ABSTRACT

A questionnaire survey was conducted on the actual state of manufacturing homemade pill preparations. Of the 14 facilities that responded, there were 4 pharmacies producing pills of only one type and 4 pharmacies producing more than 11 types. There were 7 facilities for handmade, manufacturing, 5 facilities for machinery manufacturing, and 2 facilities for handmade and/or machinery. The manufacturing process consists of about 9 steps, and it is possible to do it in 2-3 hours if considering just the time to make the pills, but it requires several days or more including preparation, machine cleaning, maintenance, etc. Based on the above, it seems that the current technical fees for pill manufacturing are extremely low.

ABSTRACT

Objective:This study aimed to identify the characteristics of daily rehabilitation in patients with spinocerebellar degeneration (SCD), including those with multiple system atrophy, with a focus on physical status and social background.Methods:We analyzed questionnaire responses from 914 patients in Japan and conducted inter-group comparisons of backgrounds and rehabilitation-related information by dividing patients into a rehabilitation group, a self-directed exercise group, and a non-rehabilitation group.Results:Among all subjects, the rehabilitation group accounted for 67.9%, the self-directed exercise group for 17.7%, and the non-rehabilitation group for 14.3%.In the rehabilitation group, the percentage receiving services increased with a decline in the level of independent living and increase in coverage by long-term care insurance. The frequency and duration of rehabilitation showed a maximum distribution of 2-3 sessions/week for 40 minutes per session in the rehabilitation group and 7 sessions/week for 20 minutes per session in the self-directed exercise group. The intent to continue in the rehabilitation group was significantly greater than in the self-directed exercise group (p=0.018), although subjective changes with rehabilitation showed no difference between groups.Conclusion:The percentage of SCD patients who participated in rehabilitation programs including self-directed exercise was high. However, the subjective effect was not clear. We consider it necessary to develop an environment in which experts can provide rehabilitation to all SCD patients, irrespective of the disease stage.

ABSTRACT

Students in their 5th and 6th year of University of Tsukuba, School of Medicine, enter into a clinical clerkship. They record medical charts and flow sheets and they present at clinical rounds.<BR>1) From 2000 to 2005, a questionnaire survey was conducted to evaluate the clinical clerkship in the rheumatology division. We also evaluated the influence of the introduction of infliximab therapy (October 2003) on the students' experience.<BR>2) Evaluation of the clinical clerkship by the students was improved by attending the infliximab ward. Students felt more involved in the treatment of the patients by preparing drip infusion sets, disssolving infliximab, assisting with the drawing of blood and drip infusions, checking of the vital signs every 30 minutes, and recording charts.<BR>3) Students realized the improvement of rheumatoid arthritis with therapy and the changes in the patients' emotional status after the introduction of infliximab.
