ABSTRACT

PURPOSE: We compared the ocular aberration and clinical outcome between different aspheric intraocular lenses (IOL) in both eyes. METHODS: This prospective randomized controlled study was comprised of patients with bilateral cataract who received two different aspheric IOLs implanted in both eyes: negatively aspheric Tecnis® ZCB00 and spherically neutral Akreos® MI60. Total and corneal aberrations computed by Wavescan® and Pentacam® were assessed at 6 months to investigate the effects of the IOL's spherical aberration on the eye and to analyze the incidence and degree of posterior capsule opacification. By using spherical aberration of the cornea and the IOLs, values calculated via Ray-tracing software and Wavescan® were compared. Total spherical aberration was analyzed by the MATLAB program and converting the pupil size to 6.0, 4.5, 3.0 mm. RESULTS: A total of 25 patients were included. Regarding pre-operative corneal aberration, ZCB00 group was 0.232 ± 0.119 µm while MI60 group was 0.240 ± 0.117 µm, and there was no difference between the two IOLs. At 6 months after total ocular spherical aberration, MI60 group (pupil size 6.0 mm; 0.296 ± 0.097 µm, 4.5 mm; 0.094 ± 0.032 µm, 3.0 mm; 0.019 ± 0.006 µm) had more positive values than ZCB00 group (pupil size 6.0 mm; 0.051 ± 0.105 µm, 4.5 mm; 0.009 ± 0.034 µm, 3.0 mm; 0.002 ± 0.007 µm) (p < 0.001). When calculated using the ray tracing method, based on the results after surgery, MI60 group's total spherical aberrations were higher than ZCB00 group. However, from 1 month to 6 months after surgery, the uncorrected distance visual acuity, spherical equivalent and posterior capsule opacification showed no differences between the two IOLs. CONCLUSIONS: In eyes with aspheric IOLs with negative spherical aberration, spherical aberration was lower than spherically neutral aspheric IOLs. Regarding postoperative visual acuity, spherical equivalent and posterior capsule opacification, there were no significant differences between the two groups.

ABSTRACT

?AIM: To evaluate the refractive predictability achieved with an aspheric intraocular lens ( IOL ) and to develop a preliminary optimized algorithm for the calculation of its power ( PIOL ) .?METHODS:This study included 65 eyes implanted with the aspheric IOL LENTIS L-313 ( Oculentis GmbH ) that were divided into 2 groups:12 eyes (8 patients) with PIOL≥23. 0 D (group A), and 53 eyes (35 patients) with PIOL<23. 0 D ( group B ). The refractive predictability was evaluated at 3mo postoperatively. An adjusted IOL power ( PIOLadj ) was calculated considering a variable refractive index for corneal power estimation, the refractive outcome obtained, and an adjusted effective lens position ( ELPadj ) according to age and anatomical factors.?RESULTS: Postoperative spherical equivalent ranged from -0. 75 to +0. 75 D and from -1. 38 to +0. 75 D in groups A and B, respectively. No statistically significant differences were found in groups A (P=0. 64) and B (P=0. 82 ) between PIOLadj and the IOL power implanted ( PIOLReal ) . The Bland and Altman analysis showed ranges of agreement between PIOLadj and PIOLReal of +1. 11 to -0. 96 D and +1. 14 to -1. 18 D in groups A and B, respectively. Clinically and statistically significant differences were found between PIOLadj and PIOL obtained with Hoffer Q and Holladay I formulas (P<0. 01).?CONCLUSION: The refractive predictability of cataract surgery with implantation of an aspheric IOL can be optimized using paraxial optics combined with linear algorithms to minimize the error associated to the estimation of corneal power and ELP.

ABSTRACT

Abstract?AlM:To discuss the feasibility of using the visual quality analyzer KR - 1W to guide the relatively personalized aspheric intraocular lens ( lOL ) implants to make the whole eye spherical aberration close to 0. 1μm.?METHODS: ln this prospective case series study, the corneal spherical aberration with 6mm aperture of 73 patients (100 eyes) was measured with KR-1W Visual Function Analyzer 1d before surgery. For the sake of the whole postoperative spherical aberration were close to 0. 1μm, 9 cases ( 16 eyes ) with corneal spherical aberration 0. 35μm were implanted Tecnis ZA9003 lOL, named Tecnis group. Aspherical lOL was implanted after phacoemulsification through a cornea 2. 75mm incision without suture. Uncorrected visual acuity, beat corrected visual acuity, spherical aberration of the whole eye and jnternal optics (mainly lOL) at 6mm pupil diameter were examined at 3mo postoperatively. The relevant data were analyzed using t-test and variance analysis.?RESULTS: The whole ocular spherical aberration at 6mm pupil diameter in all postoperative were 0. 084 ± 0. 032μm;in Tecnis group, the data were 0. 091 ± 0. 021μm;in AO group, the data were 0. 0814-0. 013μm;lQ group were0. 093 ± 0. 042μm. There was no significantly different between the predicted value and actual value of ocular spherical aberration at 6 mm pupil diameter in all postoperative ( t = 1. 932, P = 0. 061 ) and in the three groups. The difference value in the predicted values of the preoperative spherical aberrations of the whole eye and the actual values after surgery was 0. 013±0. 041μm; there was no statistically significant difference ( F=2. 537, P=0. 091 ) . Respectively compared the uncorrected visual acuity and besta corrected visual acuity among three groups of postoperative, no significant difference were found (F=0. 897, P=0. 421;F=1. 423, P=0. 097).?CONCLUSlON: Personality selection of aspheric lOL based on preoperative corneal spherical aberration of patients is feasible and produces satisfactory target postoperative total spherical aberration.

ABSTRACT

PURPOSE: To evaluate preoperative and postoperative spherical aberrations after cataract surgery based on selecting spherical or aspheric intraocular lens (IOL) according to preoperative corneal aberration. METHODS: The medical records of patients who underwent phacoemulsification and IOL implantation in the posterior chamber by a surgeon (H.J.C) were reviewed (68 patients, 97 eyes). IOL was selected based on preoperative corneal spherical aberration measured by corneal topography (ATLAS 9000, Carl Zeiss). The target postoperative total ocular spherical aberration was set to zero (0) and one of the following lenses was chosen: Acrysof SA60AT (n = 25), Acrysof IQ (n = 36) or Tecnis(R) ZCB00 (n = 36). The Wavescan aberrometer and the corneal topography were obtained postoperatively. Absolute prediction errors of postoperative total ocular spherical aberration were analyzed. RESULTS: Preoperative corneal spherical aberration was 0.241 microm; total postoperative ocular spherical aberration was 0.0509 microm (Acrysof SA60AT: 0.0954 microm, Tecnis(R) ZCB00: 0.0374 microm, Acrysof IQ: 0.0335 microm). Postoperative corneal spherical aberration was 0.232 microm, which was not significantly different from the preoperative value (p = 0.199). Postoperative ocular spherical aberration was 0.051 microm; 0.095 microm (Acrysof SA60AT), 0.034 microm (Acrysof IQ), and 0.037 microm (ZCB00). The reducing amounts of spherical aberration were 0.185 microm (Acrysof IQ) and 0.311 microm (ZCB00). The overall absolute prediction error was 0.068 microm. The absolute prediction error of the Acrysof SA60AT group was 0.092 microm, Tecnis(R) ZCB00 group was 0.067 microm and Acrysof IQ group was 0.054 microm. There was no significant difference among the 3 groups (p = 0.089). CONCLUSIONS: Aspheric IOLs can efficiently reduce total ocular spherical aberrations according to preoperative corneal spherical aberrations.

ABSTRACT

PURPOSE: To evaluate the stability and optical performance of the newly developed single-piece aspheric intraocular lens (IOL) by comparing the clinical outcome of the aspheric IOL with the new optic profile design (HOYA iSert, HOYA iMics) and the aspheric IOL (Akreos MI60), which has been proven effective and safe. METHODS: iSert, iMics, and MI60 were inserted into 55 eyes, 60 eyes, and 50 eyes, respectively, after microincision phacoemulsification cataract surgery. Best corrected visual acuity (BCVA), refraction in spherical equivalent, anterior chamber depth (ACD), total higher order aberration (HOA), contrast sensitivity, and surgically induced astigmatism (SIA) were measured and each IOL was evaluated on the functional stability, anterior-posterior stability, centration in the capsular bag, and quality of vision. RESULTS: No statistical differences in preoperative and postoperative BCVA among the 3 IOL groups were observed, however, MI60 showed significant myopic shift postoperatively. Anterior-posterior stability assessed with postoperative change in refractive error and ACD was slightly lower in the MI60 group. In terms of vision quality, while total aberration, total HOA, coma aberration, and contrast sensitivity for the 3 IOLs were not different significantly, spherical aberration of the MI60 group was higher than the other groups at 6 months postoperative. SIA was significantly increased in eyes implanted with iSert than in eyes with iMics or MI60 at 1 month postoperatively, however, the differences were no longer evident after 3 months postoperatively. CONCLUSIONS: The new aspheric IOLs, iSert and iMics, showed good stability and visual outcome equal to MI60 at the 6-month postoperative follow-up.

ABSTRACT

PURPOSE: To compare the six-month clinical outcomes of Tek-Lens II model 872 (Tekia, Inc.) and Akreos MI-60 (Bausch & Lomb, Inc.) hydrophilic aspheric intraocular lenses (IOLs) implanted in-the-bag. METHODS: After phacoemulsification was performed by a single surgeon (JY Kim), two different hydrophilic aspheric IOLs were implanted: the Tek-Lens II IOL in 57 eyes and the MI60 IOL in 49 eyes. The best corrected visual acuity (BCVA), refractive error (RE), total high-order aberration (HOA), anterior chamber depth (ACD), and contrast sensitivity (CS) were measured preoperatively and one, three, and six months postoperatively. All parameters were analyzed using the independent t-test to compare the two IOL groups. RESULTS: There was no significant difference in the BCVA, RE, or HOA between the two groups at any of the follow-up visits. Three months postoperatively, the ACD of the TEK-Lens II group as measured by corneal topography was significantly shallower than that of the MI60 group (3.67 +/- 0.52 mm vs. 4.10 +/- 0.40 mm; p = 0.008), and most of the photopic CS in the Tek-Lens II group was significantly higher than that in the MI60 group (all p < 0.05). CONCLUSIONS: The Tek-Lens II IOL showed comparable clinical outcomes with a proven hydrophilic aspheric IOL for the postoperative six months.

ABSTRACT

PURPOSE: To evaluate the influence of pupil size on uncorrected visual acuity and spherical aberration of the TECNIS 1-piece intraocular lens (IOL). METHODS: In this study 61 eyes were implanted with a TECNIS 1-piece IOL. Pupil size was checked by standard pupil card with cobalt blue light postoperatively. Best corrected visual acuity (BCVA) and uncorrected visual acuity (UCVA) were assessed postoperatively. Total spherical aberration and high-order aberration for mesopic pupil size zone were measured. RESULTS: There were no significant differences of spherical equivalent, age, preoperative astigmatism, BCVA between two groups, but small pupil group revealed better UCVA (p = 0.02) and lower ocular aberration than large pupil group. CONCLUSIONS: After cataract surgery, pupil size greater than 5.0 mm increases total aberration. Thus, visual acuity with small pupil after aspheric TECNIS 1-piece IOL implantation show better clinical result on uncorrected visual acuity.

ABSTRACT

PURPOSE: To evaluate the postoperative outcome of Tecnis(R) ZCB00 1-Piece acrylic intraocular lens (IOL), the new single piece, aspheric IOL until 12 months. METHODS: Sixty eyes undergone cataract surgery using two different IOLs; The Tecnis(R) ZCB00 1-Piece Acrylic IOL (30 eyes) and the Tecnis(R) ZA9003 3-Piece Acrylic IOL (30 eyes). Cataract surgery was performed in the same period. All complicated cases were excluded. Best corrected visual acuity (BCVA), refractive error, photopic and mesopic contrast sensitivity, total high order aberration, spherical aberration and anterior chamber depth were measured preoperatively and at 1, 6 and 12 months after surgery. RESULTS: There were no statistically significant differences between two groups in BCVA, refractive error, total high order aberration and spherical aberration all the time after surgery. Both groups have negative ocular spherical aberration until 12 months after surgery. 1-piece and 3-piece groups had different tendency in axial movement. But there were no statistically significant differences in anterior chamber depth measured at 1 week, 1, 6 and 12 months after surgery. There was no statistically significant difference in contrast sensitivity at 1, 6 and 12 months after surgery. CONCLUSIONS: The Tecnis(R) ZCB00 1-Piece acrylic intraocular lens (IOL) showed comparable clinical outcomes to the Tecnis(R) ZA9003 3-Piece acrylic intraocular lens (IOL) until 12 months after cataract surgery. 1-piece and 3-piece IOL group had different tendency in axial movement. However, refraction remained stable during 12 months after surgery. Mostly, both IOL groups had negative ocular spherical aberration during whole follow-up period.

ABSTRACT

PURPOSE: To compare clinical outcomes between different IOL sizes after microincisional cataract surgery. METHODS: Retrospective chart review was done in 68 eyes of 68 patients who underwent phacoemulsification and implantation with two different-sized aspheric IOLs (AKREOS MI60 Bausch & Lomb, Inc., Rochester, NY). The patients were divided into 2 groups: group I consisted of 38 eyes between 15.5-22.0 diopter (D) (optic size 6.0 mm, overall size 10.7 mm), and group II consisted of 30 eyes between 22.5-30.0 D (optic size 5.6 mm, overall size 10.5 mm). The best corrected visual acuity (BCVA), refractive error, and anterior chamber depth (ACD) were measured preoperatively and postoperatively and were compared between groups. RESULTS: Postoperative 6 month BCVA was 0.08 +/- 0.10 D in group I and 0.07 +/- 0.11 D in group II. During the same period, the spherical equivalent was -0.32 +/- 0.65 D in group I and -0.16 +/- 0.59 D in group II (p > 0.05). There were no significant differences in ACD or refractive error during the postoperative period (p > 0.05). CONCLUSIONS: Comparison of postoperative visual acuity and spherical equivalent between different sizes of Akreos MI-60(TM) IOLs showed no differences.

ABSTRACT

PURPOSE:To compare the visual function among 3 aspheric intraocular lenses (IOLs) after cataract surgery. METHODS: A prospective analysis among 70 eyes of 68 patients who had undergone phacoemulsification and implantation of aspheric IOLs (Acrysof IQ, 30 eyes; Tecnis ZA9003, 20 eyes; Akreos Adapt-AO, 20 eyes) was performed. The best corrected visual acuity (BCVA), refraction, coma, trefoil, spherical aberration, root-mean-square (RMS), contrast sensitivity test, satisfaction and glare was evaluated during the follow-up period of 3 months. RESULTS: There were no significant differences in BCVA, satisfaction, glare, coma, trefoil, RMS and contrast sensitivity test among the 3 aspheric IOLs. Spherical aberration in Akreos Adapt-AO (0.04+/-0.05 microm) was higher compared with Acrysof IQ (0.03+/-0.04 microm) and Tecnis ZA9003 (0.11+/-0.03 microm)(p<0.01). The predictability of postoperative spherical equivalent in the Akreos Adapt-AO group (-0.57+/-0.22D) represented a statistically significant myopic refractive error compared with the Acrysof IQ (0.01+/-0.77D) and the Tecnis ZA9003 (-0.20+/-0.71D) groups (p=0.04). CONCLUSIONS: There were no significant differences in visual function except spherical aberration, which was highest in the Akreos Adapt-AO group.

ABSTRACT

PURPOSE: To compare postoperative visual acuity, spherical equivalent and contrast sensitivity of eyes implanted with either spherical C-flex(TM)(570C) or aspheric C-flex(TM)(970C) intraocular lenses (IOL). METHODS: Forty eyes implanted with posterior chamber intraocular lenses were divided into two groups: C-flex(TM)(570C) (20 eyes) and C-flex(TM)(970C) (20 eyes). In these two groups, postoperative best corrected visual acuity (logMAR), spherical equivalent and contrast sensitivity was performed at one month and three months, postoperatively. RESULTS: In the spherical group, the preoperative naked visual acuity was 1.25+/-0.46 (logMAR). The best corrected visual acuity at postoperative three months was 0.49+/-0.28. In the aspheric group, the preoperative naked visual acuity was 1.29+/-0.67. The best-corrected visual acuity at postoperative three months was 0.39+/-0.30. There were statistically significant differences in best-corrected visual acuity between the preoperative and the postoperative three months results in the spherical IOL group and in the aspherical IOL group. There were no statistically significant differences in spherical equivalent between preoperative and postoperative one-month results and between preoperative and postoperative three-month results in the spherical IOL group and the aspherical IOL group. In the aspherical IOL group, contrast sensitivity at postoperative one and three months were better in the all spatial frequency than in the preoperative scotopic condition. CONCLUSIONS: When we compared eyes implanted with the spherical C-flex(TM)(970C) with eyes implanted with the spherical C-flex(TM) (570C), there were no statistically significant differences in spherical equivalent or best corrected visual acuity. The aspherical IOL group showed better contrast sensitivity than did the spherical IOL group postoperatively at both one month and three months.

ABSTRACT

PURPOSE: To evaluate postoperative spherical aberration, contrast sensitivity and depth of focus after implanting 3 different aspheric intraocular lenses. METHODS: Fifty-six eyes (18 eyes for Akreos adapt Advanced Optics (AO), 20 eyes for AcrySof IQ SN60WF and 18 eyes for Tecnis Acrylic IOL ZA9003) of 48 patients were evaluated. Internal ocular aberration including spherical aberration and higher-order aberration and contrast sensitivity were evaluated 3 months after lens implantation. In addition, visual acuities at 33 cm and 1 m distance were measured with the far vision corrected state to calculate depth of focus. RESULTS: The total and internal ocular spherical aberration of the AO implanted group was slightly higher than the other groups with statistical significance. However, there was no statistically significant difference of contrast sensitivity and depth of focus among the 3 groups. CONCLUSIONS: A subtle difference of spherical aberration among the 3 groups without a statistically significant difference in other factors may not induce the differences of contrast sensitivities and depths of focus in each group.

ABSTRACT

PURPOSE: To compare postoperative wavefront aberration and visual functions between aspherical Tecnis Z9003, a new acrylic aspheric intraocular lens (IOL), and spherical AcrySof SA60AT IOL. METHODS: Fifty patients (56 eyes) who underwent cataract extraction and were implanted with spherical or aspherical IOLs were randomly evaluated by wavefront analysis, including an examination of spherical aberration and higher-order aberrations using two different types of aberrometers (ray tracing and automatic retinoscope), manifested refraction, a contrast sensitivity test, and modulation transfer function (MTF), three months after surgery. RESULT: There were no statistically significant differences of spherical equivalent and best-corrected visual acuity between the two different IOL groups. However, the aspherical IOL group showed less spherical aberration and better contrast sensitivity and MTF than the spherical IOL group. CONCLUSIONS: Tecnis Z9003 could compensate for positive spherical aberrations of the cornea and improve contrast sensitivity and MTF, thereby improving visual function.

ABSTRACT

PURPOSE: To compare spherical aberration, higher-order aberration, and contrast sensitivity of eyes implanted with aspheric (AcrySof IQ) or spherical (AcrySof Natural) intraocular lenses. METHOD: Seventy-five eyes implanted with posterior chamber intraocular lenses were divided into two groups: AcrySof IQ (35 eyes) and AcrySof Natural (40 eyes). Wavefront analysis (spherical, higher-order and total aberration) using iTace, and contrast sensitivity (under photopic and mesopic condition) using Optec 6500 were performed at 1 month and 3 months postoperatively. RESULT: One month after surgery, the IQ IOL (intraocular lens) group had lower values of spherical aberration (p<0.001) and higher-order aberration (p=0.048) than the Natural IOL group. The IQ IOL group also showed better contrast sensitivities at 1.5- and 3-cpd spatial frequencies in mesopic conditions. Three months after surgery, the IQ IOL group had less spherical aberration (p<0.001) and higher-order aberration (p=0.027) than the Natural IOL group and showed better contrast sensitivities at the 3-cpd spatial frequency in mesopic conditions. CONCLUSIONS: Aspheric AcrySof IQ IOL induced significantly less higher-order aberration and spherical aberration than AcrySof Natural IOL. The contrast sensitivity was better under mesopic conditions in the AcrySof IQ IOL-implanted group.
