ABSTRACT

ObjectiveTo explore the clinical efficacy of Huanglian Jiedutang as an adjunctive treatment for acute cerebral infarction complicated with gastric motility disorder. MethodSixty patients with acute cerebral infarction complicated with gastric motility disorder with fire toxin syndrome were randomly divided into a western medicine control group (control group) and a traditional Chinese medicine (TCM) combined treatment group (observation group), with 30 cases in each group. The control group received basic treatment for cerebral infarction and relevant western medical symptomatic treatment based on the patients' gastrointestinal symptoms. The observation group received Huanglian Jiedutang in addition to the treatment provided to the control group. The treatment course was 7 days. Neurological deficit scores and gastrointestinal dysfunction scores were assessed in both groups before treatment and on the 4th and 7th days of treatment. Gastrointestinal electrographic parameters, serum citrulline (CIT), and motilin (MTL) levels were measured in both groups before treatment and on the 7th day of treatment. Clinical efficacy was compared between the two groups. ResultCompared with the baseline in both groups, the neurological deficit scores and gastrointestinal dysfunction scores were significantly reduced on the 4th and 7th days of treatment (P<0.05). The reductions in these scores were more significant on the 7th day compared with those on the 4th day of treatment (P<0.05). On the 4th and 7th days of treatment, the observation group showed a significantly greater reduction in neurological deficit scores and gastrointestinal dysfunction scores compared with the control group (P<0.05). On the 7th day of treatment, compared with the baseline, both groups showed a significant increase in gastric antral and gastric body electric wave amplitudes as well as serum CIT and MTL levels (P<0.05), and there were no statistically significant differences in the frequency of gastric antral and gastric body electric waves. On the 7th day of treatment, compared with the control group, the observation group had a significant increase in gastric antral and gastric body electric wave amplitudes as well as serum CIT and MTL levels (P<0.05), and there were no statistically significant differences in the frequency of gastric antral and gastric body electric waves. After 7 days of treatment, the total effective rate in the observation group was 90.00% (27/30), higher than 76.67% (23/30) in the control group, but the difference was not statistically significant. ConclusionAdjunctive treatment with Huanglian Jiedutang can effectively improve the symptoms of neurological function impairment and gastrointestinal dysfunction in patients with acute cerebral infarction complicated with gastric motility disorder, increase gastric antral and gastric body electric wave amplitudes, improve gastric motility disorder, and increase serum CIT and MTL levels, thereby improving the imbalanced secretion function of the gastrointestinal tract.

ABSTRACT

[Introduction] Acupuncture has been used in patients with functional digestive symptoms. Acupuncture treatment was used on a patient with upper stomach pain and feeling of fullness accompanied by pain in knee, neck and shoulders. Details of the clinical experience have been written in this brief report and shows improvement in gastric symptoms and increase of normogastria in gastric electrical activities by acupuncture treatment.[Case] A 73 year-old female patient, who was 148 cm tall and 51 kg in weight, complained of some chronic pain at the first medical examination. However, in the 24th treatment, the patient complained of severe stomach pain and feeling of fullness accompanied by 36 points on the gastrointestinal symptom rating scale (GSRS). Therefore, electrogastrograms (EGG) were recorded, as a noninvasive clinical evaluation of gastric function, at 15 minutes before and after the 30th treatment for 10 minutes using the retaining needle technique at bilateral ST36 andST40.[Result] The decrease of GSRS showed improvement of gastric symptoms by acupuncture treatment. After treatment, the percentage of normogastria in EGG increased to 71% from 58%, thus stabilizing theincrease in power spectra.[Discussion] It has been suggested that the increase of nomogastria in EGGs show an improvement of gastric symptoms. Moreover, the patient was told about the positive changes in EGG by an acupuncturist, and explaining that the acupuncture treatment seemed to improve clinical symptoms. Thus, acupuncture treatment contributed to an improvement of symptoms in a patient with stomach pain and feeling of fullness.[Conclusion] Acupuncture treatment induced an increase in percentage of normogastria in EGG power and resulted in an improvement in gastric symptoms of a patient with upper stomach pain and feeling of fullness.

ABSTRACT

Objective To study the effect of transcutaneous electroacupuncture(TEA)on gastric accom-modation and electrogastrogram in patients with functional dyspepsia(FD)and investigate the transcutaneous elec-troacupuncture efficacy and mechanism. Methods 106 cases of FD patients were randomLy divided into TEA group(n=53)and sham-TEA group(n=53)according to the designed random table. 10 health volunteers were recruited for participation in the study for normal control group. All patients fulfilled the Rome Ⅲ criteria of FD. Informed consent was obtained from each participant. All subjects were tested in terms of gastric accom-modation (GA),namely, maximum satiety. For the patients of TEA group, the acupoints of Zusanli(ST36)and Neiguan (PC6)were stimulated. In the sham-TEA group, the sham-acupoint for PC6 was located at about 15-20 cm away from PC6 not on any meridian and the sham-point for ST36 was located at 10-15cm down from and to the lateral side of ST36 not on any meridian. The stimulation parameters used for sham-TEA were the same as the TEA group. Four weeks after treatment with double blind crossover method,we compared the two groups in view of gastric receptivity, electrogastrogram(EGG)andclinicalsymptomscores before and after treatment respectively. Results (1)The GA was 725 ± 46 mL in the normal control group and 539 ± 36 mL in the FD patients.(2)The mean total symptom score was 24.5 ± 2.9 at baseline and significantly decreased to 11.9 ± 2.1(P < 0.001)after TEA. There were significant differences in the symptom scores between the TEA group and sham-TEA group(P<0.001).(3) The percentage of normal slow waves(N%)during fasting and postprandial state in the TEA group were respectively 70.9 ± 2.4% and 68.2 ± 3.1%. The differences were statistically significant between the TEA group, baseline and sham-TEA group(P<0.001).(4)In the TEA group, the postprandial dominant power(46.23 ± 4.03)db and the fasting dominant power(35.35 ± 1.53)db were significantly increased compared to baseline before treatment and sham-TEA group(P < 0.001). Conclusions(1)Gastric accommodation in patients with FD was decreased, showing a higher visceral sensitivity. TEA can improve the GA in FD patients, reducing visceral sensitivity.(2)TEA can increase the percentage of normal slow wave in the FD patients, increase the postprandial dominant power and regulate the abnormal gastric myoelectric activity to promote the gastric motility.

ABSTRACT

BACKGROUND/AIMS: Nausea, an unpleasant symptom of diabetic gastroparesis (DMGP), has been reported to be alleviated by needleless transcutaneous electrical acupuncture (TEA). Our study was designed to utilize electroencephalography (EEG) and electrogastrography (EGG) recordings to investigate the central and peripheral responses of TEA in the treatment of nausea in DMGP patients. METHODS: Eleven DMGP subjects underwent simultaneous EEG and EGG testing while grading the severity of nausea following 30-minute intervals of: (1) baseline, (2) visual stimulation (VS) to provoke more nausea, (3) active VS together with TEA, and (4) TEA alone, and a final 15-minute recording without any intervention. RESULTS: The nausea score was increased to 5.9 ± 1.5 with VS (P < 0.05, vs 3.5 ± 1.0 at baseline), then reduced to 3.5 ± 1.2 with VS plus TEA, and to 2.5 ± 1.3 with TEA alone, while it continued at a score of 2.9 ± 1.0 post TEA (all significant, P < 0.05, vs VS without TEA). The mean percentage of normal gastric slow waves was decreased to 60.0 ± 5.7% with VS (P < 0.05, vs 66.6 ± 4.5% at baseline), then improved to 69.2 ± 4.8% with VS plus TEA, and maintained at 70 ± 3.6% with TEA alone. During initial VS, EEG signals showed right inferior frontal activity as the prominent finding, but during VS with TEA, left inferior frontal activity predominated. CONCLUSIONS: In DMGP, TEA improves gastric dysrhythmia and ameliorates nausea. TEA treatment of nausea provoked by VS resulted in a change of dominance from right to left inferior frontal lobe activity. These data provide new understandings of peripheral and central mechanisms for nausea, and potential future directions for DMGP treatment approaches.

ABSTRACT

Objective To improve the estimation and the percentage of electrogastrogram(EGG)slow wave rhythm with power spectrum estimation and calculation rule set. Methods Chirp Z-transformation was introduced for estimation power spectrum of EGG,which can obtain both high frequency resolution and accurate estimation of power;while the power proportion of main frequency components in each segment was used to substitute for conventional method in time domain. Results The variance of proposed method was about one-eighth of that of conventional method.Incorporating the power proportion of main frequency components, the proposed method gave better results especially for those EGG signals in which abnormal frequencies appeared frequently. Conclusion As compared with the conventional method, the proposed method is less affected by segmentation process in estimating the percentage of EGG slow wave rhythm, which may promote future clinical applications.

ABSTRACT

Footbath have long been used for primary health care or for nursing, however, few researches have been reported. We intended to make footbath a safe physiotherapy or care technique in the modern medicine by more profound researches on footbath. The effect of footbath on the gastric motility was studied because footbath may promote recovery from post-surgical gastric paresis. The effects of footbath on the gastric motility and subjective hunger sensation were monitored in 14 healthy adult females (32±6 yeas old). They signed informed consents and took footbaths at 38, 40, 42°C and control footbath (by a footbath machine). The experiments started after permission of the Ethical Committee of International Research Center for Traditional Medicine.<br>They took footbath after 10-min rest in a sitting position. Each footbath was 30 min long, followed by 10-min rest. The same subject participated in the studies four times at the same time of the day before taking lunch (10:00-14:00). These experiments were in a random order four days apart each other except menstruation periods. Their blood pressure, ECG R-R variability and electro-gastrogram (EGG) were monitored. The subjective hunger sensation was asked before, during and after footbath. The autonomic nervous balance was estimated from FFT analysis of the R-R variability. LF (0.04-0.15Hz) and HF (0.15-0.40Hz) components of the R-R variability were calculated. EGG was also analysed by means of FFT to calculate amplitude and frequency. The results showed the amplitude and frequency of EGG increased depending on temperature. However, the correlation between HF power of R-R variability and amplitude of EGG showed negative correlation. Regarding hunger sensation, more cases felt hunger in 38, 40°C than in control. At 42°C, the amplitude and frequency were apparently higher than in other temperatures, while the hungry cases were less than in 38 or 40°C. These discrepancies may be caused by the artifact due to sweating on the abdominal EGG leads, considering high b wave of EEG at 42°C.<br>In conclusion, it was indicated that footbath may promote gastric motility and induce hunger sensation at 38-40°C in healthy volunteers.

ABSTRACT

Objective To study and evaluate the consistency and diagnostic value of electrogastrogram(EGG) and gastroscopy in children′s gastrosis diagnosis. Methods A total of 136 children were checked with gastroscopy and EGG during the same period, then compared the diagnostic accordance rate with 2 ways, and analyzed the characteristics of EGG.Results EGG showed certain characteristics in different gastrosis.Diagnosis of EGG in the group of functional illness was consistence with gastroscopy at 95.8%;others were less than 40%.Conclusion EGG can be used as a method of special value to diagnose children′s functional gastrosis and a screening means in general gastrosis.

ABSTRACT

Objective : To investigate the effect of acupuncture stimulation at PC6 (Neiguan, Naikan) on optokinetic motion sickness, motion sickness scores were evaluated and changes in electrogastrograms (EGG) were analyzed in each subject.<BR>Methods : Thirty-six normal volunteers were randomly divided into three groups : electrical acupuncture group, retaining needle group, and no treatment group.<BR>Subjects were exposed to a rotating optokinetic drum to induce vection. EGG was recorded continuously before, during, and after the rotation exposure, each period being fifteen minutes in length. To evaluate the severity of motion sickness in each subject, subjective symptoms of motion sickness (SSMS) score and visual analogue scale (VAS) were obtained.<BR>Results : The severity of nausea which is one of main symptom of motion sickness was suppressed, and the percentage of normal 2?4 cpm band of EGG activity was increased earlier after drum rotation exposure in the retaining needle group than in the no treatment group.<BR>Conclusion : From these results, we conclude that the retaining needle stimulation of the PC6 acupoints was effective for optokinetic motion sickness possibly by suppressing abnormal gastric myoelectric activities.

ABSTRACT

Objective To observe the effect of Rhizome Atractylodis Macrocephalae (RAM) on electrogastric arrhythmia in rats and its mechanisms. Methods A total of 30 Wistar rats were randomly divided into 3 groups: control group, model group and RAM plus model group. The frequency of slow wave(FSW), coefficient of variation(CV) and abnormal rhythm index(ARI) were recorded and analyzed by computer. The substance P(SP), vasoactive intestinal peptide(VIP) and nitric oxide synthase(NOS) nerves in antral myenteric plexus were observed by immunohistochemical staining. Results Compared with those of the control group, the CV and ARI in model group markedly increased( P 0.05). The denites of SP nerves markedly decreased( P 0.05). Conclusion RAM has regulative effect on the electrogastric arrhythmia in rats and the mechanisms may be related with the increased SP nerves and decreased VIP and NOS nerves in antral myenteric plexus.

ABSTRACT

Objective To understand the intestinal nervous system(ENS) pathological change in rats with acute renal failure(ENS).Methods Rat acute renal failure model was established, the intracanthus blood was drew to determine the serum total creatinine(Scr) blood urea nitrogen(BUN) after 48h and 4th-day,then the rats was sacrificed at fourth day each rat's kidney tissue, duodenum, jejunum, ileum, colon and rectum were stained with hematoxylineosin(SE).Finaly, iNOS staining by immunohitochemstry, observed the numbers of the ENS nerve plexus, ENS nerve cell , and iNOS positive cell nerve plexus. Check whether there has hyperemia hydrops, degenerative necrosis, and inflammatory cell infiltration on the mucosa epithelium. Results ⑴Kidney histological change was occurred. The kidney tubules degenerative necrosis was found in all the ten cases of kidney failure, and the hyaline cast, mesenchyme hemorrhagic necrosis, and inflammatory cell infltration were seen. ⑵ HE coloration and microscope inspection;numbers of the ENS nerve plexus and nerve cell of the acute kidney failure group were less than the normal group, especially in the ileum and rectum .⑶The number of the iNOS positive cell nerve plexus in ENS of the rats with acute renal failure was apparently less than that of the normal group. In addition, the iNOS positive presentation was stronger than that of the normal group. Conclusions The findings suggest that toxic metabolites of uremic rat acts on the ENS,and thus leading to pathological changes of ENS,resulting in stomach and intestinal mortor function disorder.

ABSTRACT

[Objective] To observe the therapeutic effect of Gastritis Prescription Ⅰ(GP Ⅰ, mainly composed of Radix Salviae Miltiorrhizae, Rhizoma Atractylodis Macrocephalae, Herba Taraxaci, Herba Hedyotis Diffusae, Rhizoma Curcumae, Radix Angelicae Sinensis, etc.) in treating chronic atrophic gastritis (CAG). [Methods] Sixty cases of CAG were randomized into GP Ⅰ group (group A, n = 30) and multi-enzymes group (group B, n=30) . The two groups were given the corresponding drugs orally for 14 weeks. Symptoms score, serum gastrin (Gas) level and body surface electrogastrogram (EGG) were detected before and after treatment. [Results] In group A, symptoms such as abdominal distention, epigastric pain, anorexia, loose stool, lassitude, gastric discomfort, acid regurgitation, and belching were much relieved; serum Gas level and post-prandial EGG amplitude were increased ( P
