β-collagen degradation product (β-CTX). MethodSixty-six ONFH patients with the syndrome of liver and kidney deficiency in Zhengzhou Traditional Chinese Medicine Hospital of Orthopedics from December 2021 to September 2022 were selected. The patients were randomized into an experimental group and a control group by the parallel control method, with 33 patients in each group. The experimental group received Bushen Huoxue prescription orally, while the control group received Xianlinggubao Capsules orally, with a treatment cycle of 6 months. The visual analogue scale (VAS) score, Harris score, Association Research Circulation Osseous (ARCO) staging, imaging changes, quantitative scores of TCM symptoms, and serum levels of PINP and β-CTX were determined before and after treatment. The occurrence of adverse events and reactions was recorded. ResultThe total response rate in the experimental group was 83.87% (26/31), which was higher than that (68.75%, 22/32) in the control group (Z=-2.096, P<0.05). After treatment, the single and total scores of TCM symptoms, VAS score, and β-CTX level decreased in the two groups (P<0.05). Moreover, the decreases in the scores of hip pain, lower limb mobility, soreness of waist and knees, and lower limb flaccidity, total score of TCM symptoms, VAS score, and β-CTX level in the experimental were larger than those in the control group (P<0.05). After treatment, the imaging results showed no significant improvement in the two groups. The Harris score and PINP level in both groups increased after treatment (P<0.05), and the increases were more obvious in the experimental group than in the control group (P<0.05). No serious adverse event or adverse reaction appeared during the observation period. ConclusionBushen Huoxue prescription can relieve pain and TCM symptoms and improve the hip joint function in treating ONFH patients with the syndrome of liver and kidney deficiency. It can inhibit the development of ONFH, increase PINP, and decrease β-CTX. No obvious side effect appears during the clinical observation period, which shows that Bushen Huoxue prescription has good safety.

ABSTRACT

ObjectiveTo explore the effect of Youguiwan on the rats with adriamycin-induced nephrotic syndrome (NS) and its mechanism. MethodSD rats were randomly divided into a normal group, a model group, three Youguiwan low, medium, and high-dose groups, and a prednisone group. Rats in the model group were intravenously injected with adriamycin in the tail vein to induce the NS model. Rats in the Youguiwan low, medium, and high-dose groups were given 2.8, 5.6, 11.2 g·kg-1·d-1 of crude drugs, respectively, and rats in the prednisone group were given 6.3 mg·kg-1·d-1 of prednisone acetate. Each administration group was given continuous medicine for 6 weeks, and the normal group and model group were given an equal volume of normal saline. Bicinchoninic acid (BCA) assay was used to detect 24 h urine protein (24 h UP). Automatic biochemical analyzer was used to detect serum urea nitrogen (BUN), creatinine (SCr), albumin (ALB), total cholesterol (TC), and triglyceride (TG) levels. Hematoxylin-eosin (HE) staining was used to observe renal tissue morphology, and kit was used to detect serum advanced oxidized protein products (AOPPs) and reactive oxygen species (ROS). Western blot was used to detect the receptor of advanced glycation endproducts (RAGE) of renal tissue, nuclear factor-κB (NF-κB) phosphorylation levels, Wnt, and β-catenin protein expression. ResultAs compared with the normal group, 24 h UP, serum BUN, SCr, TC, TG, AOPPs, and ROS levels in the model group increased significantly (P<0.01), whereas ALB decreased (P<0.01). There were typical pathological injuries in the renal tissue, and the expressions of RAGE, phosphorylation(p)-NF-κB, Wnt1, and β-catenin protein were significantly increased (P<0.01). As compared with the model group, the 24 h UP, serum BUN, SCr, TC, TG, AOPPs, and ROS levels of rats in the Youguiwan low, medium, and high-dose groups significantly reduced (P<0.01), and ALB significantly increased (P<0.01). The renal tissue damage was reduced, and the expressions of RAGE, p-NF-κB, Wnt1, and β-catenin protein were significantly decreased (P<0.01) in a dose-dependent manner. ConclusionYouguiwan improves the kidney injury of rats with adriamycin-induced NS. The mechanism may be related to the reduction of AOPPs level, inhibition of RAGE/ROS/NF-κB axis, and activation of Wnt/β-catenin signal.

ABSTRACT

ObjectiveTo study the effects of Chinese herbal compound Youguiwan on angiogenesis of rats with ovarian dysfunction caused by natural aging and its relationship with chemokine interleukin 8 (CXCL8)/CXC chemokine receptor 1/2 (CXCR1/2) signaling pathway, angiopoietin 1 (Ang-1), and angiopoietin 2 (Ang-2), so as to explore its mechanism in improving the ovarian function. MethodFifty six female SD rats were randomly divided into the young control group (n=8) and modeling group (n=48, ovarian dysfunction caused by natural aging). Rats in both the young control and modeling groups were routinely fed, during which the ones in the modeling group underwent exfoliative cytology of vaginal smears for five to seven days. The ones presented with prolonged estrous cycle, followed by continuous estrus and repeated pseudopregnancy revealed by vaginal cytology during four consecutive estrous cycles indicated early aging, and the young rats with keratinocyte proliferation index higher than 50% for 10 consecutive days were classified into the young control group. The successfully modeled rats were randomly divided into the early-aged group, estrogen (65 μg·kg-1·d-1) group, Zuoguiwan (33 g·kg-1·d-1) group, as well as the low-, medium-, and high-dose (1.2, 2.4, 4.8 g·kg-1·d-1) Youguiwan groups. Rats in the young control group and the early-aged group were gavaged with the same volume of normal saline for 30 days. After the experiment, the morphological changes in rat ovary were observed by hematoxylin-eosin (HE) staining. The protein expression levels of chemokines CXCL8, CXCR1, CXCR2, Ang-1, and Ang-2 in rat ovary were detected by Western blotting and immunohistochemistry, and the mRNA expression levels of CXCL8, CXCR1, CXCR2, Ang-1, and Ang-2 by real-time fluorescence quantitative polymerase chain reaction (Real-time PCR). ResultCompared with the young control group, the early-aged group exhibited reduced number of growing follicles, corpus luteum, and blood vessels at all levels, elevated atretic follicles (P<0.01), up-regulated protein and mRNA expression of CXCL8, CXCR1, and CXCR2 in the ovarian tissue (P<0.01), and down-regulated Ang-1 and Ang-2 protein and mRNA expression (P<0.05). Compared with the early-aged group, each medication remarkably increased the number of growing follicles, corpus luteum, and blood vessels (P<0.05), lowered the number of atretic follicles (P<0.05), down-regulated the protein and mRNA expression levels of CXCL8, CXCR1, and CXCR2 in the ovarian tissue (P<0.05), and up-regulated the protein and mRNA expression levels of Ang-1 and Ang-2 (P<0.05). ConclusionYouguiwan down-regulates the levels of CXCL8, CXCR1, and CXCR2 in rat ovary and up-regulates the levels of Ang-1 and Ang-2 to promote ovarian angiogenesis and improve rat ovarian function.

ABSTRACT

<b>Objective::To observe the effect of Youguiwan on the levels of cartilage transducers and activators of transcription 3 (STAT3) and interleukin-6 (IL-6) in rats with knee osteoarthritis (KOA). <b>Method::Sixty SD rats were randomly divided into six groups: sham control group, model group, glucosamine sulfate group and Youguiwan (high, middle and low-dose) groups. The modified Hulth method was used to prepare KOA models for 6 weeks. The shame control group and the model group were treated with normal saline, Youguiwan high, middle, low-dose groups received Youguiwan 4.8, 2.4, 1.2 g·kg-1 by gavage respectively, and the glucosamine sulfate group was treated with glucosamine sulfate 0.17 g·kg-1. The rats were administrated for 8 weeks according to the dose. After intervention, each group was put to death by femoral artery blood collection, and the keen cartilages of the rats were collected. The pathological changes were observed by htoxylin eosin (HE) staining method, and Mankin score was evaluated. The expressions of STAT3, superoxide dismutase3(SOD3) and Wnt inhibitory factor 1(WIF1) in articular cartilage were detected by immunohistochemistry. The expressions of IL-6 mRNA in articular cartilage were detected by quantitative real-time fluorescence polymerase chain reaction (Real-time PCR). The expression of WIF1 in articular cartilage was detected by Western blot. <b>Result::Compared with the sham control group, the Makin score was obviously increased in the model group, the protein expression of STAT3 was increased significantly, the mRNA of IL-6 was raised significantly, but the protein expression of WIF1 was decreased significantly (P<0.01), articular cartilage was seriously damaged, and chondrocytes were arranged in disorder. Compared with the model group, the Makin score was declined obviously in the high-dose Youguiwan group, the protein expression of STAT3 was significantly reduced, the mRNA expression of IL-6 was significantly reduced in Youguiwan treatment group, while the protein expression of WIF1 was significantly increased (P<0.05, P<0.01), the cartilage structure returned to be normal, the chondrocytes distribution was uneven, and articular cartilage surface was not smooth. <b>Conclusion::Youguiwan could significantly improve the articular cartilage degeneration of KOA rats, and inhibit the inflammation of chondrocytes, which may be related to the suppression of STAT3 and IL-6 expression.
