ABSTRACT

La lepra o enfermedad de Hansen es una infección crónica causada por Mycobacterium leprae (M. leprae) o Mycobacterium lepromatosis (M. lepromatosis), es una enfermedad infecciosa curable que es endémica en más de 140 países de todo el mundo. La Organización Mundial de la Salud lo declaró eliminado como un problema mundial de salud pública en el año 2000, sin embargo, en 2017 se informaron aproximadamente 200 000 casos nuevos en todo el mundo. A pesar de la terapia farmacológica combinada disponible, continúa como un importante problema de salud pública, que conlleva un fuerte estigma. En Cuba, según el Anuario Estadístico de Salud de 2020, se reportaron durante ese año 116 casos, las provincias más afectadas fueron Santiago de Cuba con 25 nuevos casos y Guantánamo con 17 nuevos casos. La presentación de caso se trata de una joven de 20 años, en edad reproductiva, con manifestaciones generales de astenia, pérdida del cabello, adenopatías, poliartritis, lesiones cutáneas y fiebre, que hizo sospechar en una enfermedad del colágeno. En estos momentos con buena evolución y en seguimiento por atención primaria de salud de su provincia(AU)


Leprosy or Hansen's disease is a chronic infection caused by mycobacterium leprae (M. leprae) or mycobacterium lepromatosis (M. lepromatosis). It is a curable infectious disease, endemic in more than 140 countries around the world. Despite being declared eradicated as a global public health problem by the World Health Organization in 2000, approximately 200,000 new cases were reported worldwide in 2017. Despite available combination drug therapy, it remains a major public health problem, carrying strong stigma. In Cuba, according to 2020 health statistical yearbook, 116 cases were reported during that year. The most affected provinces were Santiago de Cuba (25 new cases) and Guantánamo (17 new cases). The case to be reported concerns a 20-year-old young woman of reproductive age, with general manifestations of asthenia, hair loss, adenopathies, polyarthritis, skin lesions, and fever; she suspected was a collagen disease. Currently, she is well and she is followed up by primary health care in her province(AU)

ABSTRACT

Pancreatic cancer is one of the most malignancy tumor in digestive system with extraordinarily poor prognosis owing to its difficulty in early diagnosis and complex treatments.Recently,great progress has been made in surgical techniques,adjuvant treatment and perioperative management of pancreatic cancer.However,the long-term survival of patients with pancreatic cancer has not been significantly improved. American Society of Clinical Oncology released clinical practice guidelines for potentially curable pancreatic cancer in 2016 and updated it for the first time in 2017.In2019,American Society of Clinical Oncology renewed its guidelines in order to modify the program of postoperative adjuvant therapy for pancreatic cancer and recommend FOLFIRINOX as the first choice for postoperative adjuvant therapy for patients with pancreatic cancer in order to improve the prognosis of patients with pancreatic cancer.

ABSTRACT

Why are some dental practices more successful than others? Skill, though undoubtedly important is not the complete answer.Simply put, a successful practice is directly proportional to the number of satisfied patients. And for a patient to be satisfied, it his needs that have to be met – voiced and unvoiced! Thus to be able to deliver effective dental care, needs as perceived by the patient and dentist need to be in sync. But to achieve this first and foremost a dentist must acknowledge the existent gulf that exists between professional and lay perception of needs and then proceed to bridge the gap between the two. This article looks at various ways in which the science of psychology can aid the dentist overcome this ubiquitous impasse. Psychology helps not only in assessing the underlying concerns of a patient but also in addressing them. And this in turn lays the foundation for a long lasting dentist-patient relationship which is what lies at the heart of any successful practice!

ABSTRACT

Sexually transmitted infections (STIs) represent a public health problem worldwide. The aim of this study was to investigate the prevalence of curable STIs caused by Neisseria gonorrhoeae, Chlamydia trachomatis and Ureaplasma urealyticum in female sex workers in a population from Zulia State, Venezuela. Seventy eight (78) women attended a health monitoring sanitary controls were evaluated, and PCR amplification assays were used to detect the three microorganisms in endocervical samples. In 33.3% of the samples, at least one microorganism was detected: U. urealyticum was found more frequently (25.6%), followed by N. gonorrhoeae (18%), and C. trachomatis (12.8%). A significant association between N. gonorrhoeae and C. trachomatis was found (p <0.0001). STIs cases represented 42.9% and 28% for symptomatic and asymptomatic groups, respectively. In the symptomatic group, N. gonorrhoeae was 2.4 times (28.6%) more frequent than in the asymptomatic one (12%) (p=0.015), particularly associated with mucopurulent discharge (p=0.025). No association was found between C. trachomatis (p=0.078), and U. urealyticum (p=0.432) with clinical manifestations. Prevalence of curable STIs in the study population was relatively low compared with other high-risk populations worldwide. The results support the possible association between C. trachomatis and N. gonorrhoeae, therefore, treatment would be indicated against both pathogens when one of them is detected in vulnerable populations.


Las infecciones de transmisión sexual (ITS) representan un problema de salud pública a nivel mundial. El objetivo de este estudio fue investigar la prevalencia de ITS curables causadas por Neisseria gonorrhoeae, Chlamydia trachomatis y Ureaplasma urealyticum en trabajadoras sexuales de una población del estado Zulia, Venezuela. Se evaluaron 78 mujeres que asistieron a jornadas de control sanitario y se utilizaron ensayos de amplificación por PCR para detectar los tres microorganismos en muestras endocervicales. En 33,3% de las muestras, se detectó al menos un microorganismo: U. urealyticum fue encontrado con mayor frecuencia (25,6%), seguido de N. gonorrhoeae (18%) y C. trachomatis (12,8%). Se encontró asociación significativa entre N. gonorrhoeae y C. trachomatis (p<0,0001). Los casos de ITS representaron porcentajes de 42% y 28% para los grupos sintomático y asintomático, respectivamente. N. gonorrhoeae fue 2,4 veces más frecuente en el grupo sintomático (28,6%) que en el asintomático (12%) (p=0,015), particularmente asociado con secreción mucopurulenta (p=0,025). No se encontró asociación entre C. trachomatis (p=0,078) y U. urealyticum (p=0,432) con manifestaciones clínicas. La prevalencia de ITS de la población en estudio fue relativamente baja comparada con otras poblaciones de alto riesgo a nivel mundial. Los resultados apoyan la posible asociación entre C. trachomatis y N. gonorrhoeae, por lo tanto se debería considerar el tratamiento contra ambos patógenos, cuando uno de ellos sea detectado en poblaciones vulnerables.

ABSTRACT

Microcystic meningioma, a distinct morphological variant of meningiomas, is histologically characterized by a vacuolated appearance with multiple cystic spaces lined by vacuolated or stellate-shaped tumor cells. This unusual variant was originally described by Masson, who labeled it "humid". The clinical and morphological findings point toward a benign course. And these unusual morphological variants of meningioma are potentially curable tumor. But there is no definitive method for preoperatively differentiating microcystic meningioma from the malignant glioma. We report a case of microcystic meningioma occurring in the left frontal area of 74 year old woman, with malignant clinical nature.

ABSTRACT

The authors report unusual three cases of large, peri- or intratumoral cystic lesions associated with intracranial meningiomas. These meningiomas may lead to an incorrect presumptive diagnosis because of minic appearance of a glial or metastatic tumor in computed tomography(CT) or magnetic resornance imaging(MRI) finding. Radiological evaluation and recognition are important for the surgical removal of these potentially curable neoplasms.

Renal artery embolism is an uncommon event, but a correct diagnosis and appropriate treatment are often delayed. We report a case in which viability was sustained by surgical intervention 18 hours after an episode of embolism that produced hypertension.
