ABSTRACT

Objective·To analyze the choice of smoking cessation methods and its influencing factors in moderate-to-severe nicotine-dependent population.Methods·A retrospective clinical analysis was conducted for170 moderate-to-severe nicotine-dependent smoking-quitters (Fagerstr(o)m scale ≥ 4).Different smoking cessation methods (pharmacotherapy and non-pharmacotherapy) and abstinence rates of the selected population (7 d point prevalence abstinence rates and sustained abstinence rates 1,3,and 6 months after treatment) were compared and possible influencing factors were analyzed.Results·The differences in choosing the smoking cessation method in moderate-to-severe nicotine-dependent population with different ages,levels of education,and employment status were statistically significant (P=0.000).The Logistics regression analysis showed that the level of education was the major factor for choosing the smoking cessation method (P=0.006).The 7 d prevalence abstinence rate was much lower for pharmacotherapy than for non-pharmacotherapy 1 month after treatment (P=0.000) and was much higher for pharmacotherapy than for non-pharmacotherapy 3 and 6 months after treatment (P=0.000).The sustained abstinence rate was much higher for pharmacotherapy than for non-pharmacotherapy 3 and 6 months after treatment (P=0.002,P=0.000).Conclusion·For moderate-to-severe nicotine-dependent smoking-quitters,attention should be paid to the formulation and implementation of personalized strategies.The choice of smoking cessation methods depends on the age,level of education,and employment status.Reasonable pharmacotherapies can significantly increase the abstinence rate.

ABSTRACT

Objective To explore pre-hospital delay factor of coronary reperfusion therapy for ST-elevation acute myocardial infarction (STEAMI) patients presenting with non-chest pains. Methods A retrospective observation was conducted. The clinical data of STEAMI patients underwent emergency percutaneous coronary intervention (PCI) admitted to Luoyang Central Hospital Affiliated to Zhengzhou University from August 2013 to August 2015 were analyzed. The patients were divided into chest pain group and non-chest pain group according to the presence of chest pain or not. Clinical characteristics were compared between the two groups, and incidence of major adverse cardiac events (MACE), door-to-balloon time, door-to-electrocardiograms (ECG) time and ECG-to-balloon time were evaluated. Influencing factors of pre-hospital delay was analyzed by logistic multiple stepwise regression. Results A total of 259 patients with STEAMI were enrolled, including 154 patients with chest pain and 105 presented with non-chest pains. Compared with chest pain group, the patients in the non-chest pain group were older (years: 68.12±8.93 vs. 62.34±7.12, P < 0.05), less female (26.67% vs. 42.20%, P< 0.05), and had a higher past history of angina, stroke and heart failure (27.61% vs. 13.63%, 31.42% vs. 18.83%, 26.67% vs. 11.68%, respectively, all P < 0.05), and higher percentage of Killip ≥ Ⅲ patients (15.24% vs. 6.49%, P < 0.05), the lower ambulance use (26.67% vs. 44.81%, P < 0.01), longer hospitalization time (days: 12.50±2.89 vs. 9.50±2.67, P < 0.05), higher incidence of MACE (19.05% vs. 9.09%, P < 0.05), longer door-to-balloon time and door-to-ECG time (minutes: 159.01±51.21 vs. 115.31±36.74, 53.06±18.17 vs. 30.35±9.93, both P < 0.01). It was shown by logistic multivariate regression analysis that no-chest pain [odds ratio (OR) = 5.14, 95% confidence interval (95%CI) = 2.34-10.81, P < 0.001], age ≥ 65 years old (OR = 1.43, 95%CI = 0.93-2.99, P = 0.022), diabetes (OR = 1.57, 95%CI = 0.66-2.15, P = 0.015) and no-ambulance transport (OR = 1.55, 95%CI = 0.73-2.75, P < 0.001) were risks factors of coronary reperfusion delay ≥ 2 hours. Conclusions STEAMI patients presenting without chest pain showed higher incidences of MACE, longer time of ECG obtained and initial PCI time delay. Clinicians should try to reduce the delay time of the patients in order to improve patient survival rates.

ABSTRACT

Abstract? AlM: To study the effect of pterygium excision combined with limbal stem cell on visual acuity and refractive status.?METHODS:A total of 120 cases ( 178 eyes ) pterygium cases were enrolled in our hospital from March 2012 to May 2014 and divided into observation group ( 60 cases, 90 eyes) and control group (60 cases, 88 eyes) according to different treatment methods. Patients in observation group received pterygium excision combined with limbal stem cell transplantation, those in control group received pterygium excision. Vision levels, astigmatism degrees were compared. The effects of age, working environment and medial rectus muscle insertion distance from corneal limbus on postoperative visual acuity were analyzed.? RESULTS: Before operation, acuity levels and astigmatism degrees of two groups had no difference ( P>0. 05 ); 1, 2 , 3, 4wk after operation, visual acuity in observation group were higher than that in control group (0.49±0.09 vs 0.34±0.07, 0.76±0.11 vs 0.42±0.04, 0.95± 0. 14 vs 0. 59±0. 08, 1. 09±0. 18 vs 0. 74±0. 10), astigmatism degrees were lower than that in control group (0. 96±0. 12 vs 1. 85±0. 28, 0. 68±0. 08 vs 1. 51±0. 23, 0. 54±0. 08 vs 1. 23± 0.19,0.42±0.07 vs 0.91±0.14); visual acuity of patients older than 45 years old, indoor work, medial rectus muscle insertion distance from corneal limbus 3-5 mm were higher, astigmatism degrees were lower.?CONCLUSlON:Pterygium excision combined with limbal stem cell transplantation is helpful to improve visual acuity and astigmatism degree; recovery of visual acuity is affected by age, working environment and medial rectus muscle insertion distance from corneal limbus.

ABSTRACT

Objective To realize compliance actuality, discuss effect factor and institute correspond countermeasure for diabete integration treatment. Method Investigate 251 patients with diabete who did colligate treatment and community with correlation factor. Results The rate of mostly complication of diabetes was higher (66.53%), and the rate of colligate treatment was lower (only 29.08%). The compliance of treatment had relation with the rate of knowing diabete, symptom and complication, economy factors, medicine insurance, and lever of doctor closely. Conclusion We should intensify diabetes knowledge education, increase medical treatment specialty level, associate with community intervene, supervise and direction, establish consummate medical treatment insurance system to increase colligate treatment of diabetes sufferer.

ABSTRACT

Objectives:To investigate the causes and effect factors of postpartum hemorrhage(PPH)in order to prevent PPH and decrease the mortality rate of gravida and parturient.Methods:Retrospective analysis was done on the 74 cases of PPH from Jan 2002 to Jan 2006 in our hospital.Results:The incidence rate of PPH during 4 years was 2.33%(74/3178).The causes were uterine inertia(43.24%),abnormal placenta(39.19%),injury of birth canal(6.67%),and dysfunction of blood coagulation(1.35%).There was significant difference among delivery style,gestational weeks,multiple pregnancy,and complications.(P
