ABSTRACT

The incidence of obesity and overweight in the world has been increasing in recent years due to poor diet and lack of physical activity; people suffering obesity and overweight, related with malnutrition due to excess, often resort to calorie restriction diets that are usually not very effective. In this context, intermittent fasting (IF) has become popular due to the possibilities for weight loss that it offers. This diet consists of alternating periods of fasting with unrestricted eating; however, its effectiveness and consequences are unknown to most users. This narrative review analyzes whether intermittent fasting contributes to the improvement of body and metabolic composition. The purpose of the review was to examine the available data on the contribution of intermittent fasting to the improvement of body and metabolic composition, in order to provide information and to define the parameters that condition safe achievement of its benefits. IF dieting triggers adaptive cell responses that cause a decrease in lipid oxidative stress markers in individuals with obesity and prediabetes. Metabolic alterations have been found to go hand in hand with the alteration of circadian rhythms; if IF contributes to this effect, it may assist in treating and preventing obesity and associated diseases. However, there are also disadvantages, such as the loss of lean muscle mass by wasting, and increased hypoglycemia(AU)


La incidencia de obesidad y sobrepeso en el mundo ha ido en aumento en los últimos años debido a la mala alimentación y la falta de actividad física; Las personas que padecen obesidad y sobrepeso, relacionadas con la desnutrición por exceso, suelen recurrir a dietas de restricción calórica que suelen ser poco efectivas. En este contexto, el ayuno intermitente (AI) se ha popularizado debido a las posibilidades de pérdida de peso que ofrece. Esta dieta consiste en alternar períodos de ayuno con alimentación sin restricciones; sin embargo, su eficacia y consecuencias son desconocidas para la mayoría de los usuarios. Esta revisión narrativa analiza si el ayuno intermitente contribuye a la mejora de la composición corporal y metabólica. El objetivo de la revisión fue examinar los datos disponibles sobre la contribución del ayuno intermitente a la mejora de la composición corporal y metabólica, con el fin de aportar información y definir los parámetros que condicionan la consecución segura de sus beneficios. Se ha encontrado que las alteraciones metabólicas van de la mano con la alteración de los ritmos circadianos; si AI contribuye a este efecto, puede ayudar a tratar y prevenir la obesidad y las enfermedades asociadas. Sin embargo, también existen desventajas, como la pérdida de masa muscular magra por atrofia y el aumento de la hipoglucemia(AU)

Abstract The present study was designed to investigate the effects of Gundelia tournefortii L. plant extract on different tissues in terms of DNA damage, biochemical and antioxidant parameter values in rats with high-calorie diets. With this aim, Wistar albino male rats were divided into 4 groups containing 6 rats each and the study was completed over 12 weeks duration. At the end of the implementation process over the 12 weeks, rats were sacrificed and blood and tissue samples were obtained. Analyses were performed on blood and tissue samples. According to results for DNA damage (8-OHdG), in brain tissue the OG2 group was significantly reduced compared to the NC group. For MDA results in liver tissue, OG1 and OG2 groups were determined to increase by a significant degree compared to the control group, while the OG2 group was also increased significantly compared to the obese group. In terms of the other parameters, comparison between the groups linked to consumption of a high calorie diet (HCD) and administration of Gundelia tournefortii L. in terms of antioxidant activities and serum samples obtained statistically significant results. Gundelia tournefortii L. plant extracts had effects that may be counted as positive on antioxidant parameter activity and were especially identified to improve DNA damage and MDA levels in brain tissues. Additionally, consumption of Gundelia tournefortii L. plant extract in the diet may have antiobesity effects; thus, it should be evaluated for use as an effective weight-loss method and as a new therapeutic agent targeting obesity.


Resumo O presente estudo foi desenhado para investigar os efeitos do extrato da planta Gundelia tournefortii L. em diferentes tecidos em termos de danos ao DNA, valores de parâmetros bioquímicos e antioxidantes em ratos com dietas hipercalóricas. Com esse objetivo, ratos Wistar albinos machos foram divididos em 4 grupos contendo 6 ratos cada e o estudo foi concluído ao longo de 12 semanas de duração. No final desse processo de implementação, os ratos foram sacrificados e amostras de sangue e tecido foram obtidas. As análises foram realizadas em amostras de sangue e tecido. De acordo com os resultados para danos ao DNA (8-OHdG), no tecido cerebral o grupo OG2 foi significativamente reduzido em comparação com o grupo NC. Para os resultados de MDA no tecido hepático, os grupos OG1 e OG2 aumentaram significativamente em comparação ao grupo controle, enquanto o grupo OG2 também aumentou significativamente em comparação ao grupo obeso. Quanto aos demais parâmetros, a comparação entre os grupos ligados ao consumo de dieta hipercalórica (DC) e à administração de Gundelia tournefortii L. em termos de atividades antioxidantes e amostras de soro obteve resultados estatisticamente significativos. Os extratos de plantas de Gundelia tournefortii L. tiveram efeitos que podem ser considerados positivos na atividade dos parâmetros antioxidantes e foram especialmente identificados para melhorar os danos ao DNA e os níveis de MDA nos tecidos cerebrais. Além disso, o consumo de extrato vegetal de Gundelia tournefortii L. na dieta pode ter efeitos antiobesidade; portanto, deve ser avaliado para uso como um método eficaz de perda de peso e como um novo agente terapêutico voltado para a obesidade.

ABSTRACT Introduction: The long-term energy imbalance between intake and consumption is the main reason for obesity in college students. Therefore, weight loss should also combine an inversely proportional intervention. Based on this premise, a growing number of studies combine physical training and diet to restore physical health in this population group. Objective: Study the impacts of endurance training associated with a low-calorie diet on the constitution and metabolism of obese college students. Methods: Through a questionnaire survey and a 4-week experimental endurance training program and hypocaloric diet, 20 obese college students volunteered for the intervention project. The exercise protocol for the training composition and the diet menu followed the updated recommendations in the scientific literature. At the end of the experiment, the relevant data collected were statistically analyzed and discussed. Results: After the diet intervention associated with endurance training, the BMI index of college students reduced from 28,075 to 26,378; the basal metabolic rate increased from 1581,046 kcal to 1681,317 kcal. Conclusion: Endurance training associated with a hypocaloric diet significantly reduced fat in obese college students. The efficacy of this association promoted a better joint effect on the outcome, compared to individual weight loss plans. RESUMO Introdução: O desequilíbrio energético entre ingestão e consumo a longo prazo é o principal motivo da obesidade nos universitários, portanto, a perda de peso também deve combinar uma intervenção inversamente proporcional. Valendo-se dessa premissa, são crescentes os estudos que combinam treinos físicos e dietas com o propósito de restaurar a saúde física nessa parcela da população. Objetivo: Estudar os impactos do treino aeróbico associado a dieta hipocalórica sobre a constituição e o metabolismo de estudantes universitários obesos. Métodos: Através de uma pesquisa por questionário e um programa experimental com duração total de 4 semanas de treino aeróbico e dieta hipocalórica, 20 universitários obesos voluntariaram-se para o projeto de intervenção. O protocolo de exercícios para a composição do treino e o cardápio da dieta seguiram as recomendações atualizadas da literatura científica. Ao final do experimento, os dados relevantes coletados foram analisados estatisticamente e discutidos. Resultados: Após a intervenção da dieta associada ao treino aeróbico, o índice IMC de estudantes universitários reduziu de 28.075 para 26.378; o índice metabólico basal aumentou de 1581.046 kcal para 1681.317 kcal. Conclusão: O treino aeróbico associado à dieta hipocalórica demonstrou um efeito significativo na diminuição da gordura dos universitários obesos. A eficácia dessa associação promoveu um melhor efeito conjunto no resultado, em comparação a planos individuais de emagrecimento. Resumen Introducción: El desequilibrio energético entre la ingesta y el consumo a largo plazo es la razón principal de la obesidad en los estudiantes universitarios, por lo tanto, la pérdida de peso también debe combinar una intervención inversamente proporcional. Aprovechando esta premisa, cada vez son más los estudios que combinan entrenamiento físico y dietas con el fin de restablecer la salud física en esta parte de la población. Objetivo: Estudiar los impactos del entrenamiento aeróbico asociado a dieta hipocalórica sobre la constitución y el metabolismo de estudiantes universitarios obesos. Métodos: Mediante una encuesta con cuestionario y un programa experimental con una duración total de 4 semanas de entrenamiento aeróbico y dieta hipocalórica, 20 estudiantes universitarios obesos se presentaron como voluntarios para el proyecto de intervención. El protocolo de ejercicios para la composición del entrenamiento y el menú dietético siguieron las recomendaciones actualizadas de la literatura científica. Al final del experimento, se analizaron y discutieron estadísticamente los datos pertinentes recogidos. Resultados: Tras la intervención dietética asociada al entrenamiento aeróbico, el índice de IMC de los estudiantes universitarios se redujo de 28,075 a 26,378; el índice metabólico basal aumentó de 1581,046 kcal a 1681,317 kcal. Conclusión: El entrenamiento aeróbico asociado a una dieta hipocalórica mostró un efecto significativo en la reducción de grasa en estudiantes universitarios obesos. La eficacia de esta asociación promovió un mejor efecto conjunto sobre el resultado en comparación con los planes individuales de pérdida de peso. Resumen Introducción: Los edulcorantes no calóricos (ENC) se utilizan para sustituir azúcares en alimentos y bebidas. Para que este reemplazo sea beneficioso para la salud, la ingesta de cada ENC no de bería superar su ingesta diaria admisible (IDA). El objetivo principal de este estudio es determinar el consumo de aspartamo, acesulfame-K, sucralosa y sacarina en la población argentina de 15 a 65 años y evaluar este consumo en relación con la IDA. Otros objetivos son estratificar el consumo en función de distintas variables sociodemográficas y determinar las principales fuentes de ENC consumidas por la población argentina. Métodos: La muestra fue de 1266 individuos (población urbana 15-65 años), estratificada por región, edad, sexo y nivel socioeconómico. Los datos de ingesta fueron recolectados con dos recordatorios de 24 horas. Resultados: En Argentina, el consumo promedio de sacarina, aspartamo, acesulfame-K y sucralosa está muy por debajo de su respectiva IDA: 8.4%, 3.2%, 2% y 0.3% de la IDA respectivamente. Los consumos máximos reportados tampoco superan la IDA. Hay una mayor proporción de mujeres consumidoras de ENC. La proporción de consumidores de ENC aumenta con la edad. Las regiones noreste y sur presentan el menor porcentaje de consumidores de ENC. Las bebidas constituyen la principal fuente de ENC, seguidas por los edulcorantes de mesa. La contri bución de los alimentos al consumo de ENC es despreciable. Discusión: En Argentina existe un buen margen de seguridad para la reformulación de productos azucarados tendiente a disminuir el exceso de calorías y el consumo de azúcares añadidos.


Abstract Introduction: Non-caloric sweeteners (NCS) are used to replace added sugars in foods and bev erages. For this replacement to be a health benefit, the intake of each NCS should not exceed its Acceptable Daily Intake (ADI). The main objective of this study is to determine the consumption of aspartame, acesulfame-K, sucralose, and saccharin in the argentine population aged 15 to 65 years and to evaluate this consumption in relation to the ADI. Other objectives are to stratify the consumption based on different sociodemographic vari ables and to determine the main sources of NCS consumed by the argentine population. Methods: The sample consisted of 1266 individuals (urban population aged 15-65), stratified by region, age, sex, and socioeconomic level. Intake data was collected with two 24-hour recalls. Results: In Argentina, the average consumption of saccharin, aspartame, acesulfame-K and sucralose is well below their respective ADI: 8.4%, 3.2%, 2% and 0.3% of the ADI, respectively. The maximum reported consumptions do not exceed the ADI either. There is a higher proportion of women who consume NCS. The proportion of NCS consumers increases with age. The Northeast and South regions have the lowest percentage of NCS consumers. Beverages constitute the main source of NCS, followed by tabletop sweeteners. The contribution of food to NCS consumption is negligible. Discussion: In Argentina there is a good safety margin for the reformulation of sugary products aimed at reduc ing the excess calories and added sugars consumed by the population.

ABSTRACT

The advancement of industrialization and communications has made man more sensitive and critical, resulting in a variety of health issues. Sedentary lifestyles are becoming more common as a result of the progress of industrialization and communication, resulting in chronic non-communicable diseases such as diabetes, hypertension, cancer, ischemic heart disease, cerebro-vascular accidents, atherosclerosis, varicose veins, and so on. Obesity is a risk factor for several diseases, thus preventing it will reduce the likelihood of developing them. Obesity, like other disorders, is a complex disorder caused by a combination of factors such as food, physical activity, mental health, and overall lifestyle. In Ayurveda, Sthaulya is the closest clinical entity for obesity. Sthoulya's causality is described in the Asta-Nindita-Purusha chapter. The etiopathogenesis of obesity is caused by an excessive intake of calories combined with a decrease in expenditure. A key weight-loss approach that can be used anywhere to reduce energy density. In Ayurveda, Pathya and Apathya are given specific attention in terms of Ahara, Vihara, and Manasika Entities, so that the ailment can be treated holistically. When these guidelines are followed, the treatment yields a better result in the treatment of the condition. This article attempts to review the pathya and apathy of obesity, as well as discuss the possible causes.

ABSTRACT

Objective:To evaluate the effect of short-term very low-calorie restriction(VLCR) on glycemic control in overweight/obese patients with type 2 diabetes, and to explore mechanisms through identifying markers of gut microbiota.Methods:This trial was conducted in 14 adult overweight/obese patients with type 2 diabetes. They received VLCR for 9 days in the hospital(calorie intake 300-600 kcal/d). Before and after VLCR, body weight(BW), waist circumference(WC), blood pressure(BP), and heart rate(HR) were measured, and body mass index(BMI) was calculated according to their height and weight. Fasting blood glucose(FBG), 2 h postprandial blood glucose(2hPBG), fasting insulin(FINS), triglycerides(TG), total cholesterol(TC), high-density lipoprotein-cholesterol(HDL-C), and low-density lipoprotein-cholesterol(LDL-C) were determined, and yielded the homeostasis model assessment for insulin resistance(HOMA-IR). Additional lab tests such as liver and kidney function and electrolytes were performed. The estimated glomerular filtration rate(eGFR) was calculated to evaluate renal function. All data were analyzed using the SPSS Sample Power software. Feces samples were collected before and after VLCR. Fecal samples were tested for microbial diversity using 16S rDNA technology. Professional software was used to analyze the differences of gut microbiota in feces before and after VLCR.Results:After 9 days of VLCR, BW, BMI, WC, BP, HR, FBG, 2hPBG, FINS, HOMA-IR, alkaline phosphatase, TG, and blood urea nitrogen of 14 overweight/obese patients with type 2 diabetes were significantly reduced( P<0.05). No effect was seen on serum alanine aminotransferase, aspartate amino transferase, gamma glutamyl transferase, TC, HDL-C, LDL-C, creatinine, eGFR, uric acid, albumin, calcium, and phosphorus( P>0.05). The gut microbiota diversity did not differ before and after VLCR. The abundance of Bacteroidetes increased significantly, and the Firmicutes/Bacteroidetes ratio decreased from 11.79 to 4.20. Between groups analysis showed the abundance of Parabacteroides distasonis increased significantly after VLCR. Conclusion:VLCR can improve body weight and glucose and lipid metabolism in overweight/obese patients with type 2 diabetes, with no serious adverse events. Parabacteroides distasonis may be a marker of VLCR.

ABSTRACT

Objective:To explore the effects of low-calorie diet intervention and aerobic exercise intervention on insulin levels and body composition in obese patients with type 2 diabetes.Methods:A total of 300 obese patients with early type 2 diabetes who were diagnosed and treated in the Department of Endocrinology, Xuanwu Hospital of Capital Medical University from July 2016 to July 2019 were selected as the research objects. According to the random sampling method, they were divided into a control group and an observation group with 150 cases each. Calorie diet intervention was given to control group, while the observation group was given low-calorie diet intervention and concentrated aerobic exercise intervention. Observation and evaluation of fasting blood glucose (FBG), fasting insulin (FINS), homeostasis model assessment of insulin resistance (HOMA-IR), body mass index (BMI), body fat percentage, body weight, triacylglycerol (TG), total cholesterol (TC), high-density lipoprotein cholesterol (HDL-C), low-density lipoprotein cholesterol (LDL-C) were conducted.Results:After 4 weeks of intervention, FBG, FINS, HOMA-IR, BMI, body fat percentage, body mass, TG, TC, HDL-C, LDL-C indicators of the observation group were (6.15±0.92) mmol/L and (14.12±1.11) mU/L, 2.67±0.32, (25.01±1.75) kg/m 2, (27.45±1.92)%, (70.01±3.56) kg, (3.01±0.30) mmol/L, (5.25±0.88) mmol/L, (2.25 ±0.42) mmol/L, (3.15±0.41) mmol/L. The control group were (8.18±1.28) mmol/L, (16.78±1.85) mU/L, 3.78±0.78, (27.36±2.45) kg/m 2, (29.78±2.39)%, (72.98±5.62) kg, (3.49±0.52) mmol/L, (6.23±1.08) mmol/L, (1.88±0.30) mmol/L, (3.98±0.89)mmol/L. The difference between the two groups was statistically significant ( t values were 5.47-16.13, all P<0.05). After 8 weeks of intervention, FBG, FINS, HOMA-IR, BMI, body fat percentage, body mass, TG, TC, HDL-C, LDL-C indicators of the observation were (5.06±0.45) mmol/L, (12.78±0.69) mU/L, 2.01±0.12, (23.25±1.18) kg/m 2, (25.05±1.19)%, (66.02±2.45) kg, (2.21±0.12) mmol/L, (4.03±0.41) mmol/L, (3.08 ±0.72) mmol/L, (2.65±0.15) mmol/L,while in the control group were (6.07±0.88) mmol/L, (14.09±1.05) mU/L, 2.95±0.45, (26.98±2.08) kg/m2, (27.18±2.06)%, (70.98±4.02) kg, (2.98±0.28) mmol/L, (5.16±0.71) mmol/L, (2.41±0.51) mmol/L, (3.29±0.39) mmol/L. The difference between the two groups was statistically significant ( t values were 5.47-30.96, all P<0.05). Conclusions:Low-calorie diet intervention combined with concentrated aerobic exercise intervention is more conducive to improving patients′ blood sugar and blood lipid levels, as well as reducing body weight.

Abstract Bitter gourd (Momordica charantia L.) fruit is good source of many nutraceutical compounds and possess antioxidant, anti-diabetic and hypoglycaemic activities. However, its utilization in the preparation of beverages is limited due to its bitter after taste. Therefore, to realize the functional and therapeutic benefits of bitter gourd, an attempt was made to optimize nutritious and low caloriebitter gourd based beverage by blending with kiwifruit (Actinidia deliciosa), a store house of bioactive compounds and substituting sugar with stevioside (steviol glycoside). The standard (sugar sweetened) bitter gourd (BG)-kiwifruit (K) blended beverage was developed by utilizing 30% fruit part of BG:K blended juice (80: 20) with 40oB TSS and 1.3% acidity. Further, to develop the low calorie beverage, sucrose (table sugar) was replaced with 25, 50, 75 and 100% equi-sweetness level of stevioside (steviol glycoside). Results revealed that 75% substitution of sucrose with stevioside resulted in shelf stable beverage with identical taste, good antioxidant potential (68.80%) and strong antimicrobial activity (26 mm ZOI) with reduced calorie values (28.5 Kcal/100g) compared to the sugar sweetened control sample (150.60 Kcal/100g). Hence, the developed beverage can be commercialized as low calorie beverage with additional health benefits of natural compounds of bitter gourd and kiwifruit with highest bioactivity.

ABSTRACT

Objective To explore the formulation and adjustment of nutritional therapy by nutrition support pharmacists in patient with acute prerenal failure complicated with urinary tract infection, and to provide a reference for nutritional therapy in such patient. Methods With nutrition support pharmacists participated in nutrition treatment management and case analysis of a patient with acute prerenal failure complicated with urinary tract infection, we explored the nutritional support treatment plan for this type of patient. Results Nutrition support pharmacists provided pharmaceutical care throughout the course for patient with acute prerenal failure and designed an individualized low-calorie and high-protein nutritional support treatment according to the change of patient's condition, to increase the patient's serum albumin level and maintain at 35 g/L, at the same time the infection was effectively controlled and the creatinine value decreased to normal. Conclusion Small-volume, low-calorie, high-protein nutritional support solution ≤1 000 ml, calorie intake is about 1, 000 kcal, and protein is maintained at 1.2 g/(kg·d) throughout the course of acute prerenal renal failure with urinary tract infected patients can obtain better clinical outcomes and prognosis, and can better maintain clinical operability, and fully improve the safety, effectiveness and economics of nutritional support treatment.

ABSTRACT

Background@#Patients admitted due to severe COVID-19 pneumonia are at high risk for malnutrition and worsening of their clinical condition. Patients with type 2 diabetes admitted for COVID-19 pneumonia have an increased risk for poor clinical outcomes. Adequate nutrition is recommended to augment a strong immune response. The American Society of Parenteral and Enteral Nutrition (ASPEN), Philippine Society of Parenteral and Enteral Nutrition (PHILSPEN), European Society of Parenteral and Enteral Nutrition (ESPEN) recommend an energy intake goal of 15-20 kcal/kg actual body weight (ABW) per day or 70-80% of caloric requirements after the acute phase of critical illness, with recommended protein intake of 1.2-2.0 g/kg ABW per day. This study aims to provide an association between calorie and protein intake with negative clinical outcomes.@*Methods@#This is a retrospective cohort study of 55 mechanically ventilated SARS COV-2 RT-PCR positive patients admitted in the critical unit of Chinese General Hospital between April 1, 2020 to December 30, 2020. Clinical profile taken include: sex, age, height weight, BMI, comorbidities, and components of the modified SOFA score and APACHE II score. Calorie and protein intake from day 3 to day 7 of ICU admission were taken. Clinical outcome data were in-hospital mortality, number of days of ICU stay, hospital admission, vasopressor use and mechanical ventilation.@*Results@#Majority of the patients included in the study were male, elderly, overweight, and with comorbidities such as hypertension, diabetes, and chronic kidney disease. Majority of the patients were on mechanical ventilation and on vasopressors for more than 1 week. In-hospital mortality accounted for 65.5% of cases. For patients without diabetes, calorie intake was 16.9 kcal/kg/day and protein intake was 0.72 g/kg/day, while patients with diabetes had a calorie intake of 20.2 kcal/kg/day and protein intake of 0.86 g/kg/day. Based on the 70% cutoff, patients with diabetes were noted to have been provided with more adequate protein (P-value= 0.027). Higher caloric intake was inversely associated with in-hospital mortality among patients younger than 75 years old (P-value=0.026) and among patients with diabetes (P-value=0.003). Higher calorie intake was also significantly associated with decreased duration of pressors among patients with diabetes (P-value=0.021). Higher protein intake positively associated with the number of days admitted among patients with lower modified SOFA scores (P-value=0.041) and among patients with diabetes (P-value=0.021). All other associations did not display significant results (all P-values>0.05).@*Conclusion@#Based on this study, increased caloric intake was associated with increased survival among patients less than 75 years, but no association was found in patients 75 years and older. Among patients with lower mortality risk, increased protein intake was associated with longer duration of hospital stay, however patients with higher risk had higher in-hospital mortality regardless of protein and calorie intake. Among patients with diabetes, higher calorie intake was associated with increased survival and decreased duration of pressor requirement, while increased protein was associated with longer length of hospital stay. Treatment for COVID-19 pneumonia, however, was not established at this time and there was significant in-hospital mortality among these mechanically ventilated patients.

ABSTRACT: Based on the data of "employment and food demand of urban migrant workers", this paper empirically analyzed the impact of urban pension insurance on the nutritional intake structure of migrant workers. The results showed that participating in urban pension insurance can change the nutritional intake structure of migrant workers. Additionally, fat and protein replace carbohydrate as the main nutrition sources for migrant workers. After controlling the income and labor intensity of migrant workers and other factors, urban pension insurance has a positive effect on the intake of fat and protein of migrant workers for they increase by 13.5% and 8.8% respectively. There is no significant effect on the intake of carbohydrates of migrant workers. The calorie intake of migrant workers increases by 6.8% accounting for the change of nutritional intake structure. Endogenous and robustness tests showed that the above conclusions are robust. Heterogeneity analysis showed that there is no significant difference in the effect of urban pension insurance on calorie intake of migrant workers in different income levels and age groups.


RESUMO: Este artigo usa os dados da pesquisa de "Emprego e demanda alimentar dos trabalhadores migrantes urbanos" para analisar empiricamente o impacto do seguro patrimonial urbano na estrutura de ingestão nutricional dos trabalhadores migrantes. O estudo constatou que: o seguro-pensão urbano mudou a estrutura de ingestão nutricional dos trabalhadores migrantes. Gordura e proteína substituíram os carboidratos e se tornaram a principal fonte de nutrição dos trabalhadores migrantes. Depois de controlar fatores como a renda e a intensidade do trabalho dos trabalhadores migrantes, o seguro de pensão urbana aumentou significativamente a ingestão de gordura e proteína dos trabalhadores migrantes em 13,5% e 8,8%, respectivamente, enquanto a ingestão de carboidratos dos trabalhadores migrantes não foi afetada. Significativamente afetado. A transformação da estrutura de ingestão nutricional dos trabalhadores migrantes aumentou sua ingestão calórica em 6,8%. Os testes de endogenia e robustez mostram que as conclusões acima são robustas. A análise de heterogeneidade mostra que não há diferença significativa no efeito do seguro previdenciário urbano sobre o aumento da ingestão de calorias para trabalhadores migrantes de diferentes níveis de renda e diferentes grupos etários.

Resumo Fundamento A influência de intervenções não farmacológicas como restrição calórica e exercício físico sobre a saúde e prevenção de enfermidades cardíacas tem sido documentada em estudos clínicos e experimentais. Objetivo Analisar a influência da combinação entre dieta intermitente e exercício físico sobre a capacidade funcional, metabolismo glicêmico e remodelação cardíaca. Métodos Foram utilizados 60 ratos Wistar machos distribuídos em quatro grupos: Controle (C), Exercício Físico (EF), Dieta Intermitente (DI) e Exercício Físico e Dieta Intermitente (EDI). Durante 12 semanas, enquanto C e EF foram tratados diariamente com dieta comercial padrão ad libitum, DI e EDI receberam dieta similar em dias alternados com dias de jejum. Os grupos EF e EDI foram submetidos a protocolo de corrida em esteira rolante. Posteriormente, foram analisadas capacidade funcional, comportamento nutricional e metabolismo glicêmico. Além da morfologia do coração, a expressão proteica das proteínas extracellular signal-regulated kinase (ERK) e c-Jun N-terminal kinase (JNK) no coração foi avaliada por Western-blot. A análise dos resultados foi feita por meio de Two-Way ANOVA e teste de Student-Newman-Keuls. O nível de significância considerado foi de 5%. Resultados O exercício físico aumentou a capacidade funcional nos grupos EF e EDI, e acarretou fibrose cardíaca. A combinação entre dieta intermitente e exercício físico resultou em menor área sob a curva de glicemia e menores medidas de área e interstício cardíaco no EDI em relação ao EF. A expressão de proteínas ERK e JNK foi similar entre os grupos (p>0,05). Conclusões Dieta intermitente se associa com melhor tolerância glicêmica e atenua o processo de remodelação cardíaca decorrente do exercício físico. (Arq Bras Cardiol. 2020; 115(2):184-193)


Abstract Background The effects of non-pharmacological interventions such as calorie restriction and exercise training on health and prevention of cardiovascular diseases have been investigated in clinical and experimental studies. Objective To analyze the influence of intermittent fasting and exercise training on functional fitness, glycemia and cardiac remodeling. Methods Wistar rats (n=60) were randomly divided into four groups: control, exercise training (ET), intermittent fasting (IF) and exercise training plus intermittent fasting (ETI). Over 12 weeks, control and ET animals were fed daily a standard commercial diet ad libitum , while IF and ETI animals were fed every other day. In addition, the ET and ETI groups were submitted to a running protocol on a treadmill. After this period, functional fitness, nutritional parameters and blood glucose levels were analyzed. In addition to heart morphology, myocardial protein expression of extracellular signal-regulated kinase (ERK) and c-Jun N-terminal kinase (JNK) was assessed by Western-blot. The results were analyzed using two-way ANOVA and Student-Newman-Keuls test. The level of significance considered was 5%. Results Exercise training increased functional fitness in the ET and ETI groups and promoted cardiac fibrosis. The combination of intermittent fasting and exercise training resulted in a smaller area under the blood glucose curve and reduced cardiomyocyte cross-sectional area and interstitial collagen fraction in the ETI group compared to ET. ERK and JNK expression levels were similar among groups (p>0.05). Background: This study was conducted to assess the role of Partial Parenteral Nutrition (PPN) and a specially prepared 'mixed fluid formula' used as PPN in sick surgical neonates.Methods: This single institution based Randomised Control Trial was performed in Surgical NICU of a tertiary care Centre. Surgical neonates who required bowel rest for >3 days like Esophageal Atresia (22), Duodenal Atresia (38), Jejuno-Ileal Atresia (50), Necrotising Enterocolitis (29) and others (11), were chosen. As a CONTROL population half (75) neonates were allowed maintenance fluid only (Isolyte P) and rest (75) were given a combination of maintenance fluid (Isolyte P) and PPN (specially prepared mixed fluid). This mixed fluid each 100 ml was prepared with 65ml Isolyte P, 15 ml 25% Dextrose, 10 ml Normal Saline, 7 ml Astymin 3, 2 ml Multivitamin and 1 ml Injection KCl. Fluid was continued till enteral feeding was established.Results: Neonates according to their body weight were divided in three groups <1 kg (50), 1-2.5 kg (71) and >2.5 kg (29) and their post-operative outcome was assessed as Clinical Positive/Negative and Laboratory Positive/Negative. Results were separately assessed in 3 body weight groups under 'Control' and 'Study' which showed both Clinical and Biochemical improvement in 'Study group'.Conclusions: The composition of mixed fluid, its nutritional value, calorie supplementation, compatible osmolarity for peripheral venous administration and most importantly its easy and cost-effective preparation were assessed thoroughly and we recommend this 'mixed fluid' preparation as a very useful PPN for sick surgical neonates.

This study aimed to evaluate changes in beige adipocytes at different times of melatonin administration, in the morning (ZT01) or in the evening (ZT11), at 30 mg/kg daily by gavage for 7 weeks or continuously with drinking water in the term of high-calorie diet-induced obesity (HCD). Melatonin received at ZT11 or with drinking water resulted in an increased area of the browning zone in the subcutaneous white adipose tissue (sWAT), even in rats with HCD (compared with Control or HCD, respectively). The beige adipocyte and lipid droplet area after melatonin use were reduced compared to those with HCD and Control, in all administration modes (group ZT01 showed smaller changes compared to ZT11 or with drinking water groups). The fibrosis level decreased and significantly differed in HCD ZT01, HCD ZT11, and HCD water compared to that in HCD; moreover, the lowest value determined in HCD water, reached the control parameters. Furthermore, the IL-1b and IL-8 level was decreased in the HCD groups under melatonin treatment at ZT11 or with drinking water compared to that in HCD. The obtained results suggest that melatonin promotes sWAT browning in rats with diet-induced obesity and influences morphological signs of normal rats depending on the time of administration. Different functional activity of beige adipocytes was observed after melatonin was used depending on the time of administration, resulting in heat production and lipolysis (the relative mass of visceral fat was likewise diminished). More rapid browning was observed when melatonin treatment was performed at 1 h before lights-off (ZT11) or continuously via drinking water. Melatonin acted on beige adipocytes of obese rats through changing some parameters such as the area of adipocytes and lipid drops, the number of lipid drops, the relative area browning of sWAT, and the level of tissue fibrosis.


Este estudio tuvo como objetivo evaluar los cambios en los adipocitos beige en diferentes momentos de la administración de melatonina, en la mañana (ZT01) o por la noche (ZT11). Se administraron 30 mg/kg diariamente por sonda durante 7 semanas o continuamente con agua potable durante el periodo de obesidad inducida por una dieta alta en calorías (HCD). La melatonina recibida en ZT11 o con agua potable resultó en un aumento de área dorada en tejido adiposo blanco subcutáneo (sWAT), incluso en ratas con HCD (en comparación con Control o HCD, respectivamente). El área de gotas de lípidos y adipocitos de color beige después del uso de melatonina se redujo en comparación con aquellos con HCD y Control, en todos los modos de administración (el grupo ZT01 mostró cambios más pequeños en comparación con ZT11 o con grupos de agua potable). El nivel de fibrosis disminuyó y difirió significativamente en HCD ZT01, HCD ZT11 y agua HCD, en comparación con el HCD; además, el valor más bajo determinado en agua HCD alcanzó los parámetros de control. Además, el nivel de IL-1b e IL-8 disminuyó en los grupos HCD bajo tratamiento con melatonina en ZT11 o con agua potable en comparación con el de HCD. Los resultados obtenidos sugieren que la melatonina promueve el dorado sWAT en ratas con obesidad inducida por la dieta e influye en los signos morfológicos de las ratas normales dependiendo del momento de la administración. Se observó una actividad funcional diferente de los adipocitos de color beige después de usar melatonina dependiendo del tiempo de administración, dando como resultado la producción de calor y lipólisis (la masa relativa de grasa visceral también disminuyó). Se observó un ennegrecimiento más rápido cuando el tratamiento con melatonina se realizó 1 h antes de apagar las luces (ZT11) o de forma continua en grupos de agua potable. La melatonina actuó en los adipocitos beige de ratas obesas al cambiar algunos parámetros, como el área de adipocitos y gotas de lípidos, el número de gotas de lípidos, el área relativa de ennegrecimiento de sWAT y el nivel de fibrosis tisular.

Resumen Los edulcorantes son sustancias capaces de sustituir el dulzor característico del azúcar y en la actualidad son muy utilizados por su capacidad edulcorante, superior a la de la sacarosa, lo que hace que se requieran menos cantidades de estos en los alimentos. Esta característica ha despertado el interés de muchos investigadores por sus posibles beneficios para pacientes con diabetes y obesidad, entre otras morbilidades. En este artículo se hace una revisión acerca de la relación de los edulcorantes no calóricos con el control del peso, el síndrome metabólico y la aparición de eventos cardiovasculares.


Abstract Sweeteners are substances capable of replacing the characteristic sweetness of sugar. They are currently widely used for their sweetener capacity, which is greater than that of saccharose, and which means lower quantities are required in foods. This characteristic has aroused the interest of many researchers due to their possible benefits in diabetes and obesity, as well as in other disorders. A review is presented in this article on the relationship between calorie-free sweeteners and weight control, metabolic syndrome, and the appearance of cardiovascular events.
