ABSTRACT

Abstract The present study involves the chemical and bacteriological analysis of water from different sources i.e., bore, wells, bottle, and tap, from Peshawar, Mardan, Swat and Kohat districts of Khyber Pakhtunkhwa (KP) province, Pakistan. From each district, 50 water samples (10 samples from each source), regardless of urban and rural status, were collected from these sources and analysed for sulphates, nitrates, nitrites, chlorides, total soluble solids and coliforms (E. coli). Results indicated that majority of the water sources had unacceptable E. coli count i.e.> 34 CFU/100mL. E. coli positive samples were high in Mardan District, followed by Kohat, Swat and Peshawar district. Besides this, the some water sources were also chemically contaminated by different inorganic fertilizers (nitrates/nitrites of sodium, potassium) but under safe levels whereas agricultural and industrial wastes (chloride and sulphate compounds) were in unsafe range. Among all districts, the water quality was found comparatively more deteriorated in Kohat and Mardan districts than Peshawar and Swat districts. Such chemically and bacteriologically unfit water sources for drinking and can cause human health problems.

ABSTRACT

Background : Reproductive Tract Infection (RTI) due to poor hygiene caused by faecal Bacteria or viruses which goes into the vaginal canal during and after defecation is not only damaging to the reproductive organ, but also causes indirect or direct impact on Socio-economic status of the patient and their family as well. Objective : This study highlights the importance of faecal hygiene in preventing damage to the reproductive tract and curbing the impact on social and economic problems in patients and their families. Materials and Methods : This study was conducted at GICE Nursing Home, Kalyani, Nadia, WB, between April 2016 and March, 2022. During this period, 11,872 cases were reported at GICE Nursing Home of which 10,000 cases were selected. Discussion : It was interesting to observe that those who wipe from back to front or vice versa are more likely suffering from Vaginitis (85%), Cervicitis (92%), Endometritis (55%), PID (60%), UTI (70%) and Tubal Block (17%) as compared to those who used Hand shower (10%) or Tissue Paper (9%). This is an indication that proper faecal hygiene can prevent damage to the reproductive system. Further analysis revealed that the inability to pay for medical expenses created a Socio-economic crisis among family members. Conclusion : It is concluded from this study that maintenance of proper faecal hygiene by women in Rural, Urban and Industrial areas, can not only help with prolonged illnesses but also prevent financial losses. As a result, Guardians, Teachers, Health Care Providers and Local Government should conduct outreach programs to teach the importance of faecal hygiene at home, in Schools, Colleges, Public health institutions and so on, etc

ABSTRACT

Microbiome is associated with a wide range of diseases. The gut microbiome is also a dynamic reflection of health status, which can be modified, thus representing great potential to exploit the mechanisms that influence human physiology. Recent years have seen a dramatic rise in gut microbiome studies, which has been enabled by the rapidly evolving high-throughput sequencing methods (i.e. 16S rRNA sequencing and shotgun sequencing). As the emerging technologies for microbiome research continue to evolve (i.e. metatranscriptomics, metabolomics, culturomics, synthetic biology), microbiome research has moved beyond phylogenetic descriptions and towards mechanistic analyses. In this review, we highlight different approaches to study the microbiome, in particular, the current limitations and future promise of these techniques. This review aims to provide clinicians with a framework for studying the microbiome, as well as to accelerate the adoption of these techniques in clinical practice.

ABSTRACT

Phage based therapeutics have shown promising results against the infections caused by the drug resistant bacteria. To combat the problem of antibiotic resistance posed by diarrhoeagenic E. coli, here, we identified and characterized 38 E. coli phages which were isolated from 70 solid sources (goat-faeces and soil). The in vitro lytic range of phage isolates (n=38) against 439 isolates of E. coli was found between 16 and 53%. Three phage isolates with highest host range showed lytic efficacy against 53, 48 and 46% of E. coli isolates, respectively. A preparation with above three phages was developed, and the phages of the preparation were found stable at wide range of temperature, pH and chloroform treatment. Endotoxin content of the preparation was found below the threshold level and it also passed safety and sterility tests. a total of 40 diarrheic goat kids were administered orally with the therapeutic phage preparation for two days twice daily. Total 21 diarrheic goat-kids were successfully treated using the therapeutic phage preparation, whereas 19 kids could not be treated (success rate: 52.5%; 21/40). The results of the current study provide insight for using lytic bacteriophages for therapeutic interventions against drug resistant E. coli responsible for colibacillosis in neonatal goat kids

ABSTRACT

Background: Cholera is a preventable disease, still it remains a major public health problem among developing countries like India. Access to safe water and a sanitary environment can easily control many gastrointestinal infections including Cholera. In the last week of June 2021 large number of acute diarrhoeal cases were reported which was higher than expected subsequently confirmed as an outbreak of Cholera. Objective: To verify the diagnosis, identify risk factors and institute appropriate control measures to control the outbreak. Materials and Methods: A cross-sectional study was undertaken to identify the time, place and personal distribution of the disease. Based on the findings of the descriptive study, sanitary survey and environmental examination a hypothesis was formulated. A case-control study was carried out to test a hypothesis Results: A total of 158 cholera cases were reported with one death. Male and females were affected equally. All age groups affected, 18.35% of the cases were reported among children aged 0-5 years old. It was a common source epidemic and water was source of infection. The water sample reported faecal contamination. Those households not using water purification methods were 1.454 times more affected (OR=1.454) than those using water purification methods. Conclusion: The Vibrio Cholerae bacterium caused the cholera outbreak in Nadiad. Infection was caused by contaminated drinking water.

ABSTRACT

Aims: This study aimed to investigate the levels of efficacy of anthelmintics that were most commonly used in sheep farms in Nziih locality. Study Design: Longitudinal prospective study. Place and Duration of Study: The study was carried out in Nziih locality in August 2019 and September 2020. Methodology: Resistance against Albendazole (12.5 mg/kg) and Ivermectin (0.2 mg/kg) was evaluated using faecal egg count reduction test in naturally occurring gastrointestinal strongyles of sheep in Nziih. A total of 300 young animals were randomly selected from 5 farms in 2019 and in 2020. Faecal samples were collected from each animal on the first day of treatment (D0) and collected again 14 days later (D14). Faecal Egg Counts were performed using the Willis and Mac Master techniques and resistance was declared when faecal egg count reduction (FECR) was less than 95 % and the lower limit of confidence interval (LCI) less than 90%. Pooled faecal samples for each treatment group were set for D0 and D14 to identify resistant strongyles genera by calculating persistent larval rate (PLR). Results: Resistance against Ivermectin was detected while there was none against Albendazole. Moreover, it was declared only in 2 farms out of the 5 selected. From 2019 to 2020, the status of resistance has shifted from suspicion to resistance for ivermectin and from absence of resistance to suspicion to Albendazole. Resistance was recorded in 2 sites: Pastorale1 (FECR= 85 %, LCI= 42.61 %) and Pastorale 3 (FECR=90.5 %, LCI= 72.26 %) in large flock herds and in farms where sheep were dewormed at least 4 times per year. Trichostrongylus were more persistent than Haemonchus and Oesophagostomum/Chabertia with PLR of 21.22 %, 13.71 %, and 7.40 % respectively. Conclusion: Anthelmintics must be used correctly in conjunction with farmer education in Nziih.

ABSTRACT

BACKGROUND In order to study the influence of long-term growth process and evolution environment on intestinal bacteria of different breeds, the intestinal bacteria and volatile fatty acids among the faeces of Min, Landrace and Yorkshire pigs were analysed by Illumina high-throughput sequencing of the 16S-rDNA and gas chromatography. RESULTS The shared core microbiota of Landrace, Yorkshire and Min pig were 1273, accounting for 69.56% of total abundance of organisms. The proportion of Firmicutes in Min pig faeces (57.89%) was significantly higher than that in Landrace and Yorkshire pig faeces (47.01% and 46.40%, respectively) (P < 0.05), but that of Bacteroidetes was exactly opposite. Moreover, Min pig presented more highly efficient membrane transport, environmental adaptation, carbohydrate transport, and metabolism than Yorkshire pig (P < 0.05). The acetic acid/total volatile fatty acid ratio in Min pig was significantly higher than that in Landrace pig (P < 0.05), and the isobutyric acid/ total volatile fatty acid ratio in Min pig was significantly larger than that in Yorkshire pig (P < 0.05). Furthermore, the content of branched chain volatile fatty acids in Min pig was significantly higher than that in Yorkshire pig (P < 0.05). CONCLUSION This study demonstrated that Min pig, as an excellent breed in the cold area of China, possessed special intestinal floral structure compared to the imported pigs in order to satisfy their phys iological and metabolic demands, which may influence their characteristics such as resistance to cold, diseases, and crude feeding, and the ability to deposit intramuscular fat.

ABSTRACT

Objective:This study aims to evaluate the clinical effect of Changyanqing mixture combined with mesalazine enteric-coated tablets in the maintenance treatment of ulcerative colitis(UC) in remission period. Method:The 140 patients with UC in remission period were randomly divided into control group (70 cases) and observation group (70 cases). The 61 patients in control group completed the therapy (6 cases lost or lost to follow-up and 3 were eliminated), 63 patients in observation group completed the therapy (5 cases lost or lost to follow-up and 2 were eliminated). Both groups′ patients got treatment of lifestyle adjustment, and they also took mesalazine enteric-coated tablets orally, 0.5 g/time, 3 times/day. Patients in observation group took Changyanqing mixture orally for a month in the morning and evening every day, 150 mL/time, and then changed to 150 mL/time, 1 time/day, for 3 consecutive months, finally changed to once every other day for 8 months. Patients in control group took simulated medicine of Changyanqing mixture orally in the same way as observation group. The treatment was continued for 12 months. When UC recurred during the treatment, patients took mesalazine enteric-coated tablets orally at 1 g/time, 3 times/day until remission, when the above intervention plan was continued to be adopted. The recurrence rate, first recurrence time within 12 months (duration from remission to Mayo≥3) and the degree of disease activity at recurrence were recorded. Scores of traditional Chinese medicine(TCM)syndrome and inflammatory bowel disease questionnaire (IBDQ) were evaluated once every 2 months. Before treatment, and at the 6th and 12th month after treatment, colonoscopy and mucosal histology were performed once, enteroscopic mucosal scores, Geboes index of mucosal histology were evaluated, and fecal calprotectin(FC) levels were detected. Also, safety evaluation was conducted. Result:During 12 months, the recurrence rate in observation group was 20.63% (13/63), lower than 39.34% (24/61) in control group(P<0.05), the frequency of recurrence and the first recurrence duration in observation group were all less than those in control group(P<0.01). All these meant the disease activity of patients in observation group was lighter than that in control group (<inline-formula><alternatives>χ2<graphic specific-use="big" xmlns:xlink="http://www.w3.org/1999/xlink" xlink:href="alternativeImage/444CE72A-DE9D-4013-BB0B-F487F60C8DC8-M003.jpg"><?fx-imagestate width="3.30200005" height="3.64066648"?></graphic><graphic specific-use="small" xmlns:xlink="http://www.w3.org/1999/xlink" xlink:href="alternativeImage/444CE72A-DE9D-4013-BB0B-F487F60C8DC8-M003c.jpg"><?fx-imagestate width="3.30200005" height="3.64066648"?></graphic></alternatives></inline-formula>=5.947, P<0.05). After repeated measurements of variance analysis, scores of TCM syndrome, enteroscopic mucosal scores, Geboes index and FC levels in two groups gradually increased(P<0.05), and scores of IBDQ gradually decreased (P<0.05) during the 12-month period. At the second, fourth, sixth, eighth, tenth and twelfth month, scores of TCM syndromes in observation group were lower than those in control group (P<0.01), and scores of IBDQ were higher than those in control group (P<0.01). At the sixth and twelfth month after treatment, intestinal endoscopic mucosal scores, Geboes index and FC levels in observation group were all lower than those in control group (P<0.01). And there were no adverse reactions related to Changyanqing mixture. Conclusion:Changyanqing mixture combined with mesalazine enteric-coated tablets in the maintenance treatment of patients with UC in remission can control the FC level, further reduce the recurrence rate, delay the recrudescence-time, reduce the frequency of UC and the disease activity, maintain the good remission state of UC, stabilize the quality of life of patients, and ensure the safety of clinical use.

ABSTRACT

ABSTRACT: This study evaluated the effects of different levels of substitution of ground corn by sweet potato flour on intake, digestibility, microbial protein synthesis and total Nitrogen (N) retention in sheep fed mixed diets based on corn silage. Eight castrated male sheep were used, kept in metabolic cages. The animals received fixed proportions of oat hay, corn silage and soybean meal. The control group (0%) received only ground corn, while the other groups received different levels of inclusion of sweet potato flour (SPF) to replace ground corn, in the proportions of 33%, 66%, and 100%. The 0% and 33% groups had lower neutral detergent fiber (NDF) intake than the 66% and 100% groups. There was a trend (P = 0.07) of effect of treatments on crude protein (CP) intake with a significant positive linear effect (P = 0.03). There was a trend (P = 0.07) for treatment effects in N intake with a significant positive linear effect (P = 0.03). The other parameters as urine N excretion (P = 0.91) and feces N excretion (P = 0.16) were not affected by the replacement of ground corn by SPF. Moreover, there was no effect on N retention (P = 0.43) and on efficiency of N use (P = 0.69). Adding SPF to a diet instead of corn did not alter the microbial protein parameters. The inclusion of sweet potato flour did not cause changes in the protein metabolism of animals, suggesting it can be used instead of corn.

ABSTRACT

OBJECTIVE Epimedium is rich in a variety of beneficial active ingredients, and has been widely used in the ethnopharmacological practices, however, its biotransformation in gastrointestinal digestions remain unclear. This study aimed to investigate the dynamic changes of components and biological activity of Epimedium in the in vitro simu? lated digestion and subsequent human faecal fermentation. METHODS The models of in vitro simulated saliva, gastric and intestinal digestion, as well as colonic fermentation were constructed to simulate the digestion process of Epimedium. The dynamic changes of components of Epimedium during the simulated digestions in vitro and subsequent human faecal fermentation were investigated by UPLC-MS, HPLC-DAD combined with principal component analysis (PCA) and multi-ingredient quantitative analysis. RESULTS A variety of metabolites with high contents were produced after 0.5 h of intestinal digestion and colonic fermentation 0.5 h. Application of PCA to HPLC data showed the obvious separation of colonic fermentation 0.5 h stage samples from other colonic fermentation stages samples (24, 48 and 72 h). Addition? ally, non-digestion and saliva digestion stage samples clustered together, and there was obvious separation between intestinal digestion samples and gastric digestion samples. The contents of epimedium C, icariin and baohuside I all increased significantly after intestinal digestion [58.70 ± 7.08, 47.15 ± 5.68 and (12.78 ± 0.55) mg · g-1] compared with gastric digestion [29.00 ± 5.65, 17.40 ± 4.55 and (2.77 ± 0.19) mg·g-1]. There were significant differences between sample after 0.5 h of colonic fermentation [64.22 ± 9.32, 51.26 ± 6.33 and (16.68 ± 3.19) mg·g-1] and other time points (24, 48 and 72 h) in components and the contents of active ingredient, and the content of these components all decreased with the fermentation time. The ability of scavenging ABTS free radicals [IC50=(0.29 ± 0.02) g · L-1] increased significantly compared with gastric digestion [(1.57 ± 0.02) g·L-1], and after 0.5 h of colonic fermentation, the ability also increased significantly. CONCLUSION Gastrointestinal digestion had a significant impact on the contents of active components in Epimedium, and the metabolism of these components mainly occurred in the colon. The intestinal digestion and colonic fermentation significantly improved the anti-ABTS activity of epimedium.

ABSTRACT

Abstract In neonates, rapid change in diet imbalances gut health allowing colonization of opportunistic pathogens that confer harmful effects on animal health causing reduced digestion and malabsorption of nutrients. In this milieu, probiotic feeding can be a promising approach in promoting animal health and stabilization of gastrointestinal microbiota. Hence, the present study was designed to investigate the effect of Lactobacillus acidophilus NCDC15 enriched fermented milk on nutrient digestibility, faecal biomarkers and immune response in Murrah buffalo calves. Twenty-four, neonatal calves (5-7 days) were randomly allocated into four groups for 90 days. The control group (CT) was provided a basal diet of calf starter and green fodder (maize and jowar), without any probiotic fermented milk (PFM) supplementation. Basal diet was supplemented with probiotic fermented milk at 100, 200 and 300 mL/calf/day, in PFM100, PFM200 and PFM300 groups, respectively. Nutrient digestibility remained similar among the groups. Faecal acetate was higher (P<0.05) in PFM200 and PFM300, while, faecal butyrate was increased (P<0.05) with all levels of probiotic supplementation than control. Faecal Lactobacillus and Bifidobacterium count were increased (P<0.05) with a concomitant reduction in coliform population (P<0.05) among all the treatments. Cell-mediated and humoral immune response were higher (P<0.001) in PFM200 and PFM300 than CT. Overall, it can be concluded that inclusion of Lactobacillus acidophilus NCDC15 in the form of fermented milk upto 300 mL/calf/day improved immunity and faecal biomarkers in Murrah buffalo calves without any adverse effect on nutrient utilization which may positively impact growth performance in Murrah buffalo calves.

ABSTRACT

Aim: The present study was carried out to identify and explore novel areas of semiochemical based pest management like oviposition deterrents.Methodology: The oviposition deterrents were identified from larval faecal pellets of pink bollworm using methanol as solvent and analysed in GC-MS. Three fatty acids were identified and evaluated for oviposition deterrent effect. Results: In the present study, three major compounds namely; oleic, linoleic and palmitic acids were identified for the first time from larval faecal pellets of pink bollworm. Their oviposition deterrent effect was confirmed in bioassays carried out with different concentrations of identified compounds. The avoidance index (Ai) 0.78 ± 0.05 and per cent effective deterrency (PED) 87.42% was recorded in oleic acid at highest concentration followed by linoleic acid (Ai: 0.77 ± 0.03; PED: 86.61%) in reducing the egg laying by conspecific female. This clearly showed the role of these compounds as oviposition deterrent. Interpretation: The compounds, oleic and linoleic acids evaluated in laboratory showed oviposition deterrent effect on female pink bollworm reducing egg laying considerably. However, further field studies need to be conducted to validate these observations.

ABSTRACT

Background: Fistula in ano is a common problem in patients presenting to surgical OPD. Various procedures have been described for the treatment of anal fistula, including fistulectomy, fistulotomy and use of a cutting seton. Surgical treatment of anal fistula is associated with a significant risk of recurrence and faecal incontinence due to damage to anal sphincter. The introduction of cyanoacrylate glue to close fistula tracts using an occlusive material and with no risk of incontinence (as there is no sphincter damage). The study was designed to evaluate the role of cyanoacrylate glue in the management of fistula in ano. Methods: Here, 40 patients were enrolled in study as day cases. Patients were examined clinically and subjected to MRI pelvis where internal opening couldn’t be palpated on digital rectal examination (DRE). Fistula tract was mapped using fistula probe and washed with diluted hydrogen peroxide and normal saline. The excess granulation tissue at the external opening was curetted. The glue was then injected slowly into fistulous tract through 8 F infant feeding tube. Patients were further examined in the OPD until 6-months.Results: Here, 32 patients got healed after first instillation of glue with stoppage of discharge from the fistulous tract. The other 2 patients required second instillation of glue and showed no signs of discharge thereafter. ABSTRACT

Coronavirus disease 2019 (COVID-19), caused by the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), was found initially in Wuhan, China in early December 2019. The pandemic has spread to 216 countries and regions, infecting more than 23310 000 people and causing over 800 000 deaths globally by Aug. 24, 2020, according to World Health Organization (https://www.who.int/emergencies/diseases/ novel-coronavirus-2019). Fever, cough, and dyspnea are the three common symptoms of the condition, whereas the conventional transmission route for SARS-CoV-2 is through droplets entering the respiratory tract. To date, infection control measures for COVID-19 have been focusing on the involvement of the respiratory system. However, ignoring potential faecal transmission and the gastrointestinal involvement of SARS-CoV-2 may result in mistakes in attempts to control the pandemic.

ABSTRACT

Objective: To investigate the effect of modified fecal drainage device on avoiding the occupational exposure infection of the medical staffs in the treatment of corona virus disease 2019 (COVID-19), and to provide the reference for the clinical treatment of COVID-19 and avoiding the occupational exposure infection of the medical staffs. Methods: The clinical data of a critical COVID-19 patient with diarrhea as the main symptom were collected. The modified fecal drainage device of F18 silicone gastric tube connected with disposable negative pressure drainage device was uesd to treat the fecal excrement of the patient. The general data of the medical staffs, containing 16 doctors and 48 nurses∗ were collected. The COVID-19 serological antibodies and pharyngeal swabs of the medical staffs were tested every 2 weeks. Results: The 78-year-old woman patient was admitted to hospital due to diarrhea∗ cough and expectoration for 15 d, chest distress and shortness of breath for 10 d» and fever for 1 d. The test result of COVID-19 pharyngeal swabs of the patient was positive. After the feces were collected with the modified fecal drainage device, the average operation time of medical staffs was reduced from 20 min to 10 min, the patient's perianal skin flushing subsided, and no incontinence-associated dermatitis ( IAD ) occurred. The patient was cured but remained in hospital for the other underlying diseases. The test results of COVID-19 serological antibodies and pharyngeal swabs of 64 medical staffs were all negative, all the medical staffs had no infection. Conclusion: The modified fecal drainage device has better stability, which can effectively prevent IAD and the spread of COVID-19 and reduce the risk of occupational exposure infection of medical staffs, and it is suitable for clinical promotion application.

ABSTRACT

Bovine tuberculosis (bTB), a chronic infection in cattle caused by Mycobacterium tuberculosis/bovis, that impacts productivity and represents a major public health threat. Although the considerable economic costs and zoonotic risk consequences associated with the disease, accurate estimates of bTB prevalence are lacking in many countries, including India. Therefore, in the current study for collection of tubercular lesions the postmortem examination of 100 cattle was conducted. All major viscera and regional lymph nodes were examined and incised. Histopathology was performed in the cases where gross lesions were suggestive of tuberculosis. PCR was performed on the tissue and faecal samples by using IS6110 insertion sequence, Mycobacterium tuberculosis/bovis complex PCR kit. In 12 animals, nodular lesions with casseating mass suggestive of tuberculosis were observed in the lung tissue. All the 12 lung impression smear and only five faecal smear showed acid fast bacilli stained by Ziehl-Neelsen staining. Histologic features comprised a classic granuloma as a characteristic lesion of tuberculosis composed of a central caseous necrosis with mantle of macrophages, lymphocytes, plasma cells, epithelioid macrophages and Langhan’s giant cells and were observed in all 12 cases. All the tissue samples and 11 faecal samples were positive for the Mycobacterium tuberculosis complex using IS6110 sequence. 8 tissue samples and 4 faecal samples were positive by using Mycobacterium tuberculosis/bovis complex PCR kit. It can be concluded that there was good agreement between histopathology, acid fast staining and PCR. It can also be concluded that faecal samples which are easier to collect should be preferred for diagnosis of TB by PCR in cattle

ABSTRACT

Background: The war against multidrug-resistant bacteria is challenging and of global concern. Hospitals are increasingly plagued by resistant gram negative pathogens. Bacteria of the family Enterobacteriaceae such as Escherichia coli and Klebsiella pneumoniae are part of the normal human intestinal flora but are also often responsible for community- and healthcare-associated infections. These bacteria are prone to acquiring resistance genes. Methods: Rectal swabs/swabs from the peri-anal area of the patients who were admitted in the Intensive Care Unit (ICU) of the accident and emergency department of this teaching hospital. Swabs were collected first on day 1 of admission, then day 4, and thereafter weekly during the period of stay in the ICU. All the swabs were immediately inoculated into trypticase soy broth with one 10μg  meropenem disc and were incubated overnight at 35±2ºC, ambient air. Next day, the broth was vortexed, and then sub-cultured onto a MacConkey agar plate. On the third day, MacConkey agar plates were examined for lactose fermenting (pink-coloured) colonies. The representative isolated colonies were subjected to conventional antimicrobial susceptibility testing by the Kirby Bauer Disc diffusion method following the CLSI guidelines to know the susceptibility to carbapenem and other antimicrobial agents. Carbapenemase production was done by a Modified Hodge Test (MHT) and Imipenem-EDTA test. Results: Out of 89 patients, carbapenem resistant Klebsiella pneumoniae and E. coli isolates were recovered from 35 (39.3%) patients i.e. Klebsiella pneumoniae isolates from fifteen patients and carbapenem resistant E. coli isolates from twenty patients. Prevalence of carbapenemase producing isolates was found to be 1.42%.  Conclusions: Surveillance for CRE can definitely help reduce rates of healthcare associated infections.

ABSTRACT

Gut microbiota are microorganisms that inhabit the gut; they coexist peacefully with the host, thereby contributing to the health and well-being of individuals. Bacteroidetes and Firmicutes largely dominate the gut microbial flora. The intestinal flora promotes intestinal mucosal integrity, provides essential nutrients such as vitamins and enzymes, protects the body against pathogens and produces antimicrobial peptides such as defensins, C-type lectins, cathelicidins, they also play an active role in the innate and adaptive immune system. Gut microbial flora plays an active role in the synthesis of short-chain fatty acids such as butyrate, propionate and acetate. Gut microbiota also plays a significant role in the cognitive and behavioural functions of the host. A balanced gut microbiota shifts to dysbiosis, due to intake of high fat or sugar or other factors like sedentary lifestyle. The dysbiosis of the gut results in increased permeability, endotoxaemic, insulin resistant, systemic inflammation, adiposity and metabolic disorders such as type 2 diabetes mellitus, non-alcoholic fatty liver disease, non-alcoholic steatohepatitis, irritable bowel disorder, colorectal cancer, etc. A prudent lifestyle modification, added on with use of probiotics and prebiotic restore the normal flora of the gut, especially in patients with Clostridium difficle-associated diarrhoea, inflammatory bowel syndrome, liver disease and colon cancer. Faecal microbial transplant is an important therapeutic tool in many illness related with the gut. Thereby, understanding the gut microbial signatures in various diseases yields various novel therapeutic targets. Human gut microbiota has a prognostic, diagnostic and therapeutic potential which is recognised worldwide.
