ABSTRACT

Resumen El objetivo de esta investigación fue analizar el papel que los factores de personalidad pertenecientes a los Cinco Grandes y la Tríada Oscura -maquiavelismo, narcisismo y psicopatía- tienen en la conducción agresiva. La muestra se compuso de 318 estudiantes universitarios con permiso de conducir, quienes contestaron a una batería de pruebas que evaluaba los factores de personalidad de los Cinco Grandes (TIPI), la Tríada Oscura (DD) y la conducción agresiva (DAS y DAX). Los análisis de regresión jerárquica controlando las variables de edad y sexo, respaldan la utilidad predictiva de los factores de personalidad de los Cinco Grandes y la Tríada Oscura. Los resultados mostraron cómo la afabilidad, la estabilidad emocional y la apertura a la experiencia predicen de manera significativa diversos aspectos de la conducción agresiva. El maquiavelismo y el narcisismo son predictores significativos de diversas formas de expresión de la ira en la conducción, una vez controlados los efectos de los Cinco Grandes.


Abstract The aim of this paper was to analyze the role that personality traits belonging to the Big Five and the Dark Triad (Machiavellianism, narcissism, and psychopathy) have on aggressive driving. 318 undergraduate students with a driver's license completed a battery of tests assessing Big Five personality traits (TIPI), Dark Triad (DD) and aggressive driving (DAS and DAX). Hierarchical regressions analyses controlling for respondents' age and gender variables supported the predictive utility of the Big Five personality traits and the Dark Triad. Results showed that agreeableness, emotional stability and openness to experience predict various aspects of aggressive driving. Machiavellianism and narcissism are significant predictors of various forms of anger expression in driving after controlling for the effects of the Big Five personality traits.

ABSTRACT

gressive violent behavior and apprehension anxiety are two common mental health issues that sports persons often experience. These issues can have negative effects on the sports persons' performance, as well as their overall well-being. This paper aims to explore the roots of these mental health issues and their relationship with sports participation. There are various factors that contribute to aggressive violent behavior among sports persons. One major factor is the intense competition that is inherent in sports. Sports persons often feel pressure to win, which can lead to frustration and aggression. Additionally, some athletes may have a predisposition towards aggression due to genetics or past experiences. Apprehension anxiety is another issue that many sports persons face. This anxiety can be caused by a variety of factors, including fear of failure, fear of injury, or fear of public speaking. Sports persons may also experience anxiety due to the high expectations placed on them by coaches, fans, and teammates. Furthermore, social and cultural factors can contribute to these issues among sports persons. The culture of sports may glorify aggression, which can lead sports persons to believe that aggression is necessary to succeed. Additionally, social pressures can lead sports persons to suppress their emotions, leading to increased levels of anxiety and aggression. It is important for sports organizations and coaches to recognize the causes of these mental health issues and to implement strategies to prevent and address them. By addressing these issues, sports organizations can help sports persons to perform at their best while also maintaining their mental well-being.

ABSTRACT Aggressive periodontitis (AP) is the most serious entity of periodontal disease (stage III/IV, grade C periodontitis according to the latest classification, 2017). Aim: to enhance knowledge of periodontal microbiota in AP in native Argentine patients and describe the effect of a combined pharmacological-mechanicalperiodontal treatment on clinical and microbiological parameters. Materials andMethod: The study analyzed 42 periodontal sites in 11 patients diagnosed with AP. Clinical periodontal parameters were recorded at baseline, 45, 90 and 180 days. Microbiological samples were taken before treatment and at 180 days. PCR was used to determine presence of the periodontopathic bacteria Aggregatibacter actinomycetemcomitans (Aa), Porphyromonas gingivalis (Pg), Tannerella forsythia (Tf), Treponema denticola (Td), Prevotella intermedia (Pi) and Fusobacterium nucleatum (Fn). Patients underwent periodontal therapy including antibiotics (Amoxicillin 500mg + Metronidazole 250mg; 8hs/7 days), and were reevaluated at 45, 90 and 180 days. Results: Mean age was 28.4 ± 7.9 years. The initial PCR detected the following frequencies: Aa 14.3%, Pi 61.9%, Pg 71.4%, Tf 81.0%, Fn 95.2% and Td 97.6%. Baseline microbiological samples revealed significantly higher prevalence of Pg over Aa (p=0.012). Clinical parameters improved significantly after treatment (73.8% PS<5 mm; PS, NIC, SS p<0.001). At 180 days, a significant decrease in microbiological detection rates was observed (Fn, Td, Tf, Pi, Aa p<0.05). Aa was no longer detectable while Pg did not decrease significantly (p=0.052). Fn was the only study species detected in 100% (n=11:42) of residual pockets (PS>5 mm) (p=0.053). Conclusion: In the initial samples, there was significant prevalence of Pg over Aa. Significant clinical improvement was achieved after the mechanical-pharmacological treatment, with undetectable levels ofAa, while Fn persisted in residual pockets, and Pg was present at most of the treated sites.


RESUMEN La periodontitis agresiva (PA) es la entidad más grave de la enfermedad periodontal (clasificación 2017: periodontitis estadio III/IV, grado C). Objetivo: mejorar el conocimiento sobre la microbiota periodontal de la PA en sujetos nativos argentinos y describir el efecto de un tratamiento mecánico-farmacológico periodontal sobre los parámetros clínicos y microbiológicos. Materiales y Método: se estudiaron 42 sitios periodontales correspondientes a 11 pacientes con PA. Los parámetros clínicos se registraron a 0, 45, 90 y 180 días. Las tomas microbiológicas se realizaron antes de iniciar el tratamiento y a los 180 días. La determinación de especies periodontopáticas (Aggregatibacter actinomycetemcomitans (Aa), Porphyromonas gingivalis (Pg), Tannerella forsythia (Tf, Treponema denticola (Td), Prevotella intermedia (Pi) y Fusobacterium nucleatum (Fn)) se realizó por PCR. Los pacientes iniciaron terapia básica periodontal junto con antibioticoterapia (Amoxicilina 500 mg + Metronidazol 250 mg; 8 hs/7 días) y fueron evaluados a los 45, 90 y 180 días. Resultados: la edad media fue 28,4 ± 7,9 años. Las detecciones iniciales fueron: Aa 14,3%, Pi 61,9%, Pg 71,4%, Tf 81,0%, Fn 95,2% y Td 97,6%. En las muestras iniciales la prevalencia de Pg sobre Aa fue significativamente superior (p=0,012). Los pacientes tuvieron una respuesta clínica favorable al tratamiento (73,8% PS<5 mm; PS, NIC, SS p<0,001). A 180 días, se observó una disminución estadísticamente significativa en la detección microbiana (Fn, Td, Tf, Pi, Aa p<0,05). En igual plazo, Aa no fue detectado, mientras que Pg mostró una disminución no significativa (p=0,052). Fn fue el único detectado en el 100% (n=11:42) de las bolsas periodontales residuales (PS>5 mm) (p=0,053). Conclusión: Las muestras iniciales evidenciaron prevalencia significativa de Pg sobre Aa. El tratamiento logró una significativa mejora clínica con niveles indetectables de Aa. La persistencia de Fn en las bolsas residuales y de Pg en la mayoría de los sitios tratados, caracterizaron la muestra poblacional estudiada.

Introduction: Periodontitis is an inflammatory disease that affects the supporting tissues of teeth, the effects of excess of nitric oxide, may contribute to the symptoms of periodontitis. Objective: To determine the serum nitric oxide concentration in generalized chronic and aggressive periodontitis patients and to compare it with a healthy subject group from the Mexican population. Materials and methods: A case and control study was performed. Sixty-nine individuals were recruited from the Clínica de Posgrado de Periodoncia of the Centro Universitario de Ciencias de la Salud, Universidad de Guadalajara, México. Patients with clinical features of generalized chronic periodontitis (GCP group, n=19), generalized aggressive periodontitis (GAP group, n=11), and a group of healthy subjects (HS group, n=39) were included in the study. Informed consent was obtained from each subject, and serum nitric oxide concentration was measured by an enzyme-linked immunosorbent assay. Results: Nitric oxide concentration in the study groups was greater in the GCP group (462.57 ± 16.57 µmol/L) than in the GAP group (433.84 ± 18.61 µmol/L) and the HS group (422.46 ± 12.07 µmol/L). A comparison using Student's t-test (one-tailed) between healthy subjects and generalized chronic periodontitis showed borderline significance (p<0.04), whereas no significant differences were observed in HS and GAP groups, with a p-value of 0.64, and the GAP vs. GCP p-value was 0.33. Conclusion: The serum nitric oxide concentration observed in the present study suggests that nitric oxide plays a major role in the inflammatory process, which cannot necessarily be linked to the severity of the disease and periodontal tissue destruction.


Introducción. La periodontitis es una enfermedad inflamatoria que afecta los tejidos de soporte dental; los efectos del exceso de óxido nítrico pueden contribuir a los síntomas de la periodontitis. Objetivo. Determinar la concentración de óxido nítrico en el suero de los pacientes con periodontitis agresiva y crónica generalizada, y compararla con la de individuos sanos de población mexicana. Materiales y métodos. Se trata de un estudio de casos y controles. Se incluyeron 69 individuos de la Clínica de Posgrado de Periodoncia del Centro Universitario de Ciencias de la Salud de la Universidad de Guadalajara. Se dividieron en tres grupos: pacientes con periodontitis crónica generalizada (GCP, n=19), pacientes con periodontitis agresiva generalizada (GAP, n=11) e individuos sanos periodontalmente (HS, n=39). Se obtuvo el consentimiento informado de todos los participantes. Se utililizó la prueba ELISA para medir la concentración de óxido nítrico en suero. Resultados. Las concentraciones de óxido nítrico observadas fueron mayores en el grupo GCP (462,57 ± 16,57 µmol/L) que en los grupos GAP (433,84 ± 18,61 µmol/L) y HS (422,46 ± 12,07 µmol/L). La comparación entre HS y GCP mediante la prueba estadística t de Student (una cola), mostró diferencias significativas (p<0,04), y no se observaron diferencias entre los grupos HS y GAP (p=0,64), ni entre GAP y GCP (p=0,33). Conclusiones. La concentración de óxido nítrico en suero, observada en el presente estudio, sugiere que el óxido nítrico desempeña un importante papel en el proceso inflamatorio, lo que no necesariamente está ligado a la gravedad de la enfermedad ni a la destrucción del tejido periodontal.

OBJECTIVE@#To explore and summarize the clinical characteristics and treatment of aggressive NK-cell leukemia (ANKL), and provide new insights for clinical diagnosis and treatment of this disease.@*METHODS@#The clinical data of 7 patients with ANKL admitted to the First Affiliated Hospital of Wannan Medical College from March 2014 to July 2021 were retrospectively analyzed, and their clinical characteristics, laboratory and imaging results, treatment and outcomes were analyzed.@*RESULTS@#Among the 7 patients, 5 were males and 2 were females, with a median age of 47 (33-69) years old. The morphology of bone marrow cells in 7 patients showed similar large granular lymphocytes. Immunophenotyping revealed abnormal NK cells in 5 cases. By the end of follow-up, 6 cases died and 1 case survived, with a median survival time of 76.9 (4-347) days.@*CONCLUSION@#ANKL is a rare disease with short course and poor prognosis. If combined with hemophagocytic syndrome (HPS), the prognosis is even worse. There is no unified treatment method at present, and the use of PD-1 inhibitors may prolong the survival in some patients.

ABSTRACT

Objective:To summarize the clinical features, treatments and prognoses of aggressive natural killer cell leukemia (ANKL) in children.Methods:Clinical data and follow-up results were retrospectively reviewed for one hospitalized case of ANKL in June 2019.Through a literature search, the relevant items were retrieved from the databases of China National Knowledge Infrastructure, WanFang and PubMed using the Chinese and English keywords of "aggressive natural killer cell leukemia" and "children" up to December 2021.Results:This 8-year-old girl was diagnosed with ANKL by flow cytometric immunophenotype and immunohistochemical stain.Fever was the initial manifestation accompanied by sallow complexion, fatigue, enlargement of liver, spleen and lymph node and hematopenia of three lines.Allogeneic hematopoietic stem cell transplantation (allo-HSCT) was performed after chemotherapy.As of April 2022, the child stayed in a disease-free survival state after follow-ups for over 2 years.The literature search finally yielded 7 eligible Chinese and 10 English reports with a total of 17 pediatric ANKLs.In this group, there were fever (n=15), rash (n=1), perineal mass (n=1) and diarrhea, vomiting and other digestive tract symptoms (n=1). Six cases were misdiagnosed during an early stage of disease.4 cases received chemotherapy alone, 3 cases received chemotherapy regimen for acute lymphoblastic leukemia, 1 child died and one death occurred after received chemotherapy regimen of "cisplatin + vincristine + doxorubicin + ifosfamide". Allo-HSCT was performed in 5 patients after remission with chemotherapy and one child died from multiple organ failure at 9 months after allo-HSCT.Nine cases gave up treatment.Conclusions:ANKL has a rapid disease progression, diverse clinical manifestations, easy misdiagnosis and poor prognosis.For suspected ANKL cases, clinicians perform multiple bone perforations at multiple sites and immunophenotype by flow cytometry as soon as possible to confirm the diagnosis.Currently allo-HSCT offers a long-term survival of ANKL patients.

Abstract Periodontitis Stage III-IV, Grade C (PerioC) is a severe form of Periodontitis. The individual genetic background has been shown to be an important etiopathogenic factor for the development of this disease in young, systemically healthy, and non-smokers patients. Recently, after exome sequencing of families with a history of the disease, PerioC was associated with three single nucleotide variations (SNVs) - rs142548867 (EEFSEC), rs574301770 (ZNF136), and rs72821893 (KRT25) - which were classified as deleterious or possibly harmful by prediction algorithms. Objective Seeking to validate these findings in a cohort evaluation, this study aims to characterize the allele and genotypic frequency of the SNVs rs142548867, rs574301770, and rs72821893 in the Brazilian population with PerioC and who were periodontally healthy (PH). Methodology Thus, epithelial oral cells from 200 PerioC and 196 PH patients were harvested at three distinct centers at the Brazilian Southern region, their DNA were extracted, and the SNVs rs142548867, rs574301770, rs72821893 were genotyped using 5′-nuclease allelic discrimination assay. Differences in allele and genotype frequencies were analyzed using Fisher's Exact Test. Only the SNV rs142548867 (C > T) was associated with PerioC. Results The CT genotype was detected more frequently in patients with PerioC when compared with PH subjects (6% and 0.5% respectively), being significantly associated with PerioC (odds ratio 11.76, p=0.02). Conclusion rs142548867 represents a potential risk for the occurrence of this disease in the Brazilian population.

Describir las situaciones de violencia por parte de los pacientes percibidas por el personal de enfermería en una institución de salud pública de Corrientes capital durante 2021. Metodología: Estudio observacional, descriptivo, transversal. Muestreo de tipo consecutivo. La población personal de enfermería asistencial de un hospital. Variables: edad, género, instrucción, antigüedad laboral, área de trabajo, turno, identifcación de maltrato físico, verbal y asedio sexual, principal modo de actuar e impacto frente a situaciones de violencia. Recolección de datos mediante cuestionario Google Forms con consentimiento informado anónimo, que fue validado por prueba piloto. Análisis con Microsof Excel y Epidat 4.1. Resultados: Población estudiada 115 trabajadores de enfermería; 69,57% mujeres; rango etario 22-56 años, promedio 35,5 años. Formación: 58,26% enfermeros, 22,61% licenciados en enfermería y 19,13% auxiliares en enfermería. Turno: 37,39% mañana, 30,43% tarde, 17,39% noche y 14,78% rotativo. Área laboral: cuidados intensivos 27,83%, clínica médica 21,74%, clínica quirúrgica 15,65%, emergencias 15,65%, consultorios 10,43%, coronaria 8,70%. De los entrevistados, 89,56% había percibido algún tipo de violencia, de estos 61,17% estaba entre 22 y 38 años. La percepción de violencia mostró que 92,53% eran enfermeros, 88,46% licenciados y 81,81% auxiliares de enfermería. Los tipos de violencia identificados fueron maltrato verbal en el 86,09%, maltrato físico en el 72,17% y asedio sexual en el 24,35%. El principal género afectado por los hechos violentos fue el femenino y el turno de trabajo con mayor identificación de situaciones violentas fue el de la mañana. En cuanto a los modos de proceder para comunicar a la institución las situaciones vividas, el 66,96% lo reportaron a un superior Los hechos de violencia según área de trabajo se reportaron por 80% de los de unidad coronaria, 84,38% de unidad de cuidados intensivos, 88,89% de emergencias, 92% de clínica médica, 94,44% de clínica quirúrgica y 100% de consultorios externos. Conclusión: La violencia hacia enfermería está presente en el hospital y afecta más a las mujeres. La forma más frecuente es la verbal, seguida de la física y del asedio sexual. En general frente a las agresiones optan por no hacer nada[AU]

Work area: intensive care 27.83%, medical clinic 21.74%, surgical clinic 15.65%, emergencies 15.65%, doctor's ofces 10.43%, coronary 8.70%. Of those interviewed, 89.56% had perceived some type of violence, of these 61.17% were between 22 and 38 years old. Te perception of violence showed that 92.53% were nurses, 88.46% licensed and 81.81% nursing assistants. Te types of violence identifed were verbal abuse in 86.09%, physical abuse in 72.17% and sexual harassment in 24.35%. Te main gender affected by violent acts was female, and the work shif with the highest identifcation of violent situations was the morning shif. Regarding the ways of proceeding to communicate the situations experienced to the institution, 66.96% reported it to a superior Te acts of violence according to work area were reported by 80% of those in the coronary unit, 84.38% of intensive care unit, 88.89% emergencies, 92% medical clinic, 94.44% surgical clinic and 100% outpatient clinics. ABSTRACT

Ameloblastoma is a slow-growing and locally invasive epithelial odontogenic tumor of the jaw that runs a benign course in most cases. Granular cell ameloblastoma is a rare subtype of ameloblastoma, showing the granular transformation of its cytoplasm. It is considered as an aggressive variant of ameloblastoma. Herein, we report the case of a 34-year-old male patient who presented with a swelling in the left cheek for a 1-year duration with a recent rapid increase in size. Examination showed a 4 � 3 cm growth involving the left buccal mucosa and retromolar area. Imaging studies showed expansile lytic lesion mandible. Biopsy revealed neoplasm with odontogenic epithelial islands showing peripheral palisading of tall columnar cells with reversal of polarity and the center of the islands showing stellate reticulum-like cells which were markedly replaced by granular cells. Granular cells can appear in various odontogenic and non-odontogenic tumors. When there is extensive granular cell change in ameloblastoma, it should be differentiated from other oral lesions with granular cells including granular cell odontogenic tumor, granular cell tumor, and congenital epulis.

Background: The role of dermoscopy in distinguishing the histopathological subtypes of basal cell carcinoma (BCC) is not fully elucidated. Aims: To determine the accuracy of dermoscopy in diagnosing different BCC subtypes. Methods: The dermoscopic features of 102 histopathologically verified BCCs were studied retrospectively. The tumours were classified as superficial (n=33,32.3%), nodular (n=46,45.1%) and aggressive (n=23,22.6%) BCCs by histopathology. Statistical analysis included Cohen’s kappa test, proportion of correlation, measures of diagnostic accuracy, diagnostic odds ratio and the credibility ratio of positive (LR+) and negative (LR?) tests. Results: The highest value in all performed tests was seen in superficial BCCs (kappa 0.85; proportion of correlation 93%; diagnostic accuracy 93.1%), good correlation was noted in nodular BCCs (kappa 0.62, proportion of correlation 80%; diagnostic accuracy 80.4%) but dermoscopic correlation with histopathology was low for aggressive BCCs (kappa 0.13; proportion of correlation 79%; diagnostic accuracy 78.4%). Short, fine telangiectasias (83.3%) showed the greatest importance for the diagnosis of superficial BCCs, blue-grey ovoid nests (61.8%) had the highest diagnostic accuracy in nodular BCCs, while arborising vessels (79.4%) was the most significant dermoscopic feature for the diagnosis of aggressive BCCs. Limitations: This was a retrospective analysis and included only Caucasian patients from a single centre. Conclusion: The highest agreement of dermoscopic features with the histologic type was found in superficial BCCs. We did not find any specific dermoscopic structure that could indicate a diagnosis of aggressive BCC. The presence of relevant dermoscopic features in the evaluated cases was determined by the depth of tumour invasion and not by its histology

O Cartoon Test é um instrumento que analisa a percepção de crianças e adolescentes acerca do significado de bullying por meio de cenas de interação entre pares no ambiente escolar. Este estudo qualitativo exploratório constitui a segunda etapa de validação do instrumento no Brasil, com os objetivos de verificar se os participantes compreendem as imagens e legendas dos cartoons e que palavras são utilizadas para descrever os comportamentos inadequados. Foram realizados oito grupos focais, com 52 alunos, com idades de oito e 14 anos, de duas cidades do Rio Grande do Sul. Os instrumentos utilizados foram: Questionário sociodemográfico, Folha de registro do pesquisador e Cartoon Test. Realizou-se análise de conteúdo para tratamento dos dados. Os resultados indicaram que o instrumento foi compreendido pelos participantes e o termo bullying foi o mais utilizado para descrever situações agressivas. Para as crianças, sugere-se a redução no número de cartoons, visando evitar a exaustão.(AU)


The Cartoon Test is an instrument that analyzes the perception of children and adolescents about the meaning of bullying through scenes of interaction between peers in the school environment. This qualitative exploratory study is the second phase of validation of the instrument in Brazil. The aim is to verify whether the participants understand both the images and captions of the cartoons and to record the words used to describe the inappropriate behaviors. Eight focus groups were carried out, with 52 students, aged between eight and 14 years, from two cities in Rio Grande do Sul. The instruments used were: sociodemographic questionnaire, record sheet and Cartoon Test. Content analysis was performed. The results showed that the instrument was understood by all the participants and bullying was the word most used to describe the aggressive situations. A reduction in the number of cartoons administered to children is suggested, in order to avoid exhaustion.(AU)


El Cartoon Test es un instrumento que analiza la percepción de niños y adolescentes a respeto del significado de bullying por medio de una escena de interacción entre pares en el ambiente escolar. Este estudio cualitativo exploratorio constituye la segunda etapa de validación del instrumento en Brasil, con el objetivo de verificar que los participantes comprenden las imágenes y leyendas de los cartoons y qué palabras son utilizadas para describir comportamientos inapropiados. Fueron realizados ocho grupos focales, con 52 alumnos, con edades entre ocho y 14 años, de dos ciudades de Rio Grande do Sul. Los instrumentos utilizados fueron: Cuestionario Sociodemográfico, Ficha de Registro del Investigador y Cartoon Test. Se llevo a cabo un análisis de contenido para procesar los datos. Los resultados indicaron que el instrumento fue comprendido por los participantes y el termo bullying fue lo más empleado para describir situaciones agresivas. ABSTRACT

Chondroid syringomas are similar to mixed parotid tumors and present in both benign and malignant forms. Malignant chondroid syringoma (MCS) is a rare skin tumor that has a predilection for extremities, particularly in young women. It is even rarer to present as a scalp tumor with very few reported cases in the literature. We present a middle-aged woman, with a history of increasing fatigability of her right arm for the past 3 months who, on examination, was found to have scalp swelling and matted right posterior triangle lymph nodes. The working diagnosis on her was a large sebaceous cyst with secondary in the neck from an occult primary/non-Hodgkin抯 lymphoma. Preliminary fine-needle aspiration was inconclusive. Imaging followed by wide excision of the tumor and the nearby occipital node was done. The final histopathological diagnosis was MCS with secondary in the lymph node. These tumors are aggressive and metastasize early. Radical surgery is the only hope of cure as adjuvant treatment is yet to be standardized

Background: Periodontal diseases are characterized by the presence of bleeding, inflammation, sensitivity, discomfort, mobility and tooth loss. Plenty of studies supported the assumption that vitamin D deficiency might be a risk factor for periodontal diseases. Our study aimed to investigate the true association between serum vitamin D level and the presence of periodontal disease. Materials and methods: The study evaluated 40 patients (age group 19-35 years) attending the outpatient department, Department of Periodontics and Implantology, Govt. Dental College and Hospital, Srinagar, equally divided into two groups (Group I and Group II) according to the inclusion criteria. Group I included generalized aggressive periodontitis patients while Group II included non periodontitis patients. Clinical parameters and serum 25-hydroxyvitamin D level were assessed. Results: Serum vitamin D levels of periodontitis patients compared to non-periodontitis patients presented non statistically significant differences (p-value = 0.878). Conclusion: Although the serum vitamin D level in aggressive periodontitis patients was lower than the healthy controls but the difference seemed to be statistically insignificant and hence it can be concluded that the serum vitamin D deficiency is not a risk factor for periodontitis.

Background and Aim: Anaplastic lymphoma kinase (ALK)-positive large B-cell lymphomas (ALK+-LBCLs) are aggressive CD20-negative lymphomas, accounting for <1% of diffuse LBCLs. Being rare and with peculiar immunophenotypic characteristics, these can be easily misdiagnosed. We present 11 cases of ALK+-LBCLs diagnosed over a period of 11 years at a tertiary care hospital in South India to analyze the clinical, morphological, and immunophenotypic profile of these tumors. Subjects and Methods: ALK+-LBCL cases diagnosed from September 2009 to August 2020 were included. Clinical details were obtained from stored electronic records and summarized. Available hematoxylin and eosin (H and E) stained slides and immunohistochemistry slides were reviewed and observations tabulated. Results: Eleven patients (nine males and two females) were diagnosed with ALK+-LBCLs in the study period with seven presenting primarily with extranodal disease manifestations. Tumors in the lymph nodes showed diffuse architecture effacement and variable sinusoidal invasion. All tumors showed immunoblastic and plasmablastic-type large lymphoid cells with scattered anaplastic/multinucleate large cells, including rare Reed–Sternberg-like cells. Cytoplasmic granular ALK-1 staining, CD20 negativity, and immunohistochemical features of plasmablastic differentiation were noted in all. Of eight patients treated, only one achieved remission with multi-agent chemotherapy but relapsed after 6 months. Two patients died of disease and five others had progressive/persistent disease and were lost to follow-up. Conclusion: Although rare, these tumors should always be in the differential diagnoses of tumors with plasmablastic and immunoblastic morphology, especially in extranodal sites to avoid diagnostic delay/misdiagnosis.

ABSTRACT

Introduction: An aggressive variant of central giant cell granuloma in a paediatric patient is a localized benign osteolytic prolif- eration. The lesion consist of fibrous tissue with haemorrhage, hemosiderin deposits and presence of osteoclast-like giant cells with reactive bone formation. Clinically shown with rapid growth, resorption of roots, displacement of teeth and thinning or perforation of cortical bone, and marked recurrence. Case presentation: A 9-year-old boy reported with a 3 days history of asymptomatic soft tissue swelling in left mandibular pos- terior region which was diagnosed as central giant cell granuloma based on clinical, radiological and histopathological features. Management and Prognosis: Surgical resection was performed without any signs of recurrence on 9 months follow up. Conclusion: Prompt diagnosis and treatment of Central giant cell granuloma can significantly improve morbidity and long- term outcomes.

Ameloblastoma is one of the most common benign tumours of odontogenic origin which developed from epithelial cells and its elements and dental tissues in their various phases of development. Patients with Giant Ameloblastomas are very rare but they are widely found and diagnosed in developing countries because of painless growth and patient’s fear of Surgery leading to delayed treatment. This paper presents a case of large Ameloblastoma of left side of mandible in a 25-year-old female patient which was successfully resected in toto. Patient refused for free fibula flap re-construction of the defect because of her apprehension for donor site morbidity

ABSTRACT

The revised Olweus Bully/Victim Questionnaire (rBVQ) is an internationally used instrument to identify bullying, but due to Brazilian students' low proficiency in reading and writing, it had to be adapted for audio-guided use through smartphones. We investigated construct validity, using Confirmatory Factor Analysis (CFA), under a two-correlated factor solution. Participated 1,742 fifth graders and 2,316 seventh graders from 30 public schools in the city of São Paulo. We found excellent fit indices models for both grades (fifth grade: CFI=0.985, RMSEA=0.020; seventh grade: CFI=0.990, RMSEA=0.015), as well as factor loading higher than 0.4 for all items, indicating their relevance to the construct. The rBVQ demonstrated excellent psychometric properties and may help future studies that aim to investigate bullying in populations with similar profiles. In addition, this study innovates by evaluating an audio-guided instrument and using mobile technology. (AU)


O Questionário revisado de bullying de Olweus (rBVQ) é um instrumento internacionalmente utilizado para identificar bullying, mas devido às dificuldades de estudantes brasileiros na leitura e escrita, foi adaptado para uso áudio-guiado em smartphones. Investigou-se a validade do construto por meio de análise confirmatória fatorial em uma solução de dois fatores. Participaram 1742 e 2316 alunos de quintos e sétimos anos de trinta escolas públicas de São Paulo. Encontrou-se excelentes índices de adequação para ambos os anos escolares (quintos: CFI = 0,985; RMSEA = 0,020; sétimos: CFI = 0,990; RMSEA = 0,015) e cargas fatoriais maiores que 0,4 para todos os itens, indicando sua relevância para o construto. O rBVQ demonstrou excelentes propriedades psicométricas e pode ajudar estudos futuros que objetivam investigar bullying em populações com perfis semelhantes. Se investigó la validez del constructo mediante el Análisis Factorial Confirmatorio, bajo un modelo de dos factores. Participaron 1742 y 2316 estudiantes de quinto y séptimo curso de 30 escuelas públicas de São Paulo. Excelentes tasas de adecuación fueron encontradas para ambos casos (quinto año: CFI=0,985, RMSEA=0,020; séptimo año: CFI=0,990, RMSEA=0,015), además de cargas factoriales superiores a 0,4 para todos los ítems, lo que indica su relevancia para el constructo. El instrumento rBVQ ha presentado excelentes propiedades psicométricas y puede ayudar a futuros estudios pretendan investigar el bullying en poblaciones con perfiles similares. Introducción. Los tumores desmoides son lesiones de los tejidos blandos, histológicamente benignas, poco frecuentes y con gran agresividad local y carencia de potencial metastásico. Se relacionan estrechamente con antecedentes traumáticos o quirúrgicos, como la cesárea, y su tratamiento generalmente es quirúrgico. Métodos. Presentamos una serie de tres pacientes intervenidas en nuestro centro durante el año 2020. Se revisan sus antecedentes y se describe su tratamiento. Resultados. En todas nuestras pacientes se encontró algún antecedente quirúrgico, dos cesáreas y una resección de un disgerminoma. El tratamiento empleado fue la resección quirúrgica con márgenes libres y reparación del defecto mediante malla. Conclusiones. El tumor desmoide es una patología poco frecuente, su diagnóstico se realiza mediante exámenes imagenológicos y se confirma con el estudio histológico; es importante hacer el diagnóstico diferencial con el sarcoma. La cirugía radical sigue siendo el tratamiento de elección, aunque algunos autores proponen el tratamiento conservador.


Introduction. Desmoid tumors are soft tissue lesions, histologically benign, rare and with great local aggressiveness and lack of metastatic potential. They are closely related to traumatic or surgical history such as caesarean section. Their treatment is generally surgical. Methods. We present a case series of three patients operated on in our center during the year 2020. Their history is reviewed and their type of treatment is presented. Results. In all our patients, surgical history was found (two caesarean sections and one resection of a dysgerminoma). The treatment used was surgical resection with free margins and mesh repair of the defect. Conclusions. Desmoid tumor is a rare pathology; its diagnosis is made by imaging studies, and confirmed by histology. It is important to make a differential diagnosis with sarcoma. Radical surgery remains the treatment of choice, although some authors propose conservative treatment.

Objective:To explore the ultrasonographic features of desmoid fibromatosis and provide evidence for clinical management.Methods:The ultrasonic images and clinical data of 22 cases of desmoid fibromatosis confirmed by pathology in the Union Hospital Affiliated to Tongji Medical College of Huazhong University of Science and Technology from 2016 to 2021 were retrospectively analyzed. The location, size, shape, boundary, internal echo, posterior acoustic enhancement, relationship with surrounding tissues and blood flow of the lesion were evaluated.Results:The maximum diameter of lesions in 22 patients ranged from 0.8 cm to 11.3 cm (5.2±2.4)cm. (1) There were 17 cases of extra-abdominal shape: 10 cases of irregular shape, 4 cases of spindle shape, and 3 cases of oval shape. In 10 cases, the lesion boundary was not clear, and the lesion was infiltrated along the muscle, fascia or adipose tissue. Uneven hypoechogenicity was found in 11 cases, and strip or patchy strong echo was found in 4 cases. 5 cases with posterior echogenicity enhancement; color Doppler flow imaging (CDFI) blood flow classification: grade 0 in 2 cases, grade 1 in 7 cases, grade 2 in 7 cases, and grade 3 in 1 case. (2) Abdominal wall type in 5 cases: oval shape in 2 cases, spindle shape in 2 cases, irregular shape in 1 case; In 3 cases, the boundary was not clear and was infiltrative along the muscle. Inhomogeneous hypoecho was found in 4 cases. Posterior echogenicity was enhanced in 1 case. CDFI blood flow classification: grade 1 in 2 cases, grade 2 in 2 cases, grade 3 in 1 case.Conclusions:The sonograms of typical desmoid fibromatosis have certain features. Combined with the patient′s history and clinical manifestations, they can provide an important basis for the clinical management of the disease.
