ABSTRACT

Objective:To prepare a peptide fluorescent probe based on aggregation-induced emission and to investigate its application in the detection of early caries.Methods:Eight aspartate-serine-serine (DSS) were combined with aggregation-induced emission material to prepare peptide fluorescent probes, and an artificial demineralization model was established in vitro. The samples were immersed in the peptide fluorescent probe solution for 1 min, and a fluorescence imaging system was applied to examine the tooth samples and collect images and fluorescence data. Scanning electron microscopy was also applied to observe the phenotype of the teeth, and electron microscopy was applied to detect the calcium-phosphorus ratio on the enamel surface of the teeth. Polarized light microscopy was also applied to observe the enamel area of the teeth. Results:The fluorescence intensity of demineralized teeth was clearly observed to be lower than that of normal teeth in the peptide fluorescent probe-treated area, and the difference was statistically significant ( P < 0.05). The results of scanning electron microscopy showed that the enamel surface of the demineralized group had more irregular pores, while the enamel surface of the undemineralized group was flatter with only some irregular accumulation of flakes. The results of polarized light microscopy showed that a clear birefringence could be observed in the enamel region of normal teeth, while a black area or the disappearance of the birefringence effect accompanied by a partial black dark shadow could be observed in the enamel region of demineralized teeth. Conclusions:An aggregation-induced luminescence-based peptide fluorescent probe was successfully prepared, which can precisely localize the enamel and show some application value in early caries detection.

ABSTRACT

Objective To observe the effect of ammonia silver nitrate on early caries.Methods 100 cases of children with early caries treated in our hospital from April 2015 to December 2016 were admitted to conduct research and analysis.The children were divided into the control group and the observation group by double blind,and random method 50 cases in each group.The control group was treated with caries removal,the observation group was treated with carious worn out joint ammonia silver nitrate.After treatment, the two groups of children suffering from dental pulp tissue TNF-a, IL-8 concentration, the recurrence rate, filling rate falling, the rate of suffering from pulpitis, pain conditions were analyzed.Results After treatment, the observation group with TNF-α, IL-8 concentrations were lower than the control group(t=21.961,t=23.836), the difference was statistically significant (P<0.05);The recurrence rate, suffering from pulpitis rate was significantly lower than the control group, and the difference was statistically significant (P<0.05);the observation group after treatment in children with severe pain and painless was significantly lower than the control group, and the difference was statistically significant (P<0.05).Conclusion Ammonia silver nitrate is an effective and safe treatment for early caries, which can relieve the pain and discomfort in children, it is a safe and effective method of treatment and should be promoted and used in the clinic practice.

ABSTRACT

<p><b>OBJECTIVE</b>This study aimed to evaluate the potency of optical coherence tomography (OCT) to detect early occlusal caries compared with clinical visual examination.</p><p><b>METHODS</b>Approximately 97 sites of occlusal fissures on 77 extracted accessional human teeth were scored by three examiners using conventional visual examination and OCT. Results of histological examination on these sites obtained by polarimicroscope served as a gold standard to analyze the sensitivity (SE), specificity (SP), positive predictive value (PPV), and negative predictive value (NPV). Results of the area under receiver operating characteristic (ROC) curve (AUC) by visual examination and OCT were also analyzed. The Spearman's rank correlation coefficient with histology and the inter-examiner reliability were compared.</p><p><b>RESULTS</b>For sites of enamel demineralization limited to the outer 1/2 of the enamel layer, the detection rate of OCT (14/25) was obviously higher than that of the clinical and visual examination (3/25). SE, SP, PPV and NPV of OCT during diagnosis of the early occlusal caries (0.83, 0.64, 0.87 and 0.57) were higher than that of the visual examination (0.79, 0.60, 0.85 and 0.50). AUC (95%CI) of OCT and the visual examination were 0.737 (0.569-0.822) and 0.696 (0.614-0.859), respectively. No statistically significant difference was observed between the results. Results of OCT correlated well with histology (r=0.559, P<0.05). The inter-examiner reliability of OCT was medium.</p><p><b>CONCLUSIONS</b>OCT can accurately detect early occlusal lesions atraumatically with high sensitivity and effectiveness. OCT exhibits the potential of auxiliary clinical diagnosis enhancing detection rate and finally implementing early diagnosis and early intervention of early occlusal lesions in clinical practice.</p>

ABSTRACT

OBJECTIVES: To compare the QLF-D method and the ICDAS and DIAGNOdent techniques for in vitro quantification of occlusal caries and to assess the histological features of the caries. METHODS: One hundred and twenty-two extracted permanent teeth were selected, and the site of interest on the occlusal surface was examined using each detection method. The occlusal sites were classified according to the ICDAS II criteria based on the decision taken by two investigators, who have taken the ICDAS E-learning course. The examined site was then measured using the DIAGNOdent, and the peak value was recorded. In addition, by using the QLF-D, the occlusal site was photographed to obtain the DeltaFmax value. After all assessments were performed, the occlusal sites were vertically sectioned in order to assess the histological features. This was considered the gold standard. The histological criteria were graded using a 4-point scale as follows: S=sound (n=21), E1=limited enamel caries (n=27), E2=caries extending to the dento-enamel junction (n=49), D=caries involving the dentine (n=25). RESULTS: An ICDAS code between 0 and 4 was assigned to all the occlusal sites, and this revealed the QLF-D value, which was between -95 to 0. The DIAGNOdent value was between 8 and 99. The correlation values of QLF-D, ICDAS, and DIAGNOdent with the histological features were 0.68, 0.58, and 0.46, respectively (P<0.01). A highly significant correlation was observed between QLF-D and the gold standard, which showed a moderate correlation and an acceptable correlation was observed with ICDAS (r=0.75, P<0.01). A statistically significant difference was observed in the average QLF-D values of each histological grade i.e., -28.5 (S), -53.7 (E1), -68.1 (E2), and -84.4 (D). CONCLUSIONS: The QLF-D showed a significant correlation with the ICDAS and histological features. Therefore, visual inspection with QLF-D would improve the detection accuracy and ensure early diagnosis of dental caries.

ABSTRACT

Objective Quantitative analysis of optical coherence tomography (OCT) images of naturally occurred early enamel caries was performed to find quantitative parameter indicator for detecting early caries by OCT.Methods Three human premolars with enamel natural caries in D0,D1,and D2 stages were selected respectively and scanned by OCT.The ratios of the average gray value of the caries region (object region) and sound region (background region),namely,the object-background contrast (OBC) were calculated in OCT images and selected as the quantitative parameters to analyze the OBC trend of different degree of enamel caries.Results Within the fixed depth,the OBC value showed a trend of first increase and then decrease with the severity of dental caries,indicating that OBC value increased with the severity of dental caries from normal to D1 stage,and then decreased from D1 to D2 stage.Conclusion The OBC value can be used as a new quantitative indicator for OCT to estimate the severity of naturally occurred early caries.

ABSTRACT

The definition and diagnostic threshold of early caries have been changed to even earlier stage of subclinical level in recent years. This article tries to introduce the significant changes in the area of caries research, including the raise of the concept, the diagnostic standard, the problems and challenges facing, the interference and the treatment effect assessments of early stage subclinical caries. The prospect of the research is discussed as well to enhance the level of awareness and diagnosis and interference capability.

ABSTRACT

O uso da luz para o tratamento e prevenção de doenças na área médica e odontológica tem sido muito utilizado atualmente e pode ser chamado de fotobiologia. O DIAGNOdent (KaVo Biberach, Alemanha) consegue detectar, através da fluorescência do laser de diodo 655 nm, lesões iniciais que vão desde mínimas perdas minerais até lesões ocultas na dentina. Com a superfície do esmalte intacta consegue, também, determinar a atividade da cárie, ou seja, se no local da lesão há bactérias ativas ou inativas. A diminuição da prevalência da doença cárie em crianças e adolescentes tem exigido métodos de diagnóstico cada vez mais precisos, que nos forneçam dados complementares e que nos ajudem a compreender melhor todo o processo da doença cárie. O método de detecção por fluorescência a laser, DIAGNOdent, visa detectar precocemente as primeiras manifestações clínicas desta patologia, em uma fase que possamos evitar sua progressão através de métodos conservadores, numa abordagem atual de promoção da saúde.


Photobiology is the use of light in the treatment and prevention of diseases in the medical and dental areas. DIAGNOdent (KaVo Biberach, Alemanha), through fluorescence of diode laser 655 nm, is able to detect early lesions, from minimal mineral loss to lesions hidden in the dentine with intact enamel surface. It is also able to determine the caries activity, i.e., if there are active or inactive bacteria in the lesion site. Prevalence of caries disease in children and adolescentes has decreased and thus demands more precise diagnostic methods to furnish additional data to help understand the ,whole process. DIAGNOdent a laser fluorescence detection method, detects the clinical manifestations of this pathology at a very early stage. Thus, the progress of the disease is deterred, avoiding the use of conservative methods, which is in accordance with the current approach to buccal health promotion.
