ABSTRACT

Resumo Objetivo Avaliar o nível de incapacidade funcional e identificar os fatores associados em pessoas após Acidente Vascular Cerebral Isquêmico. Métodos Coorte prospectiva, realizada em hospital de referência em neurologia, com 224 pessoas com acidente vascular cerebral isquêmico. A coleta de dados ocorreu entre março a outubro de 2019. Os participantes foram acompanhados durante a internação, quando as variáveis sociodemográficas e clínicas foram levantadas empregando-se instrumentos específicos e contatados após 90 dias, por ligação telefônica, para aplicação do Índice de Barthel modificado. Na análise, aplicou-se estatística descritiva e o teste Qui-quadrado de Pearson. Adotou-se significância estatística de 5%. Resultados A maioria apresentou algum grau de incapacidade funcional (58,5%), sendo que 29,5% apresentaram dependência moderada e 29,0% severa ou grave. As variáveis sexo feminino (p=0,011), tempo de chegada ao hospital de referência maior ou igual a 4,5h (p=0,017), Acidente vascular cerebral prévio (p=0,031), não ter realizado trombólise (p=0,023), ter hipertensão arterial (p=0,032) e maior gravidade estimada pela National Institute of Health Stroke Scale (p=0,000) foram associadas a maior nível de incapacidade. Conclusão Predominou a dependência de moderada a grave. A gravidade do evento, evento prévio, hipertensão, não submissão à trombólise, retardo à chegada ao hospital e sexo feminino foram associados a maior nível de incapacidade funcional.


Resumen Objetivo Evaluar el nivel de incapacidad funcional e identificar los factores asociados en las personas después de un accidente cerebrovascular isquémico. Métodos Cohorte prospectiva, realizada en un hospital de referencia en neurología, con 224 personas con accidente cerebrovascular isquémico. La recopilación de datos se llevó a cabo entre marzo y octubre de 2019. Se acompañó a los participantes durante la internación, momento en que se recopilaron las variables sociodemográficas y clínicas mediante la utilización de instrumentos específicos, y se los contactó 90 días después, por teléfono, para aplicar el Índice de Barthel modificado. En el análisis se aplicó estadística descriptiva y la prueba χ2 de Pearson. Se adoptó significación estadística de 5 %. Resultados La mayoría presentó algún nivel de incapacidad funcional (58,5 %), de los cuales el 29,5 % presentó dependencia moderada y el 29,0 % dependencia severa o grave. Las siguientes variables fueron asociadas a un mayor nivel de incapacidad: sexo femenino (p=0,011), tiempo de llegada al hospital de referencia mayor o igual a 4,5 h (p=0,017), accidente cerebrovascular previo (p=0,031), no haber realizado trombólisis (p=0,023), tener hipertensión arterial (p=0,032) y mayor gravedad estimada por la National Institute of Health Stroke Scale (p=0,000). Conclusión Predominó la dependencia de moderada a grave. La gravedad del evento, evento previo, hipertensión, no realización de trombólisis, retraso de la llegada al hospital y sexo femenino fueron las variables asociadas a un mayor nivel de incapacidad funcional.


Abstract Objective To assess the functional disability level and identify associated factors in people after Ischemic Cerebral Vascular Accident. Methods A prospective cohort, carried out in a reference hospital in neurology with 224 people with ischemic stroke. Data collection took place between March and October 2019. Participants were followed up during hospitalization, when sociodemographic and clinical variables were collected using specific instruments and contacted after 90 days, by telephone call, to apply the Modified Barthel Index. In the analysis, descriptive statistics and Pearson's chi-square test were applied. Statistical significance of 5% was adopted. Results Most had some degree of functional disability (58.5%), with 29.5% having moderate dependence and 29.0% having severe dependence. The variables being female (p=0.011), time of arrival at the reference hospital greater than or equal to 4.5 hours (p=0.017), previous stroke (p=0.031), not having undergone thrombolysis (p=0.023), having high blood pressure (p=0.032) and greater severity estimated by the National Institute of Health Stroke Scale (p=0.000) were associated with a higher disability level. Conclusion Moderate to severe dependence predominated. The severity of the event, previous event, hypertension, non-submission to thrombolysis, delay in arriving at the hospital and female gender were associated with a higher functional disability level.

RESUMO Objetivo Analisar a associação da independência funcional com aspectos clínicos de comprometimento neurológico, a localização e extensão do dano neuronal e os fatores sociodemográficos em pacientes na fase aguda do AVC. Método Estudo analítico de recorte transversal, realizado com 90 pacientes adultos e idosos acometidos por AVC isquêmico, que tiveram admissão no ambiente hospitalar nas primeiras 24 horas após o evento vascular. A coleta dos dados referentes aos aspectos clínicos e fatores sociodemográficos foi realizada pelo prontuário eletrônico e/ou entrevista para descrever o perfil dos pacientes, Oxfordshire Community Stroke Project, Alberta Stroke Programme Early CT Score, National Institute of Health Stroke Scale e a Medida de Independência Funcional. Resultados O comprometimento neurológico, de acordo com a National Institute of Health Stroke Scale, foi associado à funcionalidade nas primeiras 24 horas após o AVC. Além disso, a presença de hipertensão arterial, idade, trabalho inativo, tabagismo e extensão do dano neuronal estiveram associados à dependência funcional, mas não permaneceram no modelo final deste estudo. Conclusão A dependência funcional está associada à hipertensão arterial, idade, trabalho inativo, tabagismo, extensão do dano neuronal e grau de comprometimento neurológico nas primeiras 24 horas após o evento vascular. Além disso, um nível mais elevado de comprometimento neurológico foi independentemente associado a níveis aumentados de dependência funcional.


ABSTRACT Purpose To analyze the association of functional independence with clinical aspects of neurological impairment, the location and extent of neuronal damage and sociodemographic factors in patients in the acute phase of stroke. Methods Analytical cross-sectional study in 90 adult and older patients affected by ischemic stroke, admitted to the hospital within 24 hours of the vascular event. Sociodemographic factors and clinical aspects data were collected from electronic medical records and/or interviews in order to depict the patients'profile, Oxfordshire Community Stroke Project, Alberta Stroke Programme Early CT Score, National Institute of Health Stroke Scale, and Functional Independence Measure. Results Neurological impairment, according to the National Institute of Health Stroke Scale, was associated with functioning in the first 24 hours after the stroke. Furthermore, the presence of arterial hypertension, age, inactive work, smoking and extent of neuronal damage were associated with functional dependence, but did not remain in the final model of this study. Conclusion Functional dependence is associated with arterial hypertension, age, inactive work, smoking, extent of neuronal damage, and degree of neurological impairment in the first 24 hours after the vascular event. Furthermore, a higher level of neurological impairment was independently associated with increased levels of functional dependence.

ABSTRACT

Os sintomas persistentes da síndrome pós-COVID-19 acarretam impactos negativos na saúde, qualidade de vida e produtividade. O objetivo deste trabalho foi descrever os sintomas persistentes da síndrome pós-COVID-19, principalmente neurológicos, e as repercussões cognitivas, emocionais, motoras, de qualidade de vida e de custos indiretos, 12 meses após a infecção. Pacientes com primeiros sintomas entre janeiro e junho de 2021 evoluíram com síndrome pós-COVID-19 e procuraram atendimento na unidade da Rede SARAH de Hospitais de Reabilitação em Fortaleza, Ceará, Brasil. As informações foram obtidas no início do acompanhamento e por entrevista telefônica após 12 meses da infecção. Participaram do estudo 58 pessoas, com idade média de 52,8 anos (±10,5) e 60% permaneceram em UTI. Os sintomas mais frequentes na admissão foram: fadiga (64%), artralgia (51%) e dispneia (47%); e após 12 meses: fadiga (46%) e alteração de memória (39%). Foram identificadas alterações nas escalas/testes funcionais: PCFS, MoCA, HAD, FSS, SF-36, TLS5x, Timed up and go, caminhada de 6 minutos e preensão manual. Os custos indiretos totais foram de USD 227.821,00, com 11.653 dias de absenteísmo; 32% dos pacientes não voltaram a trabalhar. Melhores resultados de TLS5x e SF-36, nas dimensões capacidade funcional, aspecto físico, vitalidade e dor, demonstraram associação com retorno laboral (p ≤ 0,05). Os sintomas persistentes mais frequentes foram: fadiga, artralgia, dispneia, ansiedade e depressão, com repercussões negativas na funcionalidade cognitiva, emocional, motora e qualidade de vida. Esses sintomas perduraram por mais de um ano, principalmente fadiga e alteração de memória, sendo esta última mais relatada tardiamente. Houve dificuldade importante de retorno laboral e custos indiretos de USD 4.847,25 por pessoa/ano.

El objetivo fue describir los síntomas persistentes del síndrome post-COVID-19, sobre todo neurológicos, y las repercusiones cognitivas, emocionales, motoras, de calidad de vida y los costos indirectos, 12 meses tras la infección. Pacientes que tuvieron los primeros síntomas entre enero y junio de 2021, desarrollaron síndrome post-COVID-19 y buscaron atención en la unidad de la Red SARAH de Hospitales de Rehabilitación en Fortaleza, Ceará, Brasil. Las informaciones se obtuvieron al inicio del seguimiento y por entrevista telefónica 12 meses tras la infección. Participaron 58 personas, con edad de 52,8±10,5 años y el 60% permaneció en la UTI. Los síntomas más frecuentes al ingreso fueron: fatiga (64%), artralgia (51%) y disnea (47%); tras 12 meses fueron: fatiga (46%) y alteración de memoria (39%). Se identificaron alteraciones en las escalas/testes funcionales: PCFS, MoCA, HAD, FSS, SF-36, TLS5x, timed up and go, caminar por 6 minutos y prensión manual. Los costos indirectos totales fueron USD 227.821,00, con 11.653 días de absentismo. El 32% de los pacientes no volvió a trabajar. Mejor TLS5x y SF-36 en las dimensiones capacidad funcional, aspecto físico, vitalidad y dolor demostraron una asociación con el retorno al trabajo (p ≤ 0,05). Los síntomas persistentes más frecuentes fueron fatiga, artralgia, disnea, ansiedad y depresión, con repercusiones negativas en la funcionalidad cognitiva, emocional, motora y calidad de vida. Estos síntomas continuaron por más de un año, sobre todo la fatiga y la alteración de la memoria, siendo esta última reportada con más frecuencia tardíamente. Hubo una dificultad importante en el retorno al trabajo y costos indirectos de USD 4.847,25 persona/año.


The persistent symptoms of post-COVID-19 syndrome negatively impact health, quality of life, and productivity. This study aimed to describe the persistent symptoms of post-COVID-19 syndrome (especially neurological ones) and their 12-month post-infection cognitive, emotional, motor, quality of life, and indirect cost repercussions. Patients showing the first symptoms of COVID-19 from January to June 2021 who developed post-COVID-19 syndrome and sought care at the Fortaleza Unit (Ceará, Brazil) of the SARAH Network of Rehabilitation Hospitals were included in this study. Information was obtained at the baseline follow-up and by telephone interview 12 months post-infection. In total, 58 people participated in this study with an average age of 52.8±10.5 years, of which 60% required an ICU. The most frequent symptoms on admission included fatigue (64%), arthralgia (51%), and dyspnea (47%), whereas, after 12 months, fatigue (46%) and memory impairment (39%). The following scales/functional tests showed alterations: PCFS, MoCA, HAD, FSS, SF-36, TLS5x, timed up and go, 6-minute walk, and handgrip. Indirect costs totaled USD 227,821.00, with 11,653 days of absenteeism. Moreover, 32% of patients were unable to return to work. Better TLS5x and higher SF-36 scores in the functional capacity, physical functioning, vitality, and pain dimensions were associated with return to work (p ≤ 0.05). The most frequent persistent symptoms referred to fatigue, arthralgia, dyspnea, anxiety, and depression, which negatively affected cognitive, emotional, and motor function and quality of life. These symptoms lasted for over a year, especially fatigue and memory alteration, the latter of which being the most reported after COVID-19 infections. Results also show a significant difficulty returning to work and indirect costs of USD 4,847.25 per person/year.

ABSTRACT. Frailty is defined as a recognizable state of increased vulnerability resulting from age-associated decline of function in various physiological systems, such that the ability to deal with acute or everyday stressors is compromised. Objective: The aim of the study was to characterize the sample of older adults with cognitive impairment, according to the frailty status indirectly assessed by family members, other clinical and sociodemographic variables; and to assess the overlap of clinical conditions evaluated in this sample with cognitive impairment. Methods: Data were extracted from the follow-up database of the Frailty in Brazilian Older Adults (FIBRA) study (2016-2017). The sample consisted of 130 elderly people with cognitive impairment assessed by the Mini Mental State Examination (MMSE). The scores for the Clinical Dementia Scale (CDR), Cornell Scale for Depression in Dementia and Functional Activities Questionnaire were described. Frailty was indirectly measured through questions answered by family members about the five criteria that compose the frailty phenotype. Results: The sample consisted mostly of older women (n=91) with a mean age of 82.4 (SD=5.3) years, mean schooling of 3.3 years (SD=3.07), widowed (47.7%) and who lived with children and/or grandchildren (68%). More than half had multimorbidity (74.90%), 39.5% had depression symptoms suggestive of major depression, 57% had impaired functionality, 49.3% were frail, 37.6% pre-frail, and 13.10% robust. Conclusion: Among older adults with cognitive impairment, frailty and functional limitations are common.


RESUMO. A fragilidade é definida como um estado reconhecível de vulnerabilidade aumentada resultante do declínio da função associado à idade em vários sistemas fisiológicos, de modo que a capacidade de lidar com estressores agudos ou cotidianos fica comprometida. Objetivo: Caracterizar uma amostra de pessoas idosas com comprometimento cognitivo, segundo o estado de fragilidade, avaliado de forma indireta por familiares, assim como outras variáveis clínicas e sociodemográficas; e avaliar a sobreposição das condições clínicas avaliadas nesta amostra com o comprometimento cognitivo. Métodos: Os dados foram extraídos do banco de dados de acompanhamento do estudo Fragilidade em Idosos Brasileiros (FIBRA - 2016-2017). A amostra foi composta por 130 idosos com comprometimento cognitivo avaliado pelo Mini-Exame do Estado Mental (MEEM). Foram descritos os escores da Escala Clínica de Demência (CDR), da Escala Cornell de Depressão em Demência e do Questionário de Atividades Funcionais. A fragilidade foi mensurada indiretamente por meio de questões respondidas junto aos familiares sobre os cinco critérios que compõem o fenótipo de fragilidade. Resultados: A amostra foi composta em sua maioria por mulheres idosas (n=91) com idade média de 82,4 (DP=5,3) anos, escolaridade média de 3,3 anos (DP=3,07), viúvas (47,7%) e que viviam com filhos e/ou netos (68%). Mais da metade apresentava multimorbidade (74,90%), 39,5% apresentavam sintomas depressivos sugestivos de depressão maior, 57% tinham funcionalidade prejudicada, 49,3% eram frágeis, 37,6% pré-frágeis e 13,10% robustos. Conclusão: Entre idosos com alterações cognitivas, é comum a co-ocorrência de fragilidade e de limitações funcionais.

Introduction: The functional status of older adults (OA) is usually used as an indicator of their health. Increased dependence raises the need for an informal caregiver (IC), leading to a state of work overload, which was frequently observed during the COVID-19 confinement. Objective: To assess the relationship between physical dependence of OA, job burnout of their IC, and the confinement conditions in the context of the COVID-19 pandemic. Materials and methods: The participants included 77 OA together with their respective IC. The OA filled out a sociodemographic data sheet and a Confinement Conditions Questionnaire. In addition to these two forms, the IC filled out the Zarit Caregiver Burnout Scale and the ABVD Barthel Scale. Results: Statistically significant correlations were found between: physical dependence and job burnout (rho=0.475, p<0.01); physical dependence and confinement degree (rho=0.441, p<0.01); and job burden and confinement degree (rho=0.344, p<0.01). Conclusion: Caregivers develop a greater job burnout as a consequence of the OA's dependence. Furthermore, it seems that this burnout is more related to the confinement conditions experienced by older adults than to the caregiver's own conditions.


Introducción: El estado funcional de las personas adultas mayores (PAM) suele tomarse como indicador de salud; la presencia de dependencia incrementa la necesidad de un cuidador informal (CI) que puede desarrollar sobrecarga, lo cual se agudizó durante el confinamiento por COVID-19. Objetivo: Evaluar la relación de la dependencia física de las PAM, la sobrecarga de su CI y las condiciones de confinamiento en el contexto de la pandemia por COVID-19. Materiales y métodos: Participaron 77 diadas conformadas por una PAM y su CI. Las PAM contestaron una ficha de datos sociodemográficos y Cuestionario de Condiciones de Confinamiento. Los CI, diligenciaron una ficha de datos sociodemográficos, Cuestionario de Condiciones de Confinamiento, Escala de Carga del Cuidador de Zarit y Escala de Barthel de ABVD. Resultados: Se encontró correlaciones estadísticamente significativas entre la dependencia física y la sobrecarga (rho=0,475, p<0,01); la dependencia física y el grado de confinamiento (rho=0,441, p<0,01); y la sobrecarga y el grado de confinamiento (rho=0,344, p<0,01). Conclusión: Los cuidadores de PAM desarrollan mayor sobrecarga frente a su dependencia física; además parece ser que la sobrecarga del cuidador está más relacionada con las condiciones de confinamiento de las personas adultas mayores, que con las propias condiciones de confinamiento del cuidador.


Introdução: O estado funcional do idoso (PAM) é geralmente tomado como um indicador de saúde; A presença de dependência aumenta a necessidade de um cuidador informal (CI) que pode desenvolver sobrecarga, que se agravou durante o confinamento da COVID-19. Objetivo: Avaliar a relação entre a dependência física dos PAM, a sobrecarga do seu CI e as condições de confinamento no contexto da pandemia de COVID-19. Materiais e métodos: Participaram 77 díades compostas por uma PAM e seu CI. O PAM respondeu a uma ficha de dados sociodemográficos e a um Questionário de Condições de Confinamento. Os CI preencheram ficha de dados sociodemográficos, Questionário de Condições de Confinamento, Escala de Sobrecarga do Cuidador de Zarit e Escala Barthel (ABVD). Resultados: Foram encontradas correlações estatisticamente significativas entre dependência física e sobrecarga (rho=0,475, p<0,01); dependência física e grau de confinamento (rho=0,441, p<0,01); e sobrecarga e grau de confinamento (rho=0,344, p<0,01). Conclusão: Os cuidadores do PAM desenvolvem maior sobrecarga diante da dependência física; além disso, parece que a sobrecarga do cuidador está mais relacionada com as condições de confinamento dos idosos do que com as próprias condições de confinamento do cuidador.

INTRODUCCIÓN: La enfermedad cerebral de pequeño vaso es una causa principal de pérdida funcional, discapacidad y deterioro cognitivo. OBJETIVO: Determinar la prevalencia de la enfermedad de pequeño vaso y características clínicas que se asocian a mayor deterioro funcional, cognitivo y afectivo en adultos mayores con enfermedad cerebrovascular atendidos en el Servicio de Neurología del Hospital Carlos Andrade Marín en el período 2020 ­ 2021. METODOLOGÍA: Estudio observacional, analítico transversal con 80 pacientes mayores de 65 años con enfermedad cerebrovascular previamente diagnosticada. Se determinó cuáles presentaban enfermedad cerebral de pequeño vaso. Se compararon los dos grupos el de enfermedad cerebro vascular isquémico con y sin enfermedad cerebral de pequeño vaso. Se midió el grado de deterioro funcional con escala de Barthel; Lawton y Brody. El deterioro cognitivo con test de Montreal Cognitive Assessment ­Basic, estado afectivo con escala de Yesavage. Se utilizó razón de momios y se consideró significativo un valor p <0,05. Se utilizó el programa Statistical Package for Social Sciences versión 25. RESULTADOS: Los hombres representaron el 51,2%. La edad promedio fue 76,2 años. Prevalencia de enfermedad cerebral de pequeño vaso (87,5%). Escala de Fazekas grado 1 (46,3%), Factores asociados con enfermedad cerebral de pequeño vaso: tabaquismo [RR: 7,27; IC 95%: 1,69-31,3); enfermedad renal crónica [RR: 4,0; IC 95%: 1,01-15,7]. Dependencia moderada [RR: 6,42; IC 95%: 1,02-40,3]. Factores asociados con pérdida funcionalidad: gravedad del ictus. Factores asociados con deterioro cognitivo: infarto con doble territorio. Factores asociados con deterioro afectivo: infarto con doble territorio y síndrome metabólico (p<0,05). CONCLUSIÓN: La enfermedad cerebral de pequeño vaso tiene una elevada prevalencia entre los adultos mayores con enfermedad cerebrovascular y representó un deterioro cognitivo, funcional y afectivo considerable, en relación a los pacientes sin esta enfermedad.


INTRODUCTION: Cerebral small vessel disease is a leading cause of functional loss, disability, and cognitive impairment. OBJECTIVE: To determine the prevalence of small vessel disease and clinical characteristics associated with greater functional, cognitive and affective impairment in older adults with cerebrovascular disease attended at the Neurology Service of the Carlos Andrade Marín Hospital in the period 2020 - 2021. METHODOLOGY: Observational, analytical cross-sectional study with 80 patients over 65 years of age with previously diagnosed cerebrovascular disease. It was determined which patients had cerebral small vessel disease. The two groups of ischemic cerebrovascular disease with and without cerebral small vessel disease were compared. The degree of functional impairment was measured with the Barthel, Lawton and Brody scales. Cognitive impairment was measured with the Montreal Cognitive Assessment-Basic test, and affective state with the Yesavage scale. Odds ratio was used and a p value <0,05 was considered significant. Statistical Package for Social Sciences version 25 was used. RESULTS: Males represented 51,2%. Mean age was 76,2 years. Prevalence of cerebral small vessel disease (87,5%). Fazekas scale grade 1 (46,3%), Factors associated with cerebral small vessel disease: smoking [RR: 7,27; 95% CI: 1,69-31,3); chronic kidney disease [RR: 4,0; 95% CI: 1,01-15,7]. Moderate dependence [RR: 6,42; 95% CI: 1,02-40,3]. Factors associated with loss of function: severity of stroke. Factors associated with cognitive impairment: infarction with double territory. Factors associated with affective impairment: dual territory infarction and metabolic syndrome (p<0.05). CONCLUSION: Cerebral small vessel disease has a high prevalence among older adults with cerebrovascular disease and represented a considerable cognitive, functional and affective deterioration, in relation to patients without this disease.

ABSTRACT

Objetivo: Verificar a prevalência da intolerância ao exercício e a correlação com o perfil sociodemográfico, clínico, de hospitalização e função física na alta de pacientes hospitalizados pela COVID-19. Métodos: Estudo transversal com 52 pacientes internados por COVID-19 no Hospital Santa Casa de Misericórdia de Vitória, Espírito Santo. Coletou-se informações no momento da alta quanto ao perfil sociodemográfico, clínico, de hospitalização e função física, através de questionário semiestruturado, prontuário eletrônico, escalas e testes. Utilizou-se o Medical Research Council Modificado (mMRC) para graduar a dispneia, o Medical Research Council (MRC) para avaliar força muscular periférica, a manovacuometria, através das pressões inspiratória e expiratória máximas, para mensurar força muscular respiratória e a Medida de Independência Funcional (MIF) para funcionalidade. A variável desfecho, tolerância ao exercício, foi medida pelo Teste de Sentar e Levantar de 1 minuto. Resultados: Todos participantes apresentaram intolerância ao exercício no momento da alta hospitalar. O desempenho no teste de sentar e levantar foi inversamente correlacionado com o grau de dispneia e diretamente com a força muscular inspiratória, pontuação total da Medida de Independência Funcional, domínios de autocuidado, locomoção, mobilidade (p 0,019), e controle de esfíncter Conclusão: A prevalência da intolerância ao exercício na alta por COVID-19 foi de 100%, e estava correlacionada com o grau de dispneia, força muscular inspiratória e funcionalidade para autocuidado, locomoção, mobilidade e controle de esfíncter. Destaca-se a avaliação da tolerância ao exercício como importante preditor de sequelas pós-COVID-19, capaz de avaliar a interação entre diversos sistemas orgânicos.


Objective: To verify the prevalence of exercise intolerance and the correlation with the sociodemographic, clinical, hospitalization and physical function profile at discharge of patients hospitalized for COVID-19. Methods: Cross-sectional study with 52 patients hospitalized for COVID-19 at Hospital Santa Casa de Misericórdia in Vitória, Espírito Santo. Information was collected at discharge regarding the sociodemographic, clinical, hospitalization and physical function profile, through a semi-structured questionnaire, electronic medical record, scales and tests. The Modified Medical Research Council (mMRC) was used to grade dyspnea, the Medical Research Council (MRC) was used to assess peripheral muscle strength, manovacuometry, through maximal inspiratory and expiratory pressures, to measure respiratory muscle strength and the Independence Measure Functional (MIF) for functionality. The outcome variable, exercise tolerance, was measured by the 1-minute Sit and Stand Test. Results: All participants presented exercise intolerance at discharge. Performance in the 1-minute Sit and Stand Test was inversely correlated with the degree of dyspnea and directly with inspiratory muscle strength, total score on the Independence Measure Functional, self-care domains, locomotion, mobility and sphincter control. Conclusion: The prevalence of exercise intolerance at discharge due to COVID-19 was 100%, and was correlated with the degree of dyspnea, inspiratory muscle strength and functionality for self-care, locomotion, mobility and sphincter control. The assessment of exercise tolerance stands out as an important predictor of sequelae after COVID-19, capable of assessing the interaction between different

Objetivo: Analisar a evolução da independência funcional de pacientes com Síndrome de Guillain-Barré (SGB) internados para reabilitação. Método: Estudo retrospectivo e longitudinal. Foram analisados prontuários de pacientes com diagnóstico de SGB internados para reabilitação, de janeiro de 2015 a março de 2020, que possuíam a Medida de Independência Funcional (MIF) na admissão e alta hospitalar devidamente preenchida, sem distinção de idade. A comparação da MIF antes e depois da internação para reabilitação foi feita por meio do teste t de Student e McNemar. Resultados: A amostra foi composta por 26 pacientes, com média de idade de 41,96 ± 19,67 anos. Os participantes tinham em média 66,07 ± 69,56 dias entre os primeiros sintomas e a admissão para internação para reabilitação. O tempo médio de internação foi de 38,96± 28,36 dias. Houve diferença significativa entre as médias das pontuações, na admissão e alta, nos domínios motor (37,58 - 59,62; p<0,001) e cognitivo (23,19 - 33,35; p<0,001) e escore total da MIF (60,77 - 92,96; p<0,001). Observou-se também aumento do número de pacientes com independência completa após internação para reabilitação (2 - 15; p<0,001). Conclusão: Pacientes com SGB internados para reabilitação apresentam melhora da independência funcional. É importante acesso a esta modalidade terapêutica multiprofissional aos pacientes com SGB.


Objective: To analyze the evolution of the functional independence of patients with Guillain-Barré Syndrome (GBS) hospitalized for rehabilitation. Method: Retrospective and longitudinal study. Medical records of patients with diagnosis of GBS hospitalized for rehabilitation were analyzed, from January 2015 to March 2020, who had the Functional Independence Measure (FIM) at admission and hospital discharge duly filled, regardless of age. The comparison of FIM before and after hospitalization for rehabilitation was performed using Student's t-test and McNemar's test. Results: The sample consisted of 26 patients, with a mean age of 41.96 ± 19.67 years. Participants had a mean of 66.07 ± 69.56 days between the first symptoms and hospital admission for rehabilitation. The mean length of stay was 38.96 ± 28.36 days. There was a significant difference between the mean scores, at admission and discharge, in the motor domain (37.58-59.62; p<0.001) and cognitive domain (23.19-33.35; p<0.001) and total FIM score (60.77-92.96; p<0.001). There was also an increase in the number of patients with complete independence after hospitalization for rehabilitation (2-15; p<0.001). Conclusion: Patients with GBS hospitalized for rehabilitation show improved functional independence. Access to this multiprofessional therapeutic modality is important for patients with GBS.

Objetivo: Estabelecer a capacidade física e alterações do controle postural (COP) durante o apoio unipodal estático e dinâmico em mulheres com Síndrome de Dor no Grande Trocânter (SDGT). Métodos: Foram avaliadas 36 mulheres, sedentárias, maiores de 45 anos. Destas, 18 tinham diagnóstico de SDGT (Grupo Dor - GD) e 18 sem queixas álgicas (Grupo Controle - GC). Todas responderam ao questionário Victorian Institute of Sports Assessment for Gluteal Tendinopathy (VISA-G) para análise da capacidade física relacionada a dor lateral do quadril, e foram submetidas à avaliação do controle postural na plataforma de força, em apoio unipodal estático e dinâmico (mini agachamentos). Os dados foram comparados e correlacionados, com significância estatística estabelecida 5%. Resultados: As participantes do GD apresentaram alto índice de dor (7), por 10 meses e baixa capacidade funcional (54,44 pontos no VISA-G). Na análise do controle postural estático, GD mostrou piores resultados para a área de oscilação do COP (p= 0,04) e maior amplitude de oscilação médio-lateral (p= 0,03). Na avaliação dinâmica, os resultados da amplitude médio-lateral (p= 0,02) e velocidade antero posterior (p= 0,04) foram maiores no GD, mas o COP foi pior no GC (p= 0,01). Conclusão: Mulheres com SDGT tem baixa capacidade funcional e pior controle postural estático e dinâmico. Estas variáveis devem ser avaliadas para estabelecer novas estratégias de prevenção e reabilitação em mulheres com SDGT


Objective: To establish physical capacity and changes in postural control (PCO) during static and dynamic single-legged support in women with Greater Trochanter Pain Syndrome (GTPS). Methods: A total of 36 sedentary women over 45 years of age were evaluated. Of these, 18 had a diagnosis of GTPS (Pain Group - DG), and 18 had no pain complaints (Control Group - CG). All participants answered the Victorian Institute of Sports Assessment for Gluteal Tendinopathy (VISA-G) questionnaire to analyze their physical capacity regarding lateral hip pain. Therefore, were submitted to the evaluation of postural control on the force platform in static and dynamic single-legged support (mini squats). The data were compared and correlated, with statistical significance established at 5%. Results: The participants in the DG presented a high pain index (7) for 10 months and low functional capacity (54.44 points in the VISA-G). In the analysis of static postural control, DG showed worse results for the Center of Pressure (COP) oscillation area (p= 0.04) and greater amplitude of mediolateral oscillation (p= 0.03). In the dynamic evaluation, the results of the mediolateral amplitude (p= 0.02) and anteroposterior velocity (p= 0.04) were higher in the DG, but the COP was worse in the CG (p= 0.01). Conclusion: Women with GTPS have lower functional capacity and worse static and dynamic postural control. These variables should be evaluated to establish new prevention and rehabilitation strategies for women with GTPS

BACKGROUND: Physical fitness assessment of older adults is essential because it is a key component of functional independence and healthy aging. AIM: To establish physical fitness reference values for physically active older Chilean adults of both sexes and identify the variables associated with the deterioration of their physical condition. MATERIAL AND METHODS: Cross-sectional study that included 342 older adults aged 60 and over. Their physical fitness was assessed with the Senior Fitness Test (SFT). The timed up and go (TUG), chair stand (CS), arm curl (AC), and aerobic resistance (2 min) tests were evaluated. RESULTS: Performance in the physical fitness tests by age group decreased in all tests as older adults advanced in age. Scores for men were more evenly distributed across the different age groups. The main risk factors for the deterioration of physical fitness were age, sex, and body mass index (BMI) (p-value < 0.05). The primary risk factor for men was age and for women age and BMI. CONCLUSIONS: Performance of both men and women in the different SFT tests decreased as older adults aged. Age, sex, and BMI were the main risk factors for the deterioration of the physical fitness of physically active older adults.

SUMMARY OBJECTIVE: Therapeutic exercises are well documented for the treatment of osteoarthritis; there is less evidence on what the effect of closed kinetic chain exercises is for knee osteoarthritis. The aim of this study was to investigate the effects of open kinetic chain exercises and closed kinetic chain exercises on pain, muscle strength, functional status, and quality of life in patients with knee osteoarthritis. METHODS: The study included a total of 60 patients with primary unilateral knee osteoarthritis grade I and II. The patients were categorized into three groups as open kinetic chain exercises (n=20), closed kinetic chain exercises (n=20), and control group (n=20). The outcome measures, including pain, isokinetic muscle strength, functional status, and quality of life, were collected at baseline and at the end of 6 and 12 weeks. RESULTS: Closed kinetic chain exercises and open kinetic chain exercises had significant improvement in pain, muscle strength, WOMAC, and SF-36 scores after the treatment and at their 6th and 12th week follow-ups compared to their baseline values and compared to the control group (p<0.05). CONCLUSION: The changes in all outcome measures were similar between closed kinetic chain exercises and open kinetic chain exercises (p>0.05). Closed kinetic chain exercises and open kinetic chain exercises were similar for knee osteoarthritis grade I and II. Closed kinetic chain exercises could be safely added to the exercise programs of patients with low-grade knee osteoarthritis.

ABSTRACT Hip replacement (HR) is a surgery that replaces the injured joint with a prosthesis. The postoperative period can reduce functional independence and affect the subjective experience of pain in older adults. This study evaluated the association between the level of functional independence and the subjective experience of pain in older adults who underwent HR. This cross-sectional descriptive quantitative association study used the functional independence measure (FIM) to assess the level of functional independence and the visual analogue scale (VAS) to analyze the subjective experience of pain. Spearman's correlation test was used to evaluate the association between FIM and VAS and simple linear regression to analyze the influence of one variable on another (p≤0.05). This study evaluated 48 older adults, with a mean age of 71.5±9 years, of which 37.5% (n=18) were men and 62.5% (n=30) were women. The mean VAS and FIM values were 3.5 (±1.6) and 107 (±17.82), respectively, and they had a significant and inverse association (p=0.006; r=−0.389; R2=0.142). This study showed an inversely proportional relationship between the subjective experience of pain and the level of functional independence in older patients who underwent HR. Therefore, health professionals should pay more attention to pain in these patients in order to optimize their functional recovery and increase their functional independence.


RESUMEN La artroplastia de cadera (AC) es un procedimiento quirúrgico que reemplaza la articulación lesionada por una prótesis. El postoperatorio puede reducir la independencia funcional y afectar la percepción subjetiva del dolor en el anciano. Este estudio evaluó la asociación entre el nivel de independencia funcional y la percepción subjetiva del dolor en ancianos sometidos a AC. Se trata de un estudio transversal, descriptivo, cuantitativo y de asociación. La medida de independencia funcional (MIF) se utilizó para evaluar el nivel de independencia funcional, y la escala visual analógica (EVA), para estimar la percepción subjetiva del dolor. Se aplicaron la prueba de correlación de Spearman para verificar la asociación entre MIF y EVA, y una regresión lineal simple para analizar la influencia de una variable sobre la otra (p≤0,05). Participaron 48 ancianos, de edad media de 71,5±9 años; de los cuales el 37,5% (n=18) eran del sexo masculino y el 62,5% (n=30) del sexo femenino. La media de la evaluación de EVA fue de 3,5 (±1,6), mientras que la de MIF, de 107 (±17,82). Hubo una asociación significativa e inversa con p=0,006 y con r de −0,389, con R2 de 0,142. Este estudio demostró una relación inversamente proporcional entre la sensación subjetiva de dolor y el nivel de independencia funcional de los ancianos en el postoperatorio de AC. Por lo tanto, es necesario que los profesionales de la salud tengan mayor atención al dolor de esta población, con el objetivo de optimizarle la recuperación funcional y, así, aumentar su independencia funcional.


RESUMO A artroplastia de quadril (AQ) é um procedimento cirúrgico que consiste na substituição da articulação lesionada por uma prótese. O pós-operatório pode reduzir a independência funcional e repercutir na percepção subjetiva da dor em idosos. O estudo avaliou a associação entre o nível de independência funcional e a percepção subjetiva da dor em idosos submetidos à AQ. Trata-se de um estudo transversal, descritivo, quantitativo e de associação. A medida de independência funcional (MIF) foi utilizada para avaliar o nível de independência funcional, e a escala visual analógica (EVA) foi empregada para avaliar a percepção subjetiva da dor. O teste de correlação de Spearman foi utilizado para verificar a associação entre a MIF e a EVA, e uma regressão linear simples, para analisar a influência de uma variável sobre a outra (p≤0,05). Um total de 48 idosos foram avaliados, com média de idade de 71,5±9 anos, em que 37,5% (n=18) pertenciam ao sexo masculino e 62,5% (n=30) ao feminino. A avaliação da EVA mostrou uma média de 3,5 (±1,6), enquanto a MIF, 107 (±17,82). Observamos uma associação significante e inversa com p=0,006 e com r de −0,389, com R2 de 0,142. SUMMARY OBJECTIVE: The purpose of this study was to assess exercise capacity, lung and physical function in COVID-19 survivors, and the association of lesion-level characteristics assessed by chest computed tomography, probable sarcopenia, and percentage of diffusing capacity of the lung for carbon monoxide with clinical and functional variables. METHODS: This study was conducted in Salvador, Bahia, Brazil. All patients had a laboratory-confirmed SARS-CoV-2 infection. The sociodemographic characteristics, COVID-19 exposure history, pulmonary function, computed tomography, and functionality of the participants between 1 and 3 months of diagnosis of the disease were collected. RESULTS: A total of 135 patients after COVID-19 recovery were included in this study. Probable sarcopenia, reduction in percentage of diffusing capacity of the lung for carbon monoxide, and a lower 6-min walk distance were observed after COVID-19 infection. Computed tomography>50% was associated with a longer length of stay and a lower percentage of diffusing capacity of the lung for carbon monoxide. Probable sarcopenia diagnosis was associated with a worse percentage of the predicted 6-min walk distance in relation to the predicted, absolute 6-min walk distance (m), percentage of diffusing capacity of the lung for carbon monoxide, and percentage of total lung capacity. CONCLUSION: Muscle disability and lung dysfunction are common in COVID-19 survivors. Hospitalization was associated with the worst muscle force and diffusing capacity of the lung for carbon monoxide. Computed tomography characteristics could be a marker of prolonged hospital stay after the acute phase of COVID-19. Additionally, the probable diagnosis of sarcopenia could be a marker of impact on walking distance. These results highlight the need for long-term follow-up of those patients and rehabilitation programs.

Objetivo: identificar o perfil de admissões e o impacto nos desfechos de crianças internadas em uma terapia intensiva pediátrica e comparar os escores de gravidade, funcionalidade e de desconforto respiratório avaliados respectivamente pelas escalas (PIM II, FSS e BSA). Métodos: Estudo de caráter descritivo, retrospectivo, quantitativo de amostragem censitária. Ferramentas de avaliação: Pediatric Index of Mortality - PIM II, Functional Status Scoret -FSS, Boletim de Silverman-Andersen -BSA e avaliação de prontuários médicos e assistenciais. Resultados: 257 crianças menores de 12 anos foram incluídas no estudo durante todo o ano de 2019. A maioria 56% (n 143) eram menores de um ano e masculino 64 % (n 164) por causas respiratórias 60,05 % (n 155). A mortalidade foi de 9,8% (n 25), e a quantidade média de dias de ventilação mecânica foi de 4,57 dias ± 1,31. A idade não influenciou na quantidade de dias de ventilação mecânica (p<0.05), e o BSA avaliado isoladamente, não se associou a necessidade imediata de intubação (p<0.05), os pacientes do desfecho óbito ficaram em média 8,88 e ±13,04 dias internados, e no desfecho alta 4,73 ±6,63 dias. O PIM II pode ser utilizado para o risco de óbito (p <0,05) e valores maiores ou iguais a 21,58 % foram associados a óbitos e menores ou iguais a 6,65 % foram associados à alta. A FSS dos 257 pacientes na admissão foram: normal (147), disfunção leve (37), moderada (47) e grave (26); na alta hospitalar a FSS foi: normal (178), leve (21), moderada (25) e grave (8) mostrando que grau de funcionalidade normal e leve na admissão esta significativamente associado com a alta hospitalar (p< 0,001). Conclusão: O escore de gravidade PIM II foram compatíveis com os desfechos óbito ou alta, as variações no escore BSA para necessidade de ventilação mecânica não estão associados com a idade e com grau do escore. A funcionalidade mais adequada na admissão está associada ao desfecho alta, e os pacientes em sua maioria saem funcionais.

Results: 257 children under the age of 12 years were included in the study throughout 2019. The majority 56% (n 143) were under one year and male 64 % (n 164) from respiratory causes 60.05 % (n 155). Mortality was 9.8% (n 25), and the average amount of days on mechanical ventilation was 4.57 days ± 1.31. Age had no influence on the number of days of mechanical ventilation (p<0.05), and the BSA assessed alone was not associated with the immediate need for intubation (p<0.05), the patients in the outcome death were hospitalized for an average of 8.88 ±13.04 days, and in the discharge outcome 4.73 ±6.63 days. The PIM II can be used for the risk of death (p <0.05) and values greater than or equal to 21.58 % were associated with death and less than or equal to 6.65 % were associated with discharge. The FSS of the 257 patients at admission were: normal (147), mild (37), moderate (47) and severe (26) dysfunction; at hospital discharge the FSS was: normal (178), mild (21), moderate (25) and severe (8) showing that the degree of normal and mild functionality at admission was significantly associated with hospital discharge (p < 0.001). Conclusion: The PIM II severity scores were compatible with the outcomes death or discharge, the variations in BSA score for mechanical ventilation need were not associated with age and score level. The most adequate functionality at admission is associated with the outcome discharge, and most patients leave the hospital functional.

Métodos: Se trató de un estudio descriptivo, retros- pectivo y cuantitativo con muestreo censal. Instrumentos de evaluación: Índice de Mor- talidad Pediátrica - PIM II, Functional Status Scoret -FSS, Boletín de Silverman-Ander- sen -BSA y evaluación de historias clínicas y asistenciales. Resultados: 257 niños meno- res de 12 años fueron incluidos en el estudio a lo largo de 2019. La mayoría 56% (n 143) eran menores de un año y varones 64% (n 164) de causas respiratorias 60,05% (n 155). La mortalidad fue del 9,8% (n 25) y la media de días con ventilación mecánica fue de 4,57 días ± 1,31. La edad no influyó en el número de días de ventilación mecánica (p<0,05), y el BSA evaluado por sí solo no se asoció con la necesidad inmediata de intu- bación (p<0,05), los pacientes en el resultado muerte estuvieron hospitalizados una media de 8,88 ±13,04 días, y en el resultado alta 4,73 ±6,63 días. El PIM II se puede utilizar para el riesgo de muerte (p <0,05) y los valores mayores o iguales a 21,58 % se asociaron con la muerte y menores o iguales a 6,65 % se asociaron con el alta. La SFS de los 257 pacientes al ingreso fue: normal (147), disfunción leve (37), moderada (47) y grave (26); al alta hospitalaria la SFS fue: normal (178), leve (21), moderada (25) y grave (8) mos- trando que el grado de funcionalidad normal y leve al ingreso se asoció significativamente con el alta hospitalaria (p < 0,001). Conclusiones: Las puntuaciones de gravedad del PIM II fueron compatibles con los resultados muerte o alta hospitalaria, las variaciones en la puntuación del BSA para la necesidad de ventilación mecánica no se asociaron con la edad y el nivel de puntuación. La funcionalidad más adecuada al ingreso se asocia con el resultado alta, y la mayoría de los pacientes salen del hospital funcionales.

ABSTRACT BACKGROUND: Stroke is a major cause of death and functional disability worldwide. Knowledge of the associated factors is essential for defining education, management, and healthcare strategies. OBJECTIVE: To analyze the association between arrival time at a neurology referral hospital (ATRH) and functional disability in patients with ischemic stroke 90 days after the event. DESIGN AND SETTING: Prospective cohort study conducted at a public institution of higher education in Brazil. METHODS: This study included 241 people aged ≥ 18 years who presented ischemic stroke. The exclusion criteria were death, inability to communicate without companions who could answer the research questions, and > 10 days since ictus. Disability was assessed using the Rankin score (mR). Variables for which associations showed a P value ≤ 0.20 in bivariate analysis were tested as modifiers between ATRH and disability. Significant interaction terms were used for multivariate analysis. Multivariate logistic regression analysis was performed with all variables, arriving at the complete model and adjusted beta measures. The confounding variables were included in the robust logistic regression model, and Akaike's Information Criterion was adopted to choose the final model. The Poisson model assumes a statistical significance of 5% and risk correction. RESULTS: Most participants (56.0%) arrived at the hospital within 4.5 hours of symptom onset, and 51.7% presented with mRs of 3 to 5 after 90 days of ictus. In the multivariate model, ATRH ≥ 4.5 hours and females were associated with more significant disability. CONCLUSIONS: Arrival at the referral hospital 4.5 hours after the onset of symptoms or wake-up stroke was an independent predictor of a high degree of functional disability.

As quedas em idosos representam um importante problema de saúde pública devido à associação com a morbimortalidade. Objetivos: Descrever o perfil epidemiológico de idosos com fratura de fêmur proximal, bem como associar o tempo de espera para a cirurgia e os desfechos clínicos com as variáveis físico-funcionais. Métodos: Estudo transversal analítico, de idosos com fratura de fêmur proximal de origem traumática. Os aspectos físico-funcionais foram avaliados pelo Índice de Barthel, Escala de Lawton, Medical Research Council e Dinamometria de Força de Preensão Palmar em dois momentos distintos, admissão e alta hospitalar. Foi realizado análise estatística descritiva e inferencial adotando-se p<0,05. Resultados: A amostra foi composta por 64 indivíduos, sendo 48 (75%) do sexo feminino, com média de idade de 77,8 anos (±8,73). Os pacientes com maior dependência funcional no Índice de Barthel na admissão (U= 282,000; p<0,05) e na alta hospitalar (U= 248,000; p<0,05) aguardaram mais tempo para o procedimento cirúrgico. O principal desfecho foi a alta, de 55 pacientes (85,9%), no entanto aqueles que evoluíram a óbito apresentaram piores pontuações na Escala de Lawton (t(62)= -2,060; p<0,05) e no Índice de Barthel (U= 145,500; p<0,05) no momento da admissão. Conclusão: O perfil de idosos com fratura de fêmur proximal são mulheres, na transição para a oitava década de vida, vítimas de queda da própria altura. Idosos com maior dependência funcional aguardaram mais tempo para a cirurgia e apresentaram piores desfechos


Falls in the elderly represent an important public health problem due to the association with morbidity and mortality. Objectives: To describe the epidemiological profile of elderly people with proximal femur fractures, as well as to associate the waiting time for surgery and clinical outcomes with physical-functional variables. Methods: Analytical cross-sectional study of elderly patients with fractures of the proximal femur of traumatic origin. Sociodemographic, trauma and clinical information were obtained via electronic medical records. The physical-functional aspects were assessed using the Barthel Index, Lawton Scale, Medical Research Council and Hand Grip Strength at two different times, admission and hospital discharge. Descriptive and inferential statistical analysis was performed, adopting p<0.05. Results: The sample consisted of 64 individuals, 48 (75.0%) of which were female, with a mean age of 77.8 years (±8.73). The median waiting time for surgery was 12 (8-15) days. Patients with greater functional dependence on the Barthel Index at admission (U= 282.000; p<0.05) and at hospital discharge (U= 248.000; p<0.05) waited longer for the surgical procedure. The main outcome was discharge 55 (85.9%), however, patients who evolved to death had worse scores on the Lawton Scale (t(62)= -2.060; p<0.05) and on the Barthel Index (U = 145.500; p<0.05) at admission. Conclusion: The profile of elderly people with proximal femur fractures are women, in the transition to the eighth decade of life, victims of a fall from their own height. Elderly people with greater functional dependence waited longer for surgery and had worse outcomes

@#Introduction: Detection of anti-cyclic citrullinated peptide (anti-CCP) antibodies in patients with rheumatoid arthritis (RA) is associated with higher disease activity and lower functional ability. This study investigated the presence of the new generation of anti-CCP antibodies (anti-CCP2 IgG, anti-CCP2 IgA and anti-CCP3.1 IgG/IgA) and their association with disease severity and functional status of RA patients. Methods: A total of 46 RA patients and 40 healthy controls participated in this cross-sectional study that was conducted at the Rheumatology Clinic, Hospital Universiti Sains Malaysia. Blood samples were taken from all participants for anti-CCP2 IgG, anti-CCP2 IgA, and anti-CCP3.1 IgG/IgA analysis. Disease severity and functional status of RA patients were measured using the Disease Activity Score-28 (DAS28) and the modified Health Assessment Questionnaire (mHAQ) respectively. Results: Significantly higher proportion of RA patients were found with positive anti-CCP2 IgG (63.0%), anti-CCP2 IgA (37.0%), and anti-CCP3.1 IgG/IgA antibodies (63.0%) than the healthy controls. No significant association was found between anti-CCP antibodies status and mean DAS28 score of the RA patients. However, RA patients with negative anti-CCP2 IgG status had higher mean mHAQ score than patients with positive anti-CCP2 IgG status. Conclusion: Our study has demonstrated detection of the new generation anti-CCP antibodies in RA patients, supporting the use of autoantibodies in RA diagnosis. While no significant association was found between the presence of anti-CCP antibodies and disease severity of RA patients, the absence of anti-CCP2 IgG was associated with worse function and greater disability of the patients.

Abstract Objective: The authors investigated the functional status at ICU admission and at hospital discharge, and the impact of dysfunctions on survivors' lifespan. Method: Single-center retrospective cohort. The FSS (Functional Status Scale) was calculated at ICU admission and at hospital discharge. A new morbidity was defined as an increase in FSS ≥ 3. Results: Among 1002 patients, there were 855 survivors. Of these, 194 (22.6%) had died by the end of the study; 45 (5.3%) had a new morbidity. Means in the motor domain at admission and discharge were 1.37 (SD: 0.82) and 1.53 (SD 0.95, p = 0.002). In the feeding domain, the means were 1.19 (SD 0.63) and 1.30 (SD 0.76), p = 0.002; global means were 6.93 (SD 2.45) and 7.2 (SD 2.94), p = 0.007. Acute respiratory failure requiring mechanical ventilation, the score PRISM IV, age < 5 years, and central nervous system tumors were independent predictors of new morbidity. New morbidity correlated with lower odds of survival after hospital discharge, considering all causes of death (p = 0.014), and was independently predictive of death (Cox hazard ratio = 1.98). In Weibull models, shortening in the life span of 14.2% (p = 0.014) was estimated as a new morbidity. Conclusions: New morbidities are related to age, disease severity at admission, and SNC tumors. New morbidities, in turn, correlate with lower probabilities of survival and shortening of the remaining life span. Physical rehabilitation interventions in this population of children may have the potential to provide an increase in lifespan.

SUMMARY OBJECTIVE: In-hospital cardiac arrest is a critical medical emergency. Knowledge of prognostic factors could assist in cardiopulmonary resuscitation decision-making. Frailty and functional status are emerging risk factors and may play a role in prognostication. The objective was to evaluate the association between reduced mobility and in-hospital cardiac arrest outcomes. METHODS: This retrospective cohort study included patients over 18 years of age with in-hospital cardiac arrest in Botucatu, Brazil, from April 2018 to December 2021. Exclusion criteria were patients with a do-not-resuscitate order or patients with recurrent in-hospital cardiac arrest. Reduced mobility was defined as the need for a bed bath 48 h before in-hospital cardiac arrest. The outcomes of no return of spontaneous circulation and in-hospital mortality were evaluated. RESULTS: A total of 387 patients were included in the analysis. The mean age was 65.4±14.8 years; 53.7% were males and 75.4% had reduced mobility. Among the evaluated outcomes, the no return of spontaneous circulation rate was 57.1%, and in-hospital mortality was 94.3%. In multivariate analysis, reduced mobility was associated with no return of spontaneous circulation when adjusted by age, gender, initial shockable rhythm, duration of cardiopulmonary resuscitation, and epinephrine administration. However, in multiple logistic regression, there was no association between reduced mobility and in-hospital mortality. CONCLUSION: In patients with in-hospital cardiac arrest, reduced mobility is associated with no return of spontaneous circulation. However, there is no relation to in-hospital mortality.

Resumo Objetivo Analisar o perfil do estado funcional e os fatores associados em pessoas idosas residentes na Ilha de Fernando de Noronha/Pernambuco, Brasil. Método Tratou-se de um estudo seccional, de caráter analítico, e a coleta de dados foi conduzida entre agosto e outubro de 2019. A população do estudo compreendeu 176 pessoas idosas residentes na ilha. O protocolo de coleta foi aplicado por meio de entrevista para resposta ao questionário de variáveis demográficas, sociais, clínicas e ambientais, e aos instrumentos de avaliação. O estado funcional foi determinado a partir da Análise de Classes Latentes, avaliada pelos protocolos dos domínios: cognição, humor, atividades instrumentais de vida diária, mobilidade e comunicação. Resultados O modelo da análise escolhido possuiu duas classes, nomeadas como: "Boa autonomia e independência" e "Moderada autonomia e dependência parcial". A maioria das pessoas idosas apresentou bom estado funcional e os fatores associados foram: pertencer ao sexo masculino, ser separado ou divorciado, viver sozinho, visitar os amigos ou parentes, ter trabalhado mais de 36 anos, ser alfabetizado, não ter deficiência visual ou auditiva, fazer uso de uma ou duas medicações e ter história de queda. Conclusão O estado funcional das pessoas idosas insulares se apresentou como um fenômeno multifatorial, pois fatores demográficos, sociais e clínicos estiveram associados de forma independente à boa funcionalidade. É importante que a manutenção do estado funcional seja incentivada na implementação de políticas públicas voltadas às especificidades das populações residentes em ilhas.


Abstract Objective To analyze the functional status profile and associated factors in older persons living on Fernando de Noronha Island/Pernambuco, Brazil. Method This was a cross-sectional, analytical study, and data collection was conducted between August and October 2019. The study population comprised 176 older persons living on the island. The collection protocol was applied through an interview to respond to the behavior of demographic, social, clinical and environmental variables, and to the assessment instruments. The functional status was determined from the Analysis of Latent Classes, evaluated by the protocols of the domains: cognition, mood, instrumental activities of daily living, mobility and communication. Results The chosen analysis model had two classes, named as: "Good autonomy and independence" and "Moderate autonomy and partial dependence". Most older persons had good functional status and the associated factors were: being male, being separated or divorced, living alone, visiting friends or relatives, having worked more than 36 years, being literate, not having a visual or hearing impairment, taking one or two medications and having a history of falls. Conclusion The functional status of island older persons was presented as a multifactorial phenomenon, as demographic, social and clinical factors remained independently associated with good functionality. It is important that the maintenance of the functional state is encouraged in the implementation of public policies aimed at the specificities of populations residing on islands.
