ABSTRACT

<p><b>OBJECTIVE</b>This study aimed to establish a SD rat model of chronic periodontitis (CP) merged with hyperlipidemia (HL), perform periodontal treatment, detect the expression of partial C-reactive protein (CRP) and Porphyromonas gingivalis (P. gingivalis) in the rat carotid artery, and explore the relationship between periodontitis and atherosclerosis.</p><p><b>METHODS</b>SD rats were randomly divided into three groups: control group (A), HL group (B), and CP+HL group (C). Group C rats were divided into natural process group (C1), scaling and root planning group (C2), and tooth extraction group (C3). Group C2 rats were randomly divided into C2-1 (scaling and root planning group) and C2-2 (scaling and root planning+minocyline+systemic antibiotics group). Group C3 rats were randomly divided into C3-1 (tooth extraction group) and C3-2 (tooth extraction+systemic antibiotic group). One rat from group B was randomly selected and sacrificed after 15 weeks. Subsequently, the carotid vascular tissue was collected for oil red O staining. Modeling was successful when foam cell formation was observed. Periodontal treatments were conducted twice, and euthanasia was performed after the experiment. Moreover, double-carotid artery bifurcation was carried out to detect the expression of CRP and P. gingivalis. Immunohistochemical and 16sRNA semiquantitative methods were used to detect the CRP expression and the relative contents of P. gingivalis, respectively.</p><p><b>RESULTS</b>Immunohistochemical results showed that the CRP-positive expression in groups B and C was significantly higher than that in group A (P<0.05). The CRP-positive expression in other group C rats were significantly lower than that in group C1 (P<0.05). The CRP-positive expression in group C2-2 was the lowest among the groups (P<0.05). The relative quantity of P. gingivalis in group C1 was the highest and significantly higher than that in groups A and B (P<0.05). The relative quantities of P. gingivalis in groups C2-1, C2-2, C3-1, and C3-2 were significantly lower than that in group C1 (P<0.05), and the quantity in group C3-2 was the lowest (P<0.05).</p><p><b>CONCLUSIONS</b>Rats with CP associated with HL will increase the CRP expression and oral bacteria quantity on carotid artery, and lesions will gradually aggravate. Interventions, such as periodontal basic treatment and tooth extraction, could improve carotid artery lesions. The basic treatment with local and systemic anti-inflammatory drugs exerts the most satisfactory effect on local CRP expression. Tooth extraction with antibiotics is an effective method on reducing oral bacteria in carotid artery. Periodontal basic treatment associated with local and systemic antiflammatory drugs can obviously improve the effect.

ABSTRACT

Objective To investigate the local or systemic effects of initial periodontal therapy on peritoneal dialysis in patients with chronic kidney disease and periodontitis. Methods Sixty-one patients with both periodontitis and regular peritoneal dialysis were selected in this study and were randomly divided into observation group (n=31) and control group (n=30). Patients in observation group were given periodontal initial therapy (ultrasonic supragingival scaling and ultrasonic subgingival irrigation, drug, root planing, oral health education) and peritoneal dialysis treatment. Patients in control group were given only peritoneal dialysis treatment. Clinical parameters including plaque index (PLI), gingival index (GI) and probing depth (PD) were detected before and one month after treatment in two groups. The concentrations of high-sensitivity C-reactive protein (hs-CRP) in gingival sulcus fluid and serum samples were measured by immune transmission turbidity method in two groups. Results There were no significant differences in PLI, GI, PD, hs-CRP of gingival sulcus fluid, and serum hs-CRP before treatment between the two groups. After one-month initial therapy, all the indexes were decreased in observation group, which were significantly different compared with those before treatment (P<0.05). And all the indexes were significantly lower in observation group than those of control group. There were no significant differences in PLI, PD, GI, hs-CRP of gingival sulcus fluid, and serum hs-CRP between control group and observation group before treatment. Conclusion Periodontal initial treatment can improve the periodontal inflammation in patients with chronic kidney disease, and reduce the concentration of systemic inflammatory factor hs-CRP, decreasing the risk of infection.

ABSTRACT

Objective To explore the short-term effect of oral antibiotic at different times on the treatment of invasive periodontitis. Methods 78 patients with invasive periodontitis from November 2015 to March 2017 were enrolled in this study. Randomized method was used to divide the patients into the observation group and the control group, 39 cases in each group. On the basis of the basic treatment, the observation group were received amoxicillin and metronidazole after oral prophylaxis, the control group were received amoxicillin and metronidazole after subgingival scaling. The plaque status before and after the basic treatment and the efficacy in the two groups after 8 weeks were compared. Results The plaque in the two groups was significantly improved after basic treatment (P<0.05). There was no significant difference in the probing depth and the loss of attachment between the two groups. The bleeding index in the observation group was significantly lower than that in the control group (P <0.05). The probing depth in the observation group was significantly better than that in the control group (P<0.05). Conclusion On the basis of basic therapy, taking antibiotics has a better therapeutic effect on the patients with invasive periodontitis. Antibiotics is use after oral prophylaxis is better than it used after subgingival scaling, which is worthy to be popularized in clinical practice.

ABSTRACT

OBJECTIVE:To observe the efficacy and safety of Huanglian shangqing soft capsule in the adjuvant treatment of in-flammation around the implant. METHODS:40 patients(72 implants)with inflammation around the implant were randomly divid-ed into control group (20 cases) and observation group (20 cases). Control group received full mouth supragingival scaling tech-nique,subgingival curettage,root planing,occlusal adjustment,loose teeth fixation or removal can not be reserved teeth using ul-trasonic scaler row scaling postoperative with 3% hydrogen peroxide,0.9% Sodium chloride injection alternately flushing periodon-tal pocket,removing residue until the inflammation control after the break-adjusted+appropriate amount of Minocycline hydrochlo-ride ointment,along with fine needles inserted into the periodontal pocket wall slowly until bag after pushed into the bottom of Mi-nocycline hydrochloride ointment,slow retreated the drug overflow until the gingival margin,no gargle and drinking water within half an hour,once a week. Observation group additionally received 1.2 g Huanglian shangqing soft capsules,twice a day. 4-week was regarded as 1 course,and it last 2 courses. Clinical efficacy,PLI,PD,SBI,IL-1β,IL-6,IL-2 and IL-17 levels before and af-ter treatment,and the incidence of adverse reactions in 2 groups were observed. RESULTS:The total effective rate in observation group was significantly higher than control group,the difference was statistically significant (P<0.05). After treatment,the PLI, PD,SBI,IL-1β,IL-6,IL-2 and IL-17 levels in 2 groups were significantly lower than before,and observation group was lower than control group,the differences were statistically significant(P<0.05). The incidence of adverse reactions in observation group was significantly higher than control group,the difference was statistically significant (P<0.05),but the symptoms were mild. CONCLUSIONS:Based on conventional treatment,Huanglian shangqing soft capsule shows significant efficacy in the adjuvant treatment of inflammation around the implant,it can reduce inflammatory cytokine levels in gingival sulcus fluid,but with more ad-verse reactions.

ABSTRACT

Objective Periodontal clinical effects of hyperbaric oxygen therapy were analyzed and discussed the foundation .Methods Our hospital from 56 patients with periodontal disease in August 2013 admitted -August 2014 period, were randomly divided into experimental and control groups , and the control group underwent drug treat-ment on a regular basis , the experimental group on the basis of the control group , the line hyperbaric oxygen therapy , compared to its clinical effects .Results The symptoms were improved , but the effect more pronounced in the exper-imental group, which was significantly better than the control group of patients , compared to significantly different sta-tistically significant ( p<0.05 ) .Conclusion Hyperbaric oxygen combined basis have better clinical treatment of periodontal disease , which should be widely applied , pending further improved and refined to improve the outcome .
