ABSTRACT

Face to various disaster and dilemma, the tasks of national rescue team were fast and effective rescue, and reducing the harm of life and damage of property, and decreasing each damage to the lowest point. The ability of national rescue team could reflect the strength and developed degree of a national, it further more manifested the levels of medical rescue team. Face to major disasters, it was vital to provide humanitarian assistance for the affected people in disaster areas at the first time. Many developed countries have established national rescue teams and equipped various kinds of rescue equipment, and the disaster relief of medical team was regarded as the main force in emergency health rescue for disaster. refer to the constitution and equipment of national rescue team of developed countries has an important guiding role for perfection of emergency medical rescue in China.

ABSTRACT

Objective To explore the present situation,application and prospect of the medical shelter of the PLA.Methods The application,advantages and disadvantages of the medical shelter were introduced,and some suggestions were proposed for its futural development.Results It's suggested that standardization mechanism and maintenance system be established for modular medical shelter system and medical equipment.Conclusion Military medical service facilities have to execute medical shelter modular allocation to enhance the field hospital in rapid response and treatment.

ABSTRACT

After the outbreak of the Korean war, the Kingdom of Sweden, a permanent neutral nation, dispatched the Swedish Red Cross Field Hospital(SRCFH) instead of armed forces for humanitarian support to the allied forces in South Korea. The Hospital consisted of about 170 Swedes, all volunteers. From the early part of the Korean War, SRCFH took part in the medical assistance in Busan. When the frontline advanced to northern Korea, the number of inflowing casualties to this field hospital decreased. At that time, earnest medical aid for civilians commenced, and many Koreans were treated in available beds in SRCFH. After the armistice in July 1953, SRCFH became the Swedish Hospital in Busan, serving not only the military but also civilians, and continued its humanitarian mission until April 1957 for the Korean who were suffering from a collapsed medical system inthe midst of war. When the Hospital returned to Sweden, it had treated over two million patients from twenty countries, including wounded UN allied force, Korean (south and north), Chinese prisoner of war and Korean civilian. Moreover, it left a transformative legacy, the National Medical Center in Seoul which was established in collaboration with other Scandinavian countries who dispatched medical assistance during the Korean War.

ABSTRACT

Objective To study the value of ultrasound in operation for traumatic brain injury (TBI) in simulation field hospitals where computed tomography (CT) equipment is unavailable.Methods The wartime conditions were simulated, and the patients at age of 18-60 years were randomly selected.According to the principles of medical ethics, all patients received CT scanning.Two neurosur-geons from the field hospital who did not know the result of CT examination cooperated with another two neurosurgeons who got the results of the CT examination to determine whether operation should be ap-plied.Forty-five patients who needed emergency craniotomy were finally selected.The operations were performed by two neurosurgeons from the field hospital, with real-time ultrasound monitoring.The results of ultrasound were compared with that of CT scanning.Results A total of 64 lesions were found by ul-trasound , and 60 of which should be treated by operation.In the meantime, 82 lesions were found by CT scanning, 64 of which should be treated operatively.Epidural hematoma and subdural hematoma had high ultrasound detection rate, followed by intracerebral hematoma, subdural effusion and brain contusion.The overall detection rate of ultrasound was 87.1%.Conclusion Ultrasound plays an important role in correctly finding the focus so as to reduce blind operation and avoid omission of the focus in field hospi-tals, where CT equipment is usually unavailable.

ABSTRACT

Objective To evaluate the injuries in Jiangyou Field Hospital first week after Wenchuan earthquake,and accumulate experience for better medical services and better-built field hospitals to meet the demands of the earthquake-stricken areas.Methods Classification was performed in 1 905 patients with emergency treatment.The medical records of those with hospital stay were retrospectively analysed,including geographical distribution,trauma type,diagnosis,initial treatment and outcomes.Results Six hundred and twenty-nine(33%)patients were hospitalized,with an average of 2 to 100 years old(median,41.2).Three hundred and eighty-four patients(61%)were admitted within the first three days after earthquake.The patients were mainly from Jiangyou City(245,39%),Beichuan County(194,31%)and Pingwu County (158,25%).The most common types of injuries were superficial lacerations(326,51.8%),fractures(202,32.1%) and soft tissue contusions/sprains(87,13.9%),among whom 15 experienced severe squeeze.There were 318 injuries to pelvis and extremities(50.6%),117 to head(18.6%)and 43 to back(6.8%),and 73 cases had multiple injuries (11.6%).Of 376 patients(59.8%)with operation,326 received debridement,21 external fixation,19 internal fixation, 11 amputation,4 laparotomy,4 fasciotomy and 2 craniotomy.The incidence of postoperative infections was 11.8%.Death occurred in 39 patients due to cerebral trauma,pelvic fracture,abdominal hemorrhage and multiple injuries.Conclusion When the earthquake has destroyed the major medical facilities,rapid organization of medical rescue teams,making full use of remaining medical resources and recovering electricity play an important role in the rescue of victims and reduction of mortality.Orthopaedic surgeons must dominate the medical teams as most of the earthquake victims suffer from injuries to the extremities.

ABSTRACT

OBJECTIVE Based on the epidemiologic data, to provide the effective measures to control nosocomial infection in field hospital. METHODS The views in literatures on medical treatments in war conditions and the practical measures for control and prevention of nosocomial infection were summarized. RESULTS Nine steps must be taken in order to effectively control and prevent nosocomial infection, including making plans and measures, preparing important materials, training medical staff, prevention educating, sterilizing, cutting off pathogenic source, monitoring, reasonable antibiotics applying, and protecting susceptible people. CONCLUSIONS In field hospital, effective prevention and control of nosocomial infection is the premises of improving healing rate and rejoining rate of the wounded.

Objective To analyze and solve problems in spreading out the power supply system in mobile field hospital during the earthquake relief.Methods According to the layout of the mobile field hospital and the need of electrical power,the sketch map of electric circuit was drawn out and spread with load as less as possible.Results Current supply and its safety in mobile field hospital are guaranteed.Conclusion This kind of power supply system meets the requirement of the mobile field hospital in emergent rescue tasks.

ABSTRACT

According to the actual situation of taking part in the earthquake relief mission followed by the mobile field hospital,the characteristics of medical support mission and the main problems in the use of medical equipment of the mobile field hospital are summarized.Aimed at the problems,equipments are suggested to be added especially used for surgery,first aid,blood transfusion and some special subjects,and improvement should be made on the equipment of ultrasonic diagnosis,inspection and X-ray examination.Finally,the necessary sorts and quantity of the medical equipments needed in the mission are listed.

Objective To analyze and solve the difficulties in rapidly expanding the mobile field hospital in earthquake relief mission.Methods Aimed at the difficulties in the speed,advance men were sent out before the expansion.Then the personnel were regrouped in the proceeding of the expansion,and the vehicles entered the district according to the predefined area and order.Results The expanding speed was raised.Conclusion The scheme meets the requirements of the first aid in the mobile field hospital.

ABSTRACT

Objective To develop a connector between anesthetic machine and oxygen provider for continual oxygen supply to anesthetic machine during field operation.Method According to the principle of three-way block and one-way qas wave,the new connector with two-routes but one-way for the anesthetic machine was made.Result This setting enable the anesthetic machine to connect two oxygen providers at one time.Conclusion With low cost and being easy to install and dissemble,it can be connected to all kinds of anesthetic machine,thus ensuring the persistence work of the anesthetic machine when changing oxygen provider.

ABSTRACT

Objective To research the advantage and disadvantage of image information management system of X-ray diag-nosis vehicle and traditional screen-film radiography in field hospital's in earthquake relief mission. Methods 465 limbs affection cases and 58 trunk affection cases were examined by the X-ray fluoroscopy. The images are printed in perspec-tive by S2001-WZ 6.0 information management system and traditional screen film radiography. Results The two methods of diagnose accordance rate is 98% for limbs disease and 73% for trunk affection. Conclusion The system has the advan-tages of rapidity, low cost, little room require ment, high diagnosis positive rate and extensive application prospect.
