ABSTRACT

Objective The aim of this study was to evaluate the clinical efficacy and hematologic toxicity of docetaxel in the treatment of metastatic breast cancer patients in different age groups. Methods The clinical data of three groups of single-agent do-cetaxel in the treatment of metastatic breast cancer patients were retrospectively analyzed. The clinical efficacy of docetaxel was ana-lyzed statistically. Results The objective response rate(ORR) and clinical beneficial rate( CBR) in the group of age < =45 year group were 30. 6% and 38. 7% ,and the ORR and CBR in the 46~59 age were 27. 5% and 35. 7% ,while ORR and CBR in the≥60 age group were 20. 0% and 27. 5% . The median progress free survival( PFS) in three groups was 6. 0(2. 9 ~9. 1) months,5. 0 (2. 9~7. 1)months and 4. 0(3. 3~4. 7) months,respectively. There was no statistical difference in three different age groups(P=0. 477,0. 492 and 0. 460,respectively). Cox regression analysis showed that the docetaxel rescue treatment of PFS had not associated with clinical stage,pathological type,ipsilateral axillary lymph node metastasis,disease free survival,unit dose of body surface area dose,number of rescue treatment lines,number of recurrence and metastasis,etc. in three different age groups of patients with meta-static breast cancer. Hematological toxicity was mainly manifested by a decrease in white blood cells and neutrophils,and docetaxel has less effect on hemoglobin and platelets. Conclusion The clinical efficacy and hematologic toxicity of docetaxel have a certain de-gree of correlation with ages. With the increase of age,the clinical curative effect is reduced and the blood toxicity is aggravated.

ABSTRACT

ObjectiveBoth BI-RADS 3 and 4A breast lumps have certain effects on the clinician's decision of proper treatment. The purpose of this study was to compare the ultrasonic BI-RADS classification and pathological results of breast lumps in patients of different ages, and explore the significance of age in ultrasound interpretation of BI-RADS 3 and 4A breast lumps.MethodsWe selected 728 patients with breast lumps treated in our hospital from Jun. 2014 to Dec. 2017 as research objects. According to different ages, the patients were divided into juvenile group (7-17 years old) , young group (18-40 years old), middle-aged group (41-65 years old) and aged group (> 65 years). Comparison was made between ultrasound results and pathological results and among the incidences of breast malignant masses of BI-RADS 3 and 4A grade of different ages to analyze possible factors that might affect the benign and malignant breast lumps of BI-RADS 3 and 4A.ResultsA total number of 728 patients was included in this study. Compared with pathological results, the rate of malignant breast tumors in BI-RADS 3 and 4A gradually increased with age. As to BI-RADS 3 breast lumps, the malignant rate of BI-RADS 3(6.54%) in aged group was higher than those of juvenile group(0.00%), young group (0%), middle-aged group (0.95%) and the differences were of statistical significance (P<0.05). As to BI-RADS 4A breast lumps, the malignant rate of juvenile group(0%) was lower than those of young group (2.06%), middle-aged group(6.54%) and aged group(25.39%), and the malignant rate in the elderly group was higher than those of the other groups(P<0.05). Logistic results showed age (OR=1.21,95%CI:1.07~1.36) and weight (OR=3.87,95%CI:1.24~12.81) were risk factors for breast cancer.ConclusionAge plays an important role in the ultrasound interpretation of BI-RADS 3 and 4A breast lumps. For young people, clinical assessment of BI-RADS 4A breast lumps by ultrasound can be properly degraded. While for elderly people, clinical assessment of BI-RADS 3 and 4Abreast lumps can be properly upgraded.

ABSTRACT

Background: Muscle relaxants are adjuncts to good anesthesia. Neonates and infants are having less developed neuromuscular junction, larger distribution volume and poor hepatic and renal function. The aim of the present study was to define dose of Vecuronium in neonates and infants in comparison of other age groups, to observe cardiovascular effect of Vecuronium, to study adequacy of intubating condition and, Adequacy of intra-operative muscle relaxation, To observe completeness of recovery after reversal. Materials and methods: 100% patients belonging to physical status of A.S.A Group I and II for operative procedures from both routine and emergency surgeries were selected randomly for study to observe clinical Comparison of Vecuronium bromide in different age groups. Patients were divided into 4 groups each consists of 25 Patients: Group I - Patients of 0-1 year of Age, Group II - Patients of 1-5 year of Age, Group III - Patients of 6-15 year of Age, Group IV - Patients of 16-50 year of Age. Results: Mean pulse rate before induction in Group I patient was 143.92/min., it increased to 151.92 /min. after intubation and then maintained throughout the course of anesthesia between 143.68 to 146.28 / min. which was not significant. The mean arterial pressure in Group I patient was maintained between 74.32 mm of Hg, 77.24 mm of Hg in Group II from 84.21 of pre-induction value – small changes were there and MAP maintained between 79.9 to 89.3 mm of Hg. Pre-induction MAP in Group III was 87.95 mm of Hg, it maintained between 89.29 to 92.66 mm of Hg throughout and in Group IV pre-induction value of MAP was 94.62, where it was maintained between 91.78 – 98.5 mm of Hg. So, there was no any pulse rate or blood pressure changes reflecting cardiovascular stability of Vecuronium. Rajani B, Brahmbhatt H, Chaudhry H, Hiren Parmar H. Page 211 Conclusion: Vecuronium is an excellent non-depolarizing muscle relaxant which is an intermediate duration of action in adult but of longer duration in neonates and infants (less than 1 year). It is having advantage of – Cardiostability, Lack of Ganglion blocking action, Lack of histamine release, Non cumulative property, Excellent conditions for intubation, Excellent muscle relaxation, Complete recovery without any complication

ABSTRACT

Objective To investigate the current status of postoperative life quality of 2-to 12-yearold children with congenital heart disease,to compare results between 2-to 4-year-old,5-to 7-year-old and 8-to12-year-old groups.Meanwhile,compare the results with foreign researches.Methods Pedsol cardiac module was adopted to investigate 255 children patients with letters in a certain heart center in Shanghai.The survey included heart problems and treatment,perceived physical appearance,treatment anxiety,cognitive problems and communication.2-to 4-year-old group was reported by parents and others were reported by both children and parents.The results underwent analysis and were compared with data of other countries.Results There existed statistical difference among three groups.Compared with foreign researches,quality of life of Chinese children was lower except heart problems and treatment.Conclusions We should provide suitable health care and family centered nursing according to physical and psychological development of children to improve children's quality of life.

ABSTRACT

Objective Chromatography fingerprint of Radix Codonopsis of different age from Shanxi province was studied to provide the reference for the science appraisal of intrinsic quality of Radix codonopsis. Method A RP-HPLC method was applied on GraceSmart-RP18 (250 mm?4.6 mm, 5 ?m) column with acetonitrile-0.1% acetic acid gradient elution as mobile phase, and the flow-rate was 0.6 mL/min. The detection wavelength was at 268 nm and the column temperature was at 30 ℃. Result Chromatography fingerprint of Radix Codonopsis can demonstrate obviously the difference and the similarity of Radix Codonopsis of different age. The RSD were lower than 3% in the precision, stability and the repetitiveness trial, and the codonopsis pilosula alkyne glucoside can be used as the reference to identify the active composition of Radix Codonopsis. Conclusion The method was simple, reproducible and reliable. The integrity of fingerprint can be used to supply the reference for the quality control of Radix Codonopsis from Shanxi provice.
