ABSTRACT
This article presents a successful orthodontic management of a 12-year-old male using a fixed orthodontic appliance (straight wire technique). His orthodontic treatment was hinged on creating enough space to accept an artificial tooth, correcting the horizontal and vertical relationships of the upper incisors to the lower and the midline shift. Ultimately, an esthetic smile was achieved; function and self-esteem were improved when the artificial tooth was finally fixed.

Background: Human saliva has got many important functions like lubrication of the oral tissues, making oral functions like speech, mastication and deglutition possible and also protecting teeth and oral mucosal surfaces in different ways. Dental Caries is one of the common complications in patients undergoing orthodontic treatment. Subjects with impaired saliva flow rate often show high caries incidence. A low flow rate combined with a low or moderate buffer effect clearly indicates poor salivary resistance against microbial attack. Aim: To investigate the changes in stimulated and unstimulated salivary flow rate, pH and buffer capacity in patients undergoing therapy with fixed orthodontic appliances. Materials and Methods: Salivary flow rate, pH and buffering capacity of 20 patients who underwent fixed orthodontic treatment at Department of Orthodontics, Ragas Dental College and Hospital, Chennai were examined using GC saliva check buffer kit. The procedure was carried out during the initiation of orthodontic treatment and subsequently, one month and six months after the placement of appliance. Results: Results showed there was a significant increase in the salivary flow rate one month and six months after placement of fixed appliance while pH and buffering capacity showed no significant changes. Conclusion: Salivary flow rate increases significantly during fixed orthodontic treatment. Salivary pH and buffering capacity showed no significant changes suggesting that they are not sole factors for demineralization in orthodontic patients.

@#This research aimed to evaluate the oral hygiene status in fixed orthodontic appliance patients when using two different slim bristles toothbrushes and to assess patients’ toothbrush perception. Twenty six fixed orthodontic appliance patients participated in the six weeks prospective, crossover clinical trial. All patients used two different slim bristles toothbrushes (Toothbrush A and Toothbrush B) for two weeks each with a washout period in between. Gingival health and plaque value were assessed based on Lӧe & Silness Gingival Index and Silness & Lӧe Plaque Index at baseline, week 2, week 4 (washout) and week 6. At the end of the trial, patients’ toothbrush perception was assessed through questionnaire. All data were analysed using SPSS version 22. The mean age of the patients was 21.5 ± 4.3 years, with female predominant (n= 17, 65.4%). More than half were Malay (n= 15, 57.7%) and had tertiary education (n=14, 53.8%). Patients could achieve good oral hygiene when using Toothbrush A (65.4%) and Toothbrush B (69.2%). However, the occurrence of gingivitis was significantly higher when using Toothbrush A (OR=1.889, 95% CI=1.207-2.957, p value < 0.05). Toothbrush B was felt to clean better (n= 14, 53.8%) while Toothbrush A was perceived to be easier to use (n=14, 53.8%). Both toothbrushes maintained patients’ oral hygiene status. However, when using Toothbrush B, oral health status was better as it significantly reduced gingivitis occurrence compared to Toothbrush A. As for the toothbrush perception, most patients preferred Toothbrush A to be taken home.

60 cases(aged 18-40 years) underwent orthodontic treatment were divided into 2 groups.30 patients in group A were treated with invisible aligners,while 30 in group B with fixed orthodontic appliance.Periodontal indices (PI,GI,SBI,PD)were examined before treatment,6 weekes,6 months and 12 months after treatment.Before and 6 weeks after treatment the indices were not statistically different between groups,6 and 12 months after treatment the indices of group A were lower than those of group B (P < 0.05).The invisalign appliance is more propitious to periodontal health than the fixed orthodontic appliance.

Objective To explore the effect of follow-up nursing intervention on preventing dental plaque during oral fixed orthodontic appliance.Methods One hundred and twenty orthodontic patients receiving oral fixed orthodontic treatment were randomly divided into control and experiment groups equally according to visit time.The control group received conventional nursing during the process and follow-up nursing intervention during the process.The two groups were compared in terms of dental plaques before orthodontic, 3 and 6 months after the orthodontic treatment.Results Before the treatment, there was no significant differences in the dental plaques between them(P>0.05), but the indices of dental plagues in the experimental group were significantly lower than those of the control group 3 and 6 months after the orthodontic appliance(P<0.001).Conclusion Follow-up nursing care during orthodontic appliance are more effective for keeping oral hygiene.

La aparatología empleada en los tratamientos de ortodoncia, favorece la retención de placa bacteriana y dificulta su eliminación por parte del paciente, aumentando el riesgo de desarrollar caries, manchas blancas y enfermedad periodontal. Diferentes cepillos dentales se han desarrollado para facilitar la mantención de una adecuada higiene oral en estos pacientes, sin embargo, se ha observado que los cepillos comunmente indicados (cepillo ortodóncico mas cepillo unipenacho), no siempre son bien manejados y difícilmente se usan ambos. El cepillo Cross Action Pro-Salud®, se ha presentado como una alternativa simple de usar y de transportar, por ser un único cepillo que combina las caracteristicas de los dos cepillos comunmente recomendados. Para evaluar la efectividad de este cepillo, en comparación con la prescripción convencional, se evaluaron 2 grupos, de 23 pacientes cada uno, portadores de aparatología fija. Un grupo utilizó la prescripción habitual y un segundo grupo utilizó cepillo Cross Action Pro-Salud®, por un período de 45 días. Los indices de higiene de O`leary, de placa en brackets y gingival modificado fueron registrados al inicio y 45 días después del uso diario de los cepillos antes mencionados. Se utilizó test-t para comparar los resultados obtenidos y se determinó que se produjo una disminución significativa en los tres indices de higiene, no existiendo diferencias en los resultados obtenidos entre ambas prescripciones. El cepillo Oral-B Cross-Action Pro-Salud® es una alternativa recomendable, ya que permite eliminar efectivamente la placa bacteriana en pacientes ortodónticos, y al ser un único cepillo, facilita y acorta el tiempo de cepillado.
Orthodontic fixed appliances include elements that allow the accumulation of bacterial plaque, making tooth brushing more difficult and increasing the risk of developing caries, white spot lesions, and periodontal disease. Several toothbrushes designs have been developed to facilitate oral hygiene in orthodontic patients; however, it has been observed that most patients do not take enough time to brush using adequately both commonly prescribed toothbrushes (Oral-B End-Tufted® and Oral-B Orthodontic® brushes). Oral B Cross Action Pro-Health® has been presented as an alternative, easier to use and to transport. To assess the effectiveness of Cross Action Pro-Health® versus commonly prescribed toothbrushes, two randomly assigned groups of 23 orthodontic patients each, were evaluated. One group of patients used commonly prescribed toothbrushes and the second group used Cross Action Pro-Health®. O`Leary, modified gingival and Bracket plaque index were measured before and 45 days after the daily use of the two mentioned toothbrushes prescriptions. T-test was used to detect statistically significant differences among the brushes for each index. The results demonstrated a significant reduction of the three indexes with the daily use of Cross Action Pro-Health® and no differences were found versus commonly used prescription. Orthodontic patients may benefit from the use of a single brush prescription (Cross Action Pro-Health®), because effective plaque removal can be achieved easier and faster.

OBJECTIVE: The purpose of this study was to evaluate the efficacy of a slim bristled toothbrush compared with a V-shaped orthodontic toothbrush in patients with fixed orthodontic appliances. METHODS: Thirty four orthodontic patients receiving edgewise treatment were randomly assigned to two groups, a slim bristled toothbrush (Nano silver slim care) and a V-shaped toothbrush (Oral-B). Plaque index, gingival index and bleeding index were recorded at the beginning of the study (baseline), 2 weeks, 4 weeks and 6 weeks after new toothbrushes were supplied and the results were analyzed statistically. RESULTS: No statistically significant differences were found for plaque index, gingival index and bleeding index between toothbrush groups during the 6 weeks. Plaque and gingival indices were decreased at 2 weeks and 4 weeks but increased at 6 weeks in both toothbrush groups. Bleeding index in the V-shaped toothbrush group showed the lowest value at 2 weeks then increased at 4 weeks and 6 weeks but in the slim bristled toothbrush group decreased from 2 weeks through 6 weeks to under baseline levels. Patients in their twenties had significantly lower values than teenagers in the slim bristled toothbrush group (p < 0.013). CONCLUSIONS: The results would suggest that both of the toothbrushes are equally effective but the use of a slim bristled toothbrush may be of benefit in promoting gingival health for fixed orthodontic appliance patients in their twenties and over.

Objective To study the influence of oral health education and the family nursing intervention on the incidence rate of gingivitis and the enamel decalcifyication for adolescent patients with fixed diorthosis.Methods Divided 146 adolescent patients who had accepted fixed diothosis into the experiment group and the control group randomly, there were 73 cases in the each group. In the experimental group, oral health education and the family nursing intervention were both used for patients and their parents, while in the control group, only traditional nursing methods were used. Compared the incidence rate of complication between the two groups at the 6th and the 12th month after the fixed diorthosis.Results The incidence rate of gingivitis and the enamel decalcifyication in the control group was significant higher than those of in the experiment group.Conclusion Oral health education and the family nursing intervention can effective decline the incidence rate of gingivitis and the enamel decalcifyication for adolescent patients who have accepted the fixed diorthosis.

The purpose of this study was to evaluate the efficacy of an electric toothbrush, with a specially designed orthodontic brush head, compared to a manual toothbrush on the periodontal health of patients with fixed orthodontic appliances. Forty patients were randomly divided into two groups, the electric and the manual toothbrush groups, 1 month after attachment of fixed orthodontic appliances. Periodontal status was measured using a plaque index, a gingival index, a bleeding index, a pocket depth and a relative attachment loss, at baseline and after 3 months, 6 months, 9 months, and 12 months. The Braun Oral-B Plak Control with Ortho OD 15-1 brush head was used as the electric toothbrush while the Butler G.U.M 124 was given as the manual toothbrush. In the manual toothbrush group, the gingival and bleeding indices showed no statistically significant differences, but the plaque index was significantly decreased (p < 0.001) and pocket depth as well as relative attachment loss were significantly increased (p < 0.01). In the electric toothbrush group, the bleeding index, pocket depth and relative attachment loss showed no statistically significant differences, but the plaque and gingival indices were significantly decreased (p < 0.001). In the case of the plaque, gingival and bleeding indices, there were no statistically significant differences between the electric and the manual toothbrush groups. On the contrary, in the case of pocket depth and relative attachment loss, there were statistically significant differences between the electric and the manual toothbrush groups: an increase in the manual toothbrush groups unlike the electric toothbrush group which kept the same state (p < 0.05). These findings suggest that an electric toothbrush is useful for orthodontic patients with fixed appliances.

The purpose of this research is to evaluate and compare the plaque removal ability of sonic brushing and manual brushing in order to present a method for continuous and effective oral hygiene control during orthodontic treatment. Nonextracted adolescent patients (male : 23 persons, female : 37 persons) and adult patients (male : 15 persons, female : 45 persons) outfitted with a fixed orthodontic appliance from their incisors to second molar, in both the maxilla and mandible, were divided into manual brushing and sonic brushing groups. A Philips oral healthcare HX-4401 sonic toothbrush was used, and the application of brushing for the two groups was standardized at two minutes. After brushing the plaque score was measured with a modified Wilkin's PHP. The following results were achieved after comparing each group's plaque score. 1. In patients with a fixed orthodontic appliance when age was not accounted for, the application of sonic brushing exerted a beneficially decreased effect on the plaque score. (adolescents : p<0.01, adults : p<0.05) 2. When comparing the adolescent and adult groups in regard to plaque score, the adolescent group showed a more decreased effect. 3. When using the same toothbrush type, there was no difference between the adolescent and adult groups.

The purpose of this study was to evaluate the effectiveness of gargling solution with 0.05% NaF and 10% Xylitol in orthodontic patients with fixed appliance. The sample consisted of 30 adolescent patients who were classified into an experimental group and a control group, 15 patients each. Experimental group was used experimental gargling solution and the control group was used with placebo solution. The change of S. mutans in saliva was observed by Cariescreen(R) SM kit at pre and post 2, 4, 6, 8 weeks. The results were as follows. 1. There were significant reduction in the number of S. mutans in saliva between pre and post 2 weeks(p<0.01), 4 weeks(p<0.05), 6 weeks(p<0001), and 8 weeks(p<0001) in experimental group. And significant reduction also were observed in the number of Ss mutans in saliva between post 2 weeks, 6 weeks(p<0.05), and 8 weeks(p<0.05), but no significant reduction were showed in control group. 2. There were significant correlation in the number of S. mutans between each measurement time(pre and post 2, 4, 6, 8 weeks) in control group. 3. There weee no correlation between pre and post 2 weeks, but significant correlation were observed between 2, 4, 6, 8 weeks in experimental group.

Dental caries is one of the most prevalent dental diseases in Korea and its prevention is very important in orthodontic therapy. For the cleansing action of saliva itself and/or tooth-brushing is lowered in patient with fixed orthodontic appliance, oral hygiene of the patient becomes worse, which provides more favorable environment for micro-organisms. Chlorhexidine, one of the series of bisguanide, has been reported to be strong antimicrobial agent and very effective on Streptococcus mutans. The purpose of this study is to evaluate the possibility of chlorhexidine as a anticariogenic agent in fixed orthodontic therapy. We used the varnish containing chlorhexidine as a main ingredient for the chemical control of salivary S.mutans in patients with fixed appliance therapy. We applied the varnish containing chlorhexidine on the labial and interproximal surface of the teeth before bonding and banding teeth of our patients(N=20) and compared to control group patients(N=20). Before the application of chlorhexidine varnish and four times periodically after the completion of fixed appliance set-up, we sampled saliva of both group patients and incuvate S.mutans for 24 hours. In the culture study of sampled saliva, counting the number of S.mutans colonies, we founded as follows: 1. In patients with fixed appliance therapy, the risk of dental caries increase when it compared to that of preorthodontic treatment; The number of salivary S.mutans increase in patient's oral cavity. 2. The experimental agent that contain chlorhexidine is effective to reduce the number of salivary S.mutans. 3. For the effect of this agent is not ever-lasting, periodical application is needed, and additional study for economical interval and number of application is needed.
