ABSTRACT

[Purpose] This study is a case report of an acupuncturist's new evaluation index for acupuncture treatment aimed at reducing or eliminating cardiac rehabilitation inhibitory factors in home medical care due to exacerbation of heart failure. [Method] Acupuncture was evaluated by heart rate variability, respiratory sinus arrhythmia, and saturation pulse O2. [Results] In severe heart failure in the evaluation index used in this study, there was no significant change in the autonomic function of the heart before and after acupuncture stimulation.[Discussion] In order for acupuncture to be accepted as a member of team medical care in the future, acupuncturists need to share objective information for multi-professional collaboration.

ABSTRACT

Atrial fibrillation (AF) is the most common arrhythmia in clinical work, accounting for about 1/3 of patients in hospital due to arrhythmia. Recently, more and more studies have shown that cardiac autonomic nerve and its remodeling are involved in the occurrence and maintenance of AF, which is one of the important mechanisms of AF. At present, the treatment methods of AF include drug therapy and radiofrequency ablation. As we all know, drug therapy has many limitations. In this paper, the mechanism of sympathetic, parasympathetic nerve and AF and the changes of cardiac autonomic nerve and its remodeling area before and after the occurrence of AF were studied, so as to explore a better treatment method of AF, and then simplify the tedious steps of radiofrequency ablation, reduce the scope of ablation, more accurately locate ectopic trigger point and vulnerable matrix, reduce the intensity of ablation, improve the success rate of operation and obtain good economic benefits.

ABSTRACT

Objective To explore the state of cardiac autonomic nerve function in schizophrenia patients with metabolic syndrome and analyze its influence factors and to reduce the occurrence of sudden cardiac death of the patients. Methods A total of 168 cases of patients according with the ICD-10 schizophrenia diagnostic criteria were divided into group A(78 cases of schizophrenia with metabolic syndrome),group B(90 cases of schizophre-nia without metabolic syndrome)and another 92 normal cases were included as control group(group C). Twenty-four hours dynamic electrocardiogram was conducted respectively and the heart rate variability (HRV) was ana-lyzed. Results Values of LF,HF,SDNN,SDANN,RMSSD and PNN50 in group A and group B were much low-er than those in group C and they were statistically significant(P < 0.05). Values of LF,HF,SDNN,SDANN, RMSSD and PNN50 in group B were much higher than those in group A and they were statistically significant(P<0.05). Excluding the influence of schizophrenia ,logistic regression analysis showed that the influence factors of HRV values were course of the disease(OR = 1.864,95%CI 1.110~3.128),age(OR = 1.170,95%CI 1.018~2.491),types of antipsychotic drugs(OR=2.419,95%CI 0.976~1.835),abdominal obesity(OR=2.425,95%CI 1.347~4.366),blood pressure value(OR=1.263,95%CI 1.575~3.937),blood glucose value(OR=3.376, 95%CI 1.359~2.923)and blood lipid value(OR = 2.178,95%CI 1.492~6.756). Conclusions Schizophrenia patients with metabolic syndrome have obvious cardiac autonomic nerve dysfunction. Excluding the influence of schizophrenia itself,the other possible risk factors include course of the disease,age,types of antipsychotic drugs,abdominal obesity,blood pressure value,blood glucose value and blood lipid value.

ABSTRACT

Background:Cigarette smoking induced increased sympathetic activity is one of the major independent risk factor for cardiac morbidity and mortality. Objective:To assess acute effects of smoking on neuro cardiovascular regulation by analysis of time domain measures of HRV in current regular healthy male cigarette smoker. Methods:This comparative analytical study was conducted in the Department of Physiology, Bangabandhu Sheikh Mujib Medical University (BSMMU), Shahbag, Dhaka from July 2011 to June 2012. 120 apparently healthy male current regular cigarette smoker aged 20-55 years were participated in the study group. Age and BMI matched 70 apparently healthy male non smoker subjects were studied as control. To observe the acute effects data were recorded 5 and 30 min after finishing a cigarette. Time domain measures of HRV were recorded by a RMS digital polyrite D. Statistical analysis was done by independent sample t test and paired sample t test.Results: Resting pulse rate, SBP, DBP, mean heart rate were significantly higher (p<0.001) and mean R-R interval, SDNN, RMSSD and total power were significantly lower (p<0.001) in all smokers in comparison to those of healthy control.In addition, all time domain parameters were significantly decreased from their corresponding baseline value just 5 minutes after smoking and returned close to their baseline value after 30 minutes of smoking but it remained significantly lower than control value. Conclusion: The result of this study concludes that cigarette smoking had acute effect on cardiac autonomic function causing depressed vagal activity and overbalance of sympathetic function..

ABSTRACT

Aim To investigate the preventive effect of Guizhi decoction on cardiac sympathetic sprouting induced by 4-Methylcatechol (4-MC) . Methods The rat models of cardiac sympathetic sprouting were in-duced by 10 mg · L-1 4-MC ( 10μg · kg-1 body weight, i. p. ) . Guizhi decoctions ( with different pro-portion between Ramulus Cinnamomi and Radix Paeon-iae Alba, 1 : 1, 1 : 2 and 2 : 1) and metoprolol were administered to the rats. Heart rate and electrocardio-gram ( ECG ) were observed, the content of norepi-nephrine (NE), growth associated protein (GAP-43), tyrosine hydroxylase ( TH ) and acetylcholine transfer-ase enzyme ( CHAT) in myocardial homogenate of left ventricular and right atrial were determined by ELISA method, and immunofluorescence assay was used to observe cardiac nerve sprouting and sympathetic distri-bution. Results 4-MC caused cardiac sympathetic sprouting and parasympathetic was not influenced. Heart rate of the model group was improved significant-ly and higher than that of the other groups . Compared with the model group, the content of NE, GAP-43 and TH in left ventricle and right atrium of the metoprolol group and Guizhi decoction group were decreased( P<0.05 ) , and the immunofluorescence result showed that the distribution of TH positive nerve was reduced sig-nificantly(P<0.05). It was demonstrated that Guizhi decoction of the proportion between Ramulus Cinnamo-mi and Radix Paeoniae Alba 1 : 1 and 1 : 2 had the best efficacy, which was similar to the efficacy of meto-prolol. Conclusion Guizhi decoction ( with the pro-portion between Ramulus Cinnamomi and Radix Paeon-iae Alba 1 : 1 and 1 : 2 ) effectively inhibits the cardi-ac sympathetic sprouting induced by 4-MC.
