ABSTRACT

Purpose: To determine the proportion of erroneously assumed ocular hypertension (OHT) among referred patients of elevated intraocular pressure (IOP) seen in glaucoma clinic of a teaching hospital in Northern India. Methods: Retrospective review of case records of referred, diagnosed patients of OHT or unspecified glaucoma seen between January 2019 and March 2020. Using an algorithmic clinical approach, including gonioscopy, Goldmann applanation tonometry (GAT), and pachymetry, underlying cause for elevated?IOP was amended and proportion of erroneously assumed OHT was calculated. Results: Of 276 patients diagnosed either as OHT or unspecified glaucoma before being seen at our glaucoma clinic, 44 (16%) had IOP within normal range (10–21 mmHg) on GAT. In 97 (35%) cases elevated?IOP was associated with angle closure. The central corneal thickness (CCT) was >550 ?m in 39 (14%) patients with elevated?IOP. The proportion of erroneously assumed OHT was 70% in this study. Conclusion: The elevated-IOP does not imply with OHT unless evident through comprehensive clinical examination and appropriate investigations.

ABSTRACT

In adult orbital diseases, thyroid associated ophthalmopathy (TAO) is of very high incidence rate, and it can seriously affect patients' appearance, eyesight and binocular visual function and so on, and significantly reduce the patients' quality of life.In addition to the common manifestations such as eyelid retraction, exophthalmos and strabismus, some TAO patients may suffer from obviously increased ocular pressure and even visual field damage, which are often ignored or missed in diagnosis and should be paid more attention to.The pathogenesis of elevated intraocular pressure in TAO is mainly related to the elevated episcleral venous pressure and the extraocular muscles.Because the elevated intraocular pressure resulted from TAO is secondary and its pathogenesis is complex, personalized treatment different from primary glaucoma therapy is needed.In this article, the epidemiology, pathogenesis and therapy of elevated intraocular pressure in TAO including medication, surgery, and radiotherapy were reviewed to provide reference for the clinical diagnosis and treatment for TAO.

ABSTRACT

Pigment dispersion and elevated intraocular pressure (IOP) with sulcus placed hydrophobic acrylic intraocular lenses (IOLs) are described and are rare with in-the-bag IOLs. We report a case of a young lady with elevated IOP and pigment dispersion in one eye following an uneventful phacoemulsification and multifocal IOL implantation. The eye had eccentric capsulorrhexis and localized capsular fibrosis with anterior displacement of the IOL optic, resulting in pigment dispersion. The uncontrolled IOP and continued pigment dispersion were managed with IOL exchange (three-piece IOL) followed by filtration surgery, which helped to resolve the condition successfully.

ABSTRACT

Objective To observe the neuroprotective effect of Copolymer 1 ( Cop 1) in rats with elevated intraocular pressure (IOP), and explore the clinical application of Cop 1 in the prevention of optic nerve atrophy induced by glaucoma. Methods Thirty adult Wistar rats were rendered for elevation of IOP in both eyes by Shareef-Sharma method. At the third week of IOP elevation, a mixture (1:1) of Cop 1 (100 μg) emulsified in complete Freund's adjuvant (CFA) was injected intracutaneously at two hind footpads in 15 rats (Cop 1 model group), and the other 15 rats were given the same dose of PBS emulsified in CFA (model control group) . Retinal ganglial cell (RGC) retrograde labeling was performed with Fluorogold 6 d later, and retina was taken after 24 h to compared the density of RGC between two groups. Another normal rats were served as blank controls. Results It was found that lymphocytes were accumulated on the histological slices of retina and optic nerve in Cop 1 model group. The rates of RGC loss of Cop 1 model group and model control group were 10. 24% ± 3.75% and 29.00% ±6.92%, respectively. The RGC density in Cop 1 model group was significantly lower than that in model control group (P <0. 05), while there was no significant difference in that between Cop 1 model group and blank control group (P >0.05). Conclusion Intracutaneous injection of Cop 1 can reduce the damage of RGC induced by elevated IOP.

ABSTRACT

Objective To observe the protective effect of Erigeron Breviscapus(Vant.)Hand-Mazz(EBHM) on retinal ganglion cells(RGCs) and optic nerve in rabbits with elevated intraocular pressure(IOP). Methods Twenty rabbits with chronic elevated IOP were divided into two groups randomly: EBHM treated group(the ocular hypertension with EBHM treated subgroup and the normal IOP with EBHM treated subgroup) and untreated group(the simple ocular hypertension subgroup and the simple normal IOP subgroup). EBHM was irrigated into the stomachs to the treated group after IOP elevated continuously for 7 d.For light and electron microscopy studies,the rabbits' eyes were made into eyeball samples and optic nerve samples after 60 d.The density of RGCs,thickness of retinal nerve fiber layer(RNFL),and optic nerve axons were observed and quantitated by computer image analysis system.The ultra-microstructure changes of RGCs and optic nerve axons were observed by electron microscope.Results(①The RNFL) thickness,RGCs density and number of axons of the two ocular hypertension subgroups were decreased,compared with those of the two normal IOP subgroups(P
