ABSTRACT

Enquanto no Norte Global se discute uma crise na Atenção Primária à Saúde, a maioria dos países nunca chegou a constituir sistemas de saúde baseados propriamente numa atenção primária robusta. Nesse cenário, o Brasil apresenta uma tendência mais favorável, com conquistas importantes para a atenção primária e a medicina de família e comunidade nos últimos dez anos. Restam desafios a serem superados para que o Sistema Único de Saúde alcance níveis satisfatórios de acesso a seus serviços, com profissionais adequadamente formados e valorizados pela população.


While the Global North is discussing a crisis in primary health care, the majority of countries have never managed to establish health systems based on robust primary care. Brazil presents a more favorable trend, with important achievements for primary care and family practice over the last ten years. There are still challenges to be overcome so that the Unified Health System achieves satisfactory levels of access to its services, with professionals who are properly trained and valued by the public.


Mientras que en el Norte Global se habla de una crisis de la atención primaria, la mayoría de los países nunca han creado realmente sistemas sanitarios basados en una atención primaria robusta. Brasil, muestra una tendencia más favorable, con importantes logros para la atención primaria y la medicina familiar y comunitaria en los últimos diez años. Aún quedan retos por superar para que el Sistema Único de Salud alcance niveles satisfactorios de acceso a sus servicios, con profesionales debidamente formados y valorados por la población.

Introdução: O conhecimento da magnitude em que a população implementa medidas de proteção emitidas pelas autoridades de saúde pública é essencial na prevenção da doença do novo coronavírus (COVID-19). A eficácia de medidas não farmacológicas de prevenção e das políticas públicas destinadas a reduzir o contágio pela COVID-19 depende de quão bem os indivíduos são informados sobre as consequências da infecção e as medidas que devem adotar para reduzir sua propagação. O entendimento, as atitudes e as práticas das pessoas em relação à COVID-19 e sua prevenção são basilares para a compreensão da dinâmica epidemiológica, demandando a realização de pesquisas sobre o cumprimento de medidas não farmacológicas de prevenção do contágio em diversos territórios. Para isso, em 2020, medidas não farmacológicas contra a COVID-19 foram divulgadas por fontes diversas, estatais e privadas, para a maior parte da população brasileira, com a finalidade de orientar comportamentos para conter a crise sanitária. As equipes da Estratégia Saúde da Família têm um papel fundamental neste processo de educação em saúde, pois compreendem elementos socioculturais das suas comunidades, alcançando-as tanto em capilaridade quanto em adequação local da informação técnico-científica. Este artigo abrange uma pesquisa de campo, parte de um projeto multicêntrico nacional. Objetivo: Avaliar se a população do território de uma unidade da Estratégia Saúde da Família da cidade de Condado-PE entende e aplica as informações que recebeu sobre medidas não farmacológicas de prevenção em suas práticas de proteção contra a COVID-19. Mais especificamente, a pesquisa visou determinar que informações foram recebidas pelos respondentes, quais as suas fontes, o grau de confiabilidade atribuído a estas, além da adesão deles às medidas não farmacológicas e sua relação com variáveis sociodemográficas. Métodos: O modelo do estudo foi observacional e descritivo, com abordagem quantitativa, a partir da coleta de dados primários com 70 usuários por entrevista presencial com questionário estruturado. Resultados: Os resultados mostraram que a população recebeu vasta informação sobre prevenção da doença. Conclusão: Com níveis variados de confiabilidade das fontes, atribuindo importância relevante às medidas de prevenção e adotou a maioria delas, com exceção do isolamento social total.


Introduction: Knowledge of the magnitude to which the population implements protective measures issued by public health authorities is essential in preventing coronavirus disease 2019 (COVID-19). The effectiveness of non-pharmacological prevention measures (NPM) and public policies aimed at reducing the spread of COVID-19 depends on how well individuals are informed about the consequences of the infection and the measures they must adopt to reduce its spread. The understanding, attitudes, and practices of people in relation to COVID-19 and its prevention are fundamental for understanding the epidemiological dynamics, demanding research on compliance with NPM to prevent contagion in different territories. To this end, in 2020, NPM against COVID-19 were released by various sources, state and private, for most of the Brazilian population, with the aim of guiding behaviors to contain the health crisis. The Family Health Strategy (FHS) teams play a key role in this health education process, as they comprise sociocultural elements of their communities, reaching them both in capillarity and in local adequacy of technical-scientific information. This article covers field research, part of a national multicenter project. Objective: To evaluate whether the population of the territory of an FHS unit in the city of Condado, Pernambuco, understands and applies the information it received about NPM prevention in their practices to protect against COVID-19. More specifically, the research aimed to determine what information was received by the respondents, what are their sources, the degree of reliability attributed to these, in addition to their adherence to the NPM and their relationship with sociodemographic variables. Methods: The study model was observational and descriptive, with a quantitative approach, based on the collection of primary data with 70 users through face-to-face interviews with a structured questionnaire. Results: The results showed that the population received extensive information on disease prevention. Conclusion: With varying levels of reliability of the sources, attributing relevant importance to prevention measures and adopted most of them, with the exception of total social isolation.


El conocimiento de la magnitud con la que la población implementa las medidas de protección emitidas por las autoridades de salud pública es fundamental en la prevención de la enfermedad por coronavirus 2019 (COVID-19). La efectividad de las medidas de prevención no farmacológicas (MFN) y de las políticas públicas dirigidas a reducir la propagación de la COVID-19 depende de qué tan bien se informe a las personas sobre las consecuencias de la infección y las medidas que deben adoptar para reducir su propagación. La comprensión, actitudes y prácticas de las personas con relación al COVID-19 y su prevención son fundamentales para comprender la dinámica epidemiológica, exigiendo investigaciones sobre el cumplimiento de las MNF para prevenir el contagio en diferentes territorios. Con ese fin, en 2020, MNF contra el COVID-19 fueron divulgados por diversas fuentes, estatales y privadas, para la mayoría de la población brasileña, con el objetivo de orientar comportamientos para contener la crisis sanitaria. Los equipos de la Estrategia de Salud de la Familia (ESF) juegan un papel fundamental en este proceso de educación en salud, ya que integran elementos socioculturales de sus comunidades, alcanzándolas tanto en la capilaridad como en la adecuación local de la información técnico-científica. Este artículo aborda una investigación de campo, parte de un proyecto multicéntrico nacional, con el objetivo de evaluar si la población del territorio de una unidad de la ESF en la ciudad de Condado-PE comprende y aplica la información recibida sobre la prevención de MNF en sus prácticas de protección contra el COVID -19. Más específicamente, la investigación tuvo como objetivo determinar qué información recibieron los encuestados, cuáles son sus fuentes, el grado de confiabilidad atribuido a estas, además de su adherencia al MNF y su relación con variables sociodemográficas. El modelo de estudio fue observacional y descriptivo, con enfoque cuantitativo, basado en la recolección de datos primarios con 70 usuarios a través de entrevistas cara a cara con un cuestionario estructurado. Los resultados mostraron que la población recibió amplia información sobre prevención de la enfermedad, con diversos niveles de confiabilidad de las fuentes, atribuyendo importancia relevante a las medidas de prevención y adoptando la mayoría de ellas, con excepción del aislamiento social total.

ABSTRACT

Objetivo: compreender a percepção de mães sobre a visitação aberta na unidade de terapia intensiva neonatal. Métodos: estudo descritivo, qualitativo, realizado por meio de entrevista semiestruturada e individualizada, em uma maternidade pública situada no interior de São Paulo, Brasil, em 2019. A amostra foi definida pelo método de saturação de dados e constou de 14 mães. Os dados foram submetidos a Análise de Conteúdo Temática. Resultados: elencaram-se duas categorias: evidenciando os benefícios da visitação aberta e desafios a serem superados. Os benefícios incluíram a satisfação em permanecer com o filho, participar dos cuidados, fortalecer o vínculo maternal, acompanhar a evolução do bebê, evidenciar a qualidade do cuidado e o envolvimento afetivo, redução de sentimentos negativos e visitação do pai no período noturno. Em contrapartida, os desafios incluíram a impossibilidade de permanecer com o filho, estar presente somente em horários pré-estabelecidos para receber informações de médicos, prioriza-las em relação as fornecidas pela enfermagem, receber informações parciais, ter receio em expressar as dúvidas e vivenciar sentimentos negativos. Conclusão: os achados deste estudo fornecem subsídios para que a equipe de saúde e os gestores promovam a adesão de mães à visitação aberta em unidades de terapia intensiva neonatais. (AU)


Objective: understand the perception of mothers about open visitation in the neonatal intensive care unit. Methods: descriptive, qualitative study, carried out through semi-structured and individualized interviews, in a public maternity hospital located in the interior of São Paulo, Brazil, in 2019. The sample was defined by the data saturation method and consisted of 14 mothers. Data were submitted to Thematic Content Analysis. Results: two categories were listed: showing the benefits of open visitation and challenges to be overcome. The benefits included the satisfaction of staying with the child, participating in care, strengthening the maternal bond, monitoring the baby's evolution, showing the quality of care and affective involvement, reducing negative feelings and visiting the father at night. On the other hand, the challenges included the impossibility of staying with the child, being present only at preestablished times to receive information from doctors, prioritizing it in relation to that provided by nurses, receiving partial information, being afraid to express doubts and experience negative feelings. Conclusion: the findings of this study provide support for the health team and managers to promote the adherence of mothers to open visitation in neonatal intensive care units. (AU)


Objetivo: comprender la percepción de las madres sobre la visita abierta en la unidad de cuidados intensivos neonatales. Métodos: estudio descriptivo, cualitativo, realizado a través de entrevistas semiestructuradas e individualizadas, en una maternidad pública ubicada en el interior de São Paulo, Brasil, en 2019. La muestra fue definida por el método de saturación de datos y estuvo conformada por 14 madres. Los datos se enviaron a Análisis de contenido temático. Resultados: se enumeraron dos categorías: mostrando los beneficios de la visita abierta y los desafíos a superar. Los beneficios incluyeron la satisfacción de quedarse con el niño, participar en los cuidados, fortalecer el vínculo materno, monitorear la evolución del bebé, mostrar la calidad del cuidado y el involucramiento afectivo, reducir los sentimientos negativos y visitar al padre por la noche. Por otro lado, los desafíos incluían la imposibilidad de quedarse con el niño, estar presente solo en horarios preestablecidos para recibir información de los médicos, priorizarla en relación a la brindada por enfermeras, recibir información parcial, tener miedo a expresar dudas y experimentar sentimientos negativos. Conclusión: los hallazgos de este estudio brindan apoyo al equipo de salud y gerentes para promover la adherencia de las madres a la visita abierta en las unidades de cuidados intensivos neonatales. básico sobre las conductas frente a los accidentes, a pesar de desconocieren el flujo de atención del servicio. ABSTRACT

O objetivo deste estudo foi revisar sistematicamente a contribuição da família e da escolano desenvolvimento de habilidades para a vida(HV)no contexto de formação esportiva. As buscas foram conduzidas em sete bases de dado se por meio da pesquisa de referências. Foram seguidas as descrições do Prisma, identificando 51 estudos. Os resultados demonstraram uma predominância de investigações no Canadá e nos Estados Unidos. Os autores dos estudos têm utilizado diferentes instrumentos, variáveis e modelos teóricos para verificar a contribuição da escola e/ou da família no desenvolvimento de habilidades para a vida. Conclui-se que o envolvimento escolar e o familiar no esporte podem contribuir com a aquisição, refinamento e transferência de características que podem ser aplicadas na vida dos indivíduos. Por fim, existe a necessidade de construir instrumentos quantitativos específicos que avaliem em conjunto a contribuição da escola e da família neste processo (AU).


The aim of this study was to systematically review the contribution of family and school in the development of life skills(LS)in the context of sports training. The searches were conducted in sevendatabases and through the reference searches.Prisma descriptions were followed, identifying 51 studies. The results showed a predominance of investigations in Canada and the United States. The authors of the studies have used different instruments, variables and theoretical models to verify the contribution of the school and/or the family in the development of skills for life. It is concluded that school and family involvement in sport can contribute to the acquisition, refinement and transfer of characteristics that can be applied in the lives of individuals. Finally,there is a need to build specific quantitative instruments that jointly assess the contribution of the school and the family in this process (AU).


El objetivo de este estudio fue revisar sistemáticamente la contribución de la familia y la escuela en el desarrollo de habilidades para la vida(HV)en el contexto del entrenamiento deportivo. Las búsquedas se realizaron en siete bases de datos y mediante la búsqueda de referencias.Se siguieron las descripciones de Prisma, identificando 51 estudios. Los resultados mostraron un predominio de investigaciones en Canadá y Estados Unidos. Los autores de los estudios han utilizado diferentes instrumentos, variables y modelos teóricos paraverificar la contribución de la escuela y/o la familia en el desarrollo de habilidades para la vida. Se concluye que la implicación escolar y familiar en el deporte puede contribuir a la adquisición, perfeccionamiento y transferencia de características que pueden ser aplicadas en la vida de los individuos. Finalmente,existe la necesidad de construir instrumentos cuantitativos específicos que evalúen de manera conjunta la contribución de la escuela y la familia en este proceso (AU).

Objetivo: Analisar a percepção de enfermeiras sobre a autonomia no exercício de suas práticas no contexto da atenção primária à saúde. Métodos: Estudo descritivo de abordagem qualitativa que analisou 108 entrevistas realizadas com enfermeiras(os) das quatro capitais da região sudeste do Brasil. As entrevistas, guiadas por roteiro semiestruturado, foram gravadas e transcritas. Os dados produzidos foram tratados e explorados com auxílio do software NVIVO®. Resultados: A maior parte das participantes eram mulheres, brancas, residiam na mesma cidade onde trabalham, graduaramse em instituições privadas. Foram organizadas duas categorias: repercussões da autonomia e seus desdobramentos para a resolutividade das necessidades em saúde dos usuários; e (des)conhecimento sobre a regulamentação das práticas da enfermeira: desafios para a autonomia. Conclusão: As enfermeiras compreendem a importância da autonomia para suas práticas, mas enfrentam interferências no cotidiano do trabalho, tanto por parte da gestão, da estrutura dos serviços ou da necessidade do uso de protocolos que garantam o exercício de sua atividade profissional de modo autônomo. Objective: To analyze nurses' perception of autonomy to exercise their practices in the context of primary health care. Methods: A descriptive study with a qualitative approach that analyzed 108 interviews carried out with nurses from the four capitals of the southeastern region of Brazil. The interviews, guided by a semi-structured script, were recorded and transcribed. The data produced were processed and explored with the help of the NVIVO® software. Results: Most of the participants were women, white, lived in the same city where they work, and graduated from private institutions. Two categories were organized: repercussions of autonomy and its consequences for solving users' health needs; and (lack of) knowledge about the regulation of nursing practices: challenges for autonomy. Conclusion: Nurses understand the importance of autonomy for their practices, but they face interference in their daily work, either by management, the structure of services or the need for protocols that guarantee the exercise of their professional activity. Se organizaron dos categorías: repercusiones de la autonomía y sus consecuencias para la solución de las necesidades de salud de los usuarios; y (falta de) conocimiento sobre la regulación de las prácticas de enfermería: desafíos para la autonomía. Conclusión: Los enfermeros comprenden la importancia de la autonomía para sus prácticas, pero enfrentan interferencias en su trabajo diario, ya sea por parte de la dirección, la estructura de los servicios o la necesidad de protocolos que garanticen el ejercicio de su actividad profesional. ABSTRACT

La distribución inequitativa del talento humano en salud afecta la capacidad de los sistemas de ofrecer servicios esenciales. En la provincia de Córdoba, el primer nivel de atención es responsabilidad de los municipios, pero el nivel provincial procura sostener la rectoría y ser garante del derecho a la salud. En ese marco, se desarrolló un programa para reducir las brechas en la distribución de médicos: el Plan Cordobés de Médicos del Interior. Acompañando ese plan se ejecutó un convenio específico con la Universidad Nacional de Córdoba para garantizar la formación en la especialidad de Medicina Familiar y General. Ingresaron al programa 170 personas, y hoy contamos con 98 médicos en formación o seguimiento. En este artículo damos cuenta de la experiencia docente, los desafíos y dificultades que supuso afrontar una formación en lugares de práctica variados, y con el aporte de las tecnologías de la información y comunicación. Esperamos que la experiencia sirva para transmitir los aprendizajes de nuestra práctica (AU)


The inequitable distribution of human talent in health affects the capacity of systems to offer essential services. In the province of Córdoba, the primary level of care is the responsibility of municipalities, but the provincial level aims to maintain leadership and guarantee the right to health. Within this framework, a program was developed to reduce gaps in the distribution of physicians: the Cordobés Plan for Interior Physicians. Accompanying this plan, a specific agreement was executed with the National University of Córdoba to ensure training in the specialty of Family and General Medicine. 170 individuals entered the program, and today we have 98 physicians in training or under supervision. In this article, we give an account of the teaching experience, the challenges, and difficulties involved in facing training in various practice settings, along with the contribution of information and communication technologies. We hope that this experience serves to transmit the lessons learned from our practice (AU)

ABSTRACT

O objetivo desse trabalho é descrever a sistematização da Assistência de Enfermagem a pessoas com agravos endócrinos e metabólicos pautado na teoria de Calista Roy. Metodologia: Trata-se de um relato de experiência através da aplicação da Sistematização da Assistência de Enfermagem (SAE), voltado para pessoas com agravos endócrinos e metabólicos no contexto hospitalar. Resultados e discussão: Foi aplicado a teoria da adaptação nas seis fases da Teoria de Calista Roy e foi elaborado diagnósticos de enfermagem para os quatro modos de adaptação: fisiológico, interdependência, autoconceito e função de papel. Considerações finais: Ao aplicar a SAE no indivíduo com agravos endócrinos e metabólicos a enfermeira deve realizá-lo em todas as suas etapas, e utilizar os protocolos para oferecer um cuidado holístico e integral, visando a promoção da saúde, prevenção de risco potencial e adaptação diante das necessidades em saúde.(AU)


The aim of this study is to describe the systematization of nursing care for people with endocrine and metabolic disorders, based on Calista Roy's theory. Methodology: This is an experience report on the application of the Systematization of Nursing Care (SNC) to people with endocrine and metabolic disorders in a hospital setting. Results and discussion: The theory of adaptation was applied in the six phases of Calista Roy's theory and nursing diagnoses were drawn up for the four modes of adaptation: physiological, interdependence, self-concept and role function. Final considerations: When applying the SNC to individuals with endocrine and metabolic disorders, the nurse must carry it out in all its stages, and use the protocols to offer holistic and comprehensive care, aimed at promoting health, preventing potential risks and adapting to health needs.(AU)


El objetivo de este estudio es describir la sistematización de los cuidados de enfermería a personas con trastornos endocrinos y metabólicos, basándose en la teoría de Calista Roy. Metodología: Se trata de un informe de experiencia sobre la aplicación de la Sistematización de los Cuidados de Enfermería (SNC) a personas con trastornos endocrinos y metabólicos en un entorno hospitalario. Resultados y discusión: Se aplicó la teoría de la adaptación en las seis fases de la teoría de Calista Roy y se elaboraron diagnósticos de enfermería para los cuatro modos de adaptación: fisiológica, interdependencia, autoconcepto y función de rol. Consideraciones finales: Al aplicar el SNC a individuos con trastornos endocrinos y metabólicos, la enfermera debe llevarlo a cabo en todas sus fases, y utilizar los protocolos para ofrecer cuidados holísticos e integrales, dirigidos a promover la salud, prevenir riesgos potenciales y adaptarse a las necesidades de salud.(AU)

ABSTRACT

Analisar como a pesquisa é vivenciada pelos enfermeiros da Estratégia Saúde da Família. Método: pesquisa com abordagem qualitativa, descritiva e exploratória realizada junto aos enfermeiros do município de Maceió, Alagoas. Resultado: o município consta com 86 enfermeiros na Estratégia, sendo todos do sexo feminino. Desses, 10 foram submetidos a entrevista semiestruturada onde foi possível a elaboração de cinco categorias: a pesquisa e a vivência na Estratégia, a pesquisa e a economia, a pesquisa e a família, a pesquisa e à docência e a pesquisa e o crescimento profissional. Conclui-se que o enfermeiro precisa se apropriar da pesquisa científica e utilizá-la no seu campo de trabalho, principalmente na Saúde Pública, pois grande parte das pesquisas realizadas por estes profissionais ainda estão concentradas na docência, nos cursos de especializações, mestrados e doutorados.(AU)


To analyze how research is experienced by nurses in the Family Health Strategy. Method: research with a qualitative, descriptive and exploratory approach carried out with nurses from the city of Maceio, Alagoas. Result: the municipality has 86 nurses in the Strategy, all being female. Analizar cómo es vivida la investigación por enfermeros en la Estrategia Salud de la Familia. Método: investigación con enfoque cualitativo, descriptivo y exploratorio realizada con enfermeros de la ciudad de Maceió, Alagoas. Resultado: el municipio cuenta con 86 enfermeras en la Estrategia, todas mujeres. De ellos, 10 fueron sometidos a una entrevista semiestructurada donde fue posible crear cinco categorías: investigación y experiencia en Estrategia, investigación y economía, investigación y familia, investigación y docencia e investigación y crecimiento profesional. Se concluye que el enfermero necesita apropiarse de la investigación científica y utilizarla en su campo de trabajo, especialmente en Salud Pública, pues gran parte de la investigación realizada por estos profesionales aún se concentra en la docencia, cursos de especialización, maestrías y doctorados.(AU)

Em 2020, a atenção à saúde sofreu o impacto da pandemia de covid-19, e a Atenção Primária não foi exceção. Para melhor compreender a reorganização desse setor no município de Salvador, Bahia, esta pesquisa descreve o perfil dos médicos e as práticas de saúde realizadas por eles na Atenção Primária no contexto da pandemia. Trata-se de um estudo de corte transversal descritivo, que se utiliza de um questionário online autoaplicável distribuído aos médicos participantes que atuam nas unidades básicas de saúde que compõem a Atenção Primária do município. Para análise dos dados, foi utilizada estatística descritiva simples. Com um total de 43 questionários respondidos, foram descritos aspectos referentes a: perfil, formação e atuação dos médicos; mudanças estruturais ocorridas e adoção de novas ferramentas de trabalho; manutenção dos cuidados primários de rotina; ações de vigilância em saúde; suporte social a grupos vulneráveis; e atuação clínica em pacientes com covid-19. Com uma maioria de médicas jovens, recém-formadas e em um período curto de atuação nas equipes onde estavam inseridas, observou-se pouco envolvimento em vigilância e suporte a grupos vulneráveis e um abrangente uso de telemedicina e reestruturação do funcionamento das unidades. Contudo, alguns cuidados primários, como acompanhamento de doenças crônicas e puericultura, resultaram em limitações no acesso e, consequentemente, menor atenção ao cuidado longitudinal.


In 2020, health care suffered the impact of the COVID-19 pandemic and primary care was no exception. To better understand the reorganization of this segment in the municipality of Salvador, Bahia, this research describes the profile of physicians and health practices performed by them in primary care in the pandemic context. This is a cross-sectional descriptive study using an online self-administered questionnaire distributed to participating physicians who work in the basic health units that are a part of primary care in the municipality. For data analysis, simple descriptive statistics was used. With a total of 43 questionnaires answered, the following aspects were described: profile, training, and performance of physicians; structural changes and adoption of new work tools; maintenance of routine primary care; health surveillance actions; social support to vulnerable groups; and clinical performance with COVID-19 patients. With mostly young females, recently graduated, and with a short period of service in the teams where they were located; little involvement in surveillance and support to vulnerable groups is observed, as well as an extensive use of telemedicine and restructuring of the operation of the units. However, some primary care, such as chronic disease follow-up and childcare, resulted in limitations in access and, consequently, less attention to longitudinal care.


En 2020, la atención sanitaria sufrió el impacto de la pandemia de la covid-19 y la atención primaria no fue la excepción. Para comprender mejor la reorganización de este sector en el municipio de Salvador, en Bahía (Brasil), esta investigación describe el perfil de los médicos y las prácticas de salud realizadas por ellos en la atención primaria en el contexto de la pandemia. Se trata de un estudio descriptivo transversal que utilizó un cuestionario autoadministrado en línea distribuido a los médicos participantes que trabajan en las Unidades Básicas de Salud que componen la atención primaria del municipio. Para el análisis de los datos, se utilizó estadística descriptiva simple. Con un total de 43 preguntas respondidas, se describieron aspectos referentes al perfil, formación y capacitación de los médicos; a los cambios estructurales ocurridos y adopción de nuevas herramientas de trabajo; al mantenimiento de los cuidados primarios de rutina; a las acciones de vigilancia en salud; al apoyo social a grupos vulnerables; y a la capacitación clínica en pacientes con covid-19. La mayoría de las médicas eran jóvenes, recién licenciadas y tenían un corto período de actuación en los equipos con los cuales trabajaban, se observó una escasa implicación en la vigilancia y apoyo a colectivos vulnerables, así como un amplio uso de la telemedicina y la reestructuración del funcionamiento de las unidades. Sin embargo, algunas atenciones primarias, como el seguimiento de enfermedades crónicas y la atención a la infancia, se tradujeron en limitaciones en el acceso y, en consecuencia, en menos atención al cuidado longitudinal.

ABSTRACT

Introduction: Family members of patients admitted to an Intensive Care Unit present high uncertainty level due to not knowing what is happening and to not having clear details about the related events; therefore, interventions are required to allow modulating those levels. Objective: To evaluate the effect of an educational Nursing intervention compared to conventional care on the uncertainty of family members of patients hospitalized in an ICU. Materials and methods: An experimental study with a sample comprised by 132 relatives of patients admitted to an ICU, randomly distributed in four Solomon groups (33 in each group). The Nursing intervention based on the concepts of the Uncertainty in Illness Theory was applied to both experimental groups and devised under the Whittemore and Grey parameters with three moments: assessment; education about the relative's hospitalization in the ICU; and accompaniment. This was done with pre-assessments for two groups and post-assessments for the four groups, using the PPUS-FM Uncertainty Scale. The data were analyzed by means of descriptive statistics and respective non-parametric analyses. The study took into account the ethical principles in research. Results: The family members in the experimental groups presented a lower final uncertainty level when compared to the control groups, with a difference of 73.04 points and a p-value of 0.001. Discussion: Standardized interventions and under a theoretical model allow reducing uncertainty in relatives of patients in ICUs. Conclusions: The Nursing intervention based on the Uncertainty theory allows reducing uncertainty in relatives of patients hospitalized in an Intensive Care Unit.

ABSTRACT

Objective To investigate the status of familial aggregation of Helicobacter pylori (Hp) infection in Jinniu District, Chengdu, and analyze its risk factors so as to provide a basis for developing prevention and control strategies of family aggregation of Hp infection. Methods A total of 172 subjects in the Second Affiliated Hospital of Chengdu Medical College · 416 Hospital of Nuclear Industry from January 2022 to January 2023 were selected as the research subjects. All subjects underwent 13C-urea breath test (13C-UBT) to diagnose whether there was Hp infection. Analyze the current situation of family aggregation of Hp infection in the region, collect general data of survey subjects, analyze the relevant factors affecting Hp family aggregation infection, and develop prevention and control strategies based on this. Results A total of 242 people from 97 households were surveyed, and the Hp family aggregation rate was 29.33%. Univariate analysis showed that there were statistically significant differences in family aggregation of Hp infection in terms of different age groups (χ2=9.719, P=0.008), marital status (χ2=8.496, P=0.014), occupations (χ2=19.462, P2=5.457, P=0.019), previous Hp test results (χ2 =4.131, P=0.042) and test results after treatment (χ2=12.000, P=0.001), with statistical significance (P<0.05). Multivariate logistic regression analysis showed that the frequency of dining out 2 days or more per week and a positive Hp test results in the past were risk factors for family aggregation of Hp infection, while the occupation of teachers/medical staff/management/technology personnel and a negative Hp results after treatment were protective factors (P<0.05). Conclusion Family aggregation of Hp infection is related to family members' occupation, frequency of dining out, previous Hp test results and Hp test results after eradication, which deserves attention in clinical practice.

ABSTRACT

Objective@#To investigate the prevalence of anxiety symptoms among high grade primary school students in Guangzhou and their correlation with family functioning and parental parenting styles, so as to provide theoretical basis and guidance for family based interventions for children s anxiety symptoms.@*Methods@#From June to September 2022, a multi stage cluster random sampling method was employed to select 5 396 students from grades 4 to 6 in 13 primary schools in Guangzhou. The Family Functioning Assessment Scale for Children, Parental Bonding Instrument, and Spence Children s Anxiety Scale Short Version were used for the survey. Inter group comparison was analysed by t test or analysis of variance. Linear regression analysis was used to explore the effects of family function and parental parenting styles on anxiety symptoms in primary school students.@*Results@#The average score for anxiety symptoms among high grade primary school students in Guangzhou was (11.63±10.88). In terms of parenting styles, mothers scored higher than fathers in the dimensions of "care" (25.63±4.92) and "control" (5.08±2.58) compared to fathers ( 24.74± 5.50, 5.00±2.51) ( t =15.80, 4.21, P <0.01). However, there was no statistically significant difference in the scores of parents on the dimension of "encouragement of independence"(12.98±4.06,13.05±3.95)( t =-1.77, P >0.05). After adjusting for general demographic characteristics, the results of linear regression analysis showed that primary school students family functioning ( B =0.47) and parental "control" ( B =0.67, 0.75) were positively associated with anxiety symptoms scores ( P <0.01). Meanwhile, parental "care" ( B =-0.53,-0.55) and "encouragement of independence" ( B =-0.62,-0.68) were negatively associated with anxiety symptom scores ( P <0.01). These associations remained statistically significant even after further adjusting for family functioning ( P <0.01).@*Conclusions@#Family functioning, parenting styles are closely associated with anxiety symptoms among high grade primary school students. Positive family functioning and parenting styles may mitigate childhood anxiety symptoms, whereas excessive parental control may increase the risk of childhood anxiety symptoms.

ABSTRACT

Discussing the cognition, attitude and influencing factors of the family members of organ donors towards advance care planning (ACP) to provide a basis for the further promotion of ACP in the field of organ donation. Using qualitative research methods, 8 family members of organ donors were selected purposefully for semi-structured interviews, and the 7-step of Colaizzi was used to analyze and summarize the themes of the interview data. The results showed that the cognition status of the family members of organ donors of ACP could be summarized into 3 themes: problems faced by ACP implementation, positive recognition of the implementation of ACP, factors affecting the promotion of ACP. Organ donors’ family members are unfamiliar with the concept of ACP and have unclear cognition, but think that the implementation of ACP is of positive significance. It is suggested to further strengthen the publicity of ACP, medical autonomy, death education, so as to promote the development of ACP and improve the willing to donate organ.

Objective@#To analyze the correlation between preschool children s emotional competence and home rearing environment in Shangrao City, so as to provide support for improving children s emotional competence development as well as their home rearing environment.@*Methods@#A total of 1 242 children aged 3-6 years old from 10 kindergartens in Shangrao City were retrospectively investigated by stratified cluster random sampling method in December 2022, and the Children s Emotional Adjustment Scale-Preschool Version (CEAS-P) and the Home Nurture Environment Scale for children aged 3-6 were surveyed on parents of preschool children. The t-test was used to test the difference, Spearman correlation analysis and multiple linear regression were used to analyze the influencing factors of preschool children s emotional competence.@*Results@#There were significant differences in emotional competence scores of preschool children for demographic indicators including age, place of residence, health status and whether they were only children ( F/t =5.98, 6.56, 38.00, 2.23, P <0.01). The emotional competence of preschool children was positively correlated with the home rearing environment ( r=0.62, P <0.01). Multiple linear regression analysis showed that diverse activities/play participation, social adaptation/self management, and emotional warmth/self expression in home rearing environment were positive predictors of children s emotional ability ( β =0.30, 0.28, 0.16), while neglect/intervention/punishment were negative predictors ( β =-0.09)( P <0.05).@*Conclusions@#The home rearing environment is a factor related to young children s emotional competence. It is suggested specific parenting initiatives such as enriching family activities and play, strengthening children s self adaptation and management, giving warmth and let children express emotions, and preventing child neglect, interference and punishment should be conducted to improve children s emotional competence.

Objective@#To analyze the status of childhood bullying bystander s behaviors among pupils and the correlation with family function,so as to provide empirical support for educational intervention in campus bullying.@*Methods@#A questionnaire survey using the cluster random sampling method was administered to 1 145 pupils from a primary school in Wushan County within Chongqing City in November 2019, including Pupil Bullying Bystander Behavior Scale and Family Adaptability and Cohesion Evaluation Scale(FACES). The χ 2 test, Spearman related analysis and binary Logistic regression were used to determine the detection rate of different bullying bystander behaviors among pupils and the association with family function.@*Results@#The proportion of bullying promoters, bullying protectors, and bystanders were 4.7%, 92.1%, and 42.1%, respectively. There were statistically significant differences in the three types of bullying bystander s behaviors among the different grades pupils( χ 2=21.45, 7.98, 27.48, P < 0.05 ). There were significant correlation between family function and its dimensions with different types of bullying bystander behavior ( | r |= 0.07-0.20, P <0.05). Binary Logistic regression analysis showed that pupils family function and grade had statistically significant impact on promoting bullying behavior, protecting victim behavior, and bystander behavior ( OR =0.98, 3.33; 1.02, 1.95 ; 0.99, 0.58, 0.41, 0.61, P <0.05).@*Conclusions@#The bullying bystanders behaviors of pupils are related to grade in school and family function. Schools should implement educational interventions for pupils of different grades and focus on improving pupils family function to effectively promote the occurrence of positive bystanders behaviors in pupils.

ABSTRACT

Objective@#To explore the relationship between cumulative family risk and depression symptoms at 2 and 4 year follow up among children and adolescents, so as to provide reference basis for promoting the mental health of children and adolescents.@*Methods@#Data were acquired from the China Family Panel Studies (CFPS) project in 2016(T1), 2018 (T2), and 2020 (T3), which included 730 children and adolescents, their families and parents. The measurement tools included the Cumulative Family Risk Scales and Center for Epidemiologic Studies Depression Scale (CES-D). Chi square test and Logistic regression were used to analyze the relationship between cumulative family risk and depression symptoms among children and adolescents.@*Results@#The detection rates of moderate and above depression symptoms among children and adolescents at T1, T2, and T3 were 14.4%, 15.9%, and 24.5%, respectively. The prevalence of depression symptoms of children and adolescents at T2 was higher in females (19.8%) than males (12.5%)( χ 2=7.28 ,P <0.01). Logistic regression analyses showed that the cumulative family risk at T1 positively predicted depression symptoms of children and adolescents at T1, T2, and T3( OR =1.73, 1.32, 1.38, P <0.05).@*Conclusions@#The cumulative family risk has a long lasting impact on depressive symptoms among children and adolescents after 2 and 4 years. Family and schools should pay attention to the changes in depression among children and adolescents,and provide timely intervention and assistance to promote their mental health development.

ABSTRACT

TCP family as plant specific transcription factor, plays an important role in different aspects of plant development. In order to screen TCP family members in tobacco, the homologous sequences of tobacco and Arabidopsis TCP family were identified by genome-wide homologous alignment. The physicochemical properties, phylogenetic relationships and cis-acting elements were analyzed by bioinformatics. The homologous genes of AtTCP3/AtTCP4 were screened, and RT-qPCR was used to detect the changes of gene expression upon 20% PEG6000 treatment. The results show that tobacco contains 63 TCP family members. Their amino acid sequence length ranged from 89 aa to 596 aa, and their protein hydropathicity grand average of hydropathicity (GRAVY) ranged from -1.147 to 0.125. The isoelectric point (pI) ranges from 4.42 to 9.94, the number of introns is 0 to 3, and the subcellular location is all located in the nucleus. The results of conserved domain and phylogenetic relationship analysis showed that the tobacco TCP family can be divided into PCF, CIN and CYC/TB1 subfamilies, and each subfamily has a stable sequence. The results of cis-acting elements in gene promoter region showed that TCP family genes contain low docile acting elements (LTR) and a variety of stress and metabolic regulation related elements (MYB, MYC). Analysis of gene expression patterns showed that AtTCP3/AtTCP4 homologous genes (NtTCP6, NtTCP28, NtTCP30, NtTCP33, NtTCP42, NtTCP57, NtTCP63) accounted for 20% PEG6000 treatment significantly up-regulated/down-regulated expression, and NtTCP30 and NtTCP57 genes were selected as candidate genes in response to drought. The results of this study analyzed the TCP family in the tobacco genome and provided candidate genes for the study of drought-resistance gene function and variety breeding in tobacco.

ABSTRACT

As one of the key enzymes in cell metabolism, the activity of citrate synthase 3 (CS3) regulates the substance and energy metabolism of organisms. The protein members of CS3 family were identified from the whole genome of apple, and bioinformatics analysis was performed and expression patterns were analyzed to provide a theoretical basis for studying the potential function of CS3 gene in apple. BLASTp was used to identify members of the apple CS3 family based on the GDR database, and the basic information of CS3 protein sequence, subcellular localization, domain composition, phylogenetic relationship and chromosome localization were analyzed by Pfam, SMART, MEGA5.0, clustalx.exe, ExPASy Proteomics Server, MEGAX, SOPMA, MEME, WoLF PSORT and other software. The tissue expression and inducible expression characteristics of 6 CS3 genes in apple were determined by acid content and real-time fluorescence quantitative polymerase chain reaction (qRT-PCR). Apple CS3 gene family contains 6 members, and these CS3 proteins contain 473-608 amino acid residues, with isoelectric point distribution between 7.21 and 8.82. Subcellular localization results showed that CS3 protein was located in mitochondria and chloroplasts, respectively. Phylogenetic analysis divided them into 3 categories, and the number of genes in each subfamily was 2. Chromosome localization analysis showed that CS3 gene was distributed on different chromosomes of apple. The secondary structure of protein is mainly α-helix, followed by random curling, and the proportion of β-angle is the smallest. The 6 members were all expressed in different apple tissues. The overall expression trend from high to low was the highest relative expression content of MdCS3.4, followed by MdCS3.6, and the relative expression level of other members was in the order of MdCS3.3 > MdCS3.2 > MdCS3.1 > MdCS3.5. qRT-PCR results showed that MdCS3.1 and MdCS3.3 genes had the highest relative expression in the pulp of 'Chengji No. 1' with low acid content, and MdCS3.2 and MdCS3.3 genes in the pulp of 'Asda' with higher acid content had the highest relative expression. Therefore, in this study, the relative expression of CS3 gene in apple cultivars with different acid content in different apple varieties was detected, and its role in apple fruit acid synthesis was analyzed. The experimental results showed that the relative expression of CS3 gene in different apple varieties was different, which provided a reference for the subsequent study of the quality formation mechanism of apple.

ABSTRACT

YABBY proteins are important transcription factors that regulate morphogenesis and organ development in plants. In order to study the YABBY of strawberry, bioinformatic technique were used to identify the YABBY gene families in Fragaria vesca (diploid) and Fragaria×ananassa (octoploid), and then analyze the sequence characters, phylogeny and collinearity of the family members. The RNA-seq data and the quantitative reverse transcription-polymerase chain reaction (qRT-PCR) technique were used to assay the expression patterns of the family members. A green fluorescent protein (GFP) was fused with FvYABBYs and transiently expressed in tobacco leaf cells for the subcellular localization. As the results, six FvYABBY genes and 26 FxaYABBY genes were identified from F. vesca and F.×ananassa, respectively. The FvYABBY genes were grouped into five clades, and five family members were orthologous with AtYABBY genes of Arabidopsis. In F. vesca, all of the FvYABBYs were basically not expressed not expressed in root and receptacle, while FvYABBY1, FvYABBY2, FvYABBY5 and FvYABBY6 were highly expressed in leaf, shoot, flower and achene. In F.×ananassa, FxaYABBY1, FxaYABBY2, FxaYABBY5 and FxaYABBY6 were expressed in achene, and all FxaYABBY were poorly or not expressed in receptacle. Additionally, under the abiotic stresses of low temperature, high salt and drought, the expression of FvYABBY1, FvYABBY3, FvYABBY4 and FvYABBY6 were down-regulated, FvYABBY5 was up-regulated, and FvYABBY2 was up-regulated and then down-regulated. In tobacco leaf cells, the subcellular localization of FvYABBY proteins were in the nucleus. These results provides a foundation for the functional researches of YABBY gene in strawberry.

ABSTRACT

In recent years, NOD-like receptor protein 3 (NLRP3) inflammasome in tumors has become a research hotspot, especially in melanoma, colorectal cancer, lung cancer, and breast cancer, and more and more evidence has shown that inflammation plays a role in the development, progression, angiogenesis, and invasion of cancer. Hepatocellular carcinoma (HCC) is the most common type of primary liver cancer, and there are still controversies over the role of NLRP3 inflammasome in the development and progression of HCC. Therefore, this article reviews the potential impact of NLRP3 inflammasome in the progression of HCC and its mechanism of action in anticancer therapy, and it is believed that NLRP3 inflammasome can be used as an effective therapeutic target for HCC patients.
