ABSTRACT

Objective:To explore the clinical application value of multi-slice spiral CT in the diagnosis.Methods: Twenty nine patients with clinically suspected rib fractures and X-ray showed no 64 row helical CT scanning in patients with chest trauma fractures after reconstruction, multiplanar reconstruction and VR imaging.Results: Twenty nine patients with occult fracture of rib on the MPR, VR imaging were rib image good, 28 cases were single, multiple rib fractures in 1 cases, the total of 4 cases with rib fracture, incomplete fracture in 25 cases.Conclusion: The 64 row helical CT for occult rib fracture detection rates, diagnostic accordance rate was superior to X-ray plain film, various post-processing techniques applied to timely, accurate, intuitive display rib fracture location, shape, extent of chest trauma, occult rib fracture first choice inspection method.

ABSTRACT

Objective To compare the outcomes of traumatic flail chest with multiple injuries treated by operative fixation versus conservative approach. Methods The clinical data of 60 patients with traumatic flail chest with multiple injuries were retrospectively analysed, and the outcomes between operation group (treated by operative fixation, n=32)and non-operation group (treated by conservative approach, n=28) were compared. Results The mean time of hospital stay, ICU stay and mechanical ventilation was significantly shorter, and the prevalences of chest wall deformity, pulmonary infection, pulmonary atelectasis and respiratory failure were significantly lower in operation group than those in non-operation group (P<0.05). Six months after discharge, the pulmonary function parameters such as inspiratory capacity, forced vital capacity, forced expiratory volume in one second, peak expiratory flow, total lung capacity and maximal midexpiratory flow were significantly higher in operation group than those in non-operation group (P<0.05). Conclusion Traumatic flail chest with multiple injuries treated by operative fixation may lead to less flail chest associated complications. Operative fixation has short- and long-term benefits to flail chest.

ABSTRACT

Objective To approach the value of combination mode of different CT post-processing technique in diagnosing rib fine fracture.Methods 98 patients suspected with rib fractures underwent multislice spiral CT(MSCT) scanning,and CT features of rib fractures were observed with the combination of three different images:(A)volume rendering(VR),curved planar reformation(CPR) and axial view;(B)maximum intensity projection(MIP),CPR and sxial view;and(C) VR+MIP,CPR and axial view.The results were respectively recorded.Results 265 rib fine fractures were found among 98 patients.The detecting rate of fine fractures with C group was higher than that with A group (χ~2=6.67,P<0.01) and B group (χ~2=6.75,P<0.01).Conclusion MSCT four step observation method can improve the detecting rate of rib fine fracture,that is of important clinical value.

ABSTRACT

To define the X ray diagnostic criteria of the rib fracture.The radiological and the clinical manifestations of 480 patients with chest blunt injury were studied retrospectively.In our study,(fracture of ribs occurred in 376 cases 78 3%)335 cases out of 376 cases had fractures more than one rib.In 348 cases,rib fracture took place on one side of the chest.In 376 cases,941 ribs were fractured.Only one fracture occurred in each rib in 903 ribs.In 623 ribs (66 2%), the fracture took place before and behind the mid axillary line.Among 376 cases,the diagnosis were mistake in 46 cases(12 2%)according to the first posterior anterior film of the chest.In case of rib fracture,besides the posterior anterior radiography,the radiography of the posterior anterior oblique position is need.Also re examination of the chest is required 1~4 weeks after of the chest injury.
