ABSTRACT

A clinical observation was made on the injuries of genitourinary tract of in-patient in the department of urology, catholic medical center during the period from Jan. 1972 to Dec. 1981. The results were as follows: 1. Of 4,167 cases hospitalized, 217 cases were injury of genitourinary tract, giving a ratio of 4.9%. 2. The favorable age was from 10 to 39 years for 65.0%. The sex ratio, male to female, was 3.8:1. 3. Traffic accident was the most frequent cause of the injury (35.1%)and the next were fall down, kick and blow etc. The urethra was involved most frequently in 37.2% and the next were kidney in 31.6%, bladder in 16.0%, external genitalia in 12.2% and ureter in 3.0%. 4. The fracture was the most frequent associated injury (90.4%)and the next were intestinal injury and hemothorax. The site of the fracture were pelvis, rib, skull, vertebrae and others. 5. The treatment of the renal injury was conservative treatment frequently (78.1%)and 16 cases were undergone operation (nephrectomy in 6, partial nephrectomy in 5, renorrhaphy in 3 and removal of subcapsular hematoma in 2). The posterior urethra was involved more commonly and the preferable treatment was immediate cystostomy and later reconstruction.

The Genito-Urinary-tract injuries were 128 cases (5.1 per cent) among 2493 war casualities. 2. As for the causes of injuries, 72 cases (56.2 per cent) were caused by gunshot, 28 (21.8 per cent) by boobytrap, 15 (11.7 per cent) by crush, and 5 (3.9 per cent) by grenade. 3. Concerning the frequency of injuries in each organ, 33 cases (25.0 per cent) were observed in bladder, 30 (23. 4 per cent) in urethra, 28 (21.8 per cent) in kidney, 27 (21.0 per cent) in scrotum and testis, 5 (3.9 per cent) in penis, 4 (3.1 per cent) in ureter, and 1 (0.7 per cent) in prostate. 4. The incidence of Genito-Urinary-tract injuries associated with the other organs was as follows. In kidney injuries, 9 cases were associated with intestine, 8 with liver, 8 with spleen, and one each with radial nerve and iliac vessel. In ureter injuries, 4 cases were associated with intestine. In bladder injuries, 7 cases were associated with intestine, 1 with spleen, 15 with pelvic bone, 3 with femur, one each with head and foot. In urethral injuries, 4 cases were associated with intestine, 13 with pelvic bone, 6 with femur, and one with head. In penis injuries, one case was associated with pelvic bone, and 2 with femur. In scrotum and testis injuries, one case was associated with intestine, 8 with pelvic bone, 7 with femur, and one each which head and foot. In prostate injury, one case was associated with intestine. 5. As for the multiplicity of Genito-Urinary-tract injuries, 9 cases were observed in scrotum, testis and urethra, 3 in scrotum, testis and penis, 3 in bladder and urethra, 1 in bladder and prostate. 6. ABSTRACT

Ninety-one cases of genitourinary tract injuries which were admitted to Severance Hospital during the period from Jan. 1960 to Aug. 1968 were analyzed clinically and statistically. And it was investigated compared with previous papers 1) The rate of genitourinary injuries accounted for 7.3 per cent to the total admission patients during eight years sixty-nine per cent of all injuries were in men and thirty-one per cent in women. The age distribution was much the same as that found in previous literature. Most favorable age was in from 20 to50 years for 72.5 percent 2) The meet common involvement site was in urethra for32.9 per cent while external genitalia showing most common site in war wound Also iatrogenic ureteral injuries was relatively high in spite of most less commonest incidence in previous paper 3) Thirty-six per cent of all injuries weredue to traffic or vehicular accident and twenty-four per cent 'due to iatrogenic. Majority of these iatrogenic injuries occurred in pelvic operation. Falls kick and blow were listed not less frequently. 4) On observation of kidney injuries. predisposed hydronephrotic kidney was injured more frequently due to mild external forces and three cases of sixteen kidney injuries had hydronephrosis dueto stone and congenital ureteral stricture in this series 5) An incidence of ureteral injury during radical hysterectomy was variable in literature. but it was noted around 10 percent generally. In reviewing 197 cases of radical hysterectomy for carcinoma of cervix, incidence of ureteral injuries was 12 cases(7.3 per cent) and it was listed 9.3 percent including bladder injuries. 6) In investigating bladder injuries, 53 percent of injuries were associated with pelvic bone fracture And 87 per cent was extraperitoneal type and 13 per cent wasin intraperitoneal rupture. 7) Also majority of urethral injuries were associatedwith pelvic bone fracture especially in car accident cases and then involved posterior urethra for 85 per cent. And bulbous urethra was injured by straddle injury for 70 per cent.
