ABSTRACT

Bayesian network Meta-analysis was employed to compare the efficacy of different oral Chinese patent medicines in treating type 2 diabetes mellitus with angina pectoris of coronary heart disease. Randomized controlled trial(RCT) of oral Chinese patent medicines in treating type 2 diabetes mellitus complicated with angina pectoris of coronary heart disease were retrieved from 8 Chinese and English databases including CNKI, Wanfang, VIP, SinoMed, EMbase, PubMed, Cochrane Library, and Web of Science with the time interval from inception to November 2022. The BUGSnet package in R 4.2.1 was used to conduct Meta-analysis. A total of 45 RCTs were included, involving 4 727 patients and 7 oral Chinese patent medicines. Network Meta-analysis showed that the conventio-nal western medicine combined with Chinese patent medicines improved the outcome indicators. Shexiang Baoxin Pills + conventional western medicine had the best effect on reducing the incidence of adverse cardiovascular events, and Yixinshu Capsules + conventional western medicine on reducing the frequency and duration of angina pectoris. The conventional western medicine combined with oral Chinese patent medicines can reduce blood glucose indicators. Yindan Xinnaotong Soft Capsules + conventional western medicine had the best effect on reducing fasting blood glucose(FBG), 2 hours postprandial blood glucose(PBG), and glycosylated hemoglobin(HbA1c). The conventional western medicine combined with oral Chinese patent medicines can reduce blood lipid indicators. Yixinshu Capsules + conventional western medicine had the best effect on reducing total cholesterol(TC) and low density lipoprotein-cholesterol(LDL-C), and Yindan Xinnaotong Soft Capsules + conventional western medicine on reducing triglyceride(TG). Current evidence suggests that the patients with type 2 diabetes mellitus complicated with angina pectoris of coronary heart disease could reasonably choose oral Chinese patent medicines on the basis of routine antiplatelet, anticoagulant, hypoglycemic, and antihypertensive therapies, which could reduce the incidence of adverse cardiovascular events, alleviate the symptoms of angina pectoris, and reduce the glucose and lipid metabolism indicators. Shexiang Baoxin Pills + conventional treatment and Yixinshu Capsules + conventional western medicine have better effect on angina pectoris, Yindan Xinnaotong Soft Capsules + conventional western medicine on lowering blood glucose, and Yindan Xinnaotong Soft Capsules + conventional western medicine and Yixinshu Capsules + conventional western medicine on reducing blood lipid. Due to the lack of direct comparative results between Chinese patent medicines and other factors, high-quality studies remain to be carried out for further verification.

ABSTRACT

OBJECTIVE@#To compare the therapeutic effect on type 2 diabetes mellitus (T2DM) complicated with angina pectoris of coronary heart disease between the combined therapy of acupuncture and western medication and the simple administration of western medication.@*METHODS@#A total of 134 patients with T2DM and angina pectoris of coronary heart disease were randomly divided into two groups, i.e. an acupuncture plus medication group (67 cases, 3 cases dropped off) and a medication group (67 cases, 4 cases dropped off). The routine western medication was used according to symptoms in the patients of both groups. In the acupuncture plus medication group, on the base of medication, acupuncture was applied to Jianshi (PC 5), Quchi (LI 11), Neiguan (PC 6), etc. The needles were retained for 20 min in each treatment and 3 treatments of acupuncture were required weekly. The treatment was given consecutively for 8 weeks in the two groups. Separately, before and after treatment, the symptom scores of TCM were observed and the indexes were detected, including glycolipid metabolism [fasting plasma glucose (FPG), 2-h plasma glucose (2hPG), glucosylated hemoglobin (HbA1c), triacylglycerol (TG) and total cholesterol (TC)], islet β cell function [homeostasis model assessment-β (HOMA-β), homeostasis model assessment-IR (HOMA-IR), fasting insulin (FINS) and insulin sensitivity index (ISI)], cardiac function indexes [cardiac output (CO), early diastolic peak velocity/late diastolic peak velocity (E/A), left ventricular end diastolic diameter (LVEDD) and left ventricular ejection fraction (LVEF)], as well as electrocardiogram QT dispersion (QTd). Besides, the clinical therapeutic effects were compared between the two groups.@*RESULTS@#After treatment, the TCM symptom scores and the values of FPG, 2hPG, HbA1c, TG, TC, HOMA-IR, FINS, E/A and LVEDD as well as QTd were all lower than those before treatment in the two groups (@*CONCLUSION@#The combined therapy of acupuncture and medication is effective in treatment of T2DM complicated with angina pectoris of coronary heart disease. Such therapy effectively improves glucolipid metabolism, islet β cell function, cardiac function and myocardial blood supply. Its curative effect is better than the simple administration of western medicine.

ABSTRACT

OBJECTIVE:To e valuate the clinical efficacy and economics of Sodium tanshinone Ⅱ A sulfonate injection combined with conventional medication versus conventional medication in the treatment of angina pectoris of coronary heart disease. METHODS :Using“Sodium tanshinone Ⅱ A sulfonate”“Danshentong”“angina pectoris ”as Chinese key words and “Sodium tanshinone Ⅱ A sulfonate”“Danshentong”“Angina pectoris ”as English keywords ,the studies were retrieved from Wanfang database ,CNKI,CBM,Cochrane Library ,Medline,Embase,ISI Web of Science and BIOSIS Previews during the inception to Apr. 2019. After literature screening and data extraction ,the included real world cohort studies were evaluated with bias risk tool of Cochrane systematic evaluator manual 5.2.0. Meta-analysis was conducted by using Stata 15.0 software,and publication bias of results was analyzed . The cost-effectiveness analysis was used for pharmacoeconomic evaluate ,and single factor sensitivity analysis and probability sensitivity analysis were carried out for the results of pharmacoeconomic evaluation. RESULTS : A total of 29 literatures were included ,involving 31 studies and 2 857 patients. Meta-analysis showed that clinical effective rate [RR =1.23,95%CI(1.18,1.28),P<0.001],ECG effective rate [RR =1.29,95%CI(1.20,1.39),P<0.001] and angina pectoris effective rate [RR =1.22,95%CI(1.15,1.29),P<0.001] of trial group were significantly higher than those of control group. The adverse reactions of the two groups were mild. The above results were likely to be biased in publication. Cost-effectiveness analysis showed that ICER was 72.02 of 2 groups. The sensitivity analysis showed that above results were stable. CONCLUSIONS : For patients with angina pectoris of coronary heart disease ,therapeutic efficacy of Sodium tanshinone ⅡA sulfonate injection combined with conventional medication is better that of conventional medication ,and the cost is also slightly higher. When the willingness to pay is higher than 7 202 yuan,the combination scheme has the advantage of cost-effectiveness.

ABSTRACT

Objective To study and observe the influence of atorvastatin for the CXCL chemokines and vascular endothelial damage indexes of patients with angina pectoris of coronary heart disease.Methods 86 patients with angina pectoris of coronary heart disease in our hospital from February 2016 to July 2017 were selected as the study objects, and all the patients were divided into the control group and the observation group with 43 cases in each group according to the random number table.The control group were treated with simvastatin on the basis of routine treatment, and the observation group were treated with atorvastatin on the basis of routine treatment.Then the treatment effect of angina pectoris, adverse reaction rates, CXCL chemokines and vascular endothelial damage indexes before and after the treatment of two groups were analyzed and compared.Results The total effective rate of observation group with angina pectoris was significantly higher than that of control group, so the difference had statistical significance (P<0.05), the adverse reaction rates of two groups were compared, so the difference had no statistical significance (P>0.05), the CXCL chemokines and vascular endothelial damage indexes of two groups before the treatment were compared, and the differenceshad no statistical significance (P>0.05), the serum PGI2 of observation group after the treatment were lower than those of control group, the serum CXCL chemokines and other vascular endothelial damage indexes were lower than those of control group, and the differences had statistical significance (P<0.05). Conclusions The treatment effect of atorvastatin in the patients with angina pectoris of coronary heart disease is better, and its safety is higher, and it has a better improvement role for the CXCL chemokines and vascular endothelial damage indexes, so it has a higher application value in the patients with angina pectoris of coronary heart disease.

ABSTRACT

OBJECTIVE:To compare the cost of Xiangdan injection and Danshen chuanxiongqin injection in the treatment of unstable angina pectoris of coronary heart disease,and to provide reference for rational and economic drug selection. METHODS:42 inpatients with unstable angina pectoris of coronary heart disease were divided into Xiangdan injection group(group A,20 cas-es) and Danshen chuanxiongqin injection group (group B,22 cases). Two groups were given conventional symptomatic treatment according to the specific clinical situation;on this basis,group A was given Xiangdan injection 20 ml intravenously,qd;group B was given Danshen chuanxiongqin injection 10 ml intravenously,qd,for 14 d. Clinical efficacy and the occurrence of ADR were compared between 2 groups,and cost-minimization analysis was conducted. RESULTS:After treatment,total effective rate of group A and B was 95.00% and 95.45%,without statistical significance(P>0.05);there was also no statistical significance in the incidence of ADR (P>0.05). The costs of 2 groups were equal in addition to the cost of drugs. According to cost-minimization analysis,the drug cost of group A was 77.28 yuan and total cost was 705.88 yuan;the drug cost of group B was 1 310.40 yuan and total cost was 1 939.00 yuan;the drug cost and total cost of group A were far more less than that of group B,with statistical significance(P<0.05). The results of sensitivity analysis were consistent with it. CONCLUSIONS:The cost of Xiangdan injection is lower and more economical than that of Danshen chuanxiongqin injection in the treatment of unstable angina pectoris of coronary heart disease.

@#Objective To observe the therapeutic effect of multidisciplinary rehabilitation therapy on patients with angina pectoris of coronary heart disease.Methods 86 patients with angina pectoris of coronary heart disease were randomly divided into the rehabilitation group(55 cases)and control group(31 cases).The patients in the rehabilitation group received routine drugs and multidisciplinary rehabilitation(psychotherapy,diet guiding,kinesitherapy,post discharged guiding etc).The patients in the control group received routine drugs for 10~14 days,activities after chest pain dispearance and natural life after discharge.The follow up period was 6 months,recording the changes of cardiac event rate,body mass index,blood lipid(glycerol,cholesterol,low-density lipoprotein).Results The rehabilitation group was significantly superior to that of control group in symptom remission velocity and remission degree(P<0.05).Cardiac event rate of the rehabilitation group was lower than that of the control group significantly within follow up period(P<0.01);body mass index and blood lipid were improve in the two groups,but the rehabilitation group was significantly superior to that of the control group(P<0.05).Conclusion The multidisciplinary rehabilitation therapy can improve clinical symptoms of patients with angina pectoris of coronary heart disease and reduce cardiac event.

ABSTRACT

Objective To carry on statistical analysis of tradtional mongolian medicine syndrome in angina pectoris of coronary heart disease, find out the significative indices for syndrome diagnosis, and establish the diagnostic criteria of tradtional mongolian medicine syndrome in angina pectoris of coronary heart disease. Methods Cases of angina pectoris of coronary heart disease were selected according to modern medical diagnostic criteria. Clinical observations were carried on and clinical information was filled in the forms of the clinical cases. Each case was differentiated and made corresponding diagnosis of syndrome according to tradtional mongolian medicine. Clinical cases were grouped according to syndrome of tradtional mongolian medicine and decided the significative indices by statistical analysis. Results Four hundred and ten cases were differentiated which contain tradtional mongolian medicine information about angina pectoris of coronary heart disease, including 358 cases of Chu Si syndrome, 23 cases of Nian Xie syndrome, and 29 cases of Wei Sha syndrome. Through statistical analysis of the various syndromes and the objective indices, diagnostic criteria of tradtional mongolian medicine syndrome in angina pectoris of coronary heart disease was established. Conclusion Diagnostic criteria of tradtional mongolian medicine syndrome in angina pectoris of coronary heart disease was summarized by combination of disease and syndrome, and provides a theoretical basis for the resonable application of mongolian herb.

ABSTRACT

AIM: To evaluate the therapeutic effect and safety of Rhadiola Extract Injection for treatment of stable angina pectoris of coronary heart disease with cariac blood stasis syndrome. METHODS: Arandomized,double-blind,positive drug parallel controlled,multi-center clinical trial was adopted.414 patients with stable angna pectoris of coronary heart disease with cariac blood stasis syndrome were randomly chosen and divided into two groups: test group(n=308 cases) and control group(n=106 cases).The test group was treated with Rhadiola Extract Injection and the control group received Xiangdan Injection.Treatment course of each group was 10 days.(RESULTS:) The therapeutic effect and changes of electrocardiogram in the test group were better than that of the control group(P0.05).The test group had no obvious side-effects. CONCLUSION: Rhadiola Extract Injection is safe and effective in treating stable angina pectoris of coronary heart disease with cariac blood stasis syndrome.
