ABSTRACT

Objective:To observe the clinical efficacy of addition and subtraction syndrome differentiation method of Huanglian Ejiaotang and Baihe Dihuangtang in the treatment of menopausal syndrome (MPS) and symptoms of deficiency of yin and hyperactivity of fire, and investigate its regulation effects on neuroendocrine network. Method:One hundred and forty-eight patients were divided into control group (74 cases) and observation group (74 cases) according to the random number table method. The patients in control group received estradiol/norethisterone acetate tablets, 3 mg/time, 1 time/day, while the patients in observation group received addition and subtraction of Huanglian Ejiaotang and Baihe Dihuangtang based on the same western medicine treatment as control group, orally 1 dose/day, with a treatment course of 3 months in both groups. Kupperman index (KI) score was evaluated before treatment and at 1 month, 2 months and 3 months after treatment. The Menopausal Quality of Life (MENQOL), self-rating depression scale (SDS), Self-rating Anxiety Scale (SAS), Pittsburgh Sleep Quality Index (PSQI) and score of deficiency of yin and hyperactivity of fire were evaluated before and after treatment. The levels of serum estradiol (E2), follicle stimulating hormone (FSH), luteinizing hormone (LH), norepinephrine (NE), 5-hydroxytryptamine (5-HT), nitric oxide (NO), endothelin-1 (ET-1) and calcitonin gene-related peptide (CGRP) were detected before and after treatment. Result:The clinical efficacy in observation group was better than that in control group (Z=2.115, P<0.05), and the KI scores in observation group at 1 month, 2 months and 3 months after treatment were lower than those in control group (P<0.01), and the decrease of KI score in observation group after treatment was larger than that in control group (P<0.01). The scores of various dimensions of MENQOL scale in observation group were lower than those in control group (P<0.01). The scores of deficiency of yin and hyperactivity of fire, SAS, SDS and PSQI in observation group were all lower than those in control group (P<0.01), and the level of E2 in observation group after treatment was higher than that in control group (P<0.01) ,while the levels of FSH and LH were lower than those in control group (P<0.01).The levels of 5-HT and NE in observation group were higher than those in control group (P<0.01).The NO level in observation group was higher than that in control group (P<0.01), while the levels of CGRP and ET-1 were lower than those in control group (P<0.01). During the study period, there were 5 drop-out cases, 2 excluded cases, and 67 completed cases in the control group. There were 7 drop-out cases, 1 excluded case, 66 completed cases in the observation group. Conclusion:On the basis of hormone replacement therapy, addition and subtraction of Huanglian Ejiaotang and Baihe Dihuangtang for patients with MPS (with symptom of deficiency of yin and hyperactivity of fire) can improve clinical symptoms, relieve depression and anxiety, improve sleep quality and enhance quality of life, and its clinical efficacy is superior to hormone replacement therapy alone. It can also regulate endocrine hormones, vasomotor factors and monoamine neurotransmitters, thereby improving clinical symptoms.

ABSTRACT

OBJECTIVE: To observe the effect of manual acupuncture intervention on the reproductive endocrine and p-Akt protein expression in polycystic ovary syndrome with insulin resistance (PCOS-IR) rats, so as to reveal its mechanism in relieving PCOS-IR. METHODS: Female Wistar rats were randomly divided into control, model, medication (Metformin) and acupuncture groups (n=8 in each). The PCOS-IR model was established by subcutaneous injection of Dehydroepiandrosterone (DHEA, 6 mg/100 g), once daily for 20 days. Rats of the medication group were treated by gavage of Metformin solution (100 mg/kg), once daily for 28 days. For rats of the acupuncture group, manual acupuncture stimulation (lifting-thrusting and rotating of needles) was applied to the bilateral "Shenshu"(BL 23), "Yishu"(EX-B 3), "Zigong" (EX-CA 1), "Sanyinjiao"(SP 6), "Fenglong"(ST 40) and "Guanyuan"(CV 4) for 5 min, once a day for 28 days. H.E. staining was used to observe the histopathologic changes of the ovary tissue. Serum levels of follicle-stimulating hormone (FSH), estradiol (E2), luteinizing hormone (LH), testosterone (T) and fasting insulin (FINS) were measured by ELISA. Fasting plasma glucose (FPG) was measured by blood glucose meter, and homeostasis model assessment-insulin resistance index (HOMA-IR) was calculated. Western blot was used to detect the expression of Akt and p-Akt proteins in ovarian tissues. RESULTS: Following modeling, the ovaries showed polycystic changes and an increase of atresia follicles with a larger diameter and no corpus luteum in the model group. After the intervention, there were various types of follicles and a small amount of corpus luteum at different stages of development in both acupuncture and medication groups. In comparison with the control group, serum FSH and E2 levels as well as ovarian Akt and p-Akt protein expression were considerably decreased (P0.05).. CONCLUSION: Acupuncture intervention can regulate the levels of reproductive endocrine hormones and reduce insulin resistance in PCOS-IR rats, which may be related to its effect in up- regulating the expression of p-Akt protein in the ovaries.

ABSTRACT

Objective To study antidepressant effect and mechanism of different polar fraction of Acorus tatarinowii Schott.on depression mouse model and explore the mechanism. Methods Different polar fractions were prepared by systematic solvent method. The adult dose recommended by Chinese Pharmacopeia ( 10 g?d-1 ) was converted to the dose of mouse. Intragastric administration was performed.A total of 80 male mice was randomly divided into normal control group,model control group and different polar fraction groups of Acorus tatarinowii Schott.. Depression mouse model was established by chronic unpredictable mild stimulation ( CUMS) with solitary feeding,and external performance of mice of different groups was observed. After 21-day feeding,blood was harvested from eyes of the mice.Concentrations of triiodothyronine (T3),tetraiodothyronine (T4), thyroid-stimulating hormone ( TSH) ,adrenocorticotrophic hormone ( ACTH) in mouse plasma and 5-hydroxy tryptamine ( 5-HT) , ACTH in brain were measured by ELISA method. Results Concentrations of T3 , T4 and ACTH in model control group [(1.203±0.042),(44.80±2.21),(11.27±0.50) ng?mL-1] were significantly increased as compared with the normal control group [(0.794±0.028),(24.87±1.25),(7.04±0.24) ng?mL-1] (P<0.05).Concentration of 5-HT in brain (146.87±10.96) was significantly decreased as compared with that of normal group ( 237. 11 ± 21. 87 ) ng?L-1 , and concentration of ACTH (58.94±4.46) ng?L-1 was significantly increased as compared with that of normal group [(38.89±2.26) ng?L-1,P<0.05)]. Concentrations of T3[(0.824±0.067),(0.812±0.051),(0.943±0.049) ng?mL-1],T4[(25.97±1.96),(27.53±1.88), (31.26±1.97) ng?mL-1] and ACTH [(7.21±0.40),(7.58±0.39),(8.69±0.42) ng?mL-1] in extraction group,cyclohexane group and chloroform group were significantly decreased as compared with model control group. At the recommendation dose of Chinese Pharmacopeia,concentrations of 5-HT in brain [(219.59±10.48),(202.19±11.95),(186.96±10.29) ng?L-1] were significantly increased,and concentrations of ACTH [(41.65±2.65),(44.87±2.95),(47.75±3.06) ng?L-1] were decreased ( P<0.05) . Conclusion Mouse model of CUMS shows typical depression symptom,and the symptom has relationship with the concentrations of endocrine hormones of T3 , T4 , ACTH, 5-HT, etc. Extraction and low polar fractions ( cyclohexane, and chloroform) of Acorus tatarinowii Schott. can partly correct incretion disorder of depression model, which may be parts of the reasons why Acorus tatarinowii Schott.exerts anti-depressant effect.
