ABSTRACT

We report two cases in which the quality of life of children with chromosomal aberration improved with Kampo treatment. Case 1 : Six-year-old boy with 5p deletion syndrome. We prescribed kambakutaisoto for the purpose of treating excitability and insomnia. After adding shokenchuto, and further using with yokukansan, the degree of excitement was obviously reduced, and he began to sleep well until morning. Case 2 : Two-year-old boy with chromosome 8 abnormality. He had failed extubation after thoracotomy, thus he needed tracheostomy and gastrostomy. Frequent sputum suction was required every 5 to 15 minutes, and he suffered chronic diarrhea. The number of suctioning was decreased after treatment with bukuryoingohangekobokuto. Moreover, after the addition of shokenchuto, suction became unnecessary for more than 2 hours, and diarrhea also improved. After surgery to close tracheostomy site, he has been receiving ogikenchuto + shoseiryuto + ninjinto and is very well enough to go to a nursery school. Although it is a small improvement not involved in life prognosis, it has contributed to the improvement of the quality of life and the reduction of burden on caregivers. We believe that it is one of the significance of providing Kampo treatment for children.

ABSTRACT

RESUMO: A fase em que se estabelece a infância é um período essencial na vida dos seres humanos, não só para o desenvolvimento físico e cognitivo como também social. Contudo, apesar do reconhecimento desta importante etapa do desenvolvimento humano, estudos sob o ponto de vista sociológico têm sido pouco explorados na literatura. Partindo deste cenário e sabendo então da necessidade de mais estudos sobre essa temática no campo educacional, o presente texto tem como objetivo contextualizar o papel da infância e de suas atividades, tais como o brincar e o jogar, no contexto do processo civilizador.


ABSTRACT: The childhood phase is an essential period to people's lives, not only for the physical and cognitive development but also to the socio-cultural development. However, despite the recognition of this important stage of human development, social studies about this subject have not been explored with intensity in academic literature. Starting from this scenario and acknowledging the need for further studies on this subject in the educational field, this paper aims to contextualize the role of childhood and its activities, such as to play and to toy in the context of the civilizing process.

ABSTRACT

Objective To observe the efficacy and safety of domestic ziprasidone injection in the treatment of excitement and agitation in acute period of schizophrenia.Methods 60 patients with schizophrenia excited were randomly divided into two groups,30 cases in each group.The treatment group was given ziprasidone 20 to 40mg/day by intramuscular injection,the control group was given haloperidol 10-20mg/day by intramuscular injection,the two groups were observed for three days.The positive and negative symptoms scale(PANSS)in the excitation factor(EC) points rate was used to assess efficacy,symptoms scale(TESS)was used to evaluate the adverse reaction.Before treatment,2,6,24,48,72 hours after treatment,the positive and negative symptoms scale of excitement factor (PANSS-EC)and symptoms scale(TESS)were evaluated.Results 72 hours after treatment,the PANSS -EC score in the ziprasidone group was obviously lower,the points rate was (69 ±18)%,the total effective rate was 83.3%,which of the haloperidol group were (71 ±26)%,86.7%,the differences between the two groups were not statistically significant (P>0.05).The incidence of adverse reactions between the ziprasidone group and haloperidol group had no statistically significant difference(P>0.05).But in terms of the incidence of vertebral body system,the ziprasidone group(4 cases)was obviously less than the haloperidol group(13 cases),the difference was statistically significant(χ2 =6.65,P<0.05).Conclusion Ziprasidone injection is effective in the treatment of agitation in acute phase of schizophrenia,with less side effect,higher safety.

ABSTRACT

Consumer satisfaction is an important parameter for assessing the quality of pharmacy. Whereas expectation for pharmacists has been increasing recently, misconception still exists. The Kano analysis is a customer-oriented quality measurement and quality improvement technique. Using this analysis, the aim of the study is to assess the patients’ satisfaction regarding the services provided by pharmacists based on basic needs, performance needs, and excitement needs. Community pharmacists will need to distinguish them to enhance patients’ satisfaction.

ABSTRACT

En el presente artículo se hace una exposición concisa de las diferentes teorizaciones sobre el concepto de ansiedad en la literatura de la terapia Gestalt. Éstas se exponen de manera sistemática; se especifican las características de cada aproximación teórica; y se describe someramente, al final de cada sección, la manera de intervención terapéutica según sea el caso. Finalmente, se hace una revisión breve de los desarrollos teóricos recientes en torno al tema.


In this paper a concise statement of the different theories on the concept of anxiety in the literature of the gestalt therapy is done. These are discussed systematically; the characteristics of each theoretical approach are specified; and the manner of therapeutic intervention, as the case may be, is briefly described at the end of each section. Finally, a brief review of recent theoretical developments on the subject becomes.

ABSTRACT

We report the case of a 17-year-old male who had involuntary movements treated with yokukansankashakuyakukoboku. He had first recognized the involuntary movement 6 years previously. His symptom could not be alleviated with neurological and psychological treatments, and he visited our clinic for treatment with Kampo medicine. His symptom gradually alleviated with yokukansankashakuyakukoboku. The ancient physician Sekki (薛已) created yokukansan in China's Ming Dynasty. Yokukansan and its various add-on combinations were used in the Edo Era of Japan. Keisetsu Ohtsuka then created yokukansankashakuyakukoboku, which is however now rarely used because there is no extract drug in Japan. There are markedly nervous patients however, who can be treated with this formula. Thus, the authors feel that more research needs to be done on the differences between yokukansan and yokukansankashakuyakukoboku.

ABSTRACT

Trata-se de uma pesquisa com abordagem qualitativa e descritiva, com a utilização da Grounded Theory, na perspectiva do Interacionismo Simbólico. Como objetivos tivemos: identificar os significados atribuídos por mulheres ao fato de experimentar, sentir ou vivenciar sensações de prazer sexual e/ou excitação sexual durante a amamentação; analisar e interpretar, na perspectiva do Interacionismo Simbólico, a experiência de sentir prazer sexual e/ou excitação sexual durante a amamentação, a partir dos significados atribuídos pelas mulheres. O estudo procedeu-se no Município do Rio de Janeiro, em lugares de grande circulação de pessoas, como praças, ruas, shoppings, além da Unidade de Saúde Milton Fontes Magarão, localizada na zona norte do referido Município. Essa variedade de cenários nos possibilitou encontrar uma diversidade de participantes, referente a condições socioeconômicas, culturais, religião, ideais e concepções sobre o assunto estudado. A coleta e análise dos dados aconteceram no período de maio a julho de 2014, observando todas as exigências da Resolução 466/2012 do Conselho Nacional de Saúde. Foram entrevistadas 17 participantes e formados dois grupos amostrais e três categorias expressas por: significando a amamentação como ato sagrado, inocente e assexuado: a socialização da amamentação; vivenciando e significando sensações de prazer ao amamentar; ressignificando a vivência de sensações de excitação sexual ao amamentar. Como resultados evidenciou-se que a socialização da amamentação influencia a maioria das mulheres, na vivência de sensações de prazer ao amamentar. Essas sensações são descritas através da simbologia estabelecida socialmente, de que o ato de amamentar é sagrado, puro, livre de erotismo, prevalecendo um prazer maternal...


It is a research with qualitative and descriptive approach, using Grounded Theory, on the Symbolic interactionism. We aimed: to identify the meanings attributed by women on the fact of trying, feeling or experiencing sexual pleasure and/or excitement sensations during breastfeeding; to analyze and interpret them. The study was developed in Rio de Janeiro, in places of great circulation of people, like squares, streets, malls, as well as a Milton Fontes Magarão Primary Care Unit, located in the North of the city. This variety of sceneries enabled us to find a diversity of participants, referring to socioeconomic and cultural conditions, religion, ideals and conceptions about the subject studied. Data collect and analysis happened in the period of may to July 2014, observing all demands from the Resolution 466/2012 from the National Health Council. 17 participants were interviewed and two group samples were formed and three categories expressed by: breastfeeding meaning as a sacred, innocent and asexual act: breastfeeding socialization; experiencing and meaning pleasure sensations on breastfeeding; resignifying experience of sexual excitement sensations on breastfeeding. The results showed that the socializing influence of breastfeeding, most women, the experience of pleasurable sensations while breastfeeding. These sensations are described by symbols socially established that breastfeeding is sacred, pure eroticism free, prevailing a maternal pleasure...

O comentário, utilizando-se da experiência clínica da autora, tematiza os dilemas vividos pelo analista, perigosamente desafiado, como seu paciente, num embate entre proposições da vida contemporânea, aparentemente excludentes, ou seja: que solução singular encontrar para desenvolver-se enquanto sujeito no confronto com a força, aparentemente invencível, da "sociedade excitada", conforme desvelada por Christoph Türcke.


The commentary, based upon the clinical experience of the author, characterizes the dilemma faced by the psychoanalyst, dangerously challenged, as his patient, in a clash between propositions of contemporary life, apparently excludent, that is: which singular solution can be found in order to promote self-development as subject in the confrontation against the strength, seemingly unbeatable, of the "excited society", as unravelled by Christoph Türcke.


El comentario, basándose en la experiencia clínica de la autora, tematiza los dilemas vividos por el analista, peligrosamente desafiado, como su paciente, en un choque entre proposiciones de la vida contemporánea, aparentemente exclusiva, o sea: que solución singular encontrar para desarrollarse como sujeto de la confrontación con la fuerza, aparentemente invencible, de la "sociedad excitada", como fue dada a conocer por Christoph Türcke.

ABSTRACT

Objective:To check the clinical effect of Multi-channel Brain Magnetic Stimulation Machine.Method:Sixteen volunteers are divided into two groups.Members of the first group which is the corresponding group listen to news material in the sober state and then answer the questionnaire.At last their EEG is collected.Members of the second group listen to the same material in the sober state and they are stimulated at the forehead and temporal area by Multi-channel Brain Magnetic Stimulation Machine at the same time.They also answer the questionnaire under the magnetic stimulation at forehead and their EEG is collected,too.Result:Answers of the second group are obviously better than the first group.The power spectrum of EEG at the forehead,temporal,parietal region and even the whole brain area in the first group is stronger than that of the second.Conclusion:Multi-channel Brain Magnetic Stimulation Machine excites brain in a way and then helps people complete the job better.

ABSTRACT

BACKGROUND: Although the pro-convulsant or anticonvulsant properties of propofol remain a matter of controversy, it is evident that propofol can produce involuntary movement. Such movement is a relatively common side effect, especially in children, and may be dose-related or injection rate-related. The goal of this study was to evaluate the effect of injection rate upon involuntary movement during propofol induction in children. METHODS: Children (age 3-14 yr) undergoing elective Eye and ENT surgery were randomly allocated to one of 4 groups based on the propofol injection rate (A, manual/15 s; B, 360 ml/hr; C, 200 ml/hr, D, 100 ml/hr) using a manual injection method and syringe pumps. No premedication was used. The induction dosage of propofol was 3 mg/kg in all groups. Fentanyl 1mcg/kg and 1% lidocaine 1-2ml were given I.V. before propofol. Involuntary movement was graded 0-2 on severity. The infused dose of propofol at movement was measured. Movement due to pain or mask fitting was not regarded as an involuntary movement. All results were analyzed using the Chi-Square Test and ANOVA. RESULTS: 595 children were studied. Age, gender, and weight were similar in the 4 groups. Involuntary movements were apparent in 179 (30.1%) of the 595 subjects. Movements were significantly less in group A (12.4%) and B (16.4%) compared to group C (46.6%) and D (45.3%). The grades of movement were not different among the 4 groups. The durations of movement in group A and B were significantly short compared to group C and D. The infused dose of propofol (mg/kg) at movement was higher in group C (2.65+/-0.62) than in A (1.99+/-0.62) and B (2.43+/-0.78). There were no significant hemodynamic and SPO2 changes during and after the propofol injection. CONCLUSIONS: We concluded that slow injection may increase the incidence of involuntary movement during propofol induction in children. Since the bolus injection rates are usually slow in most syringe pumps, manual injection for 10 15 s may be a better choice for smoother induction, as it requires fewer interventions to prevent venous catheter displacement in children.

ABSTRACT

PURPOSE: Male sexual desire and erection are directly or indirectly affected by hormone and neuroendocrine response. We investigate the changes of the serum dopamine and testosterone level by visual erotic stimulation-induced sexual excitement. MATERIALS AND METHODS: The age of volunteers was ranged from 25 to 65 years, with an average of 45.5years. Serum were collected at rest and after visual erotic stimulation. Sexual excitement induced by erotic film for 30 minutes and erectile status was monitored by Rigiscan. The response of visual erotic stimulation were divided into three groups according to degree of sexual excitement. i.e. no response (Group I), mild excitement(Group II), excitement (Group III). RESULTS: Although some changes were observed in serum dopamine and testosterone level of all 32 patients after visual erotic stimulation, it did not showed statistically signigicant change (p=0.450, p=0.058). According to excitement response, group I and II were not showed statistically significant change in both dopamine and testosterone. Only dopamine of the group III was showed statistically change (dopamine: p=0.041, testosteron: p=0.257) CONCLUSIONS: In this study, serum testosterone level is not showed significant change irrespective of the degree of sexual excitement induced by visual erotic stimulation. However, serum dopamine was changed with statistically significance only in sexual excitement group.

ABSTRACT

Objective:To investigate the effect of decoction of Fructus Mume (DFM) on the myoelectric activity of uterine smooth muscle of unpregnancy and early pregnancy rats and study its action mechanism.Methods: A pair of bipolar electrode was implanted on the serous surface of rat uterus. The DFM was injected into abdominal and the changes of uterine myoelectrical activity were observed.Results: The higher dose of DFM could enlarge the average amplitude of slow wave of uterine smooth muscle myoelectricity as well as increase the incidence and the maximal amplitude of the outbreak plexiform myoelectric wave. On the other hand, rats during early pregnancy were more sensitive to the DFM. Conclusions: The DFM enhances the myoelectricity activity of uterine smooth muscle of unpregnancy and early pregnancy rats which may result from enhacing the start cells electric activity and speeding up the action potential depolarization. Thus the DFM can serve as an effective clinical drug for anti early pregnancy.
