ABSTRACT

Fundamento: la aterosclerosis es una enfermedad de origen multifactorial que se agrava a consecuencia de un medio ambiente adverso y de estilos de vida no saludables. Las evidencias confirman su inicio desde las etapas más tempranas de la vida. Objetivo: describir la presencia de lesiones ateroscleróticas tempranas en las aortas de fetos y neonatos fallecidos hijos de madres hipertensas. Método: se realizó un estudio descriptivo de tipo transversal que incluyó a los fetos mayores de 22 semanas y neonatos fallecidos hijos de madres hipertensas, a quienes se les realizó autopsia en el departamento de anatomía patológica, del Hospital Universitario Ginecobstétrico Eusebio Hernández Pérez, en el período comprendido entre enero del 2021 hasta diciembre 2022, con un total de 18. Se recogieron los siguientes datos en las historias clínicas: sexo del fallecido, edad gestacional en el momento del parto, peso postparto, edad materna y antecedentes de hipertensión arterial (crónica o gestacional). Se analizaron las 18 aortas (torácicas y abdominales) de los fallecidos seleccionados. Resultados: del total de fallecidos estudiados el 70,6 % presentó lesiones ateroscleróticas tempranas, caracterizadas por engrosamiento de la íntima a modo de cojinete, presencia de linfocitos en la íntima o en la pared arterial cercana a ella y/o macrófagos. En algunos casos se observó una de estas variantes histológicas y en otros la combinación de dos o tres. Conclusiones: las lesiones ateroscleróticas tempranas están presentes en las aortas de la mayoría de los fetos y neonatos estudiados, con predominio en las que presentaban hipertensión gestacional.


Foundation: atherosclerosis is a disease of multifactorial origin that is aggravated as a result of an adverse environment and unhealthy lifestyles. Evidence confirms its onset from the earliest stages of life. Objective: to describe the presence of early atherosclerotic lesions in the aortas of deceased fetuses and neonates born to hypertensive mothers. Method: a descriptive cross-sectional study was carried out that included fetuses older than 22 weeks and deceased neonates born to hypertensive mothers, who underwent autopsy in the pathological anatomy department of the Eusebio Hernández Pérez Gynecobstetric University Hospital, in the period between January 2021 and December 2022, with a total of 18. The following data were collected in the medical records: sex of the deceased, gestational age at the time of delivery, postpartum weight, maternal age, and history of arterial hypertension (chronic or gestational). The 18 aortas (thoracic and abdominal) of the selected deceased were analyzed. Results: of the total number of deceased studied, 70.6 % presented early atherosclerotic lesions, characterized by thickening of the intima as a cushion, presence of lymphocytes in the intima or in the arterial wall close to it and/or macrophages. In some cases, one of these histological variants was observed and in others a combination of two or three. Conclusions: early atherosclerotic lesions are present in the aortas of the majority of the fetuses and neonates studied, with a predominance in those with gestational hypertension.

Resumen Introducción: La anemia de células falciformes se define como la hemoglobinopatía estructural autosómica recesiva más prevalente en todo el mundo, puede presentar fenómenos vasooclusivos y la anemia hemolítica que son los responsables de las principales complicaciones de esta enfermedad, una de estas es el síndrome torácico agudo, siendo una causa importante de morbilidad y mortalidad en estos pacientes. Objetivo: correlacionar los hallazgos de la autopsia médico legal, con las generalidades de la fisiopatología de la anemia de células falciformes actualizadas y disponibles. Materiales y métodos: Se realizó el análisis del protocolo de la autopsia médico legal efectuada, y revisión bibliográfica en un caso de una femenina de 37 años que en apariencia padecía de "dolor por nervio ciático", quien falleció en su casa de habitación, tras presentar un dolor intenso localizado en la espalda y disnea. Tras la autopsia médico legal se estableció como causa de muerte el síndrome torácico agudo por drepanocitosis. Conclusiones: La anemia de células falciformes se debe tener presente al momento de la valoración médico legal como una posible causa de muerte, para ello es necesario conocer la fisiopatología de esta enfermedad, y poder identificar dichas afectaciones al momento de realizar la autopsia médico legal, para esto es necesario contar con todas las herramientas posibles para poder realizar un diagnóstico certero.


Abstract Objective: To correlate the findings of the medical legal autopsy carried out with the updated and available generalities of the pathophysiology of sickle cell anemia. Materials and methods: The analysis of the protocol of the medical-legal autopsy carried out, and a bibliographic review, were carried out. A 37-year-old female who apparently suffered from "sciatic nerve pain," who died in her home after experiencing intense pain located in her back and dyspnea. After the medical-legal autopsy, acute chest syndrome due to sickle cell disease was established as the cause of death. Conclusions: Sickle cell anemia must be kept in mind at the time of the medical-legal evaluation as a possible cause of death, for this it is necessary to know the pathophysiology of this disease, and to be able to identify these effects when performing the Medical- Legal Autopsy, for this It is necessary to have all the possible tools to be able to make an accurate diagnosis.

Resumen En Costa Rica, el Reglamento de la Autopsia Hospitalaria y Médico Legal establece que la autopsia médico legal es obligatoria para toda muerte súbita. La patología cardíaca es responsable aproximadamente del 80 % de las muertes súbitas que requieren una autopsia forense; el prolapso de la válvula mitral (degeneración mixomatosa de la válvula mitral) es una de las formas más comunes de valvulopatía cardíaca, es relativamente común (2%-3% de la población general), y a menudo se considera benigno, la tasa anual de muerte cardíaca súbita (MSC) en individuos con MVP (0,2%-0,4% /año) es aproximadamente el doble que el observado en la población general (0,1%-0,2% año).


Abstract In Costa Rica, the Hospital and Legal Autopsy Regulations establish that a legal medical autopsy is mandatory for all sudden deaths. Cardiac pathology is responsible for approximately 80% of sudden deaths requiring a forensic autopsy; Mitral valve prolapse (myxomatous mitral valve degeneration) is one of the most common forms of heart valve disease, is relatively common (2%-3% of the general population), and is often considered benign, the annual rate of Sudden cardiac death (SCD) in individuals with MVP (0.2%-0.4%/year) is approximately twice that observed in the general population (0.1%-0.2%/year).

ABSTRACT

Objective:To investigate the clinicopathological features and prognosis of fetal prune belly syndrome (PBS).Methods:This retrospective study collected and analyzed the clinical data of five fetuses with PBS diagnosed through a pathological autopsy in Women and Children's Hospital, School of Medicine, Xiamen University, from December 2016 to January 2023. The clinicopathological features of these cases were summarized, and the subsequent pregnancy outcomes in their mothers were followed up. A descriptive statistical method was used for data analysis.Results:(1) Megabladder or enlarged bladder was observed in the five fetuses (four males and one female) with a maximum diameter of 4.4 cm (1.6-8.9 cm). Two fetuses were complicated by renal dysplasia, one by an absence of kidney, and ureters, one by bilateral ureterectasia and possible posterior urethral atresia, and one by the dilated cerebral ventricle, gastroschisis, exposed viscera, and scoliosis. The pregnancies were terminated at the gestational age of 15 +6 weeks (12 +5-19 +4 weeks). (2) The five fetuses exhibited distended and thin abdominal walls with excessive dilation of the bladder. There were two cases with the partially absent abdominal wall, one with diaphragmatic dysplasia, two with renal dysplasia complicated by pyeloureterectasis, two with urethratresia and anal atresia, one with hydroderma and sever seroperitoneum, and one with absent left kidney and left ureter. The muscle layers of the bladder wall, abdominal wall, and ureter tube wall were of varying thickness, and the arrangement of muscle fibers was disordered with incomplete or absent muscle layers in some areas. Moreover, the glomeruli and renal tubules were reduced to varying degrees and fibrous interstitial hyperplasia was observed. Immunohistochemical staining revealed thin muscle layers and broken muscle layers in some regions. (3) One fetus carried a compound heterozygous variant of c.907G>A/c.461C>T in the DHCR7 gene, which was found to be a pathogenic variation. The other four cases had no obvious abnormalities. (4) By August 2023, apart from one case lost to follow-up, the other four were followed up for 68 months (7-80 months). Three women were successfully conceived again after an interval of 16, 24, and 19 months, respectively, and the other one did not conceive due to being in a recovery period. Three neonates were delivered at term in good condition, and no obvious abnormality in growth or development was reported during a three-year follow-up. Conclusions:Fetal PBS is a rare congenital malformation characterized by dysplasia of the abdominal wall and the bladder smooth muscle layers. The subsequent pregnancy is generally not affected.

ABSTRACT Fatty acid oxidation defects are a heterogeneous group of disorders related to the mitochondrial fatty acid oxidation pathway. Carnitine acylcarnitine translocase (CACT) is an enzyme responsible for the unidirectional transport of acylcarnitine across the inner mitochondrial membrane. This enzyme plays a crucial role in the oxidation of fatty acids. The autopsy pathology of the CACT deficiency is described in only a few cases. We describe the autopsy pathology of a child with CACT deficiency dominantly in the form of microvesicular steatosis of the hepatocytes, renal proximal tubular epithelia, cardiac myocytes, and rhabdomyocytes. The diagnosis was further confirmed on whole exome sequencing with compound heterozygous variants in the exon 1 (c.82G>T, p.Gly28Cys; likely pathogenic) and exon 5 (c.535G>A, p.Asp179Asn; uncertain significance) of the SLC25A20 gene. This case elucidates the histopathology of the liver and the detailed autopsy of a case of CACT deficiency from India.

ABSTRACT Trisomy 13, known as Patau syndrome, is a common aneuploidy with a well-known clinical phenotype. This case report describes a trisomy 13 patient with unusual autopsy findings, including features resembling the Beckwith-Wiedemann Spectrum. Due to abnormalities of gestational ultrasounds, a prenatal karyotype of amniotic fluid cells was performed, which resulted in 47, XY+13. Autopsy microscopy studies identified leptomeningeal glioneuronal heterotopia, which was not described as belonging to Patau syndrome. Other atypical findings were diffuse hyperplasia of pancreatic islets of Langerhans and adrenals enlargement with marked adrenocortical cytomegaly, characteristically seen in the Beckwith-Wiedemann Spectrum. Molecular genetic tests were not performed for the Beckwith-Wiedemann Spectrum. Still, due to the rarity of both disorders, this report may support the evidence that trisomy 13 can affect tissue organization and lead to unusual histopathologic features resembling classic overgrowth disorders.

ABSTRACT Pulmonary arteriovenous malformations (PAVMs) are abnormal vascular connections between pulmonary arteries and veins, often associated with hereditary hemorrhagic telangiectasia (HHT). Most PAVMs are asymptomatic, but life-threatening complications like pulmonary hemorrhage, brain abscesses, and paradoxical emboli can emerge, so prompt diagnosis and treatment are crucial. We report a case of sudden pediatric death in a two-year-old female with no past medical history. Initial vomiting and fast deterioration resulted in a sudden cardiac arrest. The postmortem examination found histological evidence of consistent, extensive lung damage. The absence of the characteristic symptoms made for some challenges when it came to diagnosis, showing precisely that in early life, you could well have many difficulties in catching PAVMs. This case highlights the need to take PAVMs into account as a potential cause of sudden death, particularly when there are no conspicuous symptoms. Awareness among forensic pathologists and consideration of genetic analysis for HHT in such cases is crucial for accurate diagnosis and management.

Abstract Objective: To create a protocol for performing minimally invasive autopsies (MIA) in detecting deaths from arboviruses and report preliminary data from its application in Ceará state, Brazil. Methods: Training was provided to medical pathologists on MIA. Results: A protocol was established for performing MIA, defining criteria for sample collection, storage methods, and diagnoses to be carried out according to the type of biological sample; 43 MIAs were performed in three months. Of these, 21 (48.8%) arrived at the Death Verification Service (SVO) with arboviruses as a diagnostic hypothesis, and seven (16.3%) were confirmed (six chikungunya cases and one dengue case); cases of COVID-19 (n = 9), tuberculosis (n = 5), meningitis (n = 4), cryptococcosis (n = 1), Creutzfeldt-Jakob disease (n = 1), breast cancer (n = 1), and human rabies (n = 1) were also confirmed. Conclusion: The protocol implemented enabled identification of a larger number of suspected arbovirus-related deaths, as well as confirmation of other diseases of interest for surveillance.


Resumen Objetivo: Estabelecer un protocolo utilizado para la realización de autopsias mínimamente invasivas (AMI) para la detección de muertes por arbovirus y presentar datos preliminares de este protocolo en Ceará, Brasil. Métodos: Se llevó a cabo la capacitación de médicos patólogos en AMI. Resultados: Se estableció un protocolo para la realización de AMI, que define los criterios para la toma de muestras, métodos de almacenamiento y diagnóstico; en tres meses se realizaron 43 AMI; de estas, 21 (48,8%) llegaron al Servicio de Verificación de Óbito (SVO) con una hipótesis diagnóstica de alguna arbovirosis y siete (16,3%) fueron confirmadas (seis casos de chikungunya y uno de dengue); también se confirmaron casos de Covid-19 (n = 9), tuberculosis (n = 5), meningitis (n = 4), criptococosis (n = 1), enfermedad de Creutzfeldt-Jakob (n = 1), neoplasia de mama (n = 1) y rabia humana (n = 1). Conclusión: El protocolo implementado permitió la identificación de un mayor número de muertes sospechosas de arbovirus, además de la confirmación de otras patologías de interés.

Métodos: Médicos patologistas foram treinados em AMI. Resultados: Estabeleceu-se protocolo para AMI, definindo-se critérios para amostras a serem coletadas, suas formas de armazenamento e diagnóstico, segundo o tipo de amostra biológica; em três meses, foram realizadas 43 AMIs, das quais 21 (48,8%) chegaram ao Serviço de Verificação de Óbito (SVO) com hipótese diagnóstica de alguma arbovirose e sete (16,3%) foram confirmados (seis de chikungunya; uma de dengue); também foram confirmados casos de covid-19 (n = 9), tuberculose (n = 5), meningite (n = 4), criptococose (n = 1), doença de Creutzfeldt-Jakob (n = 1), neoplasia de mama (n = 1) e raiva humana (n = 1). Conclusão: O protocolo implantado permitiu a captação de um maior número de óbitos suspeitos de arboviroses, além da confirmação de outras patologias de interesse da vigilância.

ABSTRACT The Verbal Autopsy (VA) is a questionnaire about the circumstances surrounding a death. It was widely used in Brazil to assist in postmortem diagnoses and investigate excess mortality during the Coronavirus Disease 2019 (COVID-19) pandemic. This study aimed to determine the accuracy of investigating acute respiratory distress syndrome (ARDS) using VA. This is a cross-sectional study with prospective data collected from January 2020 to August 2021 at the Death Verification Service of Sao Luis city, Brazil. VA was performed for suspected COVID-19 deaths, and one day of the week was randomly chosen to collect samples from patients without suspected COVID-19. Two swabs were collected after death and subjected to reverse transcription-polymerase chain reaction (RT-PCR) for SARS-CoV-2 detection. Of the 250 cases included, the VA questionnaire identified COVID-19-related ARDS in 67.2% (52.98% were positive for COVID-19). The sensitivity of the VA questionnaire was 0.53 (0.45-0.61), the specificity was 0.75 (0.64-0.84), the positive predictive value was 0.81 (0.72-0.88), and the negative predictive value was 0.44 (0.36-0.53). The VA had a lower-than-expected accuracy for detecting COVID-19 deaths; however, because it is an easily accessible and cost-effective tool, it can be combined with more accurate methods to improve its performance.

ABSTRACT

ABSTRACT Hydatidosis or echinococcosis is an endemic parasitic disease caused by the ingestion of eggs of echinococcal species worldwide. In India, the annual incidence varies from 1 to 200 per one 100,000 hab., with the highest prevalence reported in the Indian states of Andhra Pradesh and Tamil Nadu. The dog is the definitive host, while humans, sheep, and cattle are intermediate hosts. The disease usually involves the liver and lungs, with the kidney and other organs rare involvement. Cardiac hydatidosis is still further rare, seen in 0.2% to 2% of the patients who remain asymptomatic until the development of its complications. Sudden deaths in cardiac echinococcosis are mostly attributed to cardiac arrhythmias, coronary artery diseases, valvular diseases, cardiomyopathies, pericarditis, and cardiac tamponade. We, herein, report a rare case of cardiac hydatid cyst incidentally found during the autopsy of a 26-year-old male who died due to electrical injuries. A single greyish-white cystic mass measuring 1.5cm X 1.2cm was detected on the left anterior ventricular wall 4 cm above the apex and was confirmed microscopically as a hydatid cyst. The cause of death was attributed to external injury.

ABSTRACT Sporadic Creutzfeldt-Jakob disease (CJD) is a rare neurodegenerative spongiform encephalopathy that causes neuronal derangement secondary to prion protein. Its initial diagnosis is often complex and challenging due to non-specific clinical presentation, lack of awareness, and low clinical suspicion. This disease is invariably fatal, and most patients die within 12 months of presentation. Definite diagnosis of prion disease requires neuropathological analysis, usually done at autopsy. Here, we present the autopsy findings of a 57-year-old male patient, illustrating the complexity of diagnosing this disease early in the clinical course and the need for a broad differential diagnosis at the onset.

Introducción. El feminicidio es un problema de salud mundial. América Latina presenta altas tasas, el Perú implementó su legislación, normativas, sanciones penales y programas preventivos; pero las estadísticas y desaparición de las mujeres siguen en aumento. Objetivo. Determinar las características sociodemográficas y personales en los protagonistas del feminicidio. Métodos. Se realizó un diseño descriptivo, de recolección de datos secundarios. Se analizó documentación forense del Instituto de Medicina Legal y Ciencias Forenses del Perú constituido por 46 protocolos de necropsia médico legal de víctimas de feminicidio en Lima Metropolitana durante los años 2019 al 2020 e informes periciales físicos y psiquiátricos de los feminicidas. Resultados. El perfil común de las víctimas fue tener 18 a 29 años, soltera, secundaria con un trabajo formal, la causa muerte fue por proyectil de arma de fuego y el lugar de muerte fue en su domicilio. El perfil de los feminicidas incluyó ser esposo/conviviente, con antecedentes policiales, familia disfuncional y tener hijos con la víctima. Conclusión. Se encontraron principales características de víctimas y sus homicidas como la edad, grado de instrucción, el domicilio de la víctima, antecedentes policiales, familias disfuncionales. En caso de los homicidas eran disociales y el argumento del homicidio fueron los celos.


Introduction. Femicide is a global health problem, Latin America presents high rates, Peru has implemented its legislation, regulations, criminal sanctions and preventive programs; but the statistics and disappearance of women continue to rise. Objectives. Determine the sociodemographic and personal characteristics of the protagonists of Femicide. Methods. Forensic documentation from the Institute of Legal Medicine and Forensic Sciences of Peru was analyzed, consisting of 46 medical legal autopsy protocols for victims of intimate femicide in Metropolitan Lima during the years 2019 to 2020, physical and psychiatric expert reports from the murderers. Descriptive design, census sampling of secondary data collection. Results. The common profile of the victims was to be 18 to 29 years old, single, high school with a formal job and the cause of death was by firearm projectile and the place of death was at his home. The profile of the murderers included being a husband/partner with a police record and a dysfunctional family and have children with the victim. Conclusion. The main characteristics of the victims and their homicides were found, such as age, level of education, the victim's address, police records, dysfunctional families. In the case of the homicides, they were dissocial and the argument for the homicide was jealousy.

Introducción: La literatura científica reporta que la autopsia psicológica desde sus inicios fue utilizada con la finalidad de ayudar a determinar las circunstancias de muerte de una persona, no obstante, en la actualidad se acoge más como concepto que como una herramienta procedimental. Objetivo: Establecer si en Colombia se ha utilizado para reconstruir los aspectos pre mortem de quienes han fallecido en extrañas circunstancias, de acuerdo con la finalidad de esta. Metodología: Se realizó una revisión sistemática de la literatura científica en 10 bases de datos; se emplearon como palabras clave "autopsia psicológica" y "Colombia", bajo una ventana de observación entre 2010 al 2021; por medio del método PRISMA. Resultados: Se evidencia que en Colombia durante los últimos 10 años el número de estudios asciende a 19, publicados en bases de datos, de los cuales 16 no emplearon el procedimiento de autopsia psicológica y solo tres de ellos sí lo hicieron. Discusión: Existe una tendencia a emplear la autopsia psicológica como concepto desde un enfoque clínico para respaldar los análisis retrospectivos, que, como técnica o procedimiento, se distancia de la posibilidad de reconstruir la criminodinámica del delito, así como la reconstrucción de aspectos de la personalidad, el estado mental y el estilo de vida de quién ha fallecido. Conclusiones: Se resalta que en Colombia la autopsia psicológica se ha utilizado exclusivamente en el campo clínico, sin el rigor científico, metodológico y sin la validez del protocolo empleado.


Introduction: The scientific literature reports that psychological autopsy from its beginnings was used with the purpose of helping to determine the circumstances of death of a person, however, it is currently embraced more as a concept than as a procedural tool. Objective: to establish if in Colombia it has been used to reconstruct the pre-mortem aspects of those who have died under strange circumstances, according to its purpose. Methodology: a systematic review of the scientific literature was carried out in 10 databases, using as keywords psychological autopsy and Colombia, under a window of observation between 2010 and 2021; using the PRISMA method. Results: they show that in Colombia during the last 10 years the number of studies published in databases amounts to 19, of which 16 did not use the psychological autopsy procedure and only three of them did. Discussion: There is a tendency to use psychological autopsy as a concept from a clinical approach to support retrospective analyses rather than as a technique or procedure, distancing us from the possibility of reconstructing the criminodynamics of the crime, as well as reconstructing aspects of the personality, mental state and lifestyle of the deceased. Conclusions: it is highlighted that in Colombia the psychological autopsy has been used exclusively in the clinical field, without the scientific and methodological rigor and without the validity of the protocol used.

Introducción:En Cuba, las enfermedades cerebrovasculares (ECV) son padecimientos crónicos no trasmisibles muy frecuentes. Objetivos: Caracterizar a pacientes de la unidad de cuidados intensivos, con diagnóstico anatomopatológico de ECV. Métodos: Estudio observacional, descriptivo y transversal en una muestra de 176 pacientes. Variables estudiadas: edad (19-39 años, 40-59 años, 60-100 años), sexo, tipo de ECV (hemorrágica, isquémica), escala APACHE II (≤ 15, > 15 puntos) y de coma Glasgow al ingreso (≤ 8, > 8 puntos), ventilación mecánica (sí, no), estadía (≤ 7, > 7 días) y causa directa de muerte. Se calcularon las frecuencias, medidas de tendencia central, pruebas de ji cuadrado y t de Student (nivel de significación el 5 por ciento). Resultados: Predominaron la enfermedad cerebrovascular hemorrágica, el sexo masculino (52,8 por ciento) y el grupo de edad de 60-100 años (64,8 por ciento). La edad media fue de 63,8 años. La media del valor de las escalas APACHE II y Glasgow fue de 21,6 y 6,5 puntos. El 97,6 por ciento recibió ventilación mecánica. La estadía media fue de 7,0 días. El edema cerebral intenso constituyó la principal causa de muerte directa (79,3 por ciento). Conclusiones: Existe predominio de la ECV de tipo hemorrágica, en pacientes del sexo masculino, de 60-100 años, con APACHE II > 15 puntos, Glasgow ≤ 8 puntos, ventilados, estadía ≤ 7 días y edema cerebral intenso(AU)


Introduction: In Cuba, cerebrovascular diseases (CVD) are very common non-communicable chronic conditions. Objectives: Characterize patients from intensive care unit with a pathological diagnosis of CVD. Methods: Observational, descriptive and transversal study in a sample of 176 patients. Variables: age (19-39 years, 40-59 years, 60-100 years), sex, type of CVD (hemorrhagic, ischemic), APACHE II scale (≤ 15, > 15 score) and Glasgow coma on admission (≤ 8, > 8 score), mechanical ventilation (yes, no), length of stay (≤ 7, > 7 days) and direct cause of death. Frequencies, measures of central tendency, chi-square and Student's t tests (significance level of 5 percent) were calculated. Results: Hemorrhagic cerebrovascular disease, male sex (52.8 percent) and the age group of 60-100 years (64.8 percent) predominated. The mean age was 63.8 years. The average value of the APACHE II and Glasgow scales was 21.6 and 6.5. 97.6 percent received mechanical ventilation. The mean stay was 7.0 days. Severe cerebral edema was the main cause of direct death (79.3 percent). Conclusions: There is a predominance of hemorrhagic CVD, in male patients, aged 60-100 years, APACHE II score > 15, Glasgow score ≤ 8, ventilated patients, stay ≤ 7 days and intense cerebral edema(AU)

ABSTRACT

La muerte súbita es aquella que ocurre dentro de las 24 horas posteriores al inicio de los síntomas y se caracteriza por ser clínicamente inexplicable, inesperada y repentina. Debido a la naturaleza de la muerte súbita, no es posible llegar a un diagnóstico preciso sin una autopsia. En esta comunicación breve, evaluaremos el caso de un empleado de crucero de 33 años, sin historial médico/farmacológico previo, el cual falleció súbitamente mientras reposaba en su camarote. Debido a las sospechas iniciales de una posible muerte causada por una sobredosis de cocaína, se le realizó un panel toxicológico abarcador el cual resultó negativo. Empero, una tomografía computarizada (TC) craneal sin contraste revirtió la hipótesis inicial y la autopsia neuropatológica -sorpresivamente- confirmó que la verdadera causa de muerte fue la ruptura de un aneurisma sacular desconocido en el polígono de Willis.


Sudden death occurs within 24 hours after the onset of symptoms and is characterized by being clinically inexplicable, sudden, and unexpected. Due to the nature of sudden death, it is not possible an accurate diagnosis without performing an autopsy. In this brief communication, we will evaluate the case of a 33-year-old cruise employee, with no prior medical/pharmacological history, who suddenly died while resting in his cabin. Due to initial suspicions of a possible cocaine overdose death, a comprehensive toxicology panel was performed, although yielding a negative result. A cranial computed tomography without contrast reversed the initial hypothesis and the neuropathological autopsy -surprisingly- confirmed that the true cause of death was the rupture of an unknown saccular aneurysm in the Circle of Willis.

ABSTRACT

El monóxido de carbono (CO) es un gas que se produce durante la combustión incompleta de diferentes materiales orgánicos. Una vez que se inhala, se absorbe hacia la sangre, ejerciendo su efecto a nivel sistémico. Se une fuertemente a la hemoglobina, y forma la carboxihemoglobina lo que provoca una disminución del transporte de oxígeno a los tejidos y dependiendo de su concentración puede ser mortal. Los hallazgos comúnmente encontrados en la autopsia son color rojo cereza en la piel y órganos, así como edema pulmonar, entre otros. El diagnóstico de intoxicación por CO se basa en la medición post mortem de carboxihemoglobina en sangre, por lo que se deben tomar muestras para cuantificar estos niveles. Con respecto al manejo en estos casos, se presenta dos casos correspondientes a la autopsia médica legal en las que se estableció como causa de muerte la intoxicación por monóxido de carbono.


Carbon monoxide (CO) is a gas that is produced during the incomplete combustion of different organic materials. Once inhaled, it is absorbed into the blood, exerting its effect at the systemic level. It strongly binds to hemoglobin, and forms carboxyhemoglobin, which causes a decrease in oxygen transport to the tissues and, depending on its concentration, can be fatal. The findings commonly found in the autopsy are cherry red color in the skin and organs, as well as pulmonary edema, among others. The diagnosis of CO poisoning is based on the postmortem measurement of carboxyhemoglobin in the blood, so samples must be taken to quantify these levels. Regarding the handling of these cases, two cases corresponding to the legal medical autopsy are presented in which carbon monoxide poisoning was established as the cause of death.

ABSTRACT

From the British era, regular medico-legal autopsies have never been done in India after sunset, except for those specially permitted by the law enforcement agencies. The Ministry of Health and Family Welfare, Government of India, issued a notification on November 15, 2021, regarding the “Conduct of post-mortem in hospitals after sunset”. This has given rise to much debate on whether post-mortems can be conducted after sunset in an ethical manner. Here, we briefly discuss the various issues related to the carrying out of post-mortems after sunset in India.

Introducción: El SARS-CoV-2 causa graves neumonías. Las gestantes experimentan cambios inmunológicos y fisiológicos, que pueden hacerlas más susceptibles a las infecciones respiratorias virales, incluida la COVID-19. Objetivo: Exponer las características de una serie de casos de muertes maternas, confirmadas con la COVID-19. Métodos: Se realizó un estudio de serie de autopsias parciales, a las puérperas confirmadas al SARS-CoV-2, revisadas por el grupo especial de trabajo de anatomía patológica para la COVID-19, en el año 2021. Se analizaron las variables edad, historia obstétrica y causas de muerte, en el Hospital Militar Central "Dr. Luis Díaz Soto". Resultados: En el 2021 fueron atendidas 425 gestantes confirmadas al SARS-CoV-2, de ellas 16 fallecieron (3,8 %). A todas se les realizó cesárea, por beneficio materno-fetal e ingresaron en la unidad de cuidados intensivos, con comorbilidades entre las cuales la obesidad y la diabetes fueron más frecuentes. La media de fecha de inicio de los síntomas fue 5,18 días, todas contacto de casos positivos; en las causas de muerte la hipoxia sistémica afectó a un tercio de las fallecidas; el edema pulmonar de permeabilidad se presentó en el 100 % de las puérperas y en todas las muertes maternas hubo daño múltiple de órganos. Conclusiones: El edema pulmonar de permeabilidad afecta a todos los casos, con impacto importante como causa de muerte, así como en la expresión de la hipoxia y la respuesta inflamatoria sistémica. La COVID-19 es la causa básica de muerte en todos los casos.


Introduction: SARS-CoV-2 causes severe pneumonias. Pregnant women experience immunological and physiological changes, which may make them more susceptible to viral respiratory infections, including COVID-19. Objective: To present the characteristics of a case series of maternal deaths confirmed with COVID-19. Methods: A serial study of partial autopsies of postpartum women confirmed with SARS-CoV-2, reviewed by the special working group of pathological anatomy for COVID-19, in the year 2021, was carried out. The variables age, obstetric history and causes of death were analyzed at the Central Military Hospital "Dr. Luis Díaz Soto". Results: In 2021, 425 pregnant women with confirmed SARS-CoV-2 were attended, 16 of them died (3.8%). All of them underwent cesarean section for maternal-fetal benefit and were admitted to the intensive care unit, with comorbidities among which obesity and diabetes were more frequent. The mean date of symptom onset was 5.18 days, all contact positive cases; in the causes of death systemic hypoxia affected one third of the deceased; permeability pulmonary edema was present in 100 % of the puerperal women and in all maternal deaths there was multiple organ damage. Conclusions: Permeability pulmonary edema affects all cases, with important impact as a cause of death, as well as in the expression of hypoxia and systemic inflammatory response. COVID-19 is the basic cause of death in all cases.
