ABSTRACT

Introduction: The female sex has been systematically identified as one of the predictors of poor quality of life in patients with cirrhosis. However, the relationship between quality of life and sexual function in women with cirrhosis has not been widely studied. Aim: To determine the relationship between quality of life and sexual function in women with cirrhosis. Materials and methods: A cross-sectional observational study. Female patients aged 18 to 69 years were included. The SF-36-V2 survey assessed health-related quality of life, and the IFSF-6 evaluated sexual function. Univariate and multivariate linear regression was used to identify the relationship between quality of life and sexual function. Statistical significance was set at a p-value < 0.05. Results: 36 patients were available for analysis. The average age was 61 years. In the SF36-V2 survey, the physical component summary mean was 58, and the mental component summary mean was 56. The IFSF-6 had an average of 10. Sexual dysfunction occurred in 80% of patients. The IFSF-6 score was positively related to health-related quality of life. Factors associated with the deterioration of sexual function were age and menopause. The Child-Pugh score and body mass index were the main determinants of health-related quality of life. Conclusions: Sexual function could be a determinant of quality of life in women with cirrhosis.


Introducción: El sexo femenino se ha identificado de forma sistemática como uno de los predictores de mala calidad de vida en pacientes con cirrosis. Sin embargo, la relación de la calidad de vida con la función sexual en mujeres con cirrosis no ha sido ampliamente estudiada. Objetivo: Determinar la relación entre la calidad de vida y la función sexual de mujeres con cirrosis. Métodos: Estudio observacional de tipo transversal. Se incluyó a pacientes femeninas de 18 a 69 años. Se utilizó la encuesta SF-36-V2 para evaluar la calidad de vida relacionada con la salud y la IFSF-6 para valorar la función sexual. Para identificar la relación entre calidad de vida y función sexual se utilizó una relación lineal uni- y multivariante. La significación estadística se fijó con un valor p < 0,05. Resultados: 36 pacientes estuvieron disponibles para el análisis. La media de edad fue de 61 años. En la encuesta SF36-V2, la media del resumen del componente físico fue de 58 y la media del resumen del componente mental fue de 56. El IFSF-6 tuvo una media de 10. La disfunción sexual se presentó en un 80% de las pacientes. El puntaje IFSF-6 se relacionó de forma positiva con la calidad de vida relacionada con la salud. Los factores relacionados con deterioro de la función sexual son la edad y la menopausia. La escala Child-Pugh y el índice de masa corporal se identificaron como los principales determinantes de la calidad de vida relacionada con la salud. Conclusiones: La función sexual podría ser un determinante de calidad de vida de las mujeres con cirrosis.

ABSTRACT

Una propiedad fundamental de los sistemas sensoriales es su capacidad para detectar estímulos novedosos en el entorno. El sistema nervioso posee neuronas que disminuyen su respuesta a los estímulos sonoros que se repiten a lo largo del tiempo y otras neuronas que aumentan su frecuencia de disparo ante estímulos novedosos, siendo la diferencia entre ambas respuestas conocida como adaptación-específica a estímulos. En las últimas décadas, se ha propuesto que el cerebro establece, continuamente, predicciones de los estímulos novedosos y del entorno basándose en sus experiencias previas y en modelos de representación internos, teoría denominada codificación predictiva. En esta revisión, abordaremos algunos conceptos de la adaptación-específica a estímulos y codificación predictiva, centrándonos principalmente en el sistema auditivo. Por último, propondremos una explicación teórica basada en el marco de la codificación predictiva para algunas disfunciones neuropsiquiátricas, auditivas y vestibulares.


A fundamental property of sensory systems is their ability to detect novel stimuli in the environment. The nervous system possesses neurons that decrease their response to sound stimuli that are repeated over time and other neurons that increase their firing rate to novel stimuli, the difference between the two responses being known as stimulus-specific adaptation. In recent decades, it has been proposed that the brain continuously makes predictions of novel stimuli and the environment based on its previous experiences and internal representational models, a theory called predictive coding. In this review, we will address some concepts of stimulus-specific adaptation and predictive coding, focusing mainly on the auditory system. Finally, we will propose a theoretical explanation based on the predictive coding framework for some neuropsychiatric, auditory, and vestibular dysfunctions.

ABSTRACT

Objetivo: Investigar o impacto dos contraceptivos orais hormonais na função sexual de mulheres. Métodos: Estudo transversal realizado por meio do questionário traduzido e validado "Índice da Função Sexual Feminina", capaz de estimar o risco de disfunção sexual feminina. Dados sociodemográficos, ginecológicos, medicamentosos e outros foram avaliados e correlacionados estatisticamente a esse escore, estimando possíveis causas da disfunção sexual, com destaque para o uso de anticoncepcional oral. O estudo foi baseado em uma amostragem por conveniência, incluindo mulheres > 18 anos em idade reprodutiva, de 04/01/2021 a 04/01/2022, obedecendo aos critérios de inclusão e exclusão. Resultados: Participaram deste estudo 105 mulheres com média e desvio-padrão de idade de 23,4 ± 3,8 anos, predominantemente heterossexuais (84,0%) e bissexuais (13,2%). A maioria delas (93,4%) utiliza métodos contraceptivos, sendo esses anticoncepcional oral (45,3%), DIU hormonal (19,8%) e camisinha (17,0%). A composição hormonal mais utilizada foi levonorgestrel (26,4%) e etinilestradiol (25,5%). Oitenta por cento das mulheres são sexualmente ativas, 69,3% delas têm parceria fixa, 42,5% tinham relações quase sempre e 33,0% referiam que as relações sexuais eram sempre satisfatórias. Houve boa adequação da amostra (0,865) e significância estatística (p < 0,0001). Utilizar ou não método contraceptivo apresentou diferença nos domínios desejo, satisfação e dor. Contudo, as questões do histórico sexual foram as que mais apresentaram relevância estatística em relação aos domínios. Conclusão: Apesar de outros estudos serem necessários para provar a hipótese de que os contraceptivos orais têm impacto negativo na função sexual feminina, é clara a importância de os profissionais de saúde já estarem cientes dessa possibilidade e saberem como abordá-la.


Objective: To investigate the impact of hormonal oral contraceptives on women's sexual function. Methods: Cross-sectional study carried out using the translated and validated questionnaire "Index of Female Sexual Function", capable of estimating the risk of female sexual dysfunction. Sociodemographic, gynecological, medication and other data were evaluated and statistically correlated to this score, estimating possible causes of sexual dysfunction, with emphasis on the use of oral contraceptives. The study was based on a convenience sample, including women > 18 years of reproductive age, from 01/04/2021 to 01/04/2022, following inclusion and exclusion criteria. Results: The study included 105 women with a mean and standard deviation of (23.4 ± 3.8) years old, predominantly heterosexual (84.0%) and bisexual (13.2%). Most of them (93.4%) use contraceptive methods, these being (45.3%) oral contraceptives, (19.8%) hormonal IUDs and (17.0%) condoms. The most used hormonal composition was levonorgestrel (26.4%) and ethinylestradiol (25.5%). Eighty percent of the women are sexually active, 69.3% of them have a steady partner, 42.5% almost always had sex and 33.0% said that sex was always satisfactory. There was good sample adequacy (0.865) and statistical significance (p < 0.0001). Using or not using a contraceptive method showed a difference in the desire, satisfaction and pain domains. However, sexual history questions were the ones that showed the most statistical relevance in relation to the domains. Conclusion: Although further studies are needed to prove the hypothesis that oral contraceptives have a negative impact on female sexual function, it is clear that health professionals are already aware of this possibility and know how to approach it.

SUMMARY OBJECTIVE: This study aimed to investigate female sexual function in patients with type 1 diabetes by comparing female sexual function index scores between women with and without type 1 diabetes. METHODS: A total of 62 women with type 1 diabetes and 69 age-matched women without diabetes but with similar backgrounds were enrolled in the patient and control groups, respectively. All participants were sexually active and had no systemic diseases other than diabetes in the patient group. RESULTS: The frequency of female sexual dysfunction was significantly higher, and the mean female sexual function index score was significantly lower in women with diabetes compared to the control group (p=0.01). There was a significant relationship between sexual dysfunction and duration of diabetes, glycosylated hemoglobin test, and body mass index (p<0.05). CONCLUSION: This study demonstrates that female sexual dysfunction is more common among women with type 1 diabetes than among women without type 1 diabetes. Patients with type 1 diabetes should be evaluated in terms of sexual health. Health professionals should give more attention to and provide guidance regarding sexual function in women with type 1 diabetes.

Abstract Objectives: to evaluate associated factors with sexual dysfunction among middle-aged women. Methods: cross-sectional and analytical study was carried out with climacteric women attended at a Family Health Strategy Unit in the city of Montes Claros, MG. Brazil. The eligible women answered a question form containing sociodemographic, behavioral, anthropometric, gynecological factors and sexuality. The Body Mass Index and waist circumference were used to assess nutritional status and metabolic risk. To evaluate quality of life and sexual performance the Menopause Rating Scale and the Sexual Quotient - Female Version were used, respectively. Bivariate analysis and hierarchical multiple regression were used to identify associated factors with sexual dysfunction in the climacteric period. Results: among 195 women, 29.6% had sexual dysfunction. The prevalence of unsatisfactory sexual performance was higher among women who reported moderate to severe climacteric symptoms (OR=2.47) and lower schooling level was also associated (OR=1.95). However, age at menarche below 12 years (OR=0.43) and non-white (OR=0.36) seem to have a protective effect for good sexual performance. Conclusion: the prevalence of sexual dysfunction was high and the level of schooling and climacteric symptomatology were associated factors with this outcome.


Resumo Objetivos: avaliar os fatores associados à disfunção sexual entre as mulheres de meia-idade. Métodos: estudo de corte transversal e analítico realizado com mulheres climatéricas atendidas em uma Unidade de Estratégia de Saúde da Família no município de Montes Claros-MG, Brasil. As mulheres elegíveis responderam a um formulário contendo questões sociodemográficas, comportamentais, antropométricas, fatores ginecológicos e sexualidade. Utilizou-se o Índice e Massa Corporal e a circunferência da cintura para avaliar o estado nutricional e risco metabólico. Para avaliar a qualidade de vida e o desempenho sexual foram utilizados a Menopause Rating Scale e o Quociente Sexual -Versão Feminina, respectivamente. Empregou-se análise bivariada e regressão múltipla hierarquizada para identificar fatores associados à disfunção sexual no climatério. Resultados: dentre 195 mulheres, 29,6% apresentaram disfunção sexual. A prevalência de desempenho sexual insatisfatório foi maior entre as mulheres que declararam sintomas climatéricos moderados a graves (OR = 2,47) e o menor grau de escolaridade (OR = 1,95). No entanto, a idade da menarca abaixo de 12 anos (OR = 0,43) e a cor de pele não branca (OR = 0,36) parecem ter efeito protetor para o bom desempenho sexual. Conclusão: a prevalência de disfunção sexual foi elevada e o nível de escolaridade e a sintomatologia climatérica foram fatores associados a esse resultado.

SUMMARY OBJECTIVE: Prealbumin has been a reliable marker to predict protein energy malnutrition and hypercatabolic state. In this analysis, we particularly aimed to investigate the potential association between serum prealbumin levels and right ventricular dysfunction in patients receiving programmed hemodialysis. METHODS: A total of 57 subjects were included in the analysis. The subjects were then categorized into two groups: right ventricular dysfunction (n=18) and non-right ventricular dysfunction (n=39) groups. In all patients, detailed transthoracic echocardiography (following hemodialysis) were performed along with the evaluation of complete blood count, routine biochemistry parameters, and, in particular, serum prealbumin levels. RESULTS: Mortality rate at 3 years was found to be significantly higher in the right ventricular dysfunction group (p=0.042). Serum prealbumin levels were also significantly lower in the right ventricular dysfunction group compared with the non-right ventricular dysfunction group (23.83±8.50 mg/dL versus 31.38±6.81 mg/dL, p=0.001). In the receiver operating characteristics curve analysis, a prealbumin cutoff value of <28.5 mg/dL was found to predict right ventricular dysfunction, with a sensitivity of 67% and a specificity of 62% (area under the curve: 0.744). In the correlation analysis, a moderate yet significant positive correlation was demonstrated between serum prealbumin and tricuspid annular plane systolic excursion (r=0.365, p=0.005). CONCLUSION: This study suggests that low serum prealbumin might serve as a potential predictor of right ventricular dysfunction (and its clinical consequences) in patients receiving programmed hemodialysis.

Los adultos mayores, por naturaleza presentan deterioro progresivo de las capacidades físico-cognitivas, donde se ven afectadas facultades como el equilibrio, la marcha, la fuerza y la movilidad funcional. El confinamiento y distanciamiento social pueden llevar a que estas personas pierdan ciertas rutinas y aumenten en ellos el riesgo de sufrir caídas y lesiones músculo esqueléticas. Objetivo. Determinar el riesgo de caídas en los adultos mayores durante el periodo de confinamiento. Materiales y métodos. Estudio de enfoque cuantitativo, alcance descriptivo, diseño no experimental y de cohorte transversal; muestra poblacional de 42 adultos mayores en la ciudad de Guayaquil, que cumplen con los criterios de inclusión, a quienes se evalúan mediante historia clínica, las pruebas Timed Up & go, Tinetti, Downton y Falls Efficacy. Resultados. Predominio del grupo poblacional de sexo femenino con edades comprendidas entre 80 y 91 años, el Test Timed Up & Go denota que el 76% presentan déficit moderado y variable de la movilidad funcional, el test de Tinetti evidencia un alto riesgo de caídas en 83% de adultos mayores, mientras que las escalas de Downton y Falls Efficacy, muestran que un 50% requiere cuidados específicos y el 48% tienen temor de caer. Conclusiones. El grupo de adultos mayores evaluados muestra una alta predisposición al riesgo de caídas, producto del déficit de la movilidad, alteración del equilibrio y marcha, como respuesta a la falta de actividad física y distanciamiento social generada durante el periodo de confinamiento de marzo a junio 2020.


Older adults, by nature, present progressive deterioration of physical cognitive capacities, where faculties such as balance, gait, strength and functional mobility are affected. Confinement and social distancing can lead to these people losing certain routines and increasing their risk of falls and musculoskeletal injuries. Objective. To determine the risk of falls in the elderly during the confinement period. Materials and methods. Study with a quantitative approach, descriptive scope, non-experimental design and a cross-sectional cohort; The population sample is of 42 older adults in the city of Guayaquil, who meet the inclusion criteria, who are evaluated through a clinical history and the tests, Timed Up & go, Tinetti, Downton and Falls Efficacy. Results. Predominance of the female population group aged between 80 and 91 years, the Timed Up & Go Test denotes that 76% present moderate and variable deficiency of functional mobility, the Tinetti test shows a high risk of falls in 83% of older adults, while the Downton and Falls Efficacy scales show that 50% require specific care and 48% are afraid of falling. Conclusions. The group of older adults evaluated shows a high predisposition to the risk of falls, product of the mobility deficit, alteration of balance and gait, in response to the lack of physical activity and social distancing generated during the confinement period from March to June 2020.


Os adultos mais velhos, por natureza, apresentam deterioração progressiva das capacidades físico-cognitivas, onde faculdades como equilíbrio, marcha, força e mobilidade funcional são afetadas. O confinamento e o distanciamento social podem levar essas pessoas a perder certas rotinas e aumentar o risco de quedas e lesões musculoesqueléticas. Objetivo. Determinar o risco de quedas em idosos durante o período de confinamento. Materiais e métodos. Estudo com abordagem quantitativa, de escopo descritivo, não experimental e com delineamento de coorte transversal; A amostra populacional é composta por 42 idosos na cidade de Guayaquil, que atendem aos critérios de inclusão, que são avaliados por meio de um histórico médico e dos testes de eficácia Timed Up & go, Tinetti, Downton e Falls. Resultados. Predominância da população feminina na faixa etária de 80 a 91 anos, o Teste Timed Up & Go denota que 76% apresentam déficit moderado e variável de mobilidade funcional, o teste de Tinetti mostra alto risco de quedas em 83% dos idosos, enquanto as escalas de Downton e Falls Efficacy mostram que 50% requerem cuidados específicos e 48% têm medo de cair. Conclusões. O grupo de adultos mais velhos avaliados apresenta elevada predisposição ao risco de quedas, produto do déficit de mobilidade, alteração do equilíbrio e marcha, em resposta à inatividade física e distanciamento social gerado durante o período de confinamento de março a junho de 2020.

Abstract BACKGROUND: Non-alcoholic fatty liver disease (NAFLD) has become a public health problem worldwide. Neck circumference (NC) is a simple anthropometric adiposity parameter that has been correlated with cardiometabolic disorders like NAFLD. OBJECTIVES: To investigate the association between NC and NAFLD, considering their obesity-modifying effect, among participants from the Longitudinal Study of Adult Health (ELSA-Brasil) baseline study. DESIGN AND SETTINGS: Cross-sectional study at the ELSA-Brasil centers of six public research institutions. METHODS: This analysis was conducted on 5,187 women and 4,270 men of mean age 51.8 (± 9.2) years. Anthropometric indexes (NC, waist circumference [WC] and body mass index [BMI]), biochemical and clinical parameters (diabetes, hypertension and dyslipidemia) and hepatic ultrasound were measured. The association between NC and NAFLD was estimated using multinomial logistic regression, considering potential confounding effects (age, WC, diabetes, hypertension and dyslipidemia). Effect modification was investigated by including the interaction term NC x BMI in the final model. RESULTS: The frequency of NAFLD and mean value of NC were 33.6% and 33.9 (± 2.5) cm in women, and 45.8% and 39.4 (± 2.8) cm in men, respectively. Even after all adjustments, larger NC was associated with a greater chance of moderate/severe NAFLD (1.16; 95% confidence interval [CI] for women; 1.05, 95% CI for men; P < 0.001). Presence of multiplicative interaction between NC and BMI (P < 0.001) was also observed. CONCLUSION: NC was positively associated with NAFLD in both sexes, regardless of traditional adiposity indexes such as BMI and WC. The magnitude of the association was more pronounced among women.

ABSTRACT

Type 2 diabetes mellitus is a progressive process. With the course of the disease progress, microvascular and macrovascular complications always happen. Thrombotic events caused by macrovascular complications, including coronary heart diseases and cerebrovascular diseases, are the main fatal factor for the patients with type 2 diabetes. Endothelial dysfunction, coagulative activation, impaired fibrinolysis, together with hyper-reactive platelets contribute to the diabetic prothrombotic state, which is strongly related to the macrovascular complications. In particular, the hyper-reactive platelets play a fundamental role among them. Type 2 diabetes is characterized by several metabolic dysfunctions such as hyperglycemia, insulin resistance and shortage, oxidative stress, systemic inflammation, obesity, and dyslipidemia. These metabolic dysfunctions work together to promote the formation of hyper-reactive platelets, which are distinctive in type 2 diabetes. The regular antiplatelet drugs, like aspirin, show limited inhibitory effect on them. Hence, studying the mechanism behind the hyper-reactive platelets could provide a brand-new view on the prevention of macrovascular complications and cardiovascular events in type 2 diabetes.

Resumen OBJETIVO: Revisar el estado actual del conocimiento acerca de la asociación entre la esterilización quirúrgica femenina y la disfunción sexual en la mujer. METODOLOGIA: Estudio retrospectivo efectuado con base en la búsqueda bibliográfica de artículos publicados en inglés y español en las bases de datos de PubMed, LILACS, MEDLINE y EBSCO entre los años 1990 y 2022. Se seleccionaron artículos con estudios de cohorte, observacionales, casos y controles, revisiones bibliográficas sistemáticas, ensayos clínicos y metanálisis. RESULTADOS: Se obtuvieron 50 artículos de los que se excluyeron 38 por duplicidad, idioma diferente al inglés o español, sin relación con el tema específico o no estaban completos. Al final se incluyeron 12 artículos que cumplieron con los criterios para la revisión. CONCLUSIONES: La asociación entre esterilización quirúrgica y disfunción sexual femenina ha sido ampliamente discutida, pero poco estudiada. Los ensayos disponibles son observacionales efectuados en contextos socioculturales diversos, con desenlaces contradictorios y con limitaciones metodológicas. Los resultados obtenidos varían según el contexto sociocultural y el papel de la mujer en la sociedad donde se estudia, las creencias religiosas y el grado de escolaridad. Es necesario emprender más estudios con mejor calidad metodológica para establecer recomendaciones más precisas y, así, repercutir en la salud sexual de las mujeres.


Abstract OBJECTIVE: Review current status of knowledge about the association between female surgical sterilization and sexual dysfunction in women. METHODOLOGY: Retrospective study based on a literature search of articles published in English and Spanish in PubMed, LILACS, MEDLINE and EBSCO databases between 1990 and 2022. Articles with cohort, observational, case-control, systematic literature reviews, clinical trials and meta-analyses were selected. RESULTS: We obtained 50 articles of which 38 were excluded due to duplicity, language other than English or Spanish, unrelated to the specific topic or not complete. In the end, 12 articles that met the criteria for review were included. CONCLUSIONS: The association between surgical sterilization and female sexual dysfunction has been widely discussed, but little studied. The available trials are observational conducted in diverse sociocultural contexts, with contradictory outcomes and methodological limitations. The results obtained vary according to the sociocultural context and the role of women in the society being studied, the religious beliefs and level of schooling. More studies with better methodological quality are needed to establish more precise recommendations and thus have an impact on women's sexual health.

Abstract In HIV-patients, the imbalance in immunological, hematological and biochemical factors can contribute to the progression to AIDS and non-AIDS comorbidities, even during combined antiretroviral therapy (cART). In this cross-sectional study, we aimed to analyze some of these parameters in 138 different asymptomatic HIV-infected patients, doing multiple comparisons between the groups, which are dichotomized in the presence / absence of cART and type of immune response (immunological responders [iR,>500cells/mL] or non-responders [iNR,<500cells/ mL]). Were analyzed cytokines and other routine laboratory parameters. Our results showed high creatine phosphokinase and low IL-10 levels in cART-patients. They also presented metabolic alterations, including elevations in total cholesterol and triglycerides, particularly in those iNR. In ART-iR an increased alanine aminotransferase was observed. Those NAÏVE-iNR presented high LDL-cholesterol, C-reactive protein and lactate dehydrogenase values. The long-term non-progressors (LTNP) showed the best laboratory results. In conclusion, many blood parameters were changed in HIV-patients, especially in those under cART. To identify LTNP individuals could be important to discussions their early therapeutic onset.

Abstract Introduction: Pelvic floor dysfunctions have an impact on women's sexual function. A Chilean study found that 74% of women have pelvic floor dysfunctions, but there is no validated tool for them. Objective: To evaluate the psychometric properties of the Pelvic Organ Prolapse/Urinary Incontinence Sexual Questionnaire (PISQ-12) for assessing sexual function in Chilean women with pelvic floor dysfunctions. Methods: Cross-sectional study of psychometrics in 217 women with pelvic floor dysfunction, age 18 or older, and sexually active (last 6 months). Non-probability, convenience sampling. Tool: PISQ-12. Experts checked content validity, construct validity with confirmatory factor analysis, reliability with Cronbach's alpha, and discriminating capacity with Pearson and McDonald's omega. Results: Population is mainly perimenopausal, highly educated with no income and with urinary incontinence (89.4%). Psychometric analysis supports a three-factor structure: sexual response, female sexual problems, and male sexual problems, with a good (α= 0.85), acceptable (α= 0.73), and poor (α= 0.63) reliability, respectively, but McDonald's omega was acceptable for all three. These were related to age (rs: -0.33), education (rs: 0.36), number of pregnancies (rs: -0.18) and vaginal births (rs: -0.25). Conclusions: PISQ-12 is valid and reliable for measuring sexual dimension and problems. Age, education, and number of pregnancies and vaginal births are moderately correlated to sexual response.


Resumen Introducción: Las disfunciones del piso pélvico impactan la función sexual de mujeres que la padecen. En un estudio chileno un 74% de las mujeres presentó disfunción sexual sin tener un instrumento validado para esta población. Objetivo: Evaluar las propiedades psicométricas del Pelvic Organ Prolapse Urinary Incontinence Sexual Questionnaire (PISQ-12) para medir la función sexual en mujeres chilenas con disfunciones del piso pélvico. Métodos: Estudio transversal psicométrico de 217 mujeres con disfunción del piso pélvico, igual /mayor a 18 años, sexualmente activas (últimos 6 meses). Muestreo no probabilístico de conveniencia. Cuestionario: PISQ-12. Se realizó juicio de expertos para validez de contenido, análisis factorial confirmatorio para validez de constructo y el α de Cronbach para confiabilidad y capacidad discriminativa con Pearson y ω de McDonald. Resultados: Población principalmente perimenopáusica, alta escolaridad sin ingresos y con Incontinencia Urinaria (89.4%). El análisis psicométrico apoyó una estructura de tres factores: respuesta sexual, limitaciones sexuales femeninas y limitaciones sexuales masculinas, con confiabilidad buena (α= 0.85) aceptable (α= 0.73) y pobre (α= 0.63), respectivamente, aunque el ω de McDonald mostró valores aceptables para los tres. Éstos se relacionaron con edad (rs: -0.33), escolaridad (0.36), número de embarazos (-0.18) y partos vaginales (-0.25). Conclusiones: El PISQ-12 es válido y confiable, midiendo la dimensión sexual y limitaciones sexuales. La edad, escolaridad, número de embarazos y partos vaginales se correlaciona con la respuesta sexual en intensidad moderada.

Abstract Objective To assess the sexual function and associated factors in postmenopausal women. Methods This a descriptive, cross-sectional study with 380 women aged 40 to 65 years, users of public health services in 2019. Questionnaires were applied on demographic characteristics, on climacteric symptoms (menopause rating scale) and on sexual function (sexual quotient, female version). Bivariate andmultiple analyses by logistic regression were performed, with adjusted odds ratios (ORad) and 95% confidence intervals (95%CIs). Results More than half (243/64%) of the participating women were at risk of sexual dysfunction, with lower scores in the domains of sexual desire and interest, comfort, orgasm, and satisfaction. Women with a partner (ORad 2.07; 95%CI 1.03-4.17) and those who reported sleep problems (ORad 2.72; 95%CI 1.77-4.19), depressed mood (ORad 2.03; 95%CI 1.32-3.10), sexual complaints (ORad 8.16; 95%CI 5.06-13.15), and vaginal dryness (ORad 3.44; 95%CI 2.22-5.32) showed greater chance of sexual dysfunction. Conclusion There was a high prevalence of sexual dysfunction, with the influence of conjugality and climacteric symptoms on sexual function.


Resumo Objetivo Avaliar a função sexual e fatores associados em mulheres na pósmenopausa. Métodos Este é umestudo descritivo, transversal, com380 mulheres de 40 a 65 anos, usuárias de serviços de saúde públicos em 2019. Foram aplicados questionários sobre características demográficas, sobre sintomas climatéricos (menopause rating scale) e sobre a função sexual (Quociente Sexual, versão feminina). Análises bivariada e múltipla por regressão logística foram realizadas, com cálculo de odds ratio ajustado (ORaj) e intervalos de confiança de 95% (ICs95%). Resultados Mais da metade (243/64%) das mulheres participantes no estudo apresentou risco de disfunção sexual, com escores mais baixos dos domínios desejo e interesse sexual, conforto, orgasmo e satisfação. Mulheres com companheiro (ORaj 2,07; IC95% 1,03-4,17) e aquelas que relataram problemas de sono (ORaj 2,72; IC95% 1,77-4,19), ânimo depressivo (ORaj 2,03; IC95% 1,32-3,10), queixas sexuais (ORaj 8,16; IC95% 5,06-13,15) e ressecamento vaginal (ORaj 3,44; IC95% 2,22-5,32) apresentaram maior chance de disfunção sexual. Conclusão Houve prevalência elevada de disfunção sexual, com influência da conjugalidade e dos sintomas climatéricos sobre a função sexual.

Abstract Objective To investigate the feasibility of pelvic floor muscle training (PFMT) through gametherapy for relieving urinary symptoms of climacteric women with stress ormixed urinary incontinence (UI). Methods Randomized clinical trial, divided into two groups: Gametherapy (G_Game) and Control (G_Control). Both groups received recommendations about unsupervised PFMT, and G_Game also received supervised PFMT through gametherapy. After 5 consecutive weeks, the feasibility was investigated considering participant adherence, urinary symptoms (evaluated by the International Consultation on Incontinence Questionnaire-Urinary Incontinence Short Form [ICIQ-UI-SF] questionnaire), and pelvic floor function (PERFECT Scheme: power, endurance, repetition and fast). The Fisher exact, Kruskal-Wallis, Wilcoxon sign paired, and Mann-Whitney U tests were used by intention-to-treat analysis, using STATA 15.1 (StataCorp, College Station, TX, USA) software. Results The present study included 20 women per group and observed a higher adherence in G_Game. In the intragroup analysis, a decrease in the ICIQ-UI-SF score was observed in both groups (14.0 to 10.0; 13.5 to 0), associated with increased endurance (2.5 to 3.5; 2.5 to 4.0) in G_Control and G_Game, respectively. Moreover, there was a concomitant increase in pelvic floor muscles (PFMs) power (2.0 to 3.0), repetition (3.0 to 5.0), and fast (10.0 to 10.0) in G_Game. In the intergroup analysis, a reduction of UI was observed (p<0.001; r=0.8), as well an increase in PFM power (p=0.027, r=0.2) and endurance (p=0.033; r=0.3) in G_Game. Conclusion The feasibility of supervised PFMT through gametherapy was identified by observing participant adherence, relief of urinary symptoms, and improvement in PFM function.


Resumo Objetivo Investigar a viabilidade do treinamento dos músculos do assoalho pélvico (TMAP) por meio de gameterapia no alívio de sintomas urinários em mulheres climatéricas com incontinência urinária (IU) de esforço ou mista. Métodos Ensaio clínico randomizado, dividido em dois grupos: Gameterapia (G_Game) e Controle (G_Controle). Ambos os grupos receberam recomendações sobre TMAP não supervisionado, e G_Gametambém recebeu TMAP supervisionado por meio de gameterapia. Após 5 semanas consecutivas, a viabilidade foi investigada considerando a aderência das participantes, sintomas urinários (avaliados pelo questionário International Consultation on Incontinence Questionnaire-Urinary Incontinence Short Form [ICIQ-UI-SF]) e função do assoalho pélvico (esquema PERFECT: power, endurance, repetition, fast). Os testes exatos de Fisher, Kruskal-Wallis, sinal de Wilcoxon pareado e Mann-Whitney U foram usados pela análise de intenção de tratar, usando o software STATA 15.1 (StataCorp, College Station, TX, EUA). Resultados O presente estudo incluiu 20 mulheres por grupo e observou uma maior adesão no G_Game. Na análise intragrupo, foi observada diminuição no escore do ICIQUI- SF emambos os grupos (de 14,0 para 10,0; de 13,5 para 0), associada ao aumento da resistência (de 2,5 para 3,5; de 2,5 para 4,0) em G_Control e G_Game, respectivamente. Além disso, houve um aumento concomitante no power (de 2,0 para 3,0), repetition (de 3,0 para 5,0) e fast (de 10,0 para 10,0) dosmúsculos do assoalho pélvico (MAPs) no G_Game. Na análise intergrupos, foi observada redução da IU (p<0,001; r=0,8), assim como do power (p=0,027; r=0,2) e da endurance (p=0,033; r=0,3) dos MAPs no G_Game. Conclusão A viabilidade do TMAP supervisionado por meio de gameterapia foi identificada pela observação da aderência das participantes, pelo alívio dos sintomas urinários e pela melhora da função dos MAPs.

Resumen Objetivo: Realizar una reflexión sobre la relación etiológica recíproca entre la disfunción sexual femenina y la drogodependencia, y sus implicaciones prácticas e investigativas. Materiales y métodos: Se presenta una descripción de los efectos y las consecuencias a corto y a largo plazo del uso de drogas en mujeres y se analiza si el uso de drogas es la causa de la disfunción sexual o si, por el contrario, la disfunción sexual conduce al uso de drogas. Asimismo, se discute la necesidad de ahondar en la investigación que relaciona estas dos variables y sus implicaciones clínicas. Conclusión: El consumo de drogas afecta la función sexual femenina, por lo que es pertinente un diagnóstico inicial y la rehabilitación sexual tras el uso crónico de sustancias psicoactivas; asimismo, se hace indispensable implementar medidas profilácticas para disminuir el uso de drogas en la actividad sexual y sus consecuencias asociadas, y ampliar la investigación de esta área del conocimiento médico y psicológico.


Abstract Objective: To reflect on the reciprocal etiologic relationship between female sexual dysfunction and drug abuse, and its implications for practice and research. Materials and Methods: A description of the effects and short-term and long-term consequences of drug use in women is presented together with an analysis of whether drug use is the cause of sexual dysfunction or on the contrary, if sexual dysfunction leads to drug abuse. The need to conduct further research into the relationship between these two variables and their clinical implications is also discussed. Conclusion: Drug use affects female sexual function, hence the importance of initial diagnosis and sexual rehabilitation following chronic use of psychoactive substances; Implementing prophylactic measures in order to reduce drug use during sexual activity and its associated consequences; and expanding research in this area of medical and psychological knowledge.

Introdução: As mulheres, entre 40 e 65 anos, passam por mudanças fisiológicas que interferem na sua sexualidade, caracterizado pelo climatério. Objetivo: Verificar a prevalência das disfunções sexuais em mulheres climatéricas contribuindo com evidências para profissionais que lidam com a saúde da mulher. Métodos: Estudo de corte transversal descritivo e analítico, realizado na clínica especializada da mulher em Caruaru/PE, com 99 mulheres, de 40 a 65 anos e que tinham vida sexual ativa. Foram avaliadas através dos questionários: Sociodemográfico, Questionário da Saúde da Mulher, Quociente Sexual Versão Feminina e Índice de Função Sexual Feminino. Resultados: 44,44% têm indicativo para disfunção sexual. 52,52% possuem bom desempenho sexual, cerca de 58,58% tem alteração na lubrificação e 51,51% dor no ato sexual. 63,63% tem alterações na satisfação e orgasmo, 69,69% têm alterações no desejo e a falta de excitação foi o maior índice amostral, representado por 74,74%. Conclusão: A maioria apresenta bom desempenho sexual, entretanto possuem baixa qualidade de vida e alto indicativo para disfunções sexuais. Sendo assim, propõe-se desenvolvimento de pesquisas, gerando conhecimentos para profissionais que lidam com essa temática, visando saúde e qualidade de vida. (AU)


Introduction: Women, between 40 and 65 years old, undergo physiological changes that interfere with their sexuality, characterized by the climacteric. Objective: To verify the prevalence of sexual dysfunction in climacteric women, contributing with evidence for professionals who deal with women's health. Methods: Descriptive and analytical cross-sectional study, carried out at the women's specialized clinic in Caruaru/PE, with 99 women, 40 to 65 years old and who had an active sex life. They were evaluated through questionnaires: Sociodemographic, Women's Health Questionnaire, Female Version Sexual Quotient and Female Sexual Function Index. Results: 44.44% are indicative of sexual dysfunction. 52.52% have good sexual performance, about 58.58% have changes in lubrication and 51.51% have pain during sex. 63, 63% have changes in satisfaction and orgasm, 69.69% have changes in desire and lack of excitement was the highest sample rate, represented by 74.74%. Conclusion: Most have good sexual performance, however they have low quality of life and high indications for sexual dysfunction. Therefore, it is proposed to develop research, generating knowledge for professionals who deal with this theme, aiming at health and quality of life. (AU)

RESUMEN Objetivo: definir la incidencia de deseo sexual hipoactivo (DSH) en mujeres que planifican con el implante subdérmico de levonorgestrel (LNG) después de 12 meses, y conocer otros efectos adversos. Materiales y métodos: estudio de cohorte, cuyas participantes fueron mujeres mayores de 18 años sin hijos ni embarazos previos y con pareja estable, a quienes se les había insertado el implante de LNG al menos tres meses previos en una clínica privada de carácter universitario en Armenia (Quindío), Colombia, entre 2014 y 2019. Se excluyeron mujeres con antecedentes de disfunción sexual, infecciones de transmisión sexual en los últimos 6 meses, peso mayor a 89 Kg y dificultades para el seguimiento. El muestreo fue consecutivo. Se describieron las características de la población estudiada, se estimó la incidencia acumulada de DSH y efectos adversos relacionados con el implante subdérmico de LNG Resultados: participaron 238 mujeres cuya edad media fue de 24,69 (DE ± 5,82) años. Se determinó una incidencia de deseo sexual hipoactivo del 18,82%. El 60,25% de las mujeres fueron diagnosticadas con DHS en los primeros 6 meses de seguimiento. Los efectos adversos se hicieron presentes desde los primeros 3 meses de la inserción del implante, el sangrado irregular (62,34%) fue el más frecuente, seguido de la amenorrea (38,91%) y el aumento de peso (33,89%). Conclusiones: es importante que los médicos y las usuarias se informen sobre la presencia del trastorno de deseo sexual hipoactivo y los efectos adversos que pueden surgir con el uso del implante subdérmico de levonorgestrel. Las futuras investigaciones deben confirmar estos hallazgos, así como su eventual base fisiológica.


ABSTRACT Objective: To determine the incidence of low libido in women using the levonorgestrel (LNG) sub-dermal implant for contraception after 12 months and to identify other adverse effects. Materials and methods: Cohort study that included women over the age of 18, with no children or prior gestations and in a stable relationship, with an LNG implant inserted at least three months before in a private teaching hospital in Armenia (Quindío), Colombia, between 2014 and 2019. Women with a history of sexual dysfunction, sexually transmitted infections in the past six months, weighing more than 89 kg and difficult to follow were excluded. Consecutive sampling was used. The characteristics of the study population were described, the cumulative incidence of low libido and the adverse effects related to the LNG sub-dermal implant after 12 months of follow-up were estimated. The tool used was the Female Sexual Dysfunction Index (FSFI). Descriptive statistics were applied. Results: The participants were 238 women with a mean age of 24.69 (SD ± 5.82) years. An 18.82% incidence of low libido was found. In 60.25% of the women were diagnosed with low libido in the first 6 months of follow-up. Adverse effects manifested as early as 3 months after the insertion of the implant, the most frequent being irregular bleeding (62.34%), followed by amenorrhea (38.91%) and weight gain (33.89%). Conclusions: It is important for practitioners and users to become aware of the presence of low libido disorder and the adverse events that may occur with the use of the levonorgestrel subdermal implant. Future research is required to confirm these findings and the underlying physiological cause.

A partir del caso de una paciente con trastorno por deseo sexual hipoactivo durante su climaterio y a través del resumen de los resultados de dos revisiones sistemáticas, los autores de este artículo revisan la evidencia sobre la suplementación con andrógenos para el tratamiento de esta condición clínica. Concluyen que su uso sería relativamente seguro a corto plazo, aunque su eficacia no alcanzaría la relevancia clínica y no contamos aún con mayor información sobre la seguridad en el largo plazo. Los autores destacan además que el abordaje de las pacientes con este problema de salud debería ser realizado en forma integral, incluyendo opciones terapéuticas no farmacológicas e informando sobre las incertidumbres todavía presentes. Based on the case of a patient with hypoactive sexual desire disorder during her climacteric period and through the summary of the results of two systematic reviews, the authors of this article review the evidence supporting androgen supplementation for the treatment of this clinical condition. They conclude that its use would be relatively safe in the short term, although its efficacy would not reach clinical relevance and no further information on long-term safety is available. The authors also highlight that patients with this health problem should be approached comprehensively, including non-pharmacological therapeutic options and providing information on the uncertainties still present. ABSTRACT

OBJECTIVE Cranial radiotherapy is clinically used in the treatment of brain tumors;however, the conse?quent cognitive and emotional dysfunctions seriously impair the life quality of patients. LW-AFC, an active fraction combi?nation extracted from classical traditional Chinese medicine prescription Liuwei Dihuang decoction, can improve cogni?tive and emotional dysfunctions in many animal models;however, the protective effect of LW-AFC on cranial irradiation-induced cognitive and emotional dysfunctions has not been reported. Recent studies indicate that impairment of adult hippocampal neurogenesis (AHN) and alterations of the neurogenic microenvironment in the hippocampus constitute crit?ical factors in cognitive and emotional dysfunctions following cranial irradiation. Here, our research further investigated the potential protective effects and mechanisms of LW-AFC on cranial irradiation-induced cognitive and emotional dys?functions in mice. METHODS LW-AFC (1.6 g·kg-1) was intragastrically administered to mice for 14 d before cranial irra?diation (7 Gyγ-ray). AHN was examined by quantifying the number of proliferative neural stem cells and immature neu?rons in the dorsal and ventral hippocampus. The contextual fear conditioning test, open field test, and tail suspension test were used to assess cognitive and emotional functions in mice. To detect the change of the neurogenic microenvi?ronment, colorimetry and multiplex bead analysis were performed to measure the level of oxidative stress, neurotrophic and growth factors, and inflammation in the hippocampus. RESULTS LW-AFC exerted beneficial effects on the contex?tual fear memory, anxiety behavior, and depression behavior in irradiated mice. Moreover, LW-AFC increased the num?ber of proliferative neural stem cells and immature neurons in the dorsal hippocampus, displaying a regional specificity of neurogenic response. For the neurogenic microenvironment, LW-AFC significantly increased the contents of superox?ide dismutase, glutathione peroxidase, glutathione, and catalase and decreased the content of malondialdehyde in the hippocampus of irradiated mice, accompanied by the increase in brain-derived neurotrophic factor, insulin-like growth factor-1, and interleukin-4 content. Together, LW-AFC improved cognitive and emotional dysfunctions, promoted AHN preferentially in the dorsal hippocampus, and ameliorated disturbance in the neurogenic microenvironment in irradiated mice. CONCLUSION LW-AFC ameliorates cranial irradiation-induced cognitive and emotional dysfunctions, and the underlying mechanisms are mediated by promoting AHN in the dorsal hippocampus and improving the neurogenic micro?environment. LW-AFC might be a promising therapeutic agent to treat cognitive and emotional dysfunctions in patients receiving cranial radiotherapy.

Objetivo: Esta pesquisa teve como objetivo avaliar o impacto da gestação sobre a função sexual feminina, comparando se houve diferença significativa entre os dois últimos trimestres gestacionais. Métodos: Foi realizada no serviço de Ginecologia e Obstetrícia do Hospital Universitário Evangélico Mackenzie, por meio do questionário Female Sexual Function Index (IFSF). Cento e quinze pacientes foram divididas em dois grupos (T2, correspondente ao segundo trimestre gestacional; T3, correspondente ao terceiro trimestre gestacional), apresentando taxa de disfunção sexual de 47,8%, prevalente em gestantes do grupo T3. Resultados: Houve diferen- ça significativa, entre o segundo e o terceiro trimestre, nas médias do IFSF e nas variáveis excitação, orgasmo e dor, resultando em disfunção sexual prevalente em gestantes do terceiro trimestre gestacional. Conclusão: Não houve diferença significativa nas variáveis desejo, lubrificação e satisfação.(AU)


Objective: The objective of this study was to evaluate the impact of pregnancy on female sexual function, comparing if there was significant difference between the last two gestational trimester. Methods: The research was accomplished in the Gynecology and Obstetrics of Mackenzie Evangelical University Hospital, applying the Female Sexual Function Index (IFSF). One hundred and fifteen patients were combined in two groups (T2 corresponding the second gestational trimester; T3 corresponding the third one), with sexual dysfunction rate of 47.8%, mainly the T3 group. Results: The IFSF's average between the second and the third gestational trimester was significantly different, also the sexual arouse, orgasm and pain variables are higher in the second trimester resulting in sexual dysfunction mostly of pregnant women in the third gestational trimester. Conclusion: The desire, lubrication and satisfaction variables did not have significant difference.(AU)
