ABSTRACT

【Objective】 To analyze the blood dispatching of hospitals in Shijiazhuang area, in order to provide reference for blood dispatching policy of hospitals and ensure clinical blood supply. 【Methods】 The hospital blood dispatching data from 2018 to 2021 were queried through the blood center information management system (SHINOW9.5) and TMIS system, and the blood dispatching volume, frequency, variety and blood type distribution of hospitals at all levels were analyzed by SPSS statistical software. 【Results】 From 2018 to 2021, there were 1 196 times of blood dispatching in hospitals with a total volume of 4 648.5 U, showing a downward and then upward trend two years before and after the outbreak of COVID-19. As the main blood dispatching product, erythrocyte dispatching was 3 988 U, accounting for 85.79% of the total dispatching volume, with a dispatching rate of 2.99‰. The dispatching volume of red blood cells in secondary hospitals accounted for 86.13%.The dispatching rate of erythrocyte in secondary and tertiary hospitals was 1.03% and 0.06%, respectively, with significant difference(P<0.05). The dispatching rates of type A and AB red blood cells were 3.08‰ and 4.97‰, which were significantly different from those of type B and O red blood cells. 【Conclusion】 Blood dispatching has become an effective way to avoid blood waste and ensure emergency blood use in secondary and tertiary hospitals. In the actual operation, health administration department should strengthen supervision and hospitals should improve the blood dispatching process to ensure blood safety.

ABSTRACT

China produces about 15 000 tons of expired drugs every year. Standardized destruction of expired drugs consumes a large number of secondary costs, causing huge economic losses. Improper handling will also lead to serious environmental pollution, endanger national health and even endanger public safety. This paper analyzes the hazards, sources and disposal methods, and research status of expired drugs, introduces the Shelf-Life Extension Program of United States, and put forward feasible measures for the recovery, integration and reuse of expired drugs. It is suggested to construct a scientific and reasonable recovery and disposal system of expired drugs in combination with the actual situation in China, explore the strategy of extending the expiration of drugs, and greatly reduce the waste of drug resources.

Resumen: Objetivo: Evaluar el comportamiento de las presiones intrapulmonares con la inversión del índice inspiración espiración durante la colecistectomía electiva videolaparoscópica. Material y métodos: Se realizó un ensayo clínico controlado fase tres acerca de tales presiones intrapulmonares, durante operaciones de colecistectomía laparoscópica, en el Hospital Universitario «Manuel Ascunce Domenech¼, en el período de septiembre del 2016 a febrero del 2020. El universo estuvo constituido por los pacientes que cumplieron con los criterios de inclusión, y una muestra de 106 de ellos, los cuales se dividieron en dos conjuntos de 53 personas cada uno, por método aleatorio simple. La fuente primaria de obtención de datos la constituyeron las historias clínicas y el resultado de una encuesta. Los datos se recogieron en un modelo diseñado en correspondencia con la bibliografía. Resultados: Se presentó aumentos de la presión inspiratoria pico y presión meseta luego del neumoperitoneo, con descenso de ambas tras la transposición del índice I:E, además de hipertensión arterial, arritmias e hipotensión arterial, sin guardar relación con la aplicación del indicador I:E invertido. Conclusiones: La inversión de la inspiración espiración contribuyó a mejorar los efectos del neumoperitoneo sobre dichas presiones intrapulmonares, manteniendo una oxigenación adecuada y nulos efectos cardiovasculares.


Abstract: Objective: Evaluating the behavior of intrapulmonary pressures with the inversion of the inspiration expiration index, during elective videolaparoscopic cholecystectomy. Material and methods: A phase three controlled clinical trial was conducted on intrapulmonary pressures during laparoscopic cholecystectomies at the «Manuel Ascunce Domenech¼ University Hospital, from September 2016 to February 2020, the universe consisted of patients who met the inclusion criteria, from which a sample of 106 patients was selected, by a simple randomizing method. The primary source of data collection was the medical records and the result of a survey. These data were collected in a model designed in correspondence with the bibliography reviewed. Results: Measured by variation in peak inspiratory pressure and plateau pressure after pneumoperitoneum, with significant decrease of both after inversion of I:E index. More than half of the patients presented arterial hypertension, arrhythmias and arterial hypotension, without them being related to the application of the inverted I:E index. Conclusions: The inversion of inspiratory expiration contributed to improve the effects of pneumoperitoneum on intrapulmonary pressures, with adequate oxygenation and no cardiovascular effects.

ABSTRACT: The glass ionomer cements (GICs) is a generic name given to a group of materials widely used in clinical dentistry which if used after the specified expiration date, material properties may be affected. to evaluate the Vick ers microhardness, surface morphology and the energy dispersive X-ray microanalysis (EDX), of GICs with different expiration dates that were stored at room temperature. specimens of highly viscous glass ionomer cement (HVGIC) (Ketac Cem and Ketac Molar) and resin-modified glass ionomer cement (RMGIC) (Vitrebond) with different expiration dates (current, close to their expiration and expired) were prepared for Vickers microhardness test and scanning electronic microscopy (SEM) with EDX, measuring 5 mm in diameter and 2 mm length, per the manufacturer's instructions, in standard cylindrical teflon molds. For the comparison of obtained values, the ANOVA test was used, while Tukey test was used for the multiple comparisons. In all the GICs used, the microhardness decreased as the expiration date approached, finding a significant statistical differen ce (P<0.05) in Ketac Molar and Vitrebond. SEM sample analysis revealed similar cohesive cracks in all tested materials. The EDX analysis revealed the presence of the elements F, Al and Si in all GICs and Ca only in Ketac Molar and Ketac Cem. The elements were found in a higher atomic percentage in the GICs with an current date and in a lower percentage in those with an expired date. HVGIC and RMGIC with an expiration date finish and that were stored at room temperature, suffer significant physical and chemical changes, which could put doubts its clinical effectiveness.


RESUMEN: El cemento de ionómero de vidrio (CIV) es un nombre genérico que se le da a un grupo de materiales ampliamente utilizados en odontología clínica que si se usan después de la fecha de vencimiento especificada, las propiedades del material pueden verse afectadas. evaluar la microdureza Vickers, la morfología superficial y el microanálisis de energía dispersa de rayos X (EDX), de CIV con diferentes fechas de caducidad almacenados a temperatura ambiente. Muestras de cemento de ionómero de vidrio de alta viscosidad (CIVAV) (Ketac Cem y Ketac Molar) y cemento de ionómero de vidrio modificado con resina (CIVMR) (Vitrebond) con diferentes fechas de vencimiento (vigente, próximo a su vencimiento y vencido) de 5 x 2 mm, fueron preparadas para microscopía electrónica de barrido (MEB) con EDX y microdureza Vickers, según las instrucciones del fabricante, en moldes de teflón cilíndricos estándar. Para la comparación de los valores obtenidos se utilizó la prueba ANOVA, mientras que para las comparaciones múltiples se utilizó la prueba de Tukey. En todos los CIV utilizados, la microdureza disminuyó a medida que se acercaba la fecha de vencimiento, encontrándose una diferencia estadística significativa (P <0.05) en Ketac Molar y Vitrebond. El análisis de la muestra en MEB reveló grietas cohesivas similares en todos los materiales probados. El análisis EDX reveló la presencia de los elementos F, Al y Si en todos los GIC y Ca solo en Ketac Molar y Ketac Cem. Los elementos se encontraron en mayor porcentaje atómico en los GIC con fecha vigente y en menor porcentaje en aquellos con una fecha vencida. Los CIVAV y CIVMR con fecha de caducidad vencida y que fueron alma- cenados a temperatura ambiente, sufrieron cambios físicos y químicos importantes, lo que podría poner en duda su efectividad clínica.

ABSTRACT

The aim of the present study was to determine the effects of expiratory effort on the magnitude and low-frequency components of force variability during submaximal isometric knee extension (IKE). Eight subjects performed 5-s IKE of the dominant leg at the intensity of 10% of maximal voluntary contraction (10% MVC). During the IKE, the subjects performed one of three forced expirations (FEs) of different effort levels. The three effort levels of FE were 0% effort (normal breathing, FE0), 50% effort (FE50) and 100% effort (FE100). In the expiratory phase of each IKE, the amplitude of the force variability was quantified by the coefficient of variation (FCV), and the low-frequency components of the force variability were evaluated by calculating the peak power values of the filtered force signal (F) and rectified EMG signal with high-pass filtering (rEMGH) of the vastus lateralis muscle. The cross-correlation between F and rEMGH was also calculated. FCV was significantly higher in FE100 than in FE0 and FE50 (p < 0.01). The peak power values of F and rEMGH was significantly higher in FE100 than in FE0 (p < 0.05). Cross-correlation between F and rEMGH with the latter preceding the former was observed in each FE condition. These results suggest that FE with maximal effort (FE100) increased not only the absolute amount of force variation but also low-frequency oscillatory components of force and EMG and that low-frequency oscillations of motor unit firing may be responsible for the increase in force variation induced by FE.

Astracts Objective: Studies have focused on use of non-expired composites. Unfortunately some clinicians still use expired composite resins without considering their effects. The objective of this in vitro preliminary research was to investigate cytotoxicity of expired(6-months) and non-expired composite resins. Materials and methods: Expired (E) and non-expired (NE) samples of one bulk-fill (Tetric N-Ceram Bulk-fill (TNB), Ivoclar Vivadent), two nano-hybrid (Tetric N-Ceram (TN), Ivoclar Vivadent; Clearfil Majesty ES-2 (CM), Kuraray) composite resins were tested on L929 fibroblast cells. Medium covering cells was removed then plastic rings (2-mm height) were filled with non-polymerized composite resins, placed in direct contact with cells and polymerized with LED light curing unit (LCU). Three samples were prepared for each group. After polymerization, removed medium was added to the cells. Cells that were left without medium (WOM) and cells that were exposed to LCU were used as positive control groups. Cells without any treatment were used as negative control group (C). Cells were incubated with tested materials for 7-days to evaluate cytotoxicity. Cell viability was calculated by sulforhodamine B test as a percentage (%). One-way ANOVA and post-hoc Tukey tests were used for statistical analyses (p0.05), except between TN NE and TN E (p0.05). All experimental groups compared with C group showed statistically significant cytotoxicity (p<0.05). A statistically significant difference existed between LCU and C groups (p<0.05). Conclusions: In clinical practice, expired composite resins should never be used. Although a correlation was found between expiration dates of nano-hybrid composite resins and cell viability, opposite data were obtained for bulk- fill composite resin. Researches are still required to evaluate biocompatibility of bulk- fill composite resins at various thicknesses with current LCUs.


Resumen Objetivo: Los estudios se han concentrado en el uso de resinas compuestas no vencidos. Desafortunadamente, algunos clínicos aún usan resinas caducadas sin considerar sus efectos. El objetivo de este estudio preliminar in vitro fue investigar la citotoxicidad de resinas compuestas caducadas (6 meses) y no caducadas. Materiales y métodos: muestras caducadas (E) y no caducadas (NE) de una resina bulk-fill (Tetric N-Ceram Bulk-fill (TNB), Ivoclar Vivadent) y dos resinas nanohíbridas (Tetric N-Ceram (TN) Ivoclar Vivadent) (Clearfil Majesty ES-2 (CM), Kuraray), se probaron en células de fibroblastos L929. Se retiraron las células que cubrían el medio, luego se llenaron anillos de plástico (2 mm de altura) con resinas no polimerizadas, se colocaron en contacto directo con las células y se polimerizaron con una unidad de fotocurado LED (LCU). Se prepararon tres muestras para cada grupo. Después de la polimerización, se añadió el medio eliminado a las células. Las células que quedaron sin medio (WOM) y las células que se expusieron a LCU se usaron como grupos de control positivo. Las células sin ningún tratamiento se utilizaron como grupo de control negativo (C). Las células se incubaron con las resinas durante 7 días para evaluar la citotoxicidad. La viabilidad celular se calculó mediante la prueba de sulforodamina B como un porcentaje (%). ANOVA unidireccional y pruebas post-hoc de Tukey se utilizaron para los análisis estadísticos (p 0.05), excepto entre TN NE y TN E (p 0.05). Todos los grupos experimentales en comparación con el grupo C mostraron citotoxicidad estadísticamente significativa (p <0,05). Existió una diferencia estadísticamente significativa entre LCU y grupos C (p <0.05). Conclusiones: En la práctica clínica, las resinas compuestas caducadas nunca deben usarse. Aunque se encontró una correlación entre las fechas de vencimiento de las resinas compuestas nano-híbridas y la viabilidad celular, se obtuvieron datos opuestos para la resina bulk-fill. Se requieren nuevas investigaciones para evaluar la biocompatibilidad de las resinas bulk-fill en distintos espesores con las LCU actuales.

Resumo Introdução Medicamentos vencidos em farmácias caseiras são potenciais agentes causadores de danos à saúde humana e ao meio ambiente. Neste sentido, são essenciais estudos que subsidiem políticas de intervenção para minimização dos riscos. Objetivo Estimar a prevalência e os fatores associados aos medicamentos vencidos em estoques caseiros. Método O estudo transversal foi realizado por meio de entrevistas nas residências dos usuários da atenção primária à saúde (APS) de uma cidade do centro-oeste mineiro, Brasil. As estimativas de prevalência e suas associações foram realizadas por meio de modelos lineares generalizados (MLGs). Resultados Nos 423 estoques caseiros, foram encontrados 4.203 medicamentos, com uma média de 10 medicamentos/residência (DP = 5,87). A prevalência de residências com medicamentos vencidos foi de 45,4%, sendo os antimicrobianos e analgésicos os mais prevalentes. A presença de medicamentos vencidos está associada à presença de medicamentos isentos de prescrição (MIPs) e antimicrobianos nas residências e ao comportamento de guardar o medicamento em desuso em seu estoque caseiro. Conclusão Aproximadamente metade das residências possui medicamentos vencidos, e a presença deles está associada à classe terapêutica e ao comportamento do usuário.


Abstract Background The storage of expired medications at home can cause potential damage to human health and the environment. Thus, studies that support intervention policies to minimize risks are essential. Objective To estimate the prevalence and factors associated with storage of expired medications at home. Method The cross-sectional study was conducted through interviews in the homes of Primary Health Care (PHC) users of a city in the center-west of Minas Gerais, Brazil. Prevalence estimates and their associations were performed using generalized linear models (MLGs). Results We found 4,203 medications stored in 423 homes, with an average of 10 medication/residence (SD=5.87). The prevalence of homes with expired medications was 45.4%, with antimicrobials and analgesics being the most prevalent. The presence of expired medications is associated with the presence of Over the Counter drugs (OTC) and antimicrobials at homes, and the behavior of keeping the unused drug in their home pharmacy. Conclusion Almost half of the homes have expired medications. And the presence of these drugs is associated with the therapeutic class and the behavior of the PHC user.

ABSTRACT

La implementación del entrenamiento respiratorio aislado en la rehabilitación y el entrenamiento de la voz es una práctica común en los países de habla hispana. Ac-tualmente, no existe ningún manuscrito en español que revise la información teórica y empírica del entrenamiento respiratorio en este contexto. El propósito de la presente revisión es entregar la evidencia actualizada del efecto del entrenamiento respiratorio aislado en la voz. El entrenamiento de la fuerza respiratoria ha demostrado tener consecuencias positivas en los parámetros medidos; no obstante, los datos disponibles hasta ahora no reporta efectos significativos en la voz, exceptuando los casos de personas con trastornos neurológicos de base y presbifonía. Si el entrenamiento respiratorio con el uso de dispositivos no ha demostrado impactar favorablemente sobre otras disfonías (no neurológicas ni presbifonía) ni en sujetos sanos profesionales de la voz, no existiría razón para esperar que los ejercicios respiratorios aislados que se suelen incluir en las rutinas de entrenadores vocales, fonoaudiólogos, logopedas y foniatras tengan un efecto positivo. Considerando que el entrenamiento de la fuerza muscular respiratoria parece actuar sobre algunos parámetros vocales en personas con alteraciones neurológicas y presbifonía, futuras investigaciones deberían considerar la exploración del posible efecto positivo en otros parámetros vocales no medidos aún en este tipo de población


Isolated breathing training in rehabilitation and voice training is a common prac-tice in Spanish-speaking countries. Currently, there are no documents in Spanish that study the theoretical and empirical information related to respiratory training in this context. The purpose of the present review is to provide updated information regarding the current evidence of the possible effect of isolated respiratory training on voice. Respiratory strength training has been shown to have positive effects on respiratory parameters, however, the available evidence does not report significant effects on the voice, except for people with underlying neurological disorders and presbyphonia. If respiratory training using devices designed for these purposes has not been shown to positively impact vocal characteristics of subjects with dyspho-nia (not neurological or presbyphonia) or of healthy professional voice users, there is no reason to expect that the isolated breathing exercises that are often included in the routines of vocal trainers and speech therapists will have a positive effect on voice variables. Considering that respiratory muscle strength training seems to positively affect some vocal parameters in people with neurological disorders and presbyphonia, future research should consider exploring the possible effect on other vocal parameters not yet measured in this type of population

Resumo Objetivos: Avaliar o grau de contaminação por fungos e bactérias e o modo de conservação destes colírios hipotensores por parte dos pacientes no ambulatório de Glaucoma da Santa Casa de Ribeirão Preto. Métodos: Foram selecionados aleatoriamente cinquenta e cinco pacientes, em seguimento no ambulatório, e após consentimento dos mesmos os colírios eram coletados e enviados via correio para análise por microbiologista e patologista em até 72 horas. Foi analisado 0,5ml aproximadamente das medicações e os pacientes respondiam a um questionário simples sobre o método de conservação e se consideravam estes adequados. Resultados: Dos 55 colírios analisados, cinco (9,01%) estavam com seu conteúdo líquido contaminado. Entre os microrganismos isolados haviam 4 bactérias Gram negativas, sendo 1 (1,8%) por Serratia marcescens, 1 (1,8%) Pseudomonas aeruginosa e 2 (3,6%) Stenotrophomas maltophilia. Um colírio estava contaminado pelo fungo Cândida ssp Todos pacientes do estudo julgam seus métodos de armazenamento e instilação adequados. Os pacientes que tiveram os colírios positivados eram convocados para exame clínico e passavam por novo questionário pelo investigador. Conclusão: O tempo de abertura dos frascos e os métodos de conservação influenciam na contaminação dos medicamentos, todos os colírios com crescimento de microrganismos no presente estudo estavam abertos entre 30 e 90 dias. O fato de que a maioria dos pacientes levam seus colírios em tarefas cotidianas, aumenta a exposição dos frascos e podem ser um fator relevante para determinar a contaminação destas medicações.


Abstract Objetives: To assess the degree of fungal and bacterial contamination of hypotensive eye drops and the way these are preserved by the patients at the Glaucoma outpatient clinic of Santa Casa Hospital in Ribeirão Preto. Methods: Fifty-five patients were randomly assigned to follow-up in the outpatient clinic and, after their consent, an eye drop was collected per patient and later sent by mail for analysis by microbiologist and pathologist in up to 72 hours. Approximately 0.5ml of the medications were analyzed and the patients were asked to answer a simple questionnaire on the method of drug conservation and whether they considered it adequate. Results: Of the 55 analysed eye drops, five (9.01%) had their liquid contents contaminated. Among the microorganisms isolated there were 4 Gram negative bacteria, 1 (1.8%) by Serratia marcenses, 1 (1.8%) Pseudomonas aeruginosa and 2 (3.6%) Stenotrophomas maltophilia. An eye drop was contaminated by the fungus Candida ssp. All the patients in the study judged their methods of storage and instillation appropriate. The patients who had the positive coliria were summoned for clinical examination and passed through a new questionnaire by the investigator. Conclusion: The time and methods of preservation influence the contamination of medicinal products. All the eye drops that presented growth of microorganisms in the present study were open between 30 and 90 days. The fact that most patients take their eye drops on daily tasks increases the exposure of the bottles and can be a relevant fact to determine the contamination of these medications.

OBJECTIVE:To provide references and suggestions for expiration date management of dismounted drugs in China.METHODS:The main contents about compliance policy for expiration date of repackaged drugs in America were summarized.The current situation and problems of expiration date management of dismounted drugs in China were analyzed to provide suggestions for standardizing expiration date management of dismounted drugs in China.RESULTS & CONCLUSIONS:The compliance policy for expiration date of repackaged drugs in America contain determination principle of expiration date of repackaged drugs,repackaged container standard,etc.In contrast,the expiration date management of dismounted drugs in China is lack of specific standards and requirements;major issues are as follows as the concept of expiration date of dismounted drugs is ambiguous and container management is not standardized.It is suggested that relevant departments can refer to compliance policy for expiration date of repackaged drugs in America,define the method of determining the expiration date of the dismounted drugs,and standardize the standards and management of packaging containers so as to standardize expiration date management of dismounted drugs in China practically.

ABSTRACT

Según la Constitución Política la acción de tutela se puede interponer en todo momento, es decir no tiene caducidad. Sin embargo, para poder garantizar la inmediatez que la caracteriza, la Corte Constitucional ha señalado que el juez que resuelve la tutela debe evaluar en cada caso, si entre el hecho que dio lugar a la presunta violación de derechos fundamentales y la interposición de la acción de tutela media un plazo razonable. El artículo tiene como objetivo analizar de qué forma el principio de inmediatez de la acción de tutela puede constituir una barrera para la protección de los derechos fundamentales en sede judicial. Para ello, se analizan las subreglas establecidas por la jurisprudencia constitucional para valorar la inmediatez en casos concretos, así como para excepcionar su aplicación por algunas causas que lo justifican. Tales subreglas son valoradas a la luz de las consideraciones frente a la inmediatez que, como jueces de tutela, tienen el Consejo de Estado, la Corte Suprema de Justicia y el Consejo Superior de la Judicatura. Los resultados sugieren que, en efecto, el principio de inmediatez sigue siendo una razón recurrente para negar el amparo, aun en contravía de la jurisprudencia constitucional.


According to the Constitution writ of protection can be used at any time so it has no expiration date. However in order to guarantee the immediacy, the Constitutional Court has ruled that the judge, who resolves the writ of protection, must assess in each case if there is a reasonable time between the event that has brought about violation of fundamental rights and the use of the writ of protection. The paper aims to analyze how the principle of immediacy of writ of protection can be a barrier to the enforcement of fundamental rights in court. For this reason, I analyze the sub-rules established by the Constitutional Court regarding to study the immediacy in specific cases and the application of exceptions when there are reasons that justify it. The sub-rules are valued in the light of the considerations regarding the immediacy that, as judges, have the Council of State, the Supreme Court of Justice and the Superior Council of the Judicature. The results suggest that, in effect, the principle of immediacy remains a recurring reason for denying protection, even in contravention of constitutional jurisprudence.


De acordo com a Constituição da ação tutela, pode ser apresentado a qualquer momento, ou seja, não tem data de validade. No entanto, a fim de garantir o imediatismo que o caracteriza, o Tribunal Constitucional decidiu que o juiz resolve a tutela deve avaliar em cada caso se entre o evento que deu origem à suposta violação dos direitos fundamentais e da apresentação do ação média tutela dentro de um prazo razoável. O artigo tem como objetivo analisar como o princípio da imediação de tutela pode ser uma barreira para a protecção dos direitos fundamentais no tribunal. Para fazer isso, sub-regras estabelecidas pela jurisprudência constitucional para avaliar a urgência em casos específicos são analisados e sua aplicação para fazer uma exceção para algumas causas que o justifiquem. Tais sub-regras são avaliadas à luz das considerações contra o imediatismo que, como juízes tutelares, tem o Conselho de Estado, o Supremo Tribunal e do Conselho Superior da Magistratura Judicial. Os resultados sugerem que, de fato, o princípio da imediação continua a ser um recorrente negar amparo, mesmo contra a razão jurisprudência constitucional.

ABSTRACT

La kinesiterapia respiratoria, es una especialidad terapéutica que tiene el rol principal de prevenir y tratar las complicaciones pulmonares de forma sencilla, sin incorporar recursos sofisticados en su ejecución. Su objetivo es optimizar la función respiratoria para lograr un adecuado intercambio de gases y mejorar la relación ventilación perfusión. Para esto se efectuan maniobras de permeabilización bronquiales, que cobran especial valor en la infancia, dado que este grupo etáreo, presenta mayor producción de secreciones y una VA de conducción muy estrecha, ambos elementos predisponen a obstrucciones que deben ser manejadas cuando se amerite. La kinesiterapia, también incluye otras técnicas como ejercicios respiratorios, movilización, posicionamientos, maniobras de reexpansión pulmonar asociados con dispositivos especialmente adaptados para este objetivo. En este escrito, se dan las bases de esta terapia física, con énfasis en la edad pediátrica, junto a la descripción global y específica de las principales técnicas, que el pediatra debiese conocer.


Chest physioterapy is a therapeutic specialty that aims to prevent and treat lung complications without adding sophisticated resources for his execution as its main role. Its goal is to optimize the pulmonary function to achieve appropriate gaseous exchange and to improve ventilation/perfusion ratio. To this end airway clearance techniques are performed, these techniques are of special value during childhood, since this age group present higher quantities of secretions and very narrow conducting airways, both elements predispose to obstructions that must be handle if the case merits. Chest physioterapy also include others techniques as deep breathing, early movilizations, lung recruitment maneuvers with specially adapted devices. This paper set the stage of this pshysical therapy with emphasis in pediatric age, adding a global and specific description of the main techniques that the pediatritian should know.

Objective To understand the expiration date of alcohol-based hand disinfectant after it is opened for use,and provide reference for rational clinical application of alcohol-based hand disinfectant.Methods 20 bottles of the same brand alcohol-based hand disinfectant which opened at the same time by the clinical departments were selected as the study object,hand hygiene compliance in theses departments ranked fourth from the end in the hospital,specimens were taken on the first day after opening,and repeated every 10 days until the disinfectant was used up.Hand specimens were also taken after disinfected by disinfectant.Qualified condition of disinfectant and hand specimens was detected.Results A total of 98 disinfectant specimens were collected,by naked eye observation,20 bottles of alcohol-based hand disinfectant were free of discoloration,precipitation,and suspended matter during the whole study period.The qualified detection rates of alcohol-based hand disinfectant within 60 days after opening were all 100%.44 hand specimens were taken and detected after disinfection,3 of which were unqualified(all were disinfected by alcohol-based hand disinfectant 50 days after opening),then detected again after disinfected by the same batch of disinfectant,all were qualified,which suggested that unqualified detection result of hand specimens was not due to disinfectant.Conclusion Alcohol-based hand disinfectant still has a good bactericidal effect on the sixth day after opening.

The theft of mobile phones as a recurrent criminal phenomenon in the Colombian capital city has become one of the most disturbing damages aff ecting citizen safety, and is demanding particular attention from the authorities in order to avoid further civilian victims and sometimes even fatal casualties resulting from the thief attempting to snatch these personal devices away from their hands. For the purpose of combating this crime and directly aff ecting the finances of the criminal organizations engage in this kind of off enses, one of the most creative strategies introduced by local authorities has been the application of the measure known as extinction of the right of property ("dominio").

ABSTRACT

OBJECTIVE: First, to investigate the diagnostic performance of fast T1-weighted sequences for lung nodule evaluation in oncologic magnetic resonance (MR)/positron emission tomography (PET). Second, to evaluate the influence of image acquisition in inspiration and expiration breath-hold on diagnostic performance. MATERIALS AND METHODS: The study was approved by the local Institutional Review Board. PET/CT and MR/PET of 44 cancer patients were evaluated by 2 readers. PET/CT included lung computed tomography (CT) scans in inspiration and expiration (CTin, CTex). MR/PET included Dixon sequence for attenuation correction and fast T1-weighted volumetric interpolated breath-hold examination (VIBE) sequences (volume interpolated breath-hold examination acquired in inspiration [VIBEin], volume interpolated breath-hold examination acquired in expiration [VIBEex]). Diagnostic performance was analyzed for lesion-, lobe-, and size-dependence. Diagnostic confidence was evaluated (4-point Likert-scale; 1 = high). Jackknife alternative free-response receiver-operating characteristic (JAFROC) analysis was performed. RESULTS: Seventy-six pulmonary lesions were evaluated. Lesion-based detection rates were: CTex, 77.6%; VIBEin, 53.3%; VIBEex, 51.3%; and Dixon, 22.4%. Lobe-based detection rates were: CTex, 89.6%; VIBEin, 58.3%; VIBEex, 60.4%; and Dixon, 31.3%. In contrast to CT, inspiration versus expiration did not alter diagnostic performance in VIBE sequences. Diagnostic confidence was best for VIBEin and CTex and decreased in VIBEex and Dixon (1.2 ± 0.6; 1.2 ± 0.7; 1.5 ± 0.9; 1.7 ± 1.1, respectively). The JAFROC figure-of-merit of Dixon was significantly lower. All patients with malignant lesions were identified by CTex, VIBEin, and VIBEex, while 3 patients were false-negative in Dixon. CONCLUSION: Fast T1-weighted VIBE sequences allow for identification of patients with malignant pulmonary lesions. The Dixon sequence is not recommended for lung nodule evaluation in oncologic MR/PET patients. In contrast to CT, inspiration versus expiratory breath-hold in VIBE sequences was less crucial for lung nodule evaluation but was important for diagnostic confidence.

ABSTRACT

In Japan, controlled devices for medical professionals have been often purchased by people other than acupuncturists or doctors, using the Internet sales system. Press Tack Needle (PYONEX<sup>®</sup>) allows easy handling by combining a plastic part with an adhesive tape. Since it has been reported to be relatively safe with limited adverse events such as adhesive-related dermatitis or similarly mild disorders, it is widely used. We report a case that when the patient removed a PYONEX<sup>®</sup> which applied by himself, the needle tip was broken and remained in his body. It might lead to an accident such as injury due to a broken needle left in the body. The patient had purchased the PYONEX<sup>®</sup> on his own judgments, storing it for a long period of time, applied it to his body 10 months after its expiration date and kept it attached for approximately 3 weeks. Plastics are characterized by deterioration and can be damaged by environmental factors including ultraviolet rays, water, and shocks. In this paper as an urgent issue, we note that when using these products, in addition to paying attention to their expiration dates, precautions should be considered since deterioration may also progress due to prolonged attachment.

Objective To investigate the stability of torasemide injection and predict its expiration date in room temperatue .Methods The contents of torasemide were determined by HPLC .The expiration date was predicted by classical constant-temperature experiment .Results The contents of torasemide degradation with time according to a first order reaction at room temperature (25 ℃) .The disintegration rate constant K was K25 ℃ =5 .186 9 × 10-6 h-1 and the expiration date of to-rasemide injection was t0 .9 =2 .32 years .Conclusion The torasemide injection expiration date was tentatively scheduled for two years .

No presente texto, discute-se a atualidade da teoria histórico-cultural elaborada por Lev Semionovitch Vigotski entre, aproximadamente, o final da segunda década e a terceira década dos anos de 1900. Adota-se atitude crítica em relação à ideia de que o conhecimento científico tem prazo de validade, procurando-se mostrar que ela decorre do processo de transformação da ciência em mercadoria. Como um contraponto a essa visão, argumenta-se que todo artifício construído pelo homem apresenta algo de permanente, quando transcende a mera funcionalidade das coisas produzidas para consumo. Ou, dizendo em outros termos, tudo o que é singular, de certa forma, tem permanência no mundo


In this paper, we discuss the topicality of the cultural-historical theory developed by Lev Semionovitch Vygotsky approximately between the end of the second decade and the third decade of the 20th century. We adopt a critical attitude towards the idea that scientific knowledge has an expiration date, and we seek to show that this idea emerges from the transformation of science into a commodity. As a counterpoint to this view, we argue that every device built by man presents something permanent which transcends the mere functionality of things produced for consumption. In other words, we argue that anything that is singular, in a way, has permanence in the world
