ABSTRACT

Objective:To explore the application of "upgraded single-entry colonoscope training method" in the primary endoscopic training of refresher doctors. Methods:A total of 71 refresher doctors who participated in the primary single endoscopy training in the Endoscopy Center of the Affiliated Hospital of Qingdao University from January 2017 to December 2019 were randomized to two groups: upgraded single-entry endoscopy training group ( n=36) and traditional single-entry endoscopy training group ( n=35). In the first week of training, all the participants learned the basic theory of single-entry enteroscopy insertion. From week 2 to week 11, each doctors of the two groups finally completed 80 to 100 cases of single-entry endoscopy. The upgraded single-entry colonoscopy training group was divided into two stages: "rectum-proximal sigmoid colon" and "descending colon-ileocecal part" with the boundary of descending-sigmoid colon as the dividing point, which was different from the traditional group. At the 12th week, each doctors were assessed by 20 cases of single-entry colonoscopy operation. the success rate of ileocecum insertion, the time to pass through sigmoid colon, the rate of loop formation and the average operation time of successful ileocecum insertion were compared between the two groups. SPSS 26.0 was used for t test or chi-square test. Results:Compared with the traditional single-entry endoscopy training group, the upgraded single-entry endoscopy training group achieved better results in the successful rate of ileocecal insertion (66.8% vs. 59.0%, P=0.005), the time to pass sigmoid colon [(6.05±3.32) min vs. (7.15±3.12) min, P<0.001], the loop rate (35.13% vs. 40.71%, P=0.035), the average operation time of successfully inserting the ileocecal part [(9.01±2.12) min vs. (10.25±3.12) min, P<0.001] and the discomfort response score [(5.19±1.41) vs. (6.70±2.15), P<0.001], with statistical significance between the groups. Conclusion:"Upgraded single-entry endoscopy training method" emphasizes no loop through sigmoid colon, which is more helpful for refresher doctors to master the operation skills of single-entry colonoscopy, shorten the teaching time, and reduce the discomfort of patients, and is suitable for popularization in the teaching.

ABSTRACT

Purpose of the work. The practice of endoscopy in Colombia was modified when Resolution 1043 of 2006 authorized specialists in general surgery, internal medicine and pediatrics to perform endoscopy after completing one year of training in endoscopy at an institution of higher education. This, together with the development of relationships with different specialties within endoscopy, generated a disordered scenario which many considered to be unjust and unequal. Training requirements became differentiated. A world of tensions and interests among specialists, scientists, health care providers and service providers led to this consensus. Starting with fundamental agreements, it makes recommendations for unification of educational features that will allow endoscopic practices which aim for quality and whose central axis is the best interest of our patients. Materials and Methods. This consensus is a descriptive, cross-sectional social research study with a mixed approach (qualitative and quantitative) based on the Delphy method. The information in this study was obtained from the event titled "Acuerdo en lo fundamental" (agreement on fundamental issues) organized on June 23, 2012 by the president of the Colombian Association of Digestive Endoscopy (ACED). Qualitative data were taken from four roundtables discussions in which 34 participants discussed 25 survey questions. The quantitative data were taken from final voting and from an individual, private electronic survey. 75% or greater agreement was defined as consensus. Qualitative analysis employed discourse analysis oriented around five variables related to formative aspects. Basic descriptive statistics centered around percentages were used for quantitative analysis.

ABSTRACT

Objective To explore the role of computer-based simulators in upper gastrointestinal endoscopy training. Methods Forty-one graduate students, residents and GI fellows for further cducation with no experience in endoscopy were randomly assigned to two groups and underwent one-month training with or without a 10-hour computer-based simulator. Each trainee performed upper endoscopy in 20 patients.Comparison was made between the two groups in terms of these performance parameters:esophageal intubation, retroflexion, pyloric intubation, intubation of the descending part of the duodenum and procedure time.Resuits There Was no significant difference in esophageal intubation(P=0.699)and intubation of the second part of the duodenum(P=0.141)between two groups. While the differences were significant in retroflexion (P<0.001),pyloric intubation(P<0.001)and procedure time(P=0.032),i.e.,the simulator group was much better in performance than the other group. Conclusion The computer-bascd simulator is effective in providing trainees with the skills needed for upper gastrointestinal endoscopy, shortening the teaching hours and lessening patients'pain.
