ABSTRACT

OBJECTIVE@#To explore the correlation between the curvature of the cervical spine and the degree of cervical disc bulging in young patients with cervical pain.@*METHODS@#The clinical data of 539 young patients with neck pain from January 2015 to December 2018 were retrospectively analyzed. ABSTRACT

Una de las infecciones virales más comunes -sobretodo en personas de mayor edad- es el Herpes Zóster (HZ). Su característica clínica, hace sospechar al médico de forma temprana, para otorgar un tratamiento adecuado y oportuno. Dentro de las complicaciones más frecuentes se encuentran las sensitivas, como la neuralgia postherpética.1 Sin embargo, existen un grupo de complicaciones motoras de menor incidencia, como lo es la Pseudohernia abdominal. Ésta corresponde a una paresia segmentaria, que se manifiesta como una protrusión de la pared abdominal sin un defecto real, que aumenta con maniobras de valsalva.1 Generalmente se puede presentar en hombres, mayores de 60 años, inmunosuprimidos o con neoplasias hematológicas.1,2,3 El diagnóstico es clínico, aunque se puede confirmar con estudio imagenológico, que evidencie una musculatura de la pared abdominal adelgazada con respecto a la contralateral y que descarta un orificio herniario por un defecto estructural. 2 La electromiografía también puede jugar un rol al evidenciar anormalidades en la conducción nerviosa. 2 La pseudohernia por HZ tiene un buen pronóstico en la mayoría de los pacientes con recuperación completa: entre 2-18 meses. 3 Su principal riesgo es la pseudobstrucción intestinal, que se puede manifestar como constipación.2,4 En el siguiente reporte de caso, se analiza a la pseudohernia abdominal como complicación motora infrecuente del HZ y sus características.


One of the most common viral infections -especially in elderly- is Herpes Zoster (HZ). Its clinical characteristic makes the doctor suspect early, to grant adequate and timely treatment. Among the most frequent complications are the sensitive ones, such as postherpetic neuralgia1 . However, there is a group of motor complications of lower incidence, such as abdominal pseudohernia. This corresponds to a segmental paresis, which manifests as a protrusion of the abdominal wall without a real defect that increases with valsalva maneuvers1 . It can generally present in men, older than 60 years, immunosuppressed or with hematological neoplasms1,2,3, The diagnosis is clinical, although it can be confirmed with an imaging study, which shows a thinner abdominal wall musculature with regard to the contralateral wall, and which rules out a hernial orifice due to a structural defect2 . Electromyography can also play a role in show abnormalities in nerve conduction2 . HZ pseudohernia has a good prognosis in most patients with complete recovery: between 2-18 months.3 Its main risk is intestinal pseudoobstruction, which can manifest as constipation2,4. In the following case report, abdominal pseudohernia is analyzed as a rare motor complication of HZ and its characteristics.

ABSTRACT

A decreased mortality rate among infants and children has been a health target for most health facilities. South Africa’s free maternal healthcare services are not adequately utilized due to the tradition of new mothers following their parents’ footsteps. The use of medicinal plant species in maternal healthcare has been perceived to be effective and reliable in preventing illness among communities. For this study, ethnobotanical data were collected by means of interviews using semi-structured questions responded to by the elderly, community adults, and youth who had been selected on the basis of referrals. Infants within the communities have been treated with traditional medicine for ailments, such as colic, nail biting, sunken, and bulging fontanelles. Hypoxis hemerocallidea Fisch., C. A. Mey. & Ave-Lall, Lippia javanica (Burm.f.) Spreng., Vachellia karroo (Hayne) Banfi & Glasso, and Annona senegalensis Pers. subsp. senegalensis were among plant species which were frequently reported as being used medicinally. The decoction was mostly prepared from the roots (79%) followed by bulbs (8%) and most of the decoction was taken orally. The study identified 13 plant species used successfully for the treatment of bulging and sunken fontanelles.

ABSTRACT

Background: The objective was to evaluate the outcome of late second trimester emergency cerclage in patients with advanced cervical dilatation with bulging membranes. Methods: Setting: department of obstetrics and gynaecology, PSG Institute of medical sciences & research, Coimbatore, Tamil Nadu, India. This is a retrospective study of case files of patients who underwent emergency late second trimester cerclage for advanced cervical dilatation with bulging membranes between January 2009 to January 2014. McDonald’s technique was used in all the cases. Results: Altogether, 7 patients (100%) underwent late second trimester emergency cerclage between 20-28 weeks of gestational age, out of which three patients (42.86%) had term deliveries (>37w), and 3 patients (42.86 %) carried on their pregnancies to more than 32 weeks resulting in healthy live born babies. Two of them delivered by normal vaginal delivery, 4 underwent LSCS, and one patient had severe abdominal pain with bleeding and draining per vaginum after 3 days of cerclage, in view of which the stitch was removed. Subsequently, the patient expelled a live foetus weighing 620gms, which died in the Neonatal Intensive Care Unit (NICU) after 3 hours. This procedure prolonged the duration of pregnancy in all patients with a mean duration of 70.4 days. The mean gestational age at the time of delivery was 34.33 weeks. The mean birth weight was 2.18 kg and ranged between 1.97 to 2.64 kg. The mean APGAR at one minute was 8/10 and the mean duration of stay in NICU was 1.66 days. All the new-born babies were healthy at the time of discharge. The live birth rate following emergency late second trimester cerclage in this series was 85.75%. Conclusion: Favourable neonatal outcome can be accomplished in patients with cervical incompetence in the second trimester of pregnancy following emergency cervical suturing, even if performed when the membranes are bulging through the cervix.

[Objective]Discussion on TCM name and pathogenesis of hypertension. [Method]To further explore and analyze the TCM name, location as wel as pathogenssis of hypertension after learning“Neijing”. [Result]Hypertension corresponding TCM diseases should be the veins bulge from“Neijing”. The location of the disease is heart,while it relates to internal organs. The main pathogenesis is that heart commanding blood abnormality. [Conclusion ]The traditional classical dialectical mode from the liver treatment of hypertension should be changed,the clinical treatment of hypertension is due to the heart according to the theory of pathogenesis which the heart commands blood vessels.Seeking the key reason is important to differential treatment of hypertension.

INTRODUÇÃO: O retalho do músculo reto abdominal, descrito por Drever em 1977, foi posteriormente modificado e, desde então, se tornou rapidamente uma importante alternativa para reconstrução da mama. Dentre as complicações do TRAM, citam-se os abaulamentos e hérnias abdominais na área doadora. Este estudo vem propor uma técnica alternativa de reparo do defeito abdominal, com o objetivo de minimizar tais inconvenientes e proporcionar melhores resultados. MÉTODO: O estudo é de caráter retrospectivo, totalizando 120 mulheres submetidas à reconstrução mamária com TRAM com colocação de tela abdominal, e seguimento mínimo de seis meses, entre Maio de 2003 e Novembro de 2012. As pacientes foram divididas em dois grupos: Grupo 1 (53 casos) reconstruídos com tela de polipropileno posicionada entre os folhetos aponeuróticos dos músculos oblíquo externo e interno, e Grupo 2 (67 casos) realizado fechamento da linha arqueada de Douglas e o remanescente inferior da aponeurose do músculo reto abdominal e reforço com tela. RESULTADOS: As pacientes foram avaliadas pelo mesmo cirurgião, obedecendo aos mesmos critérios. Os resultados obtidos foram 18,9% de abaulamentos no grupo 1 e 3% no grupo 2, estatisticamente significativo. Abaulamentos e hérnias no hipogástrio variam de 20% a 44% e 12,5% a 20% respectivamente, quando fechados primariamente. Quando associado a aplicações de telas sintéticas, esses percentuais diminuem para 1,5% a 6%. O aprimoramento técnico e a comparação de resultados têm a finalidade de minimizar tais complicações. CONCLUSÃO: A técnica descrita mostra-se vantajosa, pois permitiu significativa diminuição da complicação mais comum, permitindo satisfação estética e funcional das pacientes.


INTRODUCTION: The transverse rectus abdominis muscle (TRAM) flap, described by Drever in 1977, was subsequently modified and since then, has rapidly become an important alternative for breast reconstructions. However, bulges and abdominal hernias in the donor area are some of the complications arising from the TRAM flap. This study proposes an alternative technique for the repair of the abdominal defect that minimizes complications and provides better results. METHOD: The retrospective included 120 women who underwent breast reconstruction with TRAM with placement of an abdominal mesh between May 2003 and November 2012, with follow-up of at least six months. Patients were divided into two groups: Group 1 (53 cases) underwent reconstruction with polypropylene mesh positioned between the aponeurotic layers of the external and internal oblique muscles and Group 2 (67 cases) underwent closure of the arcuate Douglas' line and the remaining lower aponeurosis of the rectus abdominis muscle and reinforcement with mesh. RESULTS: The patients were evaluated by the same surgeon, according to the same criteria. The rate of bulging was 18.9% in group 1 and 3% in group 2, a statistically significant difference. Bulges and hernias in the hypogastrium varied from 20% to 44% and 12.5% to 20% respectively, when closed primarily. When synthetic mesh was used, these percentages decreased to 1.5% to 6%. The technical improvement and comparison of results aims to minimize such complications. CONCLUSION: The technique described is advantageous in that, it significantly reduced the occurrence of the most common complication, and increased the aesthetic and functional satisfaction of the patients.

ABSTRACT

This study was aimed to collect relevant provisions of bulging disease medical records in the Ming and Qing dynasties to excavate the syndrome differentiation, drug law and medication frequency analysis. A database was established to analyze relevant provisions. The results showed that the syndromes of liver stagnation and spleen deficiency, dampness-heat accumulation, spleen-yang deficiency are the most common type. The mainly used drugs are from the category to eliminate dampness and water, and to tonify the deficiency. The top three used drugs are poria, atractylodes, and dried citrus peel. It was concluded that medications used in the Ming and Qing dynasties were mainly targeted to the disordered zangfu-organ, which was mainly about the spleen and the stomach. The syn-drome differentiation is the mixture of deficiency and excess. And deficiency is the main part. The medication should combine tonification and reducing. And tonification should be paid attention to. The syndrome differentiation and treatment should be coordinated and the primary and secondary aspect should be identified. These rules provide ref-erence effect for the clinical practice and scientific research of bulging disease treatment.

ABSTRACT

PURPOSE: To compare clinical outcomes after management with bed rest versus cerclage for treatment of amniotic sac bulging in the second trimester. METHODS: Women with cervical incompetence with membranes at or beyond a dilated external cervical os, before 27weeks of gestation, were treated with bed rest or emergency cerclage. We analyzed the pregnancy outcome retrospectively. 25 women underwent an emergency cerclege and 35 women underwent the bed rest. RESULTS: Gestational age at time of diagnosis was 22.40 weeks in the emergency cerclage and 22.39 weeks in the bed rest group. Mean interval from diagnosis until delivery was 8.65 weeks in the emergency cerclage group and 1.18 weeks in the bed rest group (p<0.001). Mean gestational age at delivery was 31 weeks in emergency cerclage group and 23.74 weeks in the bed rest group (p<0.001). Preterm delivery before 26 weeks and 34 weeks of gestation were significantly lower in the emergency cerclage group (p<0.001). Perinatal mortality was 17.4% in the emergency cerclage group and 48.6% in bed rest group (P=0.026). CONCLUSION: Emergency cerclage reduced preterm delivery before 26 and 34 weeks and improved perinatal outcome compared with bed rest treatment.

ABSTRACT

OBJECTIVE: To determine the changes of posterior bulging of the lumbar intervertebral discs with flexion and extension movement of the spine in patients with central disc bulges or disc degeneration. METHOD: Twenty patients with low back pain were studied. Nine patients had central type disc bulging and eleven patients had disc degeneration only. The spines were scanned in neutral, flexion, and extension positions in a vertically open 0.5T MR scanner. Degree of posterior bulging of the lumbar intervertebral disc of the pathological level was measured. RESULTS: In the patients with disc bulge, posterior bulging of the disc decreased in all of the patients by 0.8 0.6 mm with flexion of the spine and increased in 77.8% of the patients by 1.0 0.8 mm with extension of the spine. In the patients with disc degeneration, posterior bulging decreased with flexion in 36.7% of the patients. With extension, posterior bulging increased in 55.6% of the patients. CONCLUSION: This study found that patients with low back pain and central disc bulges have consistent and marked discrepancies in posterior bulging with flexion-extension in comparison with our previous study with asymptomatic patients with normal MRIs.

ABSTRACT

Two patients, bulging fetal membrane accompanied with cervical dilatation & effacement in the late second-trimester, were treated by modified Shirodkar method. Pregnancies were prolonged effectively and viable fetuses were delivered in both cases. Our cases may help make objective decision prior to attempting cerclage placement when cervix is markedly dilated and something should be done to save pregnancy.

ABSTRACT

For evaluating the lumbar disc space, the fact that the postmyelography computed tomography was more accurate than computed tomography or myelography is well-known. But the technical difficulty is limitation of gantry angle. The prone position scanning may help to lessen this trouble. A prospective computed tomographic scanning was performed in both the prone and supine position on 43 patients, who had low back pain. The angle between the vertebral end plate and vertical line was measured in both position at L3-4, L4-5 and L5-S1 interspaces. The advantages of prone scanning in spinal aligment was noted at L5-S1 disc space(79.1%) and also more desirable if computed tomography was performed with metrizamide. With above advantages, the evaluation of bulging annulus was became more easier.
