ABSTRACT

INTRODUCTION: Phase angle (PhA) is a Bioelectrical impedance analysis (BIA) parameter representing an indicator of cellular health and has been suggested as a biomarker of nutritional status. OBJECTIVE: To evaluate the association between PhA and nutritional parameters in older adults. METHODS: A cross-sectional study was conducted with community-dwelling older adults. Body mass index (BMI), arm muscle circumference (AMC), calf circumference (CC), body fat percentage (BF%), appendicular skeletal muscle mass (ASMM), serum albumin, mini-nutritional assessment (MNA), and PhA were assessed. Kolmogorov­Smirnov test, Spearman's correlation coefficient, chi-square test, and Poisson regression models were performed. RESULTS: 144 participants were included in the study, and most of them were female, aged ≥80 years, and underweight. Most older adults with lower PhA were women, aged range 80­89 years, and with reduced ASMM (p<0.05). PhA presented a significant correlation with age (r=­0.417; p<0.001), ASMM (r=0.427; p<0.001), AMC (r=0.195; p=0.019) and BF% (r=­0.223; p=0.007). Older adults with lower PhA present reduced ASMM (PR: 1.25; 95%CI: 1.04­1.50), and hypoalbuminemia (PR: 1.50; 95%CI: 1.11­2.03). CONCLUSION: PhA is related to commonly nutritional indicators used in clinical practice and could be an important biomarker of muscle mass reserves in community-living older adults of both sexes.

ABSTRACT

Abstract The aim of this study was to investigate the agreement and reproducibility of a portable electrical impedance myography device (EIM - SKULPT®) for body fat percentage (BF%) estimation in young adults. Sixty young adults volunteered for the study (women, n=30, 25.0±7.7 years; 21.5±1.9 kg/m2; and men, n=30, 21.6±6.3 years; 22.5±1.8 kg/m2). Participants underwent air displacement plethysmography (ADP) and EIM measurements for agreement analysis. EIM was performed three times on the same day for the within-day reproducibility analysis. Seven days later, 37 participants repeated the EIM measurements for the between-days reproducibility analysis. Comparisons of EIM and ADP methods, and EIM repeated measurements were performed with the paired T-test or one-way ANOVA repeated measures, the Bland-Altman plot, and simple linear regressions. BF% was higher (p<0.05) when estimated by EIM (19.91 ± 5.70 for men, and 30.77 ± 5.89 for women) compared to ADP (15.28 ± 5.66 for men, and 27.31 ± 5.98 for women). The Bland-Altman analysis presented a bias of 4.4% (95%CI=-3.4-12.2) and linear regression presented an R2=0.78. For between-days reproducibility, EIM means did not differ (25.33±7.69 and 24.94±8.30, p=0.890). Thus, while the EIM device exhibited high reproducibility of BF% estimates, caution should be exercised when comparing the results with other techniques for measuring BF%. The EIM device overestimated BF% compared to ADP. However, the EIM measurements presented very good within-day and between-days reproducibility and, thus, the EIM device can be used for longitudinal monitoring of BF%.


Resumo O objetivo deste estudo foi investigar a concordância e a reprodutibilidade de um aparelho portátil de miografia por impedância elétrica (EIM - SKULPT®) para estimativa do percentual de gordura corporal (GC%) em adultos jovens. Sessenta adultos jovens foram voluntários para o estudo (mulheres, n=30, 25,0±7,7 anos; 21,5±1,9 kg/m2; e homens, n=30, 21,6±6,3 anos; 22,5±1,8 kg/m2). Os participantes fizeram medições de pletismografia de deslocamento de ar (ADP) e EIM para análise de concordância. A EIM foi realizada três vezes no mesmo dia para a análise de reprodutibilidade dentro do dia. Sete dias depois, 37 participantes repetiram as medições do EIM para a análise de reprodutibilidade entre dias. As comparações dos métodos EIM e ADP e medições repetidas do EIM foram realizadas com o teste T pareado ou medidas repetidas ANOVA de uma via, o gráfico de Bland-Altman e regressões lineares simples. O %GC foi maior (p<0,05) quando estimado pelo EIM (19,91 ± 5,70 para homens e 30,77 ± 5,89 para mulheres) em relação ao ADP (15,28 ± 5,66 para homens e 27,31 ± 5,98 para mulheres). A análise de Bland-Altman apresentou viés de 4,4% (IC95%=-3,4-12,2) e a regressão linear apresentou R2=0,78. Para a reprodutibilidade entre dias, as médias do EIM não diferiram (25,33±7,69 e 24,94±8,30, p=0,890). Assim, enquanto o dispositivo EIM exibiu alta reprodutibilidade das estimativas de %GC, deve-se ter cautela ao comparar os resultados com outras técnicas para medir %GC. O dispositivo EIM superestimou %GC em comparação com ADP. No entanto, as medidas de EIM apresentaram reprodutibilidade intradia e entre dias muito boa e, portanto, o dispositivo de EIM pode ser usado para monitoramento longitudinal de %GC.

Resumen Objetivo: Comparar la composición corporal por antropometría clásica e impedancia bioeléctrica en estudiantado universitario de una universidad peruana. Método: Estudio correlacional, transversal, conformado por 152 estudiantes (77 varones y 75 mujeres) matriculados en el ciclo 2018-II con edad promedio de 20.45 ± 3.63 años. Se calculó para el método de antropometría el índice de masa corporal, mediciones antropométricas y se utilizó la ecuación de Yuhasz (1974) para determinar el porcentaje de grasa corporal, y para el de bioimpedancia eléctrica se utilizó el analizador tetrapolar multifrecuencia de medición segmental directa Inbody 120. Para establecer la correlación se utilizó el coeficiente de Pearson y para la concordancia se utilizó el método gráfico de Bland Altman e índice de estabilidad. Los datos fueron analizados con el programa estadístico IBM SPSS Statistics, versión 27.0. Para todos los análisis se utilizó un nivel de significancia (p<0.05). Resultados: Existe una fuerte correlación estadísticamente significativa (p<0.01y r=0.95) entre los métodos de antropometría y bioimpedancia eléctrica para la determinación del porcentaje de grasa corporal; además, una buena concordancia según el método gráfico de Bland Altman, reforzada con el índice de estabilidad para complemento de la interpretación (IE= 95 %). Conclusiones: Ambos métodos son intercambiables entre sí, por lo que pueden ser usados indistintamente en este tipo de población para determinar el porcentaje de grasa corporal expresado en porcentaje.


Abstract Objective: The aim was to compare the body composition using classical anthropometry and bioelectric impedance in university students of a Peruvian university. Method: a correlational, cross-sectional study with a research sample of 152 students (77 men and 75 women) enrolled in the 2018-II period with an average age of 20.45 ± 3.63 years. The body mass index and anthropometric measurements were calculated for the anthropometric method. The Yuhasz (1974) equation was used to determine the body fat percentage. The electrical bioimpedance method implemented the tetrapolar multi-frequency direct segmental measurement analyzer Inbody 120. Pearson coefficient was used to establish the correlation, and the Bland Altman graphical method and stability index were used for concordance. The data were analyzed using the statistical software application IBM SPSS Statistics, version 27.0. A level of significance (p <0.05) was used for all analyzes. Results: there is a strong statistically significant correlation (p<0.01 and r = 0.95) between the anthropometric and electrical bioimpedance methods to determine body fat percentage. In addition, there is a good agreement according to the Bland Altman graphical method, reinforced by the stability index to complement the interpretation (IE = 95%). Conclusions: Both methods are interchangeable with each other, so they can be used indistinctly in this type of population to determine body fat percentage.

Resultados: há uma forte correlação estatística significativa (p<0,01 e r=0,95) entre os métodos de antropometria e bioimpedância elétrica para a determinação da porcentagem de gordura corporal; além disso, uma boa concordância de acordo com o método gráfico Bland Altman, reforçado com o índice de estabilidade para complementar a interpretação (IE= 95%). Conclusões: Ambos os métodos são intercambiáveis entre si, portanto, podem ser utilizados indistintamente nesse tipo de população para determinar a porcentagem de gordura corporal expressa como porcentagem.

ABSTRACT

Introduction: predictive equations to estimate body fat based on simple anthropometric parameters are easy to use in the clinical practice.Objective: to evaluate the relationship between predictive equations based on anthropometric parameters and bioelectrical impedance to estimate body fat in individuals undergoing bariatric surgery.Methods: a prospective and longitudinal study carried out with individuals undergoing bariatric surgery. Body weight, body mass index, waist circumference and body fat percentage estimated by anthropometric parameters and by impedance were evaluated at three moments, one month before, two and six months after surgery. Data were analyzed by one-way ANOVA for repeated measures with Holm-Sidak ́s post hoc or Friedman test with Tukey ́s post hoc, and Pearson or Spearman correlations, according to data distribution. Significance level adopted 5%.Results: twenty-five subjects composed the final sample. All anthropometric parameters reduced significantly over time (p<0.001). Except for Lean et al equation before surgery, the body fat percentage estimated by other formulas showed a strong correlation with impedance in all moments, with the highest correlation strength observed in Gómez-Ambrosi et al. equation.Conclusion: in the present study, the equations used showed a good correlation with bioelectrical impedance, and the Gómez-Ambrosi et al. equation as a better option to the use of bioimpedance to assess changes in body fat percentage of patients undergoing bariatric surgery for the treatment of severe obesity.


Introdução: Objetivos: analisar o consumo alimentar e os fatores associados ao estado nutricional de crianças menores de dois anos de vida.Método: Estudo de corte transversal realizado com uma amostra de 344 lactentes menores de dois anos de idade e suas respectivas mães, acompanhadas em Unidades de Saúde da Família. As variáveis sociodemográficas, antropométrica das mães e dos lactentes e o consumo alimentar dessas crianças foram avaliadas por meio de questionário estruturado. A força de associação entre as variáveis dependente e as independentes foi avaliada pelo odds ratio, tanto na análise univariada quanto na múltipla, com nível de significância de 5%.Resultados: A prevalência do estado nutricional inadequado foi de 38,08%. Observou que 29,09% das crianças menores de seis meses de idade não chegaram a receber leite materno de forma exclusiva ou o tempo de oferta foi inferior a 30 dias. Notou-se o consumo de alimentos ultraprocessados, principalmente, do suco industrializado no último mês [OR:1,96, IC 95%: 1,05-3,65], baixa ingestão de frutas e o hábito de comer assistindo televisão nos lactentes maiores de seis meses. Após ajuste para variáveis de confusão, permaneceram associadas ao estado nutricional: anemia gestacional [OR: 3,58 IC: 1,46-8,77] e trabalho materno [OR, 0,38, IC 95%: 0,18-0,80].Conclusão: A presença do estado nutricional inadequado, caracterizado pelo baixo ou excesso de peso, associou-se ao fato de a mãe trabalhar e à anemia gestacional. Ademais, constatou-se a participação precoce de alimentos ultraprocessados na alimentação das crianças menores de 24 meses de idade, substituindo alimentos considerados naturais e saudáveis, evidenciando assim práticas alimentares inadequadas frente às recomendações para a faixa etária.

SUMMARY: Bioelectrical impedance analysis (BIA) is useful for determining bioelectrical parameters and body composition. In turn, differences have been reported when comparing these variables in athletes by training status. Nevertheless, there is no evidence of bioelectrical impedance parameters in Street Workout (SW) athletes. Thus, this study aimed to compare bioelectrical parameters and body composition through BIA between trained and untrained SW athletes. Twenty-two male SW athletes were classified as trained (n=6; 26.3 y [21.0-28.9]) and untrained (n=16; 21.8 y [20.5-24.7]) based on their SW experience. A bioelectrical impedanciometer was used to estimate bioelectrical parameters and body composition. There was no difference in body composition between trained and untrained SW athletes. Regarding impedance, trained athletes had lower values in the upper limbs (right arm: p=0.049; left arm: p=0.027) and trunk (p=0.004), while phase angle values were higher in the upper limbs (right arm: p=0.004; left arm: p=0.001), and trunk (p=0.006), as well as the mean phase angle (p=0.007), than untrained athletes. Bioelectrical impedance parameter differences found between SW training level groups suggest an improvement of tissue qualities, such as muscle, with SW practice. Future longitudinal studies should corroborate if SW training modifies these parameters.


RESUMEN: El análisis de impedancia bioeléctrica es útil para determinar parámetros bioeléctricos y de composición corporal. A su vez, se han reportado diferencias al comparar estas variables en atletas según su estado de entrenamiento. Sin embargo, no hay evidencia de parámetros de impedancia bioeléctrica en atletas de Street Workout (SW). Por tanto, este estudio tuvo como objetivo comparar los parámetros bioeléctricos y la composición corporal entre atletas de Street Workout entrenados y no entrenados. Veintidós atletas masculinos de Street Workout fueron clasificados como entrenados (n=6; 26.3 años [21.0-28.9]) y no entrenados (n=16; 21.8 años [20.5-24.7]) en función de su experiencia en Street Workout. Se utilizó un impedanciómetro bioeléctrico para estimar los parámetros bioeléctricos y la composición corporal. No hubo diferencias en la composición corporal entre los atletas de SW en- trenados y no entrenados. En cuanto a la impedancia, los atletas entrenados tenían valores más bajos en los miembros superiores (brazo derecho: p=0,049; brazo izquierdo: p=0,027) y en el tronco (p=0,004), mientras que los valores del ángulo de fase eran más altos en los miembros superiores (brazo derecho: p=0,004; brazo izquierdo: p=0,001), en el tronco (p=0,006), así como la media del ángulo de fase (p=0,007) que los atletas no entrenados. Las diferencias en los parámetros de impedancia bioeléctrica encontradas entre los grupos según el nivel de entrenamiento de SW sugieren una mejora de las cualidades de los tejidos, como el músculo, con la práctica de SW. Futuros estudios longitudinales deberían corroborar si el entrenamiento SW modifica estos parámetros.

Abstract Assessment of the Nutritional Status (NS) allows screening for malnutrition and obesity, conditions associated with chronic non-communicable diseases. The fat mass index (FMI) stands out concerning traditional NS indicators. However, proposals that define thresholds for FMI are not sensitive to discriminate extreme cases (degrees of obesity or thinness). Only one proposal (NHANES), determined by total body densitometry (DXA), established eight categories of NS classification (FMI). However, DXA is expensive and not always clinically available. Our study aims to test the validity of the NHANES method using electrical bioimpedance (BIA) and skinfold thickness (ST) to classify NS. The FMI of 135 (69 women) university students aged 18 to 30 years old was determined using DXA, BIA, and ST. The agreement between the instruments (Bland-Altman) and the agreement coefficient in the NS classifications (Chi-square and Kappa index) were tested. The agreement test against DXA indicated that ST underestimated the FMI (-1.9 kg/m2) for both sexes and BIA in women (-2.0 kg/m2). However, BIA overestimated FMI (1.4 kg/m2) in men, although with less bias. There was no agreement between the NS classifications (NHANES) by FMI between DXA and BIA, or DXA and ST. The exception occurred between DXA and BIA in men who showed a slightly better consensus, considered "fair" (k = 0.214; p = 0.001). In conclusion, ST and BIA did not show enough agreement to replace DXA for NS classification, within NHANES thresholds. The FMI measurement tools for the NHANES classification of the categories of NS matters.


Resumo Avaliar o Estado Nutricional (EN) permite rastrear desnutrição e obesidade, condições associadas a doenças crônicas não transmissíveis. O índice de massa gorda (IMG) destaca-se em relação aos indicadores tradicionais de EN. No entanto, propostas que definem limiares para IMG não são sensíveis para discriminar casos extremos (graus de obesidade ou magreza). Apenas uma proposta (NHANES) estabeleceu oito categorias de classificação EN (IMG), mas foi determinada por densitometria corporal total (DXA). Porém, DXA é caro e nem sempre disponível. O objetivo foi testar a validade do método NHANES usando bioimpedância elétrica (BIA) e dobras cutâneas (DOCs) para classificar o EN. O IMG de 135 (69 mulheres) universitários com idade entre 18 e 30 anos foi obtido por DXA, BIA e DOCs. A concordância foi testada entre os instrumentos (Bland-Altman) e classificações de EN (Qui quadrado e índice Kappa). O teste de concordância com a DXA indicou as DOCs subestimarem o IMG (-1,9 kg/m2) para ambos os sexos e a BIA em mulheres (-2,0 kg/m2). No entanto, as BIA superestimaram o IMG (1,4 kg/m2) nos homens, embora com menos viés. Não houve concordância entre as classificações de EN (NHANES) pelo IMG entre DXA e BIA/DOCs. A exceção ocorreu entre DXA e BIA em homens que apresentaram concordância "razoável" (k = 0,214; p = 0,001). Em conclusão, DOCs e BIA não mostraram concordância suficiente para substituir DXA pela classificação de EN, dentro dos limites NHANES. As ferramentas diferem para medir IMG e classificar categorias de EN (NHANES).

ABSTRACT Objective We investigated the utility of the phase angle as a screening tool for sarcopenia. Methods We performed a cross-sectional study that included 169 active community-dwelling elderly women. The phase angle was determined using tetrapolar bioelectrical impedance, and sarcopenia was diagnosed based on skeletal muscle mass, muscle strength, and physical performance using bioelectrical impedance analysis, a handheld dynamometer, and the gait speed test, respectively. Receiver operating characteristic curve analysis was performed to investigate the role of the phase angle as a predictor of sarcopenia. Results The prevalence of sarcopenia was 12.4%. The median phase angle was 5.30°; elderly women with sarcopenia had lower phase angles than those without sarcopenia (p=0.006). The phase angle cutoff for the detection of sarcopenia was ≤5.15°, with an area under the curve of 0.685, sensitivity 81.0%, specificity 60.8%, and accuracy 63.31%. Elderly women with a low phase angle show a high risk of presenting with reduced muscle mass. Conclusions The phase angle was shown to be a useful screening tool in elderly women with sarcopenia.


RESUMO Objetivo Avaliar o ângulo de fase como método de triagem para sarcopenia. Métodos Estudo transversal realizado com 169 idosas ativas de comunidade. O ângulo de fase foi obtido por bioimpedância elétrica tetrapolar e a sarcopenia foi diagnosticada a partir da massa muscular esquelética, força muscular e performance física utilizando bioimpedância elétrica, dinamometria manual e o teste de velocidade de marcha, respectivamente. A curva Receiver Operating Characteristic foi construída para avaliar o ângulo de fase como preditor de sarcopenia. Resultados A prevalência de sarcopenia foi de 12,4%. A mediana do ângulo de fase foi de 5,30°; idosas com sarcopenia apresentaram valores menores do ângulo de fase em relação às não sarcopênicas (p=0,024). O ponto de corte do ângulo de fase para identificar sarcopenia foi ≤5,15°, com área sob a curva de 0,685, sensibilidade de 81,0%, especificidade de 60,8% e acurácia de 63,31%. Observou-se que idosas com ângulo de fase reduzido apresentaram maior chance de terem massa muscular reduzida e sarcopenia. Conclusão O ângulo de fase se mostrou útil para triagem de idosas com sarcopenia.

Objective:Four methods were used to count platelets to recommend suitable retest methods for abnormal blood platelet count.Methods:A total of 300 patients who received treatment in the Second People's Hospital of Lianyungang during August-September 2020 were included in this study. They were divided into low-value, median-value, and high-value groups ( n = 100/group) according to blood platelet counts determined by the electrical impedance method. The consistency in blood platelet counting was analyzed between flow cytometry and electrical impedance method, Neubauer chamber method, and modified blood smear method. Results:There was no significant difference in blood platelet count between the electrical impedance method [low-value group: (86.1 ± 10.3) × 10 9/L, median-value group: (221.8 ± 41.8) × 10 9/L, high-value group: (441.3 ± 51.4) × 10 9/L, Neubauer chamber method [low-value group: (85.2 ± 10.1) × 10 9/L, median-value group: (219.3 ± 37.7) × 10 9/L, high-value group: (443.1 ± 54.5) × 10 9/L, modified blood smear technique [low-value group: (86.1 ± 10.1) × 10 9/L, median-value group: (218.1 ± 37.7) × 10 9/L, high-value group: (442.6 ± 53.3) × 10 9/L], and flow cytometry [low-value group: (85.4 ± 10.1) × 10 9/L, median-value group: (220.7 ± 42.0) × 10 9/L, high-value group: (440.9 ± 50.0) × 10 9/L] (all P > 0.05). The Bland-Altman analysis revealed that the electrical impedance method, Neubauer chamber method and modified blood smear method, and flow cytometry showed consistency in blood platelet count. Conclusion:The modified blood smear method showed consistency with the electrical impedance method, Neubauer chamber method, and flow cytometry in blood platelet counting. It does not require a special instrument and can help observe cell morphology for blood platelet counting. In addition, the blood smears are easy to be preserved. The modified blood smear technique should be the first choice for re-checking blood platelet counts.

ABSTRACT

RESUMEN La preeclampsia es una importante causa de mortalidad materna mundial, identificar de manera temprana las gestantes con riesgo de desarrollarla, es una medida preventiva de gran impacto. La medición del agua corporal se ha documentado desde 1994, y desde 2015 se demostró su relación con el riesgo de desarrollar preeclampsia. Por lo anterior se realizó una revisión de la relación del agua corporal y la preeclampsia hasta junio de 2019, con 17 artículos seleccionados. Producto de la revisión se concluyó que la medición del agua corporal en gestantes podría detectar la aparición de preeclampsia para establecer un seguimiento estricto temprano a las mujeres con mayor riesgo de presentarla. Estas mediciones se realizan con métodos sencillos, no invasivos y de bajo costo, como la impedancia eléctrica por análisis espectral. Sin embargo, se requieren estudios con mayor rigor metodológico para el estudio de una prueba diagnóstica como la que se propone. MÉD.UIS.2021;34(2): 49-60.


ABSTRACT Preeclampsia is an important cause of maternal mortality worldwide. Early identification of pregnant women at risk of developing it is a preventive measure of great impact. Body water measurement has been documented since 1994, and since 2015 it was stablished its relationship with risk of developing preeclampsia. Therefore, a review of the relationship between body water and preeclampsia was carried out until June 2019, with 17 papers selected. As a result of the review, it was concluded that the measurement of body water in pregnant women could detect the appearance of preeclampsia to establish a strict early follow-up of women with a higher risk of presenting it. These measurements are made with simple, non-invasive and low-cost procedure, such as electrical impedance by spectral analysis. However, studies with greater methodological rigor are required for the study of a diagnostic test such as the one proposed. MÉD.UIS.2021;34(2): 49-60.

Resumo Fundamento: O agravamento da função renal (AFR) é frequentemente observado na terapia agressiva com diuréticos para o tratamento de insuficiência cardíaca aguda descompensada (ICAD) e está associado com piores desfechos em alguns estudos. Objetivo: Avaliar a relação de AFR e congestão na alta hospitalar com ocorrência de eventos (morte cardíaca ou internação por insuficiência cardíaca). Métodos: Oitenta pacientes com ICAD foram estudados. O AFR foi definido por um aumento absoluto (≥0,5 mg/dL) nos níveis séricos de creatinina a partir dos valores obtidos na admissão. Concentrações de peptídeo natriurético do tipo B (BNP) e lipocalina associada à gelatinase neutrofílica (NGAL) foram medidas na admissão e na alta hospitalar. Congestão foi avaliada na alta utilizando a análise vetorial de bioimpedância elétrica (BIVA). O desfecho primário foi o tempo para o primeiro evento, definido como uma combinação de morte cardíaca ou hospitalização por insuficiência cardíaca. Análise de curva Característica de Operação do Receptor (curva ROC) foi realizada para determinar o ponto de corte de IH mais adequado para predição de eventos. Curvas Kaplan-Meier de sobrevida livre de eventos foram construídas e comparadas usando o teste de log-rank. Modelos de riscos proporcionais de Cox foram usados para investigar a associação com eventos. O critério para se estabelecer significância estatística foi um p<0.05. Resultados: A idade média foi 60,6 ± 15,0 anos, e 48 (60%) pacientes eram do sexo masculino. A fração de ejeção média foi 35,3±7,8%. O AFR ocorreu em 37,5% da amostra. A creatinina basal associou-se com AFR (p<0,001), mas nem BNP (p=0,35) nem NGAL (p=0,18) na admissão foram preditores de AFR. Usando modelos de riscos proporcionais de Cox, o índice de hidratação na alta, estimado por BIVA, associou-se significativamente com ocorrência de eventos (HR 1,39; IC95% 1,25-1,54, p<0,0001), mas não com AFR (HR 2,14; IC95% 0,62-7,35, p=0,22). Conclusão: A congestão persistente na alta associou-se com piores desfechos. O AFR parece estar relacionado com alterações hemodinâmicas durante o processo de descongestionamento, mas não com lesões renais.


Abstract Background: Worsening renal function (WRF) is frequently observed in the setting of aggressive diuresis for the treatment of acute decompensated heart failure (ADHF) and is associated with poor outcomes in some studies. Objective: We sought to assess the relationship of WRF and congestion at discharge with events (cardiac death or heart failure hospitalization). Methods: Eighty patients with ADHF were studied. WRF was defined by an absolute increase in serum creatinine of ≥0.5 mg/dL from the values measured at the time of admission. B-type natriuretic peptide (BNP) and plasma neutrophil gelatinase-associated lipocalin (NGAL) were measured at admission and at discharge. Congestive state at discharge was assessed using bioelectrical impedance vector analysis (BIVA). Primary endpoint was time to first event defined as a combination of cardiac death or heart failure hospitalization. Receiver operating characteristic (ROC) curve analysis was used to determine the best hydration index cutoff to predict events. Kaplan-Meier event-free survival curves were constructed and compared using the log-rank test. Cox proportional hazards models were used to investigate the association with events. The criterion for determining statistical significance was p<0.05. Results: Mean age was 60.6±15 years, and 48 (60%) were male. Mean ejection fraction was 35.3±7.8%. WRF occurred in 37.5% of the sample. Baseline creatinine was associated with WRF (p<0.001), but neither admission BNP (p=0.35) nor admission NGAL (p=0.18) was predictor of WRF. Using Cox proportional hazard models, hydration index at discharge calculated with BIVA was significantly associated with events (HR 1.39, 95% CI 1.25-1.54, p<0.0001) but not WRF (HR 2.14, 95% CI 0.62-7.35, p=0.22). Conclusion: Persistent congestion at discharge was associated with worse outcomes. WRF seems to be related to hemodynamic changes during the decongestion process but not to kidney tubular injuries.

Introdução: O câncer infantil gera um grande impacto para a saúde pública. A avaliação da composição corporal pela impedância bioelétrica fornece os valores de ângulo de fase, diretamente relacionados a alterações funcionais na membrana celular e que têm sido usados para avaliar o estado nutricional e o prognóstico. Objetivo: Avaliar a correlação entre o ângulo de fase e os parâmetros do estado nutricional de pacientes oncológicos pediátricos em tratamento. Método: Estudo transversal realizado em um hospital público universitário. O estado nutricional foi verificado por meio das medidas de peso, altura, circunferência do braço, dobra cutânea tricipital, índice de massa corporal e circunferência muscular do braço. O ângulo de fase foi determinado pela impedância bioelétrica. Resultados: Foram incluídos 13 pacientes com média de idade de 103,2±39,7 meses, sendo 61,5% do sexo masculino, com prevalência da leucemia entre os diagnósticos. Constatou-se que a maioria (53,8%) dos pacientes estava em eutrofia. Houve forte correlação do ângulo de fase com os parâmetros de massa magra (r=0,923; p=0,000), massa corporal celular (r=0,911; p=0,000), massa extracelular (r=0,897; p=0,000) e peso corporal (r=0,920; p=0,000). Conclusão: Observaram-se correlações expressivas entre o ângulo de fase e os indicadores antropométricos, reforçando a hipótese de que o ângulo de fase pode antecipar a identificação de alterações na composição corporal, possibilitando uma intervenção nutricional precoce e melhor prognóstico.


Introduction: Child cancer has a major impact on public health. The evaluation of body composition by bioelectric impedance provides the values of phase angle, causally related to functional changes in the cell membrane which have been used to evaluate nutritional status and prognosis. Objective: To evaluate the correlation between phase angle and parameters of the nutritional status of pediatric oncologic patients under treatment. Method: Cross-sectional study performed in a public university hospital. The nutritional status was evaluated by weight, height, arm circumference, triceps skin fold, body mass index and arm muscle circumference. The phase angle was determined by bioelectric impedance. Results: 13 patients were included with a mean age of 103.2±39.7 months, 61.5% males with prevalence of leukemia in the diagnoses. It was found that most (53.8%) of the patients were eutrophic. There was a strong correlation of the phase angle with the parameters of lean mass (r=0.923; p=0.000), cellular body mass (r=0.911; p=0.000), extracellular mass (r=0.897; p=0.000) and body weight (r=0.920; p=0.000). Conclusion: Expressive associations between phase angle and other anthropometric indicators were observed, reinforcing the hypothesis that it can anticipate the identification of changes in body composition, allowing early nutritional intervention and better prognosis.

La evaluación de la composición corporal por impedancia bioeléctrica proporciona los valores del ángulo de fase, directamente relacionados con los cambios funcionales de la membrana celular y que se han utilizado para evaluar el estado nutricional y el pronóstico. Objetivo: Evaluar la correlación entre el ángulo de fase y los parámetros del estado nutricional de los pacientes oncológicos pediátricos en tratamiento. Método: Estudio transversal realizado en un hospital universitario público. El estado nutricional se evaluó por peso, altura, circunferencia del brazo, pliegue de la piel del tríceps, índice de masa corporal y circunferencia muscular del brazo. El ángulo de fase fue determinado por la impedancia bioeléctrica. Resultados: Se incluyeron 13 pacientes con una edad media de 103,2±39,7 meses, de los cuales el 61,5% eran hombres con prevalencia de leucemia entre los diagnósticos. Se descubrió que la mayoría (53,8%) de los pacientes eran eutróficos. Hubo una fuerte correlación del ángulo de fase con los parámetros de masa magra (r=0,923; p=0,000), masa corporal celular (r=0,911; p=0,000), masa extracelular (r=0,897; p=0,000) y peso corporal (r=0,920; p=0,000). Conclusión: Se observaron asociaciones expresivas entre el ángulo de fase y otros indicadores antropométricos, lo que refuerza la hipótesis de que puede anticiparse a la identificación de cambios en la composición corporal, lo que permite una intervención nutricional temprana y un mejor pronóstico.

ABSTRACT Objective: To explore changes in the nutritional status of pediatric cancer patients before and after chemotherapy and evaluate the correlation between deuterium oxide dilution, bioelectric impedance analysis, and anthropometry for assessment of body composition. Methods: This study included 14 children (aged 5.6 to 13.6 years) and classified them as having hematologic or solid tumors. They had their body composition analyzed according to deuterium oxide, bioelectric impedance, and anthropometric measurements before the first chemotherapy cycle and after three and six months of therapy. Results: The patients in the hematologic tumor group had an increase in weight, height, body mass index, waist, hip, and arm circumference, subscapular skinfold thickness, and fat mass with the isotope dilution technique during chemotherapy. In the solid tumor group, the children showed a reduction in fat-free mass when assessed by bioimpedance analysis. We found a positive correlation between the triceps skinfold thickness and fat mass determined by bioimpedance analysis and deuterium oxide. The arm muscle circumference correlated with the fat-free mass estimated by bioimpedance analysis and deuterium oxide. Conclusions: Patients with hematologic tumors had an increase in body weight, height, and fat mass, which was not identified in the solid tumor group. The positive correlation between anthropometry (triceps skinfold thickness and arm muscle circumference), deuterium oxide dilution, and bioelectric impedance analysis shows the applicability of anthropometry in clinical practice.


RESUMO Objetivo: Verificar mudanças no estado nutricional de crianças com câncer antes e após o tratamento quimioterápico e avaliar a correlação entre diluição de óxido de deutério, análise de impedância bioelétrica (BIA) e dados antropométricos. Métodos: Quatorze crianças (entre 5,6 e 13,6 anos de idade) foram incluídas e classificadas como tendo tumores hematológicos ou sólidos. A composição corporal foi medida pelo óxido de deutério, impedância bioelétrica e medidas antropométricas antes da primeira quimioterapia e após três e seis meses de terapia. Resultados: Os pacientes do grupo de tumores hematológicos aumentaram o peso, a estatura, o índice de massa corporal, a circunferência da cintura, quadril e braço, dobra cutânea subescapular e a massa gorda com a técnica de diluição isotópica durante o tratamento quimioterápico. No grupo de tumores sólidos, as crianças mostraram uma redução na massa magra quando avaliadas por análise de impedância. Houve uma correlação positiva entre a dobra cutânea tricipital e a massa gorda determinadas pela análise de impedância e pelo óxido de deutério. A circunferência muscular do braço correlacionou-se com a massa magra estimada pela análise de impedância e pelo óxido de deutério. Conclusões: Pacientes com tumores hematológicos tiveram aumento no peso corporal, estatura e massa gorda, o que não foi observado naqueles com tumores sólidos. A boa correlação entre a antropometria (dobra cutânea tricipital e circunferência muscular do braço), a diluição do óxido de deutério e a análise da impedância bioelétrica mostra a aplicabilidade da antropometria na prática clínica.

ABSTRACT

In the COVID-19 outbreak we are living currently, the lung ultrasound can be considered as an advantageous tool to examine patients as it can be easily used by a maximum of two trained operators, at the bedside. Likewise, the Electrical impedance tomography is useful to monitor and optimize the patient's ventilation strategies in real time. Here, we present the impact and implication of ultrasonography and electrical impedance as management tools for treating critical patients in times of COVID-19. Clinical Ultrasound has been used more and more in developing countries in Latin America such as Colombia, Argentina, and Uruguay among others, however it is still underutilized due to different issues amongst we found the absence of equipment or the lack of basic training from the medical staff and poor skills to use the machines despite their availability. The use of lung ultrasound and the Electrical impedance tomography is highlighted as a safe manner to manage COVID-19 patients, not as a replacement of a CT scan, but as an easy-to-use and fast complementary tool.


En la pandemia de COVID-19 que se vive actualmente, la ultrasonografía pulmonar puede considerarse como una herramienta ventajosa para examinar a los pacientes, ya que puede ser utilizada fácilmente al pie de cama por un máximo de dos operadores capacitados. Asimismo, la tomografía de impedancia eléctrica es útil para monitorizar y optimizar las estrategias de ventilación del paciente en tiempo real. Aquí se presenta el impacto y la implicación de la ecografía y la impedancia eléctrica como herramientas de manejo para el tratamiento de pacientes críticos en tiempos de COVID-19. El ultrasonido clínico se ha utilizado cada vez más en países en vías de desarrollo de América Latina como Colombia, Argentina y Uruguay entre otros, sin embargo, aún está infrautilizado debido a diferentes cuestiones entre las que encontramos la ausencia de equipos o la falta de formación básica del personal médico y la poca habilidad para usar las máquinas a pesar de su disponibilidad. Se destaca el uso de la ultrasonografía pulmonar y la tomografía de impedancia eléctrica como una forma segura de manejar a los pacientes con COVID-19, no como un reemplazo de una tomografía computarizada, sino como una herramienta complementaria rápida y fácil de usar.

Background: Equations for the evaluation of fat-free mass (FFM) and fat mass (FM) with Bioelectrical impedance analysis (BIA) were formulated in Caucasian populations. International recommendations suggest that population-specific equations should be formulated. Aim: To validate an equation previously formulated in Chileans adults and compare it to a new equation generated on an independent sample. Material and Methods: In 108 adult volunteers aged 38.1±14.1 years (44% males), with a body mass index (BMI) of 25.1± 4.1 kg/m2, body composition was measured by BIA (Bodystat) and dual X-ray absorptiometry (DXA: Lunar Prodigy). Body composition estimated using Schifferli equation and BIA were compared with DEXA, by the Bland-Altman method and simple linear regression. Results: FFM and FM measured by DXA were 45.2 ± 9.8 kg and 29.6 ± 11.7 % respectively. Resistance was 467.7 ± 76.3 ohm. Schifferli equation and BIA significantly overestimated FFM by 7.3 and 7.4 kg, respectively. The error was higher for high levels of FFM (slope β < 1, p < 0.01). Both equations underestimated FM measured by DXA (averages of 7.5 and 7.8%, respectively, p < 0.01), without a differential bias for Schifferli equation, but with a bias in low levels of FM measured with BIA (slope β < 1, p < 0.01). Estimation biases could be eliminated using the regression coefficients. Conclusions: Both equations behave similarly and have biases, although less with Schifferli. Statistically correcting for biases, the new adjusted equations provide clinically valid estimates of FFM and FM. Equations should not only be population-specific, but also device-specific.

INTRODUCTION: Pregnancy-related diseases have aggravated the situation of postpartum women, who end up using Mechanical Ventilation (MV) when admitted to Intensive Care Units (ICU). Although MV has benefits, it is associated with deleterious effects that can be minimized with the use of Electrical Impedance Tomography (EIT). OBJECTIVE: The aim was to analyze the epidemiological profile and ventilatory parameters of mothers, which developed HELLP Syndrome, sepsis and/or Acute Respiratory Distress Syndrome (ARDS), under MV and monitored with EIT. METHODS: The study was observational, cross-sectional, retrospective and prospective conducted between March and September 2018, using data collection forms filled from the database and sociodemographic, obstetric and ventilatory records of postpartum women admitted to adult ICU. RESULTS: The sample consisted of 13 postpartum women, 8 with sepsis (61.5%), 7 with HELLP syndrome (53.8%) and 4 with ARDS (30.8%). Five patients (38.5%) evolved with more than one of these conditions. Regarding the ventilatory parameters evaluated, VT 378.9 (±103.9) mL were observed and mean values found for PEEP 9.8 (±1.9) cmH2O and driving pressure 11.1 (±1.4) cmH2O are below recommendations in the literature, predicting lower mortality and morbidity index. CONCLUSION: The relevance of the driving pressure assessment in the MV setting was demonstrated, a parameter assessed by the EIT and directly related to static lung compliance (Cstat), PEEP, VT and optimization of regional pulmonary ventilation. It is highlighted the need for future research with greater clinical significance regarding the profile of postpartum women about the increasingly frequent diseases in this population.


INTRODUÇÃO: As doenças relacionadas à gravidez tem agravado o quadro de puérperas, que acabam fazendo uso de Ventilação Mecânica (VM) quando internadas em Unidades de Terapia Intensiva (UTI). A VM, apesar de trazer benefícios, está associada a efeitos deletérios que podem ser minimizados com o uso da Tomografia por Impedância Elétrica (TIE). OBJETIVO: Analisar o perfil epidemiológico e parâmetros ventilatórios de puérperas que evoluíram com Síndrome HELLP, Sepse e/ou Síndrome do Desconforto Respiratório Agudo (SDRA), sob VM e monitoradas com a TIE. MÉTODOS: Estudo observacional, transversal retrospectivo e prospectivo, realizado entre março e setembro de 2018, por meio de fichas de coletas preenchidas a partir de banco de dados/prontuários sociodemográficos, obstétricos e ventilatórios de puérperas internadas em UTI. RESULTADOS: Amostra composta por 13 puérperas, oito com Sepse (61,5%), sete com Síndrome HELLP (53,8%) e quatro com SDRA (30,8%), demonstrando que cinco (38,5%) pacientes evoluíram com mais de uma dessas patologias. Acerca dos parâmetros ventilatórios avaliados, observou-se VT 378.9 (± 103.9) e que valores médios encontrados para PEEP 9.8 (±1.9) e driving pressure 11.1 (±1.4) estão abaixo dos preconizados pela literatura, predizendo menores índices de mortalidade e morbidade. CONCLUSÃO: Demonstrou-se relevância da avaliação de driving pressure no cenário da VM, parâmetro avaliado por meio da TIE e diretamente relacionado à Cst, PEEP, VT e otimização da ventilação pulmonar regional. Destaca-se a necessidade de pesquisas futuras que apresentem maiores significâncias clínicas voltadas ao perfil de puérperas em relação às doenças cada vez mais frequentes nesta população.

ABSTRACT Objective To assess the utility of bioimpedance (BIA) and skinfolds thickness (SF) in body fat percentage measuring (%BF) compared to the reference method dual-energy x-ray absorptiometry (DXA) in Brazilian reproductive age women, as well as to estimate of inter- and intra-observer precision for SF. Subjects and methods 170 women aged 18-37 years with BMI between 18 and 39.9 kg/m2 were selected for this cross-sectional study. Body density was evaluated through equations proposed by Jackson, Pollock and Ward (1980) (EqJPW) and Petroski (1995) (EqPET), and %BF was estimated by BIA, DXA and Siri's formula (1961). The SF were measured by two separate observers: A and B (to determine inter-observer variability), who measured the folds at three times with 10-minute interval between them (to determine intra-observer variability - we used only observer A). Results The %BF by DXA was higher than those measured by SF and BIA (p<0.01, for all) of 90 volunteers. The Lin coefficient of agreement was considered satisfactory for %BF values obtained by EqJPW and BIA (0.55) and moderate (0.76) for sum of SF (ΣSF) values obtained by EqJPW and EqPET. No agreement was observed for the values obtained by SF (EqJPW and EqPET), BIA and DXA. Analysis of inter- and intra-observer of 59 volunteers showed that different measures of SF thickness met acceptability standards, as well as the % BF. Conclusion BIA and SF measurements may underestimate %BF compared with DXA. In addition, BIA and SF measurements are not interchangeable with DXA. However, our results suggest the equation proposed by Jackson, Pollock and Ward (three skinfolds) compared to BIA are interchangeable to quantify the %BF in Brazilian women in reproductive age. Furthermore, our results show acceptable accuracy for intra- and inter-observer skinfold measurements. Arch Endocrinol Metab. 2020;64(3):257-68

SUMMARY OBJECTIVE The current study aimed to examine the body composition of adult male ultra-trail runners (UTR) according to their level of participation (regional UTR-R, vs. national UTR-N). METHODS The sample was composed of 44 adult male UTR (aged 36.5±7.2 years; UTR-R: n=25; UTR-N: n=19). Body composition was assessed by air displacement plethysmography, bioelectrical impedance, and dual-energy X-ray absorptiometry. In addition, the Food Frequency Questionnaire (FFQ) was applied. A comparison between the groups was performed using independent samples t-test. RESULTS Significant differences between groups contrasting in the competitive level were found for chronological age (in years; UTR-R: 38.8±8.2 vs. UTR-N: 33.5±4.1); body density (in L.kg-1; UTR-R: 1.062±0.015 vs. UTR-N: 1.074±0.009); and fat mass (in kg; UTR-R: 12.7±6.8 vs. UTR-N: 7.6±2.7). CONCLUSION UTR-N were younger, presented higher values for body density, and had less fat mass, although no significant differences were found for fat-free mass. The current study evidenced the profile of long-distance runners and the need for weight management programs to regulate body composition.


RESUMO OBJETIVO O presente estudo objetivou examinar a composição corporal dos corredores de ultra-trail (UTR) e, adicionalmente, comparar dois grupos de acordo com o nível de participação (Regional vs. Nacional, respectivamente UTR-R e UTR-N). MÉTODOS A amostra foi composta por 44 corredores adultos masculinos (36,5±7,2 anos de idade; UTR-R: n=25; UTR-N: n=19). A composição corporal foi avaliada recorrendo à pletismografia de ar deslocado, bioimpedância elétrica e absorciometria de raios X de dupla energia. Adicionalmente, foi utilizado o Questionário de Frequência Alimentar. A comparação entre grupos foi realizada com base na prova t-student para amostras independentes. RESULTADOS Foram encontradas diferenças significativas por nível de competição para as seguintes variáveis dependentes: idade cronológica (em anos; UTR-R: 38,8±8,2 vs. UTR-N: 33,5±4,1); densidade corporal (em kg/L; UTR-R: 1,062±0,015 L/kg vs. UTR-N: 1,074±0,009); massa gorda (em kg; UTR-R: 12,7±6,8 kg vs. UTR-N: 7,6±2,7). CONCLUSÃO Os UTR-N tendem a ser mais jovens e apresentam valores superiores de densidade corporal e, consequentemente, valores menores de massa gorda, sendo a massa isenta de gordura semelhante entre os grupos. O presente estudo determinou o perfil dos corredores adultos masculinos de longa distância (ultra-trail), realçando a importância de uma cuidadosa regulação da massa corporal.

ABSTRACT Objective: To evaluate the hydration of obese and non-obese adolescents by vectorial bioimpedance analysis, in addition to verifying the associations between obesity and bioelectrical impedance vectorial analysis (BIVA) parameters. Methods: A cross-sectional study with adolescents between 14 and 18 years old (n=489, 300 boys and 189 girls). Electric bioimpedance (BIA; Quantum_II, RJL system, Rome, Italy) provided resistance and reactance parameters to calculate phase angle (PA), fat-free mass (FFM) and body fat (%BF). The confidence ellipses were compared, and the construction of the tolerance ellipses allowed individual and qualitative evaluation of the vectors and classification in dehydrated, normohydrated and hyperhydrated. Results: 78 obese and 411 eutrophic adolescents participated. Resistance (p<0.001) and reactance (p<0.001) and their normalization by stature (p<0.001) were reduced in the obese, whereas the PA was higher (p=0.003). %BF was 11.3% higher in obese adolescents. The main vector of the obese, both male (D=1.38; p<0.001) and female (D=1.49; p<0.001), indicated greater hydration. The ellipse of tolerance of the total sample showed that 25 (32.1%) were hyperhydrated and 02 (2.6%) vectors positioned in the sense of dehydration. A total of 17 (53.2%) girls and 16 (34.8%) boys were hyperhydrated. Logistic regression showed an inverse relation of BMI with resistance (p<0.001), reactance (p<0.001) and both normalized by stature. Adolescents with increased PA (p<0.001) were twice as likely to present obesity. Conclusions: Obese adolescents were hyperhydrated and there was an inverse relationship of BMI with resistance and direct with PA.


RESUMO Objetivo: Avaliar a hidratação de adolescentes obesos e não obesos por meio dos gráficos da análise vetorial por bioimpedância (BIVA), além de verificar as associações entre a obesidade e os parâmetros da BIVA. Métodos: Estudo transversal realizado com adolescentes entre 14 e 18 anos (n=489, 300 meninos e 189 meninas). A bioimpedância elétrica (BIA; Quantum_II, RJL system, Roma, Itália) ofereceu os parâmetros de resistência e reactância para calcular o ângulo de fase (AF), a massa livre de gordura (MLG) e a gordura corporal (GC). As elipses de confiança foram comparadas e a construção das elipses de tolerância permitiu a avaliação individual e a qualitativa dos vetores e, ainda, a classificação em desidratados, normohidratados e hiper-hidratados. Resultados: Participaram do estudo 78 adolescentes obesos e 411 eutróficos. A resistência (p<0,001), a reactância (p<0,001) e a normalização desses pela estatura (p<0,001) estavam reduzidas nos obesos, enquanto o AF foi superior (p=0,003). O percentual de gordura corporal (%GC) foi 11,3% maior nos adolescentes obesos. O vetor principal dos obesos, tanto masculino (Distância de Mahalanobis (D)=1,38; p<0,001) quanto feminino (D=1,49; p<0,001), indicou maior hidratação. A elipse de tolerância da amostra total mostrou que 25 adolescentes (32,1%) estavam hiper-hidratados e dois (2,6%) vetores posicionados no sentido de desidratação. Dezessete (53,2%) meninas e 16 (34,8%) meninos estavam hiper-hidratados. A regressão logística mostrou relação inversa do índice de massa corpórea (IMC) com a resistência (p<0,001) e a reactância (p<0,001), ambas normalizadas pela estatura. Os adolescentes com aumento do AF (p<0,001) apresentaram duas vezes mais chances de serem classificados como obesos. Conclusões: Os adolescentes obesos estavam hiper-hidratados e houve relação inversa do IMC com a resistência e direta com o AF.

Abstract Objective: To analyze the dual interference between cardiac implantable electronic devices (CIEDs) and bioelectrical impedance analysis (BIA). Methods: Forty-three individuals admitted for CIEDs implantation were submitted to a tetrapolar BIA with an alternating current at 800 microA and 50 kHz frequency before and after the devices' implantation. During BIA assessment, continuous telemetry was maintained between the device programmer and the CIEDs in order to look for evidence of possible electric interference in the intracavitary signal of the device. Results: BIA in patients with CIEDs was safe and not associated with any device malfunction or electrical interference in the intracardiac electrogram of any electrode. After the implantation of the devices, there were significant reductions in BIA measurements of resistance, reactance, and measurements adjusted for height resistance and reactance, reflecting an increase (+ 1 kg; P<0.05) in results of total body water and extracellular water in liter and, consequently, increases in fat-free mass (FFM) and extracellular mass in kg. Because of changes in the hydration status and FFM values, without changes in weight, fat mass was significantly lower (-1.2 kg; P<0.05). Conclusion: BIA assessment in patients with CIEDs was safe and not associated with any device malfunction. The differences in BIA parameters might have occurred because of modifications on the patients' body composition, associated to their hydration status, and not to the CIEDs.
