ABSTRACT

Aviator Xpro EC 225 is a new fungicide for use in cereals, containing the new active substance bixafen and the already approved active substance prothioconazole. Prothioconazole was assessed by the Norwegian Scientific Committee for Food Safety in 2006, and is therefore not included in this report. VKM was requested by the Norwegian Food Safety Authority to consider the possible health risk for operators related to the properties of bixafen used in Aviator Xpro EC 225; in particular to evaluate the relevance of the effects of bixafen on liver and coagulation parameters observed in rats and mice, thyroid tumours and reproductive effects observed in rats, and the establishment of NOAELs and reference values. VKM was also asked to evaluate the fate and behaviour of bixafen in the environment, and the ecotoxicological effects and risks related to its use. The risk assessment was finalized in a meeting on May 24. 2013 by VKM’s Scientific Panel on Plant Protection Products. VKM’s conclusions are as follows: Health: VKM concluded that the liver effects reported in sub-chronic and chronic studies in rats, dogs and mice exposed to bixafen, as well as the effects on coagulating parameters, should be considered adverse and of relevance to humans. It is the opinion of VKM that the thyroid follicular cell tumours reported in female rats exposed to bixafen is not sufficient to suggest that bixafen has a tumour inducing potential. It is further the opinion of VKM that it cannot be excluded that the reduced pup weight during lactation in the two-generation study is mediated by bixafen via the milk, and that bixafen has a direct effect on the number of stillborn pups in the F1 and F2 generation. Finally, the pup developmental variants and anomalies reported in the teratogenicity study are considered treatment related, and not as secondary effects related to maternal toxicity. VKM proposes a NOAEL of 2.0 mg/kg bw/day based on a 2 year feeding study in rats. VKM support/propose: ADI: 0.02 mg/kg bw/day AOEL: 0.13 mg/kg bw/day ARfD: 0.2 mg/kg bw/day Risk calculations show minimal risk if personal protective equipment is used. Environment: Bixafen is shown to be persistent in soil. VKM considers the results from a field study on a German site and the Finnish PEC calculator to be relevant and concludes that repeated annual applications can cause accumulation in soil under Norwegian conditions. There is also a potential for groundwater contamination from leaching of the metabolite M44. VKM has concluded that the use of bixafen as an ingredient in Aviator Xpro EC 225 with the proposed application regime in Norway will represent a minimal risk for toxic effects to terrestrial organisms. The toxicity of bixafen to aquatic organisms is high, and minimal risk of effects in surface waters can be achieved only if risk mitigation measures with bufferzones of 10 m are applied.

Resumen Objetivo: Revisar en la literatura las diferentes posturas de las agencias reguladoras de la aviación sobre la implantación de lentes intraoculares multifocales (MIOLs) en pilotos aviadores civiles que han desarrollado catarata, además de evidencia científica que podría generar una revisión de los lineamientos vigentes. Resultados: Se encontraron el 50% (3 de 6) de agencias revisadas con postura en contra de los MIOLs. Además, se revisaron dos estudios a favor de los MIOLs, en el que implantarlos brinda una mayor independencia de lentes respecto a los monofocales para las tareas visuales dentro y fuera de la cabina, así como un rendimiento sin diferencias en simuladores de vuelo versus los pilotos con lentes intraoculares monofocales. Limitaciones: Existen pocos estudios de MIOLs en pilotos aviadores. Originalidad: No se ha reportado ninguna revisión de las agencias reguladoras respecto a la prohibición o permisión de los MIOLs en pilotos aviadores, tocando evidencia que pudiera consolidar una postura unánime respecto a este tema, como en su momento fue la cirugía refractiva con láser excimer que actualmente no está restringida.


Abstract Objective: To review the literature on the different stands of aviation regulatory agencies on the implantation of multifocal intraocular lenses (MIOLs) in civilian aviators who have developed cataract, as well as scientific evidence that could generate a revision of the current guidelines. Results: 50% (3 of 6) of agencies reviewed were found to be against MIOLs. In addition, two studies were reviewed in favour of MIOLs, in which implanting MIOLs provides greater lens independence from monofocals for visual tasks in and out of the cockpit, as well as no difference in performance in flight simulators versus pilots with monofocal intraocular lenses. Limitations: There are few studies of MIOLs in aviator pilots. Originality: No review of regulatory agencies regarding the prohibition or permission of MIOLs in airline pilots has been reported, touching on evidence that could consolidate a unanimous position on this issue, as was once the case with excimer laser refractive surgery, which is currently unrestricted.

ABSTRACT

Aviator Xpro EC 225 containing the active substance bixafen was assessed by VKM in spring 2013, and it was concluded that the metabolite M44 has potential for groundwater contamination. Furthermore, VKM assessed in late 2013 the relevance of this metabolite in accordance with the EU guidance document on metabolites in groundwater, and concluded that the malformations observed in rabbits exposed to the metabolite should be considered treatment related. VKM also concluded that the data presented to evaluate the possible genotoxic properties of the metabolite was insufficient to reach a conclusion. Based on this, the Norwegian Food Safety Authority rejected the approval of Aviator Xpro EC 225. The applicant has now submitted results from an in vivo study to strengthen the basis for assessment of genotoxic properties, and also submitted new historical controls in relation to the experimental studies on foetal developmental effects in rabbits. The VKM Panel on Plant Protection Products has discussed the questions raised by The Norwegian Food Safety Authority on the basis of the new data, and has the following opinion: On the assessment of genotoxic properties of the M44 metabolite of bixafen, one of the active ingredients of Aviator Xpro EC 225. It is the view of VKM Panel on Plant Protection Products that the new in vivo mouse micronucleus study, supplemented together with a separate study demonstrating bioavailability, overrides the results of the in vitro clastogenicity studies. Taken together, it is the opinion of VKM that under the conditions studied, M44 should be considered as non-genotoxic. On the assessment of the relevance of the foetal malformations in M44 exposed animals. VKMs Panel on Plant Protection products has assessed the arguments and new historical control data presented by the applicant, intended to show that metabolite M44 is not teratogenic. It is however the opinion of the Panel that the arguments and the new historical data provided by the applicant do not alter the panel’s previous conclusion; that the malformations observed in rabbits exposed to the metabolite M44 should be considered treatment related.

@#Bilateral fracture-dislocation of the talus is a rare occurrence. It represents 0.06% of the dislocations and 2% of the traumas of the talus. We report the case of a 29-year-old patient with an exposed bilateral fracture of the talus following a plane accident. On the right ankle, the patient had a fracturedislocation Hawkin 3 Gustilo II, on the left ankle presented a Hawkin 4 Gustilo IIIB. The patient was treated within six hours from the trauma. We reduced the dislocation and performed an osteotomy of the tibial malleolus and osteosynthesis of the fracture with screws. The definitive stabilisation has been achieved in both limbs with an external fixator. We evaluated the patient at 1, 3, 6, 8, 12 and 18 months from treatment, with a radiograph and with SF-36 and Foot and Ankle Disability Index questionnaires. No infection was reported, radiographs showed a successful consolidation of the fracture in both limbs. At the one year follow-up, the patient was able to walk without aids and there were no signs of osteonecrosis on the MRI. The treatment of these lesions requires timely treatment, an anatomical reduction of the fracture and patient's collaboration. The use of external fixator with internal osteosynthesis represents a good therapeutic option in Hawkins 3 and 4 type fractures.

ABSTRACT

?AIM:To investigate and analyze the prevalence and the influencing factors on retinal arteriosclerosis in civil aviators of different ages.?METHODS:Two thousand seven hundred and seventy-eight commissioned civil aviators who received medical examination in Civil Aviation General Hospital from July 2014 to July 2015 were randomly divided into three groups: group A ( 1 311 aviators aged 22-39 ) , group B (618 aviators age 40-49 ), group C(849 aviators age 50-59). Aviators over 40 years old, were divided into two groups: group with retinal atherosclerosis and group without retinal atherosclerosis according to the results of fundus examination. Medical examination data of the aviators were analyzed.?RESULTS: Of 2 778 aviators, 334 ( 12. 02%) were detected with retinal arteriosclerosis, among which 318 aviators were classified as gradeⅠ, and 16 aviators as grade Ⅱ. The retinal arteriosclerosis prevalence of group A, B, and C increased with age, and the difference was statistically significant ( P < 0. 05 ). Biochemical tests showed that the prevalence of hypertension, hyperlipidemia, high fasting blood glucose and overweight increased in group with retinal arteriosclerosis compared with that in group without retinal arteriosclerosis, and the difference was statistically significant(P<0. 05).?CONCLUSION:The prevalence of retinal arteriosclerosis in commissioned civil aviators was 12. 02%, and most of the retinal arteriosclerosis was classified as grade Ⅰ. In aviators over 40 years old, hypertension, hyperlipidemia, high fasting blood glucose and overweight were important factors in the development of retinal arteriosclerosis.

ABSTRACT

Objective To provide an effective theoretical basis for future selection and recruitment of military flying cadets.Methods Two hundred military qualified flying cadets by comprehensive assessment samples and 200 military flying cadets by medical fitness samples were interviewed with questionnaires and subjected to a physical examination.The comprehensive assessment samples were classified by symptoms and attributive specialties for statistical analysis.A database was established, cleaned and analyzed by EpiData 3.02, SPSS 16.0 with double checking.Results There was no differ-ence between the comprehensive assessment team and the medical fitness team in age, areas, academic scores and psycho-logical performance.The ten top symptoms according to the comprehensive assessment were abnormal chest circumference, abnormal body mass,iris cyst,abnormal hypersplenotrophy,gallbladder polyps,flat foot,skin disease, intrahepatic calcifica-tion focus, joint disease and reproductive system disease.The attributed specialties included surgery, ultrasonics, ophthal-mology,ENT,radiology, ECG and internal medicine departments.The surgery and ultrasonics department contributed the most from the comprehensive assessment and accounted for 55.52%and 16.37%, respectively.Conclusion The compre-hensive assessment for military flying cadets has an important effect on military aviator selection.Continuous observation should be carried out among the military flying cadets by comprehensive assessment in their aviation training.

ABSTRACT

AlM:To research the reasonability of current criteria on distant vision anddiopter for recruiting civil aviators in China. METHODS: The data about distant vision and diopter of 1901 aviators, including flight majors and aviators in active service participating in physical examination of Civil Aviation Flight University of China from 2006 to 2013, were collected. ANOVA and LSD were used to compare the differences between distant vision and diopter among different groups. The Spearman correlation coefficients of distant vision (≥0. 1 vs ≥0. 3) and diopter (0. 00 to -3. 00D) were calculated. RESULTS: The diopter of civil aviators in China increased with distant vision decreased. The correlation between distant vision and diopter ( 0. 00 to -3. 00D ) among distant vision≥0. 3's population (0. 4 CONCLUSlON: The current eye standard of recruiting civil aviators in China is reasonable to consider both distant vision and diopter, but the standard for distant vision is higher. Further research is proposed to explore the proper standard of distant vision that matches the diopter standard.

ABSTRACT

To explore the impact of acceleration load on the temporomandibularjoint in aviators and its clinical significance.617 aviators flying aircraft of different types without previous history of temporomandibularjoint disease were studied. Abnormal manifestations of the temporomandibularjointswere analysed. The incidence of temporomandibular joint symptoms was markedly higher in the fighter plane pilots than that in the transportation planegroup. Load induced by acceleration could be one of the main causative factors of temporomandibularjoint disorder.

ABSTRACT

Aerotitis media occurs frequently among flyers. 40 cases have been treated with electro- acupuncture in our hospital with satisfactory results. Xiaguan was selected as local point and Hegu as distal point. The patients were needled once a day or every other day. The total rate of effectiveness was 90.0%. According to Western medicine, this disease is due to poor pressure-equilibrating function of the Eustachian tube. The mechanism of treating this disease was explained by traditional Chinese medicine and western medicine theory.
