ABSTRACT

【Objective】 To analyze the causes of chylemia, explore the effects of varying degrees of chylemia on subsequent blood products and the countermeasures. 【Methods】 The information of blood donors in the Central Blood Station of Xinjiang Production and Construction Corps from January 2018 to July 2022 were analyzed retrospectively, and the causes of fatty blood discarding was analyzed in terms of gender, age and times of donation. The effects of chylemia of control group, mild group, moderate group and severe group and corresponding gender and age on the hemolysis rate of suspended red blood cells(RBCs) at the end of storage and residual amount of methylene blue in the inactivated plasma were discussed. 【Results】 The incidence of chylemia in males(0.93%,84/9 047) was much higher than that in females(0.24%,9/3 781) (P<0. 05). The hemolysis rates of WBC-free suspended RBCs of mild, moderate and severe chylemia group at the end of storage(0. 19±0.08,0.28±0.06,0.66±0.25) were higher than that in control group(0.08±0.03), in males significantly higher than those in females(0.08±0.03 vs 0.06±0.02,0.21±0.09 vs 0.15±0.04,0.30±0.05 vs 0.24±0.07), in the age group of 18-30 years significantly lower than those in 31-41 years(0.07±0.02 vs 0.09±0.03) and ≥41 years(0.07±0.02 vs 0.09±0.03), P <0.05. The residual methylene blue in plasma with viral inactivation prepared from mild and moderate chylemia was significantly higher than that in control group(0.12±0.02,0.18±0.06 vs 0.06±0.02) (P<0.05). 【Conclusion】 Chylemia shall be monitored during the whole process of blood collection, and coutermeasures shall be taken to decrease the effects of chylemia on blood quality.

ABSTRACT

Objective Comparison the corrective effect of plasma exchange method and formula method for chylemia on hematology analysis. Methods A total of 10 samples without hemolysis, jaundice and lipemia were set as control group, each sample was divided into 4 parts, then 5,10,20 and 40 μl lipid emulsion was added. Each chylous samples was treated by plasma exchange method for two times, and routine blood test was reanalyzed with hematology analyzer. The comparisons before and after the exchange was made, while the test results on each exchange were analyzed. For another part, the HGB of chylous plasma was tested, the value was substituted into HGBcorrected value =HGBbefore correction-(HGBchylous plasma-HGBchylous plasma ×HCTbefore correction). Results The chylemia could lead significant increase of HGB,MCH and MCHC (P<0.05).After plasma exchanging for two times, the three parameters returned to normal and the count of WBC,RBC and PLT decline slightly with no significance. There were no differences between plasma exchange method and formula calibration method. Conclusion Plasma exchange method and formula calibration method could significantly reduce the impact of chylemia on routine blood analysis, which facilitate the clinical work with correct analysis of routine blood test.

ABSTRACT

Objective To evaluate the clinical application value of influence of chylemia specimen before and after blood plasma exchange on the detection results of hematological analyzer. Methods 22 chylemia specimens were collected, then the hematological analyzer was used to test the chylemia specimens before and after blood plasma exchange, and the results were compared with those detected by the manual microscopic counting. Results The detection results of WBC,NEU,LY,MO,RBC,HGB and PLT in the non-chylemia specimens and the original blood specimens before blood plasma exchange had no statistically significant differences (P >0.05);in 22 cases of chylemia specimens, the WBC count and classification, and the mean value of HGB and PLT before and after blood plasma exchange in the chylemia specimens had statistically significant differences (P <0.05),the WBC count and clas-sification,and PLT mean value after blood plasma exchange in the chylemia specimens by the hematological analyzer had good corre-lation with those detectd by the microscopic counting method(r2 =0.936,r2 =0.918).In the triglyceride interference test, the WBC count and classification, and the mean value of HGB and PLT could be changed with the increase of the triglyceride concentration. Conclusion Adopting the blood plasma exchange method could obviously improve the influence of chylemia specimens on the detec-tion results by the hematological analyzer and provide the accurate results for clinic.

ABSTRACT

Objective To explore the key parameters of eliminating the interference of chyle by adopting the high-speed centrifu-gation technique. Methods To establish the chylemia by adding chylomicrons to background serum, centrifuge the same model un-der five different parameters, compare the biochemical indicators of lower serum with background by matched t-test,and to analyze the relative bias of the experimental group with statistical differences. Results Compared with the background serum, for ALT, AST,TP,ALB,ALP,GGT and Fe,the correlation coefficient was higher and the differences had no statistical significance(P >0.01 )when centrifuging for 20 min by RCF 29 703 g;for CK and CHE,the correlation coefficient was higher and the differences had no statistical significance(P >0.01)when centrifuging for 15 min by 20 672 g;for Zn,the correlation coefficient was higher and the differences had no statistical significance(P >0.01)when centrifuging for 5 min by 9 168 g.For Glu,LDH,CRE,Urea,UA,Ca and P,all the experimental groups had statistically significant differences(P <0.01),but the relative bias in each group were within the acceptable range. For Ca and P,the correlation coefficient was higher when centrifuging for 15 min by 20 672 g.Conclusion It is recommended to eliminate the interference of chyle on ALT,AST,TP,ALB,ALP,GGT,Fe,Glu,LDH,CRE,Urea and UA by u-sing the centrifugal parameters(29 703 g,20 min),on CK and CHE by using the centrifugal parameters(20 672 g,15 minutes)and on Zn,Ca and P by using the centrifugal parameters(9 168 g,5 minutes).
