ABSTRACT

Objective To study the clinical effect of the combination of cataract ultrasonic emulsification and multifocal aspherical intraocular lens in cataract patients through evaluating different intraocular lenses elimi-nating spherical aberration. Methods 180 patients(180 eyes)with senile cataract were included between,2001 and 2005.The patients were divided into three group (n = 60 each),based on the admition time. Implantion of ReSTOR-SN60D3 is multifocal aspherical intraocular lens(SN60D3),implantion of SN60AT lens is single spheri-cal intraocular lens (SN60AT),and implantion of SN60WF lens is single aspherical intraocular lens (SN60WF group). Three months after operation,the refractive state,focus depth,modulation transfer function(MTF),and higher-order aberrations were measured ,and the visual quality were investigated. Results There was no signifi-cant difference in vision in three groups before operation(P > 0.05). After operation,the UCNVA of SN60D3 group was significantly better than the SN60AT group and SN60WF group(P < 0.05). Under 3 mm pupil diame-ter,spherical aberration of SN60D3 group and SN60WF Z(4,0)were less than SN60AT group(P < 0.05), SN60D3 group and SN60WF only under 5 CPD spatial frequency MTF(HOA)value was superior to the SN60AT group(P<0.05);The score of near vision,reliance on vision correction and limitation of operation of patients in SN60D3 group were higher than other two groups(P<0.05). Visual clarity and day vision fluctuations of patients in SN60D3 group is better than single focal sphere group(P<0.05). Conclusion In multifocal aspherical intra-ocular lens,general spherical aberrations and higher-order aberrations is smaller,effect of naked near vision is bet-ter and the rest of the vision is stable ,which can bring good visual quality for cataract patients after operation.

ABSTRACT

Background High order aberrations and scattered light may be the main influencing factors of visual quality after phacoemulsification combined the aspherical IOL implantation.However,the study on objectively assessing the impact of them is little.Objective This study was to assess the impact of high order aberrations and scattered light on visual quality after phacoemulsification combined the aspherical IOL implantation.Methods A series cases-observational study was performed.Sixty-seven eyes of 53 patients who finished phacoemulsification and aspherical IOL implantation for at least 3 months in Tianjin Eye Hospital from February to August 2014 were enrolled.The root mean square (RMS) of total aberrations and high order aberrations was measured under the 4 mm pupil diameter with i-Trace aberrometer,including general high order aberrations,spherical aberrations,coma,and trefoil of entire eye,cornea and internal optics.The correlations between ocular aberrations and corneal or internal optic aberrations were analyzed.Objective scatter index (OSI) and objective visual quality parameters were measured,including MTF cut off frequency (MTF cut off),strehl ratio (SR) and OQAS values (100% OV,20% OV,9% OV) with OQAS Ⅱ,and the correlations between the objective visual quality parameters and OSI,ocular aberrations,corneal aberrations or internal optic aberrations were analyzed.In addition,the correlations between OSI and ocular,corneal or internal optic aberrations were evaluated.Writen informed censent was obtained from each patient before entering the study.Results Ocular aberrations were significantly positively correlated with corneal or internal optic aberrations (r=0.255-0.586,all at P<0.05;r=0.582-0.907,all at P<0.01),and the associations between internal optic and ocular aberrations was stronger than those between corneal and ocular aberrations.The objective visual quality parameters were negatively correlated with ocular,corneal and internal optic aberrations (r=-0.247--0.675,all at P<0.05;r =-0.245--0.497,all at P<0.05;r =-0.246--0.513,all at P<0.05) as well as with OSI (r =-0.620--0.781,all at P<0.01).Most of the associations between internal optic aberrations and objective visual quality parameters were stronger than those of corneal aberrations.Furthermore,ocular,corneal and optical aberrations were all positively correlated with OSI (r =0.396-0.727,all at P<0.01;r =0.320-0.522,all at P<0.05;r =0.319-0.598,all at P<0.01).Conclusions High order aberrations and scattered light seem to interact with objective visual quality in aspherical IOL implanted eyes,and the correlation between internal optic aberrations and ocular aberrations tends to be stronger than that of corneal aberrations.

ABSTRACT

Aims: To compare the quality of vision in pseudophakic patients implanted with aspherical and spherical intraocular lenses (IOLs). Materials and Methods: Randomized prospective longitudinal intrapatient comparison between aspherical and spherical IOLs performed on 22 patients who underwent bilateral cataract surgery. Best corrected visual acuity, subjective contrast sensitivity, Strehl ratio and spherical aberrations (SA), and higher order wavefront aberrations for a 3.5 mm and a 6.0 mm pupil were measured after 3 months of cataract surgery. Results: SA (Z4,0) decreased significantly in eyes with aspherical IOL implant (P = 0.004). Modulation transfer function (MTF) and point spread function (PSF) resulted no significant difference between the two groups (P = 0.87; P = 0.32). Conclusion: Although the SA is significantly lower in eyes implanted with aspherical IOL, the quality of vision determined with MTF and PSF does not significantly differ for subjective and objective parameters that were analyzed.

ABSTRACT

Objective To compare intraindividual higher-order aberrations(HOA)after implantation of aspherical and spherical intraocular lenses(IOL).Design Prospective,case-controlled study.Participants 33 patients(66 eyes)were between 50 and 76 years of ages.The patients with bilateral senile cataracts were operated with phacoemulsification and IOL implantation.All the patients had no other ocular pathologies and no previous ophthalmic surgery.Patients with keratometric astigmatism higher than-2.0 diopters(D)were excluded from the study.Methods Wavefront aberration measurements of 33 patients(66 eyes)after implantation of a spherical IOL(Canon Staar KS-3)in one eye and an aspherical IOL(Cannon Staar KS-3Ai)in the contralateral eye were analyzed.The eyes were randomly assigned to groups of the spherical IOL or the aspherical IOL.Third-order,4th-order,5th-order,total HOA root-mean-squares(RMS),and 4th-order spherical aberrations(Z40)were compared 3 months after surgery.Main Outcome Measures Visual acuity(VA),the best corrected visual acuity(BCVA),the centrality of IOL,refraction data,diameter of pupil size,3rd-order,4th-order,5th-order and total HOA RMS,and 4th-order spherical aberration(Z40)RMS were measured.Results Fourth-order aberration RMS(0.193?0.098?m),Z40 RMS(0.037?0.099?m),total HOA RMS(0.498?0.072?m)in the aspherical IOL group were lower than those with the spherical IOL(0.403?0.155?m,0.381?0.142?m,0.737?0.164?m,respectively)at 6mm pupil diameter.For 3rd-and 5th-order RMS at 6mm pupil diameter,total HOA RMS(at 5mm pupil diameter),no significant difference was found in both IOLs.Under mesopic conditions,ten patients(30%)with an aspheric IOL had better contrast sensitivity.The average myopic shift were(-0.29?0.09)D and(-0.87?0.16)D in the aspherical and spherical IOL groups at 6mm pupil diameter(P
